Abstract Background The risk of metastasis and the survival in patients with primary extremity soft tissue sarcomas is worse when tumour size is large and the grade of malignancy is high. Such tumours may receive chemotherapy and/or radiation therapy (RTX) for optimising local control. Irradiation can either be applied preoperatively or after tumour resection. The question arises if the kind of RTX in the absence of chemotherapy influences the outcome concerning local control, metastatic disease, survival and complications. Methods We retrospectively reviewed the clinical outcome of 233 patients with a primary extremity soft tissue sarcoma treated between 1990 – 2000 with a mean follow-up of 35.8 (4–120) months in our institute. 41 patients had high grade, deep and large tumours (>8 cm), an AJCC stage III (no evidence of metastasis prior to treatment) and were treated with limb salvage surgery and irradiation but stayed without additional chemotherapy. Two groups of patients were compared: the first group received postoperative RTX after tumour resection (n = 33); the second group was treated with preoperative RTX (n = 8). Both groups did not differ concerning clinical parameters. We analysed primary and secondary outcomes. Results 56% (23/41) of the population developed metastatic disease, 24% (10/41) local recurrence. The risk of metastasis was higher in the group with preoperative irradiation (p = 0.046). The overall (p = 0.0248) and relapse free survival (p = 0.104) were worse in this group. The delay to tumour resection amounted 8 weeks on average in the preoperative group. Local control was not different (p = 0.38) in both study groups. Wound infections and other combined therapy related complications were equally distributed (p = 0.22). Conclusion Without chemotherapy there remains a high risk of metastasis in AJCC grade 3 patients. In high risk patients treated without chemotherapy the elapsed time to tumour resection after preoperative radiation might contribute to the development of metastasis. This outcome may support the thesis that a combination of RTX and offensive multimodal treatment protocols is advantageous in such a subset of patients

Osteochondral or, even more rarely, purely chondral fractures of the lateral femoral condyle following patella dislocation in adolescents are relatively infrequent. Various fixation methods are described in the literature, ranging from traditional screws to bioabsorbable constructs or the use of anchors. Each method possesses its own advantages and limitations. Here, we present a case involving a 13-year-old boy with a large purely chondral fracture of the lateral femoral condyle after patella dislocation, initially overlooked. We introduce a cost-effective suture-bridge technique for the fixation of this fracture, which eliminates the need for a second operation and has yielded favorable outcomes. BACKGROUND Supracondylar humeral fracture is a common fracture in the pediatric population. Although extension-type is the most common fracture pattern (97% to 98%), flexion-type supracondylar fractures are rarely encountered (2% to 3%). The combination of a flexion-type supracondylar humeral fracture with an ulnar nerve injury represents a real challenge for an orthopaedic surgeon. CASE REPORT We report 2 cases of flexion-type supracondylar humeral fracture with ulnar nerve injury that open reduction and fixation was necessary because closed reduction could not achieve an acceptable result. An anterior approach to the elbow joint was chosen to explore whether any neurovascular structures were entrapped between the fragments. The ulnar nerve was not found to be compressed in the fracture site. After anatomic reduction, cross K-wire fixation of the fracture was performed. At 6-month follow-up, ulnar nerve injuries (in both patients) were resolved. CONCLUSIONS These case reports enhance the existing literature that flexion-type supracondylar fractures with ulnar nerve injury are associated with higher rates of open reduction. Orthopaedic surgeons should be aware, and family members of those patients should be informed, that the likelihood of an open reduction in these types of injuries is extremely high. Open reduction is needed not only to achieve an anatomic reduction of the fracture but to make sure that the ulnar nerve is not entrapped between the proximal and distal fragment.

The purpose of this case report is to raise awareness of the early diagnosis and treatment of compartment syndrome in children. Late diagnosis can lead to irreversible outcomes, including myonecrosis, neurologic injury, functional problems, and even amputation. Grice-Green extra-articular subtalar arthrodesis is a treatment option for advanced valgus rearfoot deformity of neuromuscular origin in young patients. The purpose of this study was to evaluate long-term results of the procedure in ambulatory cerebral palsy patients. From January 1975 to December 1993, 57 operations were performed in 39 patients (22 males, 17 females) with a mean age of 7.8 ± 2.7 (range 3.9-14.4) years for excessive symptomatic hindfoot valgus. Thirty-five patients (51 feet) were followed for a mean 22.6 ± 4.6 (range 16.0-32.3) years. Preoperative GMFCS score, ambulation, hindfoot position, podoscopic view of the weight-bearing feet, and radiological examinations, along with complications and orthotic use, were compared. Based on a clinical rating scale, 39 results were excellent or good, 8 were fair, and 4 were poor. The poor results were attributed to hindfoot valgus recurrence or varus overcorrection. One case needed revisional surgery owing to slippage of the graft. Overall, the GMFCS score, level of ambulatory distance, and use of foot orthoses improved. Grice-Green subtalar arthrodesis did not result in early degenerative changes at the midterm follow-up. The procedure offers safe and long-lasting correction for severe and symptomatic hindfoot disorders in patients with cerebral palsy. Together with physiotherapy and orthotic use, it can improve ambulation. Aim: The ankle is one of the most often injured joints. About it, poor balance which for example was identified on the basis of an increased sway of the centre of pressure (CoP) while it was measured with force plates is said to be an important risk factor. In the treatment of acute distorsion, ice is recommended after the trauma and in rehabilitation ice is supposed to further the beginning of active exercise and its progression. In practice it can happen that sportsmen are sent back to training or competition directly after cryotherapy. Although the question is controversially discussed, if physical activities can be re-established without danger or if an increased susceptibility to injury exists after ice application. This investigation takes up the problems if ice application at the ankle disturbs static balance so that an enlarged risk of injury could result., Method: In a prospective, controlled crossover study the body sway of 31 healthy subjects (19 women and 12 men, 31 +/- 5 years old with a body height of 1.72 +/- 0.1m and a body weight of 68.13 +/- 11.96 kg) before and after a ten minutes ice application at the ankle (ice) or rather before and after a ten minutes rest without any treatment (con) were analysed. The measurements took place during a 30 seconds single leg stance on a PDM multifunction force plate. The sequence, whether the investigation should first been performed with or without ice application, was allocated randomly., Results and Conclusion: Within the bounds of this study and its restrictions a ten minutes ice application of the ankle did not disturb the static balance in single leg stance and does not hold any enlarged susceptibility to injury with regard to this risk factor. Conclusions about the effect on other risk factors for ankle injury can not be drawn from this study and require independent investigations.

Aim: The morbidity of fixator-assisted distraction osteogenesis should be reduced by intramedullary lengthening devices. The ISKD (intramedullary skeletal kinetic distractor) is a new, fully implantable mechanical lengthening nail. In a prospective cohort trial the possibilities and limitations of the device used on femur and tibia are examined., Methods: 22 patients with a mean age of 25 (range: 16-46) years were treated with an ISKD for femoral (n = 16) and tibial (n = 6) lengthening. The average leg length discrepancy was 48 (range: 25-80) mm. The follow-up was 21 (range: 7-37) months. Clinical and radiological results and complications were evaluated., Results: The results of femoral and tibial applications of the ISKD are different. At the tibia, in three patients a pseudarthrosis occurred and slow callus formation was observed twice. An equinus contracture became evident in 2 patients. At the femur, in one case the lengthening was not accomplished with the device. Five patients were manipulated under anaesthesia at least once to achieve the aim of distraction. Three of these patients received retrograde implantation of the ISKD. An infection or interlocking screw failure was not observed either at the femur or the tibia., Conclusion: The ISKD reduces fixator-associated problems but incorporates its own difficulties which are mainly based on the guidance of the device. Careful patient advice in monitoring the lengthening process is mandatory. At the femur 8 cm of lengthening can be achieved but the nail tends to "block". Proper reaming and osteotomy techniques are important. A lengthening of more than 1 mm/day is recommended to prevent early consolidation. At the tibia weak callus formation and soft tissue contractures occur, therefore not more than 4 cm lengthening should be planned, the distraction speed has to be reduced noticeable below 1 mm/day and the initial immobilisation should be for more than a week.
