This paper examines four challenges facing residential treatment programs: Providing short-term treatment, increasing family involvement, preparing children for life in the community, and offering a "hospital level of care." It shows that past problems in these areas have arisen from the social conditions and historical circumstances in which residential programs evolved rather than limitations inherent in the core concepts upon which the modality is based and that those concepts provide a basis for solving those problems.

This paper examines the variety of ways in which psychotherapy and residential care are integrated in the concluding phase of an intensive short-term treatment program for severely disturbed adolescents. It considers the manner in which decisions are made about termination, approaches to discharge planning, typical issues encountered in the therapy process, techniques for coordinating the work of the therapist and the residential team, ways of addressing crises around termination, the discharge process, and means of facilitating the work of outpatient treatment providers.

This paper reviews the history of residential treatment, examines the central concepts that define the therapeutic modality, and shows how those concepts provide means of addressing criticisms that have been raised about it in the past and adapting residential programs to meet challenges facing them in the future. Paul Lerner's contributions to the Rorschach test were rooted in a psychoanalytic approach to diagnostic testing that began with David Rapaport at the Menninger Clinic in the 1940s. This article reviews the work of Rapaport, Roy Schafer, and their heirs and shows how Lerner's career was devoted to advancing that tradition and assuring its continuing relevance. This final paper of our five-part series utilizes the treatment of substance abuse problems as a paradigm of ways in which residential programs can help adolescents negotiate the transition into the community in a gradual, systematic manner. It considers a range of community resources available to adolescents with chemical dependency problems, ways in which these resources can be incorporated into residential treatment, and their use in different stages of the treatment process.
