Abstract

Background Cardiopulmonary exercise testing (CPET) is essential for assessing the maximal exercise capacity of patients with chronic obstructive pulmonary disease (COPD) and for developing rehabilitation prescriptions. Body composition has been shown to be associated with CPET results, which is complex, time-consuming, contraindicated, and risky, and cannot be performed in some COPD patients, whereas body composition analysis is simple and fast. Objective This paper focuses on the correlation between body composition and CPET in patients with COPD, and provides a basis for predicting maximal exercise capacity, formulating rehabilitation prescriptions, and rational nutritional support for patients with COPD who are unable to complete cardiopulmonary exercise testing. Methods Stable COPD patients were enrolled in the outpatient respiratory clinic of China-Japan Friendship Hospital between July 2021 and March 2022, and were sequentially subjected to body composition measurements and CPET with continuous incremental loading, and the test results were analyzed to assess the relationship between body composition and CPET results, and to analyze the value of the body composition indexes in predicting the results of CPET. Results Seventy-seven eligible patients with stable COPD completed body composition measurement and exercise cardiopulmonary exercise testing. Abstract

Introduction: Center‐based rehabilitation is limited by COVID‐19 infectivity and social distancing policy. We hypothesized that discharged patients benefit from 8‐week home‐based tele‐rehabilitation (tele‐PR) using mobile phones and low‐cost instruments. Methods: The TERCOV (Tele‐rehabilitation in COVID‐19 survivors) is an investigator‐initiated, prospective, multi‐center, real‐world study. After proper assessment, 186 discharge patients received tele‐PR by smartphone, including breathing exercise, respiratory muscle training, aerobic exercise, and resistance training. Physicians, physiotherapists, and nurses provided guidance through smartphone applications. The primary outcome was six‐minute walk distance (6MWD). The secondary outcomes included hand grip, short physical performance battery, maximal inspiratory pressure, maximal expiratory pressure, self‐rating anxiety/depression scale (SAS/SDS), 36‐item short‐form health survey (SF‐36) and international physical activity questionnaire. Results: Dyspnea subgroups were more functionally impaired. After tele‐PR, improvements were observed in exercise capacity(∆6MWD: 16.80 m, 95% CI 1.18–32.42, p < 0.0001), limb muscle function (∆SPPB 0.25 points, 95% CI 0.05–0.46), respiratory muscle strength (∆MIP 16.50 cm H2O, 95% CI 9.22–23.78, p < 0.0001; ∆MEP: 12.09 cm H2O, 95% CI 3.48–20.70, p = 0.0002), health‐related quality of life (∆SF‐36 49.85, 95% CI: 21.01–78.69, p < 0.0001), physical activity(∆HEPA 13.01%, p = 0.0029). Anxiety reduced in patients with mMRC ≥ 2 (∆SAS = −4.19 points, CI −8.16 to −0.22, p = 0.03). Greater change was seen in dyspnea patients. Abstract

Immunosuppression plays a pivotal role in assisting tumors to evade immune destruction and promoting tumor development. We hypothesized that genetic variation in the immunosuppression pathway genes may be implicated in breast cancer tumorigenesis. We included 42,510 female breast cancer cases and 40,577 controls of European ancestry from 37 studies in the Breast Cancer Association Consortium (2015) with available genotype data for 3595 single nucleotide polymorphisms (SNPs) in 133 candidate genes. Associations between genotyped SNPs and overall breast cancer risk, and secondarily according to estrogen receptor (ER) status, were assessed using multiple logistic regression models. Gene-level associations were assessed based on principal component analysis. Gene expression analyses were conducted using RNA sequencing level 3 data from The Cancer Genome Atlas for 989 breast tumor samples and 113 matched normal tissue samples. SNP rs1905339 (A>G) in the STAT3 region was associated with an increased breast cancer risk (per allele odds ratio 1.05, 95 % confidence interval 1.03-1.08; p value = 1.4 × 10(-6)). The association did not differ significantly by ER status. On the gene level, in addition to TGFBR2 and CCND1, IL5 and GM-CSF showed the strongest associations with overall breast cancer risk (p value = 1.0 × 10(-3) and 7.0 × 10(-3), respectively). Furthermore, STAT3 and IL5 but not GM-CSF were differentially expressed between breast tumor tissue and normal tissue (p value = 2.5 × 10(-3), 4.5 × 10(-4) and 0.63, respectively). Our data provide evidence that the immunosuppression pathway genes STAT3, IL5, and GM-CSF may be novel susceptibility loci for breast cancer in women of European ancestry.

41. Comparison of Clinical Characteristics and Short-Term Prognoses Within Hospitalized Chronic Obstructive Pulmonary Disease Patients Comorbid With Asthma, Bronchiectasis, and Their Overlaps: Findings From the ACURE Registry

Abstract

IntroductionReal-world evidence and comparison among commonly seen chronic obstructive pulmonary disease (COPD) phenotypes, i.e., asthma–COPD overlap (ACO), bronchiectasis–COPD overlap (BCO), and their coexistence (ABCO) have not been fully depicted, especially in Chinese patients.MethodsData were retrieved from an ongoing nationwide registry in hospitalized patients due to acute exacerbation of COPD in China (ACURE).ResultsOf the eligible 4,813 patients with COPD, 338 (7.02%), 492 (10.22%), and 63 (1.31%) were identified as ACO, BCO, and ABCO phenotypes, respectively. Relatively, the ABCO phenotype had a younger age with a median of 62.99 years [interquartile range (IQR): 55.93–69.48] and the COPD phenotype had an older age with a median of 70.15 years (IQR: 64.37–76.82). The BCO and COPD phenotypes were similar in body mass index with a median of 21.79 kg/m2 (IQR: 19.47–23.97) and 21.79 kg/m2 (IQR: 19.49–24.22), respectively. The COPD phenotype had more male gender (79.90%) and smokers (71.12%) with a longer history of smoking (median: 32.45 years, IQR: 0.00–43.91). The ACO and ABCO phenotypes suffered more prior allergic episodes with a proportion of 18.05 and 19.05%, respectively. The ACO phenotype exhibited a higher level of eosinophil and better lung reversibility. Moreover, the four phenotypes showed no significant difference neither in all-cause mortality, intensive care unit admission, length of hospital stay, and COPD Assessment Test score change during the index hospitalization, and nor in the day 30 outcomes, i.e., all-cause mortality, recurrence of exacerbation, all-cause, and exacerbation-related readmission.ConclusionsThe ACO, BCO, ABCO, and COPD phenotypes exhibited distinct clinical features but had no varied short-term prognoses. Further validation in a larger sample is warranted.
