Abstract

L’hypothyroïdie acquise de l’enfant est une pathologie rare et peu connue. Elle est souvent secondaire à une thyroïdite chronique auto-immune. Son association à la maladie cœliaque est théorique mais rarement décrite. Les auteurs rapportent l’observation d’un enfant présentant à la fois une hypothyroïdie et une maladie cœliaque. Enfant âgé de 30 mois de sexe masculin, sans antécédents familiaux de pathologie auto-immune, le poids de naissance est de 5 kg, un test de dépistage de l’hypothyroïdie fait à la naissance s’est révélé négatif. A l’âge de 24 mois, on a constaté un arrêt de croissance et un ralentissement psychomoteur. Un bilan hormonal et immunologique a permis de poser le diagnostic d’hypothyroïdie acquise à l’âge de 30 mois. Après 6 mois de traitement hormonal bien conduit, l’enfant a présenté une rechute clinico-biologique de l’hypothyroïdie associée à une diarrhée chronique. Le bilan de la malabsorption avec la biopsie jéjunale effectuée à l’âge de 36 mois a montré une atrophie jéjunale totale faisant évoquer l’association de maladie cœliaque à l’hypothyroïdie acquise. L’évolution été favorable sous régime sans gluten et maintien du traitement hormonal substitutif. Abstract

Background: Streptococcus pneumoniae ( S. pneumoniae ) is the first leading cause of invasive diseases such as meningitis, bacteremia and pneumoniae in children. In this case we report an early neonatal respiratory distress revealing meningitis caused by S. pneumoniae Serotype 17F through vertical transmission, in the newborn of 3 hours of live., Case Description: A male late preterm newborn was born by vaginal delivery at a gestational age of 34 weeks. At 3 hours of life, he was admitted for early moderate neonatal respiratory distress in the Neonatal Medicine and Resuscitation Service. Cerebrospinal fluid culture yielded S. pneumoniae belonging to serotype 17F while the blood culture was negative. The same pneumococcal serotype was recovered from the high vaginal swab of the mother. Both isolates were found susceptible to all tested antibiotics except tetracycline and chloramphenicol to which the strain was resistant. Antibiotherapy management of the child included ceftriaxone at 150mg/kg/day for 21 days, in combination with gentamycin at 5 mg/kg/day for 5 days. ciprofloxacin was added at 40mg/kg/day in two doses for a period of three weeks as the baby presented a hydrocephalus., Conclusion: This finding shows that clinical manifestations of neonatal pneumococcal meningitis may be atypical and/or misleading., (© 2021 Nzoyikorera N et al.)

Abstract

Neonatal adrenal hematoma is a rare condition, most frequently caused by trauma. We report three cases of adrenal hematoma admitted to the Neonatology and Neonatal Intensive Care Unit in the A. Harouchi Children's Hospital, the Ibn Rushd University Hospital in Casablanca, Morocco, over a 2-year period from January 2011 to December 2012. The average age of these patients was 5 days. The clinical presentations were diverse; the most common manifestations were intense jaundice in one case, acute adrenal insufficiency in one case, and severe anemia in the other case. Abdominal ultrasonography was used to confirm the diagnosis and monitor adrenal hemorrhage in all the patients. Analysis of clinical, laboratory, and ultrasonography data showed a favorable prognosis in all the patients. Based on these observations, we discuss the risk factors, clinical presentations, progression and management of neonatal adrenal hemorrhage., (Copyright © 2014. Published by Elsevier SAS.)

Abstract

Gastric pneumatosis is a rare pathology. Its occurrence in the neonatal period requires looking for ulcerative-necrotizing enterocolitis in a context of prematurity or an underlying surgical obstacle. We report a case of gastric pneumatosis at a newborn child born at term, admitted on the third day of life for neonatal occlusion with a flat stomach. The abdomen without preparation showed substantial gastric distension with aspects of gastric pneumatosis and embellishes with images showing a double gastric bubble. The surgical exploration showed gastric pneumatosis, complete duodenal atresia located at the level of the second duodenal portion, and an annular pancreas.
