4. Impact of Hepatic Portal Venous Gas on the Prognosis of Traumatic Out‐of‐Hospital Cardiac Arrest: A Reason to Consider Terminating Cardiopulmonary Resuscitation.

Abstract

Background. We evaluated the prognosis of traumatic out‐of‐hospital cardiac arrest (OHCA) by assessing the presence of hepatic portal vein gas (HPVG) observed in ultrasound (US) or point‐of‐care ultrasonography (POCUS) performed during CPR. Furthermore, we aimed to understand the role of HPVG in decision‐making regarding CPR discontinuation or withholding in traumatic OHCA. Methods. The retrospective study was conducted at the level 1 trauma center of urban academic medical centers in South Korea. We included adult trauma OHCA patients who underwent CPR between January 1, 2020, and June 30, 2022. Data on traumatic OHCA patients who presented to the level I trauma center during this period were extracted from the hospital's electronic medical record system. The arrest data were separately managed through the hospital's electronic medical record system for quality control, specifically the arrest registry. US images or clips of the hepatic portal vasculature (HPV) during CPR were used to assess the presence of HPVG. These images were independently reviewed by two emergency medicine physicians with several years of US examination experience who were blinded to all clinical details and outcomes. We evaluated the prognosis of traumatic OHCA by assessing the presence of HPVG using the US. In addition, we analyzed the general characteristics and assessed the impact on the ROSC in traumatic OHCA. Results. Among the 383 cardiac arrest patients, 318 traumatic OHCA patients were included. The mean age was 54.9 ± 19.4 years, and most patients were male. The initial rhythm was mainly asystole, and falls were the most frequent cause of injury. The overall ROSC rate was 18.8%, with a survival rate of 7.2% at hospital discharge. Among the 50 patients who underwent a US examination of HPV, 40 showed HPVG. The HPVG group had a significantly lower ROSC rate and survival rate at ED discharge and hospital discharge compared to the group without HPVG. Conclusion. Traumatic OHCA with HPVG presents a significantly worse prognosis. This suggests that early consideration of termination or withholding of CPR may be appropriate in such cases. We examined the relationship between the rural, regional, and racial variables using chi-square tests and binary logistic regression associated with telemedicine use in our multivariable analysis.Results: Telemedicine use by population decreased with decreasing urbanization level, from 40.2% among adults living in large central metropolitan to 29.7% among adults living in rural area (p< 0.0001). Abstract

This research was conducted to determine the impact of high sodium intake on various obesity-related genes. Four week old Sprague-Dawley rats were purchased at Daehan Biolink. After one week of fertilization, the rats were used at 5 weeks of age. Rats were divided into three groups that received sodium orally at 0 mM(CON), 100 mM(Salt-100 mM), or 400 mM(Salt-400 mM). After 3 weeks, rats were sacrificed and used in the experiment. Triglycerides (TG) and total cholesterol (TC) were measured in the blood serum and liver. The activities of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were analyzed to test the hepatic toxicity. Genes related to lipogenesis and adipogenesis, uncoupling protein1 (UCP1), and tumor necrosis factor-alpha (TNF-α) were expressed in the liver, white adipose tissue (WAT) and brown adipose tissue (BAT). The levels of albumin and creatinine were measured in the urine. The TG and TC levels in blood serum were highest in the Salt-100 mM group, while liver TG and TC were highest in the Salt-400 mM group. Lipogenesis and adipogenesis related genes in the liver did not increase proportionally to salt concentration. In visceral and subcutaneous fat, peroxisome proliferator-activated receptor gamma (PPARγ) and CCAAT/enhancer-binding protein alpha (C/EBPα) were upregulated proportionally with increasing sodium levels. These results suggest that high sodium intake provoked adipogenesis in SD rats. In BAT, the expression of UCP1 was lower in the Salt-400 mM group than the CON group. Additionally, TNF-α in visceral fat and urine albumin and creatinine were higher in salt treated groups than the CON group. Overall, the results indicate that high sodium intake may be associated with obesity and complications.

Abstract

Objective: Despite its established benefits, palliative care (PC) is not well known among patients and family/caregivers. From a nationally representative survey, we sought to assess the following associated with PC: knowledge, knowledge sources, and beliefs.Methods: Data were drawn from the Health Information National Trends Study (HINTS 5 Cycle 2), a cross-sectional, survey of non-institutionalized adults aged 18+ years in the USA. Data were weighted and assessed by proportional comparison and multivariable logistic regression.Results: A total of 3504 respondents were identified, and approximately 29% knew about PC. In the adjusted model, less PC knowledge was associated with: lower age (those aged <50), male gender, lower education (Significance Of Results: PC knowledge was generally low (1-in-3 respondents knew of PC), with significant differences according to age, gender, education, and internet use.
