- Abstract
Background: Peritoneal dialysis (PD) has become an increasingly important treatment modality for end-stage renal disease. However, application of PD in patients with liver cirrhosis (LC) and subsequent outcomes have not been thoroughly evaluated. Methods: A total of 1,366 patients (≥ 18 years old) who started PD at 4 tertiary referral centers between January 2000 and December 2015 were initially reviewed. Among them, 45 patients with LC were finally analyzed (LC-PD). Using the multivariate Cox hazard ratio (HR) model, outcomes such as technique failure, infection, and mortality in patients with LC-PD were compared with those in non-LC-PD patients (non-LC-PD) or patients with LC who received hemodialysis (LC-HD). All of the patients were selected by 1:1 matching of age, sex, catheter insertion date, and diabetes mellitus. Results: During the mean follow-up duration of 43 ± 35.8 months, 12 patients with LC-PD experienced technique failure, and this rate was similar to that of non-LC-PD patients. In evaluating infection episodes, the most common causes for peritonitis and exit-site infection were Escherichia coli (5.8%) and Staphylococcus aureus (19.3%), respectively; these event rates of LC-PD did not differ from those of non-LC-PD. The all-cause mortality rate of the LC-PD group was not different from that of the non-LC-PD and LC-HD groups. Conclusion: Dialysis outcomes such as technique failure, infection, and mortality are not affected by the presence of LC. We describe the use of the fluorescent reporter compound CDr10b to label mid-intestinal structures in zebrafish larvae after simple immersion. CDr10b is deposited into the gut where it initially fills the lumen and is excreted. Using laser-mediated injury of the intestine, we show that CDr10b provides a useful readout of the integrity and repair of the epithelial cell barrier. In addition, CDr10b specifically labels the absorptive mid-intestine segment that is analogous to the mammalian small intestine. By perturbing retinoic acid signaling, which regulates the size of the mid-intestine segment, we show that CDr10b is a valuable tool to rapidly assess developmental malformations of the intestine in live animals. Many tumour cells express both Fas and its ligand (FasL) on their surface and it has remained a mystery why such cells do not simply kill themselves. It remains to be determined whether Fas and FasL are expressed in human hepatoblastomas and if so, what is responsible for the possible Fas resistance of these tumours. In this study, the expression of Fas and FasL was examined in 23 cases of human hepatoblastoma by immunohistochemical staining. To elucidate possible Fas resistance in hepatoblastomas, Fas‐resistance pathways including the expression of bcl‐2 and Fas‐associated phosphatase‐1 (FAP‐1), and the expression of soluble Fas (sFas) mRNA, were analysed by immunohistochemistry and in situreverse transcription‐polymerase chain reaction (in situRT‐PCR). Fas gene mutation in the death domain was also examined. Fas and FasL were expressed in all hepatoblastomas analysed. Twenty (87 per cent) and 18 (78 per cent) cases of hepatoblastoma were positive for sFas mRNA and FAP‐1, respectively, but none of the hepatoblastomas expressed bcl‐2. Mutation in the death domain of the Fas gene was not found in hepatoblastomas. Taken together, these findings demonstrated that Fas, a death receptor, and its ligand are co‐expressed in hepatoblastomas in vivo, but some inhibitors of Fas‐mediated apoptosis are also expressed in these tumours. These results suggest that it is probably due to the action of inhibitory molecules of the Fas pathway that the tumour cells of hepatoblastomas do not kill themselves in an autocrine‐driven cycle and that in this manner hepatoblastomas avoid apoptosis. 20. Peritoneal Dialysis Care for People with Diabetes, Polycystic Kidney Disease, or Advanced Liver Disease.

- Abstract
People treated with peritoneal dialysis (PD) often have complicating conditions that require careful management. Three such conditions are reviewed in this article-diabetes mellitus, polycystic kidney disease, and chronic liver disease. Each of these conditions requires an understanding of both its effect on the delivery of the PD and the effect of the PD on the condition itself. In diabetes, glucose absorption from the dialysate complicates metabolic control and affects salt and water management and patient outcome. There is particular benefit in clinical care being delivered through a multidisciplinary team that involves both kidney and diabetes experts. In relation to polycystic kidney disease, a key issue is the potential for increased intraperitoneal pressure due to the combined effect of the enlarged polycystic organs and the presence of the dialysis solution, and therefore, the PD prescription requires to be managed with a particular focus on limiting that pressure. For patients with liver disease, key issues include nutritional support because PD can add to protein losses already consequent on the liver disease itself. Considered approaches are required to manage ascites and reduce infection risk and the potential for hernias and leaks to develop. Mortality in this group is unfortunately high-however, PD may present a better management option than hemodialysis in many patients-particularly in those where the liver disease is complicated by low BP, clotting abnormalities, or troublesome ascites. Overall, the choice to use PD in patients with these complicating conditions should be based on shared decision making with the patient and their family members informed by high-quality information in which risks, benefits, and management strategies are clearly presented., (Copyright © 2024 by the American Society of Nephrology.)

The article presents problems that the concepts of God's immutability, impassibility, and simplicity pose for Christian theism. Through the critical analysis, the author indicates the roots of these ideas (mostly, but not only, in ancient Greek philosophy) and the consequences of absolutizing them for the image of God and His relationship to the world. The more general purpose of the paper is to highlight the danger of the strong dependence of religious depictions on philosophical grounds. As concluded in the article, it is a mistake to absolutize the particular solutions that appeared at the junction of fides et ratio at one of the historical stages. The awareness of this becomes extremely important when reflecting on the divine immutability, impassibility, and simplicity. The author shows some proposals to reformulate their understanding in light of contemporary analytic philosophy. Partial reinterpretation in this regard may help to rethink the concept of the Creator's interaction with creation and to reconcile the biblical image of God with the philosophical one. Background & Aims: Regorafenib demonstrated a clinical benefit for patients with unresectable hepatocellular carcinoma (uHCC) in the phase III RESORCE trial. Considering the heterogeneity of uHCC and discrepancies in its characteristics between prospective trials and daily practice, real‐life evidence is necessary. Methods: This multicentre, retrospective analysis was performed by the Korean Cancer Study Group. In total, 440 patients who received regorafenib between January 2017 and November 2019 were identified in nine tertiary referral hospitals in Korea. Results: All patients received prior sorafenib, and the median time‐to‐progression (TTP) on sorafenib was 3.9 months (range, 0.2‐71.6). Regorafenib was used as the second, third and fourth to seventh lines of therapy in 305 (69.3%), 115 (26.1%) and 20 (4.5%) patients respectively. According to the RECIST v1.1, the overall response rate was 7.7% (n = 34), and the median progression‐free survival (PFS) and overall survival (OS) were 3.2 (95% CI, 2.8‐3.5) and 12.1 (95% CI, 9.7‐14.5) months respectively. Immune checkpoint inhibitors (ICIs) were given in 115 patients (26.1%) prior to regorafenib. There were no differences in PFS and OS with regorafenib according to the prior use of ICIs (PFS, P =.61; OS, P =.63). The occurrence of hand‐foot skin reaction (HFSR) was associated with a better OS (P <.001). Conclusions: The real‐life clinical outcomes of regorafenib for patients who progressed on prior systemic therapy including ICIs were consistent with the phase III trial results. 26. Use of prophylactic silicone adhesive dressings for maintaining skin integrity in intensive care unit patients: A randomised controlled trial.

The aims of this randomised controlled study were to determine whether the application of silicone adhesive dressings with standard preventive care would reduce the impairment of skin integrity and to evaluate the subepidermal moisture (SEM) value in anatomical locations to predict impairment of skin integrity. Of the 71 intensive care unit patients enrolled, 35 and 31 were randomly assigned to the intervention and control groups, respectively. The SEM value was measured at each site (the coccyx, sacrum, and both buttocks) repeatedly three times at every dressing change. Collected data were analysed by multivariate linear regression, and least square means analysis was used to compare the SEM value at the sites of pressure injury (PI) and blanching erythema. There were fewer PIs (1 vs 9) and incidences of blanching erythema (5 vs 6) in the intervention group than in the control group (P = 0.006). The SEM value and difference of the SEM value from the initial value was higher in the group with PIs than in the group without PIs (P < 0.0001 and P = 0.001, respectively). There was a statistically significant difference in the incidence of impaired skin integrity between the groups. The SEM value may be an indicator for detecting impairment of skin integrity. Pressure ulcers result in financial losses, including the cost of unnecessary medical expenses because of extended hospital stays, treatment, and examination. This was a retrospective, observational, methodological study to develop quality indicators related to pressure ulcer development and validate risk adjustment factors for pressure ulcer development. We performed a literature review to develop risk adjustment factors, and an expert group performed a content validity test. To validate risk adjustment factors for pressure ulcer development using electronic medical records, 127 patients admitted to a long‐term care hospital in South Korea from June to September 2015 were enrolled in the study. Pressure ulcer risk factors were peripheral vascular disease, end‐stage disease, past pressure ulcer history, high risk group for pressure ulcer development, fever, haemoglobin, and albumin (all P < 0.05); only albumin (odds ratio: 0.210, P < 0.001) was significantly associated with pressure ulcer development as an independent risk factor. Further research with a large sample size is needed for the validation of risk adjustment factors. Risk‐adjusted quality indicators for pressure ulcer development can be used to evaluate the quality of nursing care and compare outcomes after preventive pressure ulcer care activities or between long‐term care hospitals. Herein, a mesoporous imine COF with densely arranged reactive vinyl groups (COF-V) was inverse-vulcanized with sulfur and used as a cathode material for Li–S batteries. Owing to the synergetic chemical covalent binding with the vinyl groups and physical confinement by the mesoporous channels, the shuttle effect of polysulfides could be efficiently suppressed. As a result, the inverse-vulcanized S-COF-V cathode demonstrated a high initial capacity of 1400 mA h g
−1 which retained 959 mA h g−1 after 100 cycles at 0.2C and can be further cycled 1000 times at 1C with a low fading rate of 0.057% per cycle. These results are among the best performances of reported COF-based sulfur cathodes. Our findings provide a new approach to develop efficient cathode materials for Li–S batteries. Mangrove plant endophytic bacteria are prolific sources of bioactive secondary metabolites. In the present study, twenty-three endophytic bacteria were isolated from the fresh roots of the mangrove plant Rhizophora apiculata. The identification of isolates by 16S rRNA gene sequences revealed that the isolated endophytic bacteria belonged to nine genera, including Streptomyces, Bacillus, Pseudovibrio, Microbacterium, Brevibacterium, Microbulbifer, Micrococcus, Rossellomorea, and Paracoccus. The ethyl acetate extracts of the endophytic bacteria's pharmacological properties were evaluated in vitro, including antimicrobial, antioxidant, α-amylase and α-glucosidase inhibitory, xanthine oxidase inhibitory, and cytotoxic activities. Gas chromatography–mass spectrometry (GC-MS) analyses of three high bioactive strains Bacillus sp. RAR_GA_16, Rossellomorea vietnamensis RAR_WA_32, and Bacillus sp. RAR_M1_44 identified major volatile organic compounds (VOCs) in their ethyl acetate extracts. Genome analyses identified biosynthesis gene clusters (BGCs) of secondary metabolites of the bacterial endophytes. The obtained results reveal that the endophytic bacteria from R. apiculata may be a potential source of pharmacological secondary metabolites, and further investigations of the high bioactive strains—such as fermentation and isolation of pure bioactive compounds, and heterologous expression of novel BGCs in appropriate expression hosts—may allow exploring and exploiting the promising bioactive compounds for future drug development. It is well known that the Chinese church (and therefore Chinese theology) has been divided in the People's Republic of China into'registered'and'unregistered'churches, and that while the state-approved church offers theologies that align with the nation's socialist values, the unregistered or house churches have tended toward an evangelical theology that is now divaricating with the resurgence of denominations in China. What is less well known is that there has been a vibrant third field of Chinese theology: Sino-Christian theology, a contested, academic discourse that makes no claims to be a confessional theology. This theology, fostered by academics in secular Chinese universities, developed in the 1990s and has grown and diversified with the increasing number of scholars researching Christianity in China. This volume offers essays on the message of Chinese Christian art, for example, alongside textual and Christological studies. The phenomenon of Sino-Christian theology, along with debates on its right to exist (if not authored by professing Christians), overlaps with broader debates on the nature of a'Chinese theology'and its unifying features.The second part of the volume draws together nine essays on theological concerns in the Chinese diaspora. These range from the nature of diasporic experience itself to studies of individual writers, and from fundamentalist beliefs in Singapore to a queer theology academy in Hong Kong. The Oxford Handbook of Pauline Studies brings together a diverse international group of experts on the apostle Paul. It examines the authentic texts from his own hand, other ancient texts falsely attributed to him, the numerous early Christian legends about him, and the many meanings that have been and still are made of these texts to give a twenty-first century snapshot of Pauline Studies. The phenomenon of South Korean Christianity is, in a word, remarkable. In less than 250 years, 29% of South Korea's population adheres to Christianity, a staggering 71% of Korean Americans identify as Christian, and the powerful zeal of Korean Christians to spread the Gospel's influence in South Korea already overshadows other established religious groups (i.e. Buddhism, Confucianism). This phenomenon-particularly the rapid growth and unique interpretation of Christianity among Koreans around the world-is intimately and inextricably tied to how Koreans appropriated the Bible in their religio-cultural and socio-political milieu from the 18th century onward. Less noted and understood, however, is the tapestry of Korean biblical interpretation that emerged from being missionized, colonized, divided, and globalized. These influences reflect a distinctive Korean-ness of biblical interpretation that relates closely to Korean perceptions of divine liberating intervention, and the Korean diaspora that seeks to move beyond oppression. This Handbook offers a comprehensive overview on how the Bible has been used by faith communities in Korea and the Korean diaspora over two centuries. In this volume, noted theologically diverse scholars present representative thinking on creative inculturations of the Bible in Korea. Some conservatively align with received western orthodoxy. In Sound Alignments, a transnational group of scholars explores the myriad forms of popular music that circulated across Asia during the Cold War. Challenging the conventional alignments and periodizations of Western cultural histories of the Cold War, they trace the routes of popular music, examining how it took on new meanings and significance as it traveled across Asia, from India to Indonesia, Hong Kong to South Korea, China to Japan. Any cutaneous insult results in wound healing and wound healing results in formation of a scar. Human cutaneous scars represent a very important problem in clinical practice. Human cutaneous scars cause both cosmetic and medical problems in people who have them. There are various types of human cutaneous scars: physiologic normotrophic scars, atrophic scars, and proliferative scars. Additionally, there are scar complications that physicians must deal with. Scar complications include unstable scars, contractures and even development of malignancy at worst. Treatment of scars is also challenging especially in case of development of abnormal scars. Human cutaneous scars have various aspects of interest such as biology, clinical significance, patient perspective, prevention, and treatment. There are various preclinical models for studying them. Scarless healing is another item of interest that is observed in fetal wound healing and in some animals. There are various studies specifically examining the biology and the treatment of proliferative scars. The cutaneous scars are very common so every physician will face a cutaneous scar-related problem in his/her medical practice. The significance is indicated by the frequency of burn injury and traffic accidents and by the importance of incision scars for any surgeon.
