To update the melanoma staging system of the American Joint Committee on Cancer (AJCC) a large database was assembled comprising >46,000 patients from 10 centers worldwide with stages I, II, and III melanoma diagnosed since 1998. Based on analyses of this new database, the existing seventh edition AJCC stage IV database, and contemporary clinical trial data, the AJCC Melanoma Expert Panel introduced several important changes to the Tumor, Nodes, Metastasis (TNM) classification and stage grouping criteria. Abstract

We report the use of a ~50-mW peak power 4.3-THz quantum cascade laser (QCL) as an illumination source for real-time imaging with a 320 times 240 element room-temperature microbolometer focal-plane array detector. The QCL is modulated synchronously with the focal-plane array for differential imaging. Signal-to-noise ratios of ~340 are achieved at a 20-frame/s acquisition rate, and the optical noise equivalent power of the detector array at 4.3 THz is estimated to be ~320 pW/radicHz. Abstract

This letter reports the fabrication and demonstration of an ErAs:GaAs interdigitated photomixer as a tunable THz source ranging from ∼20 GHz to ∼2 THz, with 12 μW maximum power typically around ∼90 GHz. Each photomixer is coupled to a composite dipole-spiral planar antenna that emits a Gaussian-type beam into free space. The beam switches from dipole to spiral antenna behavior as the frequency increases. A distributed Bragg reflector is embedded in the device beneath the photomixer to increase its external quantum efficiency. The photomixer has a 900 Å thick silicon nitride coating which serves as an antireflection and passivation layer, and also improves the reliability and heat tolerance of the device. Abstract

This letter reports the use of a photomixing sweep oscillator to characterize samples of B. subtillis spores at room temperature between 200 and 1200 GHz. Dilute (10% by mass) and concentrated samples were tested, the former using fine-grained polyethylene powder as a low-loss dielectric matrix. The dilute sample displays many weak transmission undulations, but the three centered at 418, 1037 and 253 GHz were strong enough to remain after normalization. The concentrated sample displays a much stronger broadband attenuation, precluding accurate measurements above ∼500 GHz. Two broad signatures remain between 250 and 290 GHz, and between 410 and 440 GHz, respectively. The results suggest that single-bioparticle electromagnetic resonance, perhaps from surface phonons, occurs in the dilute sample but is smeared in the concentrated sample. © 2004 American Institute of Physics. Abstract

To audit the accuracy of magnetic resonance (MR) staging of nasopharyngeal carcinoma (NPC) in daily reporting, the MR images of 101 adult patients with newly diagnosed NPC reported between December 1996 and February 2002 were reviewed retrospectively. Based on the American Joint Committee on Cancer/International Union Against Cancer (AJCC/UICC) TNM staging criteria (1997), the tumour staging obtained from the MR reports and the MR films was compared by two experienced head and neck radiologists. The number of patients being upstaged, downstaged or unchanged was noted. In all stages, the NPC staging obtained from the MR reports revealed 18 (17.8%) understaged, eight (7.9%) overstaged and 75 (74.2%) the same stage when compared to the staging obtained from the MR films based on the AJCC/UICC criteria. The percentage of patients being understaged or overstaged, in decreasing order of frequency, were stages II, III, IV and I. Magnetic resonance of NPC should be reported by radiologists who are not only familiar with the pathology of this condition and its pattern of spread but who should also base their reports on the AICC/UICC staging criteria. The necessity to improve our MR staging accuracy is largely because it determines the type of therapy to be given and this has to be appropriate and adequate for a successful treatment.
