Abstract

With the increase in laser power and finesse of optical cavities over the last decade, laboratory-size Compton sources are very promising. These sources produce X-rays through interactions between relativistic electrons and laser photons and, in term of brightness, fall between large synchrotron facilities and classical laboratory X-ray sources. The ThomX source is the French project in this field. This article first presents a state of the art of high-intensity Compton sources, then the ThomX source is briefly described, and the first results are detailed, in particular the production of the first X-rays, the acquisition of the first spectrum and the first image of the beam. Finally, the next objectives are discussed.

Abstract

ThomX is a new generation Compact Compton Source. It is currently commissioned by and at the IJCLab (Laboratoire de physique des 2 infinis - Irène Joliot-Curie (UMR9012)) at Orsay. The first beam is expected at the begining of 2021. The aim of ThomX is to demonstrate the characteristics of an intense and Compact (lab-size) X-ray source based on Compton Scattering. The performances are mostly driven by the laser optical system which is above the state of the art of stored laser power. Proof of principle of various X-ray techniques will be performed thanks to the versatile ThomX beamline. Firstly, this article presents the machine description. Secondly, the issues and limits of the laser system are discussed. Then, the ThomX beamline is described and the machine status conclude the ThomX presentation. Finally, the expected performances for the next years and the possible experiments that can be made with this new machine are detailed.

Abstract

International audience; ThomX is a new generation Compact Compton Source. It is currently commissioned by and at the IJCLab (Laboratoire de physique des 2 infinis - Irène Joliot-Curie (UMR9012)) at Orsay. The first beam is expected at the begining of 2021. The aim of ThomX is to demonstrate the characteristics of an intense and Compact (lab-size) X-ray source based on Compton Scattering. The performances are mostly driven by the laser optical system which is above the state of the art of stored laser power. Proof of principle of various X-ray techniques will be performed thanks to the versatile ThomX beamline. Firstly, this article presents the machine description. Secondly, the issues and limits of the laser system are discussed. Then, the ThomX beamline is described and the machine status conclude the ThomX presentation. Finally, the expected performances for the next years and the possible experiments that can be made with this new machine are detailed.

10. Validation of ICD-10 codes for studying foreign body airway obstructions: A health administrative data cohort study.

Abstract

Aim: To validate a case definition for foreign body airway obstructions (FBAO) using International Classification of Diseases version 10 (ICD-10) codes to accurately identify patients in administrative health databases and improve reporting on this injury., Methods: We identified prehospital patient encounters in Alberta, Canada between Jan 1, 2018 and Dec 31, 2021 by querying the provincial emergency medical services' (EMS) patient care records for FBAO-related presentations, EMS protocols, or treatments. We deterministically linked EMS patient encounters to data on emergency department visits and hospital admissions, which included ICD-10 codes. Two physicians independently reviewed encounters to determine true FBAO cases. We then calculated diagnostic accuracy measures (sensitivity, specificity, likelihood ratios) of various algorithms., Results: We identified 3677 EMS patient encounters, 2121 were linked to hospital administrative databases. Of these encounters, 825 (38.9%) were true FBAO. The combination of two ICD-10 codes (T17 = foreign body in the respiratory tract or T18.0 = foreign body in the mouth) was the most specific algorithm (96.9% [95%CI 95.8-97.8%]), while the combination of all FBAO-related ICD-10 codes and R06.8 (other breathing abnormalities) was the most sensitive (75.0% [95%CI 71.9-78.0]). We identified an additional 453 (35.4%) FBAO cases not transported by EMS (due to death or transport refusal), and therefore not linked to the hospital administrative databases. Of these unlinked encounters, 44 (9.7%) cases resulted in the patient's death., Conclusions: FBAO can be identified with reasonable accuracy using health administrative data and ICD-10 codes. Abstract

Exostosis is a solitary benign bone tumor frequently observed in children. It may be totally quiescent or provoke complications. We report a case of pseudo-aneurysm of the popliteal artery caused by an exostosis on the lower metaphysis of the femur in a 12-year-old boy. This unusual complication mostly reported in young males (mean age 19 years) occurs in the context of an initial trauma in half the cases. Surgical treatment is a semi-emergency requiring both bone and arterial repair. Preventive surgery should be discussed for all cases of exostosis with a risk of arterial damage due to the gravity of the potential vascular complications.

Abstract

Unlabelled: Isolated popliteal artery is defined as an obstruction of a superficial femoral artery with a patent popliteal segment followed by an obstructed distal popliteal artery or a patent leg artery less than 5 cm long., Purpose: The aim of this retrospective study was to report the results of surgical treatment and the causes of failures., Patients and Methods: From 1988 to 1996, 31 patients with isolated popliteal artery were operated on with femoropopliteal bypass. The age of the patients ranged from 45 to 92 years, (mean: 79 years); all had critical ischemia that threatened limb viability. All underwent preoperative arteriography and diagnosis was confirmed by intraoperative arteriography., Results: In the postoperative course, there were 22 patent bypasses (68%) with minor amputation in five patients, and nine thromboses that required a major amputation in seven patients, a trans-metatarsal amputation in one, and a medical treatment in one. With a mean 37-month follow-up, seven thromboses required a major amputation in five patients, a new bypass in one and a medical treatment in one. The death rate was 34% at two years. The actuarial patency rates of the bypasses were 51% at one year, 38% at two years and 25% at five years. The limb salvage rate was identical. The patency rates were 65% at one, two and five years for venous bypasses and 38%, 13% and 0% respectively for PTFE bypasses. Statistical analysis showed two causes of failure: the absence of a run-off branch and the use of PTFE prostheses. No other statistically significant cause of failure was demonstrated among those analysed. Favourable anatomic conditions for a bypass to a leg artery were not predictive of failure of a femoro-popliteal bypass on the isolated arterial segment., Conclusion: Bypass to isolated popliteal artery is indicated in patients whose limb viability is jeopardized. Results may be considered as satisfactory especially if there is a run-off branch and if a venous graft is available for the bypass.
