Abstract

Abstract The presence of numerous mast cells (MCs) mixed with tumor cells in the bone marrow (BM) is a hallmark of the diagnosis of Waldenström's macroglobulinemia (WM). MCs have been shown to support lymphoplasmacytic cell growth, but there is thus far no demonstration of the prognostic impact of BM MC density in WM. We investigated BM MC density by sensitive and specific digital quantification, allowing the analysis of a large area infiltrated by BM tumor cells. A total of 65 WM patients were investigated, including 54 at diagnosis and 11 at relapse. Tryptase and CD20 immunohistochemisty staining was performed on contiguous sections of deparaffinized BM trephine biopsies. After numerization of each section, the BM surface area was manually marked out, excluding the bone framework and adipocytes to limit the analyses to only hematopoietic tissue. MCs were assessed using a digital tool previously used to quantify immune‐cell infiltrates on tumor‐tissue sections. Deep next‐generation sequencing and allele‐specific PCR were used to explore the MYD88 and CXCR4 mutational status. MC density was heterogeneous among the WM patients. An optimal MC density threshold (> 56 MC.mm–2) was defined according to ROC curve analysis of overall survival. A higher MC density (> 56 MC.mm–2) was associated with greater BM involvement by WM lymphoplasmacytic cells and less hepatosplenic involvement (p = 0.023). Furthermore, MC density significantly correlated with a higher ISSWM score (p = 0.0003) in symptomatic patients. Patients with a higher MC density showed shorter median OS (56.5 months vs. nonreached, p = 0.0004), even in multivariate analysis after controlling for other predictive variables, such as age, ISSWM score, and CXCR4 mutational status. In conclusion, MC density can be accurately measured in WM patients using a specific digital tool on well‐outlined hematopoietic tissue surfaces. High MC density is associated with aggressive features and a poor clinical outcome, emphasizing the need for further investigation of the involvement of MCs in the pathophysiology of WM.

Abstract

Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare aggressive T-cell lymphoma most reported in Asia. We performed a comprehensive clinical, pathological and genomic study of 71 European MEITL patients (36 males, 35 females, median age 67 years). The majority presented with gastrointestinal involvement and had emergency surgery, and 40% had stage IV disease. The tumors were morphologically classified into two groups: typical (58%) and atypical (i.e., non-monomorphic or with necrosis, angiotropism or starry-sky pattern) (42%), sharing a homogeneous immunophenotypic profile (CD3+ [98%] CD4- [94%] CD5- [97%] CD7+ [97%] CD8+ [90%] CD56+ [86%] CD103+ [80%] cytotoxic marker+ [98%]) with more frequent expression of TCRgd (50%) than TCRab (32%). MYC expression (30% of cases) partly reflecting MYC gene locus alterations, correlated with non-monomorphic cytology. Almost all cases (97%) harbored deleterious mutation(s) and/or deletion of the SETD2 gene and 90% had defective H3K36 trimethylation. Other frequently mutated genes were STAT5B (57%), JAK3 (50%), TP53 (35%), JAK1 (12.5%), BCOR and ATM (11%). Both TP53 mutations and MYC expression correlated with atypical morphology. The median overall survival (OS) of 63 patients (43/63 only received chemotherapy after initial surgery) was 7.8 months. Multivariate analysis found a strong negative impact on outcome of MYC expression, TP53 mutation, STAT5B mutation and poor performance status while aberrant B-cell marker expression (20% of cases) correlated with better survival. In conclusion, MEITL is an aggressive disease with resistance to conventional therapy, predominantly characterized by driver gene alterations deregulating histone methylation and JAK/STAT signaling and encompasses genetic and morphologic variants associated with very high clinical risk.

Abstract

Composite and sequential lymphomas involving both classical Hodgkin lymphoma (CHL) and primary mediastinal B-cell lymphoma (PMBCL) are rare phenomena. Beyond the relevant biological interest raised by these cases, treatments and outcome data are poorly covered in the recent literature. This retrospective analysis describes the pathological and clinical characteristics of 10 composite and 15 sequential cases included after a central pathological review. At diagnosis, 70% of the composite lymphomas presented a disseminated and extranodal disease. Among the 15 sequential lymphomas, 12 were CHL at first occurrence and three were PMBCL. Based on their clinical evolution, these sequential lymphomas could be divided into early (i.e., diagnosis of second lymphoma within a year) and late [(i.e., a second lymphoma occurrence occurring after a long period of complete remission]). All composite cases were alive in complete remission after a median follow-up of 34 months. If the early sequential lymphoma presented a particularly poor outcome with a median overall survival shorter than one year, the late cases were efficiently salvaged. Further molecular studies are needed to describe the underlying biology of these rare diseases, possibly representing the extreme of tumour cell plasticity found in grey-zone lymphoma.

TO THE EDITOR : Patients with chronic lymphocytic leukemia (CLL) exhibit a variety of skin lesions including mostly non-specific cutaneous manifestations (such as cutaneous infections or exaggerated reactions to insect bite) and secondary cutaneous malignancies, as patients are at high risk of developing basal cell carcinoma, squamous cell carcinoma, melanoma, and Merkel cell carcinoma. Specific cutaneous infiltration by neoplastic B lymphocytes with clinically identifiable skin lesions, also called leukemia cutis (LC), is more uncommon and has seldom been reported in chronic lymphocytic leukemia. Abstract

The oncogenic role of TCL1 in chronic lymphocytic leukemia is well established in transgenic mice. TCL1 expression in other B-cell malignancies has been also described: post-germinal center-derived malignancies, such as multiple myeloma, classically do not express TCL1. Waldenström macroglobulinemia is a post-germinal center malignancy that is known to be similar to chronic lymphocytic leukemia in terms of its gene expression profile. TCL1 expression has not been so far assessed in Waldenström macroglobulinemia. Transcriptomic explorations show that TCL1A expression is linked to signaling pathways and biological functions that are known to be involved in Waldenström macroglobulinemia as well as to gene signatures of interest in B-cell malignancies. We investigated TCL1 expression at the protein level in the bone marrow of a series of 59 patients with Waldenström macroglobulinemia: 76% of patients expressed TCL1, which appeared to be associated with a pejorative prognostic impact. TCL1 could have an oncogenic role in Waldenström macroglobulinemia, and deserves further exploration.

Abstract

Introduction La tumeur fibreuse solitaire (TFS) est une tumeur mesenchymateuse ubiquitaire exceptionnellement retrouvee dans l’orbite. Nous en rapportons un nouveau cas que nous confrontons aux donnees de la litterature. Observation Une femme, âgee de 72 ans, presentait une inflammation conjonctivale de l’œil droit evoluant depuis 5 mois avec apparition progressive d’un comblement du fornix inferieur associe a une exophtalmie indolore non axile. L’IRM a mis en evidence une masse tissulaire extraconique homogene avec un hyposignal T1 et un hypersignal T2, depourvue de specificite. L’examen anatomopathologique de la piece operatoire obtenue par voie d’orbitotomie anterieure a permis de poser le diagnostic de TFS de l’orbite. L’evolution est favorable avec un recul de plus de 9 mois. Discussion Le diagnostic de TFS est anatomopathologique. Il s’agit d’une tumeur de nature mesenchymateuse. On note une forte expression de l’antigene CD34 et de la vimentine en immunohistochimie. Conclusion La TFS de l’orbite est une tumeur rare habituellement benigne. Elle doit etre differenciee immunohistochimiquement des autres types de tumeurs mesenchymateuses de l’orbite. Sa prise en charge repose sur l’exerese chirurgicale complete, et un suivi clinique prolonge est propose car des recidives peuvent survenir a long terme et il existe des formes malignes.

Resume Parmi les tumeurs trophoblastiques gestationnelles, la tumeur trophoblastique du site placentaire et la tumeur trophoblastique epithelioide sont des lesions rares. Elles apparaissent toutes deux dans le meme contexte et ont un profil evolutif commun. Nous en rapportons deux cas chez des patientes de 44 et 20 ans. Ces lesions atteignent des femmes en general jeunes, ayant de nombreux antecedents gestationnels. Elles se manifestent par des metrorragies, une elevation moderee du taux serique des sHCG et par une masse uterine. Ces proliferations trophoblastiques, distinctes morphologiquement, ont le plus souvent une evolution benigne, si une exerese complete est realisee.

Abstract

Les infections impliquant de nouveaux pathogenes fongiques semblent en augmentation ces dernieres annees. L’emergence de ces nouvelles especes pourrait etre expliquee par les immunodepressions de plus en plus profondes, mais egalement par l’utilisation d’outils diagnostiques de plus en plus performants. Nous rapportons ici 2 cas d’infection invasive impliquant Nannizziopsis obscura survenus dans un contexte de lymphopenie. Notre premier cas concerne une femme sejournant regulierement au Senegal et atteinte d’une leucemie prolymphocytaire refractaire a la chimiotherapie, qui decedera rapidement avant tout diagnostic, dans un contexte de decompensation œdemato-ascitique et de lesions cerebrales hypodenses au scanner. N. obscura a ete isole d’une hemoculture et a posteriori detecte par PCR panfongique dans le LCR et l’ascite. Le second cas concerne une patiente vivant au Mali et recemment diagnostiquee seropositive pour le VIH au stade SIDA. Suite a l’apparition de troubles neurologiques, le scanner cerebral revele la presence d’une importante masse abcedee. Bien que les cultures realisees sur le LCR restent negatives, la PCR panfongique rapportera la presence de N. obscura , ce qui sera confirme suite a la realisation d’une biopsie cerebrale montrant la presence de filaments myceliens. Les cultures seront positives 48 h plus tard. Les champignons du genre Nannizziopsis comprennent le complexe d’especes Nannizziopsis vriesii et sont largement connus pour leur role dans les infections cutanees chez les reptiles, pouvant conduire a des infections disseminees fatales. Au sein de ce complexe, 3 especes ( N. infrequens , N. hominis et N. obscura ) ont ete isolees chez l’Homme. Parmi les 6 cas humains rapportes dans la litterature, 3 patients etaient seropositifs pour le VIH et 4 habitaient ou avaient voyage en Afrique. Outre l’extreme rarete des cas d’infection a N. obscura , ces cas soulignent l’importance de la biologie moleculaire dans le diagnostic mycologique.

Abstract

The solitary fibrous tumor (SFT) is a spindle-cell tumor that very rarely involves the orbit. We report a new case that we compare to reports in the literature.A 72-year-old woman presented a conjunctival inflammation of the right eye developing over 5 months with progressive proptosis. Magnetic resonance imaging revealed an extraconal and homogeneous mass, which showed hypointensity on T1-weighted images and hyperintensity on T2-weighted images, without specificity. Histological examination of the lesion removed by anterior orbitotomy confirmed the diagnosis of the SFT of the orbit. The patient was doing well without recurrence after 9 months.The diagnosis of SFT is histological. It is a mesenchymal tumor. Immunohistochemically, the tumor cells are strongly positive for CD34 and vimentin.The SFT of the orbit is a very rare and generally benign tumor. It must be immunohistochemically differentiated from other spindle-cell tumors of the orbit. The treatment is a complete surgical excision, and long-term follow-up is necessary because recurrence may appear long after treatment.

Abstract

Placental site trophoblastic tumor and epithelioid trophoblastic tumor are rare gestational trophoblastic tumors. They both appear in the same context and follow the same course. We report two cases in patients aged 44 and 20 years. These lesions usually affect young women with a history of multiple gestations. Clinical manifestations include vaginal bleeding, uterine mass, and moderate elevation of serum beta human chorionic gonadotrophin levels. These morphologically distinct trophoblastic proliferations usually have a favorable course if complete excision is performed.

Abstract

Background Waldenström's macroglobulinemia (WM) is a B-cell lymphoproliferative disorder characterized by a lymphoplasmacytic infiltration of the bone marrow (BM) and a serum monoclonal IgM. WM tumor cells show variable differentiation, ranging from mature B-cells to plasma cells. T-cell leukemia/lymphoma 1 (TCL1) is a small non-enzymatic protein normally expressed only during the early stages of lymphopoiesis and known to be a coactivator of AKT and of the NF-KappaB transcription pathway. TCL1 is up-regulated and plays a critical role in the pathogenesis of chronic lymphocytic leukemia (CLL) since transgenic mice expressing TCL1 in B cells developed an aggressive form of CLL (Bichi et al., PNAS 2002). Moreover high TCL1 expression level is linked to aggressive features and poor outcome in CLL. TCL1 was also shown to be expressed in various B-cell lymphomas but not in multiple myeloma (Weng et al., Blood 2012). TCL1 might also have a potential role in the pathogenesis of WM but TCL1 expression is not so far described in WM. Aim We proposed to assess the expression pattern of TCL1 and its potential prognostic value in WM. Methods Firstly, we checked the TCL1A expression in WM by conducting an analysis using NCBI’s GEO2R tool of publically available transcriptomic data which compared gene expression profiling of CLL (n=11), MM (n=12) and WM patients (B-lymphocyte (BL) (n=10) and plasma cell (PC) (n=10)) (GSE6691 from NCBI’s GEO database, Gutiérrez et al. Leukemia 2007). Secondly, we performed histological and immunostaining analyses for TCL1 on deparaffinized whole tissue contiguous sections of 57 WM patients’ BM trephine biopsies (median age: 62 years, sex ratio M/F: 36/21, symptomatic: n=36, previously treated: n=10). Diagnostic and treatment criteria for WM fulfilled recommendations of the 2nd international WM workshop. TCL1 immunostains were graded independently by three investigators on a 4-tier scoring which had previously been used to study TCL1 in CLL (Herling et al., Leukemia 2006). TCL1 immunostains intensity was scored as follows: 1- low intensity, 2- middle intensity, 3- strong intensity; tumor infiltration patterns were defined as interstitial, nodular and interstitial, nodular or diffuse and tumor infiltration percentage was assessed according to CD20 immunostains. Results The analysis of transcriptomic data revealed that TCL1A expression level in WM is significantly higher than those found in MM and lower than those found in CLL (Figure 1). Immunohistochemistry analysis of BM showed that TCL1 protein was expressed in 45 patients (79%), with an intratumoral heterogeneous repartition and its localization appeared mostly cytoplasmic. Pattern of expression was: negative (n=12), less than 5% of tumor cells positive (n=6), positive in up to 50% of tumor cells (n=15), and positive in over 50% of tumor cells (n=24). TCL1 pattern was correlated neither with tumor infiltration pattern nor grading. TCL1 seemed not to be associated with any prognostic factors and TCL1+ and TCL1- patients were comparable in terms of clinical and lab features as for ISS. Median follow-up was 50 months. Among the 21 asymptomatic WM patients, median time to first treatment was 33 months for TCL1+ group, while none of TCL1- patients needed so far any first line therapy (p=0.24). Among the patients requiring treatment, we did not observe any impact of TCL1 expression level on ORR (63% for TCL1+ and 67% for TCL1- patients, p=0.85). TCL1+ patients presented a trend for a poorer 3 year-PFS than TCL1- patients (32% versus 62% respectively, p=0.15). Median time to next treatment (TTNT) was only 24 months for TCL1+ patients and not reached for TCL1-patients (p=0,079) (Figure 2). Median OS was 94 months in the whole population without clear influence of the TCL1. Disclosures: No relevant conflicts of interest to declare.

Abstract

Background.Waldenström's macroglobulinemia (WM) is characterized by a lymphoplasmacytic infiltration of the bone marrow (BM) along with a serum monoclonal IgM and includes MYD88L265P and CXCR4mutations in around 90% and 25% respectively (Treon, NEJM 2012; Hunter, Blood 2014). CXCR4WHIMmutations confer worse outcome and are associated with larger BM involvement and less adenopathies (Treon, Blood 2014). One hallmark and histological criteria for WM diagnosis is the presence of numerous mast cells (MC) segregating with tumor cells in the BM (Waldenström, Acta Haematologica 1958). MC have been shown to support lymphoplasmacytic cell growth, through CD154/CD40 signaling (Tournilhac, Ann Oncol 2006), but there is so far no clear demonstration of the prognostic impact of BM MC density in WM.

Abstract

Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare aggressive T-cell lymphoma most reported in Asia. We performed a comprehensive clinical, pathological and genomic study of 71 European MEITL patients (36 males, 35 females, median age 67 years). The majority presented with gastrointestinal involvement and had emergency surgery, and 40% had stage IV disease. The tumors were morphologically classified into two groups: typical (58%) and atypical (i.e., non-monomorphic or with necrosis, angiotropism or starry-sky pattern) (42%), sharing a homogeneous immunophenotypic profile (CD3+ [98%] CD4- [94%] CD5- [97%] CD7+ [97%] CD8+ [90%] CD56+ [86%] CD103+ [80%] cytotoxic marker+ [98%]) with more frequent expression of TCRgd (50%) than TCRab (32%). MYC expression (30% of cases) partly reflecting MYC gene locus alterations, correlated with non-monomorphic cytology. Almost all cases (97%) harbored deleterious mutation(s) and/or deletion of the SETD2 gene and 90% had defective H3K36 trimethylation. Other frequently mutated genes were STAT5B (57%), JAK3 (50%), TP53 (35%), JAK1 (12.5%), BCOR and ATM (11%). Both TP53 mutations and MYC expression correlated with atypical morphology. The median overall survival (OS) of 63 patients (43/63 only received chemotherapy after initial surgery) was 7.8 months. Multivariate analysis found a strong negative impact on outcome of MYC expression, TP53 mutation, STAT5B mutation and poor performance status while aberrant B-cell marker expression (20% of cases) correlated with better survival. In conclusion, MEITL is an aggressive disease with resistance to conventional therapy, predominantly characterized by driver gene alterations deregulating histone methylation and JAK/STAT signaling and encompasses genetic and morphologic variants associated with very high clinical risk.

Abstract

Background: The solitary fibrous tumor (SFT) is a spindle-cell tumor that very rarely involves the orbit. We report a new case that we compare to reports in the literature., Case: A 72-year-old woman presented a conjunctival inflammation of the right eye developing over 5 months with progressive proptosis. Magnetic resonance imaging revealed an extraconal and homogeneous mass, which showed hypointensity on T1-weighted images and hyperintensity on T2-weighted images, without specificity. Histological examination of the lesion removed by anterior orbitotomy confirmed the diagnosis of the SFT of the orbit. The patient was doing well without recurrence after 9 months., Discussion: The diagnosis of SFT is histological. It is a mesenchymal tumor. Immunohistochemically, the tumor cells are strongly positive for CD34 and vimentin., Conclusion: The SFT of the orbit is a very rare and generally benign tumor. It must be immunohistochemically differentiated from other spindle-cell tumors of the orbit. The treatment is a complete surgical excision, and long-term follow-up is necessary because recurrence may appear long after treatment.

Abstract

Placental site trophoblastic tumor and epithelioid trophoblastic tumor are rare gestational trophoblastic tumors. They both appear in the same context and follow the same course. We report two cases in patients aged 44 and 20 years. These lesions usually affect young women with a history of multiple gestations. Clinical manifestations include vaginal bleeding, uterine mass, and moderate elevation of serum beta human chorionic gonadotrophin levels. These morphologically distinct trophoblastic proliferations usually have a favorable course if complete excision is performed.
