Abstract

Hyperlipidaemia is a well-established risk factor for cardiovascular diseases and therefore, many animal model have been developed to mimic the human abnormal elevation of blood lipid levels. In parallel, extensive research for the alleviation of ischaemia/reperfusion injury has revealed that hyperlipidaemia is a major co-morbidity that attenuates the cardioprotective effect of conditioning strategies (preconditioning, postconditioning and remote conditioning) and that of pharmacological interventions by interfering with cardioprotective signalling pathways. In the present review article, we summarize the existing data on animal models of hypercholesterolaemia (total, low density and HDL abnormalities) and hypertriglyceridaemia used in ischaemia/reperfusion injury and protection from it. We also provide recommendations on preclinical animal models to be used for translations of the cardioprotective strategies into clinical practice. Several other studies have shown that soluble ST2 levels correlate with severity of heart failure (HF), left ventricular ejection fraction, creatinine clearance, B-type natriuretic peptide and C-reactive protein, and are predictors of mortality in HF. Most of these studies were not only limited to ischemic heart disease but also concentrated on left-sided HF. We therefore decided to study the relationship between soluble ST2 and some markers of right ventricular (RV) function in a cohort of hypertensive HF subjects.Patients and methods: This is a prospective cohort study of hypertensive HF patients presenting to the University of Abuja Teaching Hospital, Abuja, over a 12-month period. ST2 was measured in plasma sample by the enzyme-linked immunosorbent assay (ELISA) method. Right ventricular diameters in diastole (RVDD) and right atrial area (RAA) were obtained on echocardiography, while right ventricular systolic pressure (RVSP) was estimated from echocardiography by the addition of the pressure gradient between the right ventricle and right atrium (RA) to the pressure in the RA.Results: There was a significant correlation between RVSP and soluble ST2 (t=0.75, p

Abstract

Due to its poor capacity for regeneration, the heart is particularly sensitive to the loss of contractile cardiomyocytes. The onslaught of damage caused by ischaemia and reperfusion, occurring during an acute myocardial infarction and the subsequent reperfusion therapy, can wipe out upwards of a billion cardiomyocytes. A similar program of cell death can cause the irreversible loss of neurons in ischaemic stroke. Similar pathways of lethal cell injury can contribute to other pathologies such as left ventricular dysfunction and heart failure caused by cancer therapy. Consequently, strategies designed to protect the heart from lethal cell injury have the potential to be applicable across all three pathologies. The investigators meeting at the 10th Hatter Cardiovascular Institute workshop examined the parallels between ST-segment elevation myocardial infarction (STEMI), ischaemic stroke, and other pathologies that cause the loss of cardiomyocytes including cancer therapeutic cardiotoxicity. They examined the prospects for protection by remote ischaemic conditioning (RIC) in each scenario, and evaluated impasses and novel opportunities for cellular protection, with the future landscape for RIC in the clinical setting to be determined by the outcome of the large ERIC-PPCI/CONDI2 study. It was agreed that the way forward must include measures to improve experimental methodologies, such that they better reflect the clinical scenario and to judiciously select combinations of therapies targeting specific pathways of cellular death and injury.

Abstract

© 2016, The Author(s).In this meeting report, particularly addressing the topic of protection of the cardiovascular system from ischemia/reperfusion injury, highlights are presented that relate to conditioning strategies of the heart with respect to molecular mechanisms and outcome in patients’ cohorts, the influence of co-morbidities and medications, as well as the contribution of innate immune reactions in cardioprotection. Moreover, developmental or systems biology approaches bear great potential in systematically uncovering unexpected components involved in ischemia–reperfusion injury or heart regeneration. Based on the characterization of particular platelet integrins, mitochondrial redox-linked proteins, or lipid-diol compounds in cardiovascular diseases, their targeting by newly developed theranostics and technologies opens new avenues for diagnosis and therapy of myocardial infarction to improve the patients’ outcome.

Abstract

During pregnancy, heart rate (HR) is physiologically elevated but recovery occurs within 4 weeks of delivery. Peripartum cardiomyopathy (PPCM) is an acute condition which manifests with symptoms of heart failure late during pregnancy, or within 6 months of delivery. One of its main symptoms is elevated HR. Current standard therapy for PPCM makes use of diuretics, ACE inhibitors and beta-blockers. This approach does not satisfactorily improve HR in patients, even after 6 months of treatment. Strong evidence from both experimental and clinical studies suggests that modulation of the sino-atrial node with drugs such as ivabradine may benefi t patients suffering from PPCM. The activity of ivabradine is likely two-fold – direct with regards to heart rate and indirect with long-term structural changes affecting the heart itself, as well as the vascular and endogenous physiological systems. Large clinical trials are needed to validate this concept and further exploration of this hypothesis in an established rodent model of PPCM is required to investigate the outcome on both HR and its effects on other observable systems affected by PPCM.

Abstract

Although cytokine activation has long been recognized to associate with cardiac ischemia and reperfusion, the concept that these cytokines may enhance some cardioprotective mechanisms has only recently been considered. Ischemic preconditioning is a biologic phenomenon that activates innate cytoprotective programs in the heart. Ischemic preconditioning has been described where a transient non-lethal ischemic « trigger » or endogenous molecules produced/released by ischemia enables the tissue to become more resistant/tolerant to subsequent potentially lethal ischemia. The mechanisms and signalling events involved in this cytoprotective program still remain obscure. Recently, it has been suggested that cytokine activation including tumour necrosis factor (TNFα) may play a key role in the preconditioned phenotype. Moreover, new studies have given the evidence that the exploration of cytokine-activated sphingolipid signalling pathways may enhance our understanding of the preconditioning program. Abstract

Introduction: Heart failure (HF) is the inability of the left ventricle to fill with or eject blood. It often occurs as a side-effect of cardiovascular complications such as ischemia. At the molecular level, Signal Transducer and Activator of Transcription 3 (STAT3) is linked to HF, as STAT3 deficient (STAT3 KO) mouse models have shown development of HF with age. Hypothesis: In a mouse model, induction of heart failure consecutive to a deletion in the STAT3 gene depends on the locus of the splice-site in the gene. Material and methods: STAT3 KO and wildtype (WT) mice were aged to provoke heart failure. Mice were anesthetised for all aspects of this experiment by intraperitoneal injection of Sodium Pentobarbitone (50mg/kg body weight) . A sufficient degree of anaesthesia was considered to be obtained in the absence of the pedal reflex, after which the chest cavity was opened, and hearts were excised. Mitochondrial respiration was measured in aged hearts using permeabilized heart fibres and an oxygen-sensing Clarke-type electrode, with fatty acids (FA) as a substrate (n=4 per group). Inability to respire with FA is a typical metabolic state of a failing heart. Histology staining (picrosirius stain) was also performed on these aged mouse hearts to quantify fibrosis, a marker for heart failure (n=6 KO, n=4 WT). Results: No significant differences detected in mitochondrial respiration using a FA substrate, indicating similar metabolic profiles in both groups (see Figure 1, p>0.05). Failing hearts exposed to FA should have shown impaired mitochondrial respiration. Histology staining for fibrosis in the aged STAT3 KO hearts showed minimal fibrosis, similar to WT hearts (KO 0.83±0.203% and WT 0.57±0.104%, p> 0.05), demonstrating no overt heart failure. All data was expressed as mean ± SEM. An unpaired student's t-test was used for all statistical analysis. Conclusion: The STAT3 KO mouse model in our laboratory presents a different phenotype with aging compared to other published STAT 3 KO mouse models. This may be due to the location of the splicing in the STAT3 gene and the possibility of aberrant STAT3 proteins which contribute to HF in certain models, but not in others. It would therefore appear that the tendency to heart failure in STAT3 KO mice is dependent on the position of the deletion or the duration of aging. [ABSTRACT FROM AUTHOR]
