The identification of cardiac sarcoidosis is challenging as there is no gold standard consensually admitted for its diagnosis. The aim of this study was to evaluate the diagnostic value of the assessment of cardiac dynamic 18F-fluoro-2-deoxyglucose positron emission tomography (18F-FDG PET/CT) and net influx constant (Ki) in patients suspected of cardiac sarcoidosis. Data obtained from 30 biopsy-proven sarcoidosis patients suspected of cardiac sarcoidosis who underwent a 50-min list-mode cardiac dynamic 18F-FDG PET/CT after a 24 h high-fat and low-carbohydrate diet were analyzed. A normalized coefficient of variation of quantitative glucose influx constant, calculated as the ratio: standard deviation of the segmental Ki (min−1)/global Ki (min−1) was determined using a validated software (Carimas® 2.4, Turku PET Centre). Cardiac sarcoidosis was diagnosed according to the Japanese Ministry of Health and Welfare criteria. Receiving operating curve analysis was performed to determine sensitivity and specificity of cardiac dynamic 18F-FDG PET/CT analysis to diagnose cardiac sarcoidosis. Six out of 30 patients (20%) were diagnosed as having cardiac sarcoidosis. Myocardial glucose metabolism was significantly heterogeneous in patients with cardiac sarcoidosis who showed significantly higher normalized coefficient of variation values compared to patients without cardiac sarcoidosis (0.513 ± 0.175 vs. 0.205 ± 0.081; p = 0.0007). Using ROC curve analysis, we found a cut-off value of 0.38 for the diagnosis of cardiac sarcoidosis with a sensitivity of 100% and a specificity of 91%. Our results suggest that quantitative analysis of cardiac dynamic 18F-FDG PET/CT could be a useful tool for the diagnosis of cardiac sarcoidosis.

Dear Sir, F-Fluorodeoxyglucose (FDG) positron emission tomography (PET) is being increasingly used for assessment of early effects of conventional chemotherapy and/or tyrosine kinase inhibitors (TKI). In these settings, when using standardized uptake values (SUVs) as non-invasive quantitative imaging biomarkers in multicentre trials or in sites equipped with multiple PET scanners, adequate reproducibility is mandatory so that SUVs are comparable, regardless of the PET system used. Adequate reproducibility of SUVs can be achieved by harmonizing patient preparation as well as acquisition and reconstruction parameters [1–4], as recommended by the European Association of Nuclear Medicine Research Ltd (EARL) accreditation programme [5] and other groups [6]. Harmonization programs such as EARL are meant to reduce sources of SUV variability [7], which can be classified into technical, biological and physical factors [8–10]. The purpose of this survey was to shed light on an unreported technical cause of variability in SUVmeasurement that may hamper efforts to meet quantitative harmonizing standards. Indeed, variability in local standards of care in the use of PETacross centres has been well documented [11, 12], and compliance with PET imaging guidelines in clinical trials has been studied [13, 14]. However, although the SUV formula displayed below obviously shows that an overestimation of patient’s weight can lead to an overestimation of SUV metrics and vice versa, there is, to the best of our knowledge, no large survey about this potential confounding factor in SUV measurement.

Objectives Dementia with Lewy bodies (DLB) and Alzheimer’s disease (AD) are the two most common forms of dementia. These two diseases share some clinical and pathological similarities, yet the loss of dopaminergic neurons confirmed by 123-I-Ioflupane Single Photon Emission Computed Tomography (SPECT) is a suggestive feature of DLB. Current evidence suggests that higher education has a protective effect on the risk of developing clinical AD. However, how education influences cognitive performance and the presynaptic dopamine transporter marker in DLB is unknown. Materials and methods We reviewed 56 consecutive patients with DLB who underwent a 123-I-Ioflupane SPECT from January 2009 to August 2013 at the University Hospital of Caen. We collected clinical and neuropsychological data from medical files and 123-I-Ioflupane SPECT data for all patients. Results There was no correlation between education and global cognitive performance in patients with DLB. However, there was a positive correlation between education and tests exploring visuoconstructive functions (Rey complex figure copy and recall) and verbal retrieval strategies (Grober and Buschke free recall test). There was also a positive correlation between education and dopamine transporter binding. Higher educated patients had higher binding in the striatum, putamen and caudate nucleus (p = 0.001 for each regions of interest). Dopamine transporter binding in the striatum, putamen and caudate nucleus was lower in the subgroup of patients with REM sleep behavior disorder, but was not associated with other DLB symptoms. Conclusion Higher education may have a protective effect on visuoconstructive performance and verbal retrieval strategies and may influence dopaminergic nigrostriatal neurodegeneration in patients with DLB.

International audience; BACKGROUND: Different studies have found diminished cardiac metaiodobenzylguanidine (MIBG) uptake in Lewy body (LB) related conditions (Parkinson's disease (PD) and Lewy body dementia (LBD)). However, delayed heart/mediastinum (d-H/M) ratio diagnostic cutoff points are debated in parkinsonian syndromes.METHODS: We performed a monocentric retrospective analysis on 62 consecutive parkinsonian patients who underwent an (123)I-MIBG scintigraphy, brain imaging and dopaminergic imaging using (123)I-Ioflupane single photon emission computed tomography (SPECT) from 2009 to 2013. The optimal d-H/M ratio was determined from a Receiver Operating Characteristic (ROC) curve and the sensitivity (Se), specificity (Sp) and likelihood ratios (LR) were calculated. 42 patients were diagnosed with LB diseases (20 PD, 22 LBD) and 20 patients with other diseases.RESULTS: (123)I-MIBG scintigraphy helped to distinguish PD (p

L’amylose cardiaque (AC) se caracterise par l’accumulation de proteines fibrillaires insolubles responsable de cardiopathies restrictives. Differencier les causes les plus frequentes d’AC, les types AL et TTR, a un impact pronostique et therapeutique majeur. La scintigraphie cardiaque au 99mTc-DPD permet de distinguer ces deux formes cliniques. Nous rapportons le cas d’un patient de 78 ans, coronarien connu, hospitalise pour un tableau de decompensation cardiaque. L’echocardiographie et l’IRM cardiaque ont mis en evidence une hypertrophie myocardique concentrique, evoquant une AC. Une scintigraphie double-isotope 99mTc-DPD/201Tl a ete effectuee selon le protocole d’acquisition suivant : 3 heures apres l’injection de 740 MBq de 99mTc-DPD, une scintigraphie osseuse est realisee sur une camera double-tete (Symbia 2T, Siemens®) : hyperfixation cardiaque. Une acquisition tomoscintigraphique myocardique est effectuee sur la camera CZT (D-SPECT, Spectrum Dynamics®) : hyperfixation homogene du VG et du VD. Une tomoscintigraphie myocardique de perfusion est realisee dans un second temps, 45 minutes apres injection de 70 MBq de 201Tl : perfusion homogene du VG. In fine, l’hyperfixation myocardique identifiee apres injection de 99mTc-DPD sur la camera CZT permet d’orienter significativement vers une AC de type TRR.

17. Diagnostic value of quantitative assessment of cardiac 18F-fluoro-2-deoxyglucose uptake in suspected cardiac sarcoidosis.

Objective: The identification of cardiac sarcoidosis is challenging as there is no gold standard consensually admitted for its diagnosis. The aim of this study was to evaluate the diagnostic value of the assessment of cardiac dynamic 18F-fluoro-2-deoxyglucose positron emission tomography (18F-FDG PET/CT) and net influx constant (Ki) in patients suspected of cardiac sarcoidosis.Methods: Data obtained from 30 biopsy-proven sarcoidosis patients suspected of cardiac sarcoidosis who underwent a 50-min list-mode cardiac dynamic 18F-FDG PET/CT after a 24 h high-fat and low-carbohydrate diet were analyzed. A normalized coefficient of variation of quantitative glucose influx constant, calculated as the ratio: standard deviation of the segmental Ki (min-1)/global Ki (min-1) was determined using a validated software (Carimas® 2.4, Turku PET Centre). Cardiac sarcoidosis was diagnosed according to the Japanese Ministry of Health and Welfare criteria. Receiving operating curve analysis was performed to determine sensitivity and specificity of cardiac dynamic 18F-FDG PET/CT analysis to diagnose cardiac sarcoidosis.Results: Six out of 30 patients (20%) were diagnosed as having cardiac sarcoidosis. Myocardial glucose metabolism was significantly heterogeneous in patients with cardiac sarcoidosis who showed significantly higher normalized coefficient of variation values compared to patients without cardiac sarcoidosis (0.513 ± 0.175 vs. 0.205 ± 0.081; p = 0.0007). Using ROC curve analysis, we found a cut-off value of 0.38 for the diagnosis of cardiac sarcoidosis with a sensitivity of 100% and a specificity of 91%.Conclusions: Our results suggest that quantitative analysis of cardiac dynamic 18F-FDG PET/CT could be a useful tool for the diagnosis of cardiac sarcoidosis. 26. TEP/TDM cardiaque dynamique au 18F-FDG chez les patients suspects de sarcoïdose cardiaque

Introduction Le but de l’etude etait d’evaluer la valeur diagnostique de la TEP/TDM cardiaque dynamique au 18F-FDG evaluant la vitesse de metabolisme du glucose myocardique (MRGlu) chez les patients suspects de sarcoidose cardiaque (SC). Patients et methodes Nous avons revu les donnees de 30 patients presentant une sarcoidose prouvee histologiquement, adresses pour suspicion de SC, ayant beneficie d’une TEP/TDM cardiaque dynamique au 18F-FDG de 50 minutes apres respect d’un regime pauvre en hydrate de carbone et riche en lipide et injection de 4 MBq/kg de 18F-FDG. Un coefficient de variation normalise (NCOV) du metabolisme glucidique cardiaque a ete determine par l’utilisation d’un logiciel dedie et d’un modele a 17 segments du ventricule gauche. Le NCOV etait egal au ratio : ecart-type MRGlu segmentaire (μmol.mL−1.min−1)/MRGlu globale (μmol.mL−1.min−1). La SC etait diagnostiquee en accord avec les criteres du ministere Japonais de la Sante et du Bien-etre de 2006. Une analyse des courbes ROC a ete realisee pour determiner la sensibilite et la specificite de la TEP/TDM cardiaque dynamique au 18F-FDG pour diagnostiquer la SC. Resultats La MRGlu etait significativement heterogene chez les patients avec une SC avec des valeurs de NCOV plus elevees en comparaison aux patients sans SC (0,517 ± 0,180 contre 0,205 ± 0,080, p = 0,0007). En utilisant les courbes ROC, nous avons trouve une valeur seuil a 0,38, procurant une sensibilite de 100 % et une specificite de 91 % pour le diagnostic de SC. Conclusion Nos resultats suggerent que l’analyse quantitative de la TEP/TDM cardiaque dynamique au 18F-FDG pourrait etre un outil utile pour le diagnostic de SC.

Objectifs Decrire l’utilisation pratique du regadenoson, nouvel agent de stress commercialise en 2014 en France, ainsi que de definir sa place dans l’arsenal pharmacologique des epreuves de stress. Le regadenoson est une adenosine cardioselective A2a de 1/2 vie plasmatique courte ( Materiels et methodes Le temps de realisation de l’epreuve de stress est d’environ 5 min decompose ( Figure 1 ). Resultats Des considerations economiques nous ont conduit, au CHU de Caen, a reserver ce medicament en troisieme ligne (bicyclette ergometrique non realisable et contre-indication au dipyridamole). Les indications retenues au comite du medicament sont : asthme, BPCO, intolerance au dipyridamole, IMC > 38, prise de xanthines dans les 6 h avant l’examen. Trois cent cinquante patients ont eu une epreuve de stress dans notre etablissement en 2014 presentant des effets secondaires frequents (80 % des patients) mais brefs dus a la faible 1/2 vie du produit. Les principaux effets secondaires sont dyspnee (50 %), cephalee (12 %), vertige (10 %), nausee (4 %), diarrhee (2 %) et vomissements (2 %). Conclusions Ce nouvel agent de stress pharmacologique est relativement facile a utiliser mais couteux (> 100 € par flacon). Les effets indesirables sont tres bien toleres a la difference des autres agents de stress. Cependant, la procedure des differents temps d’administration doit etre bien maitrisee et necessite du personnel forme afin de rapidement prendre en charge les effets secondaires.

International audience; Real-time monitoring of perceived odour intensity by a sniffing subject allows the construction of an olfactogram associated with a chromatogram. This improves the matching of odorous events with VOC peaks. Analytical resolution, olfactory adaptation, odour persistence, breath frequency, assessor's awareness affect interpretation of the olfactogram. Peak height or peak area can be used to determine the odour intensity of a given odorous compound and thus determine the most odor-active compound. The number of odorous events tends to decrease with the decrease of VOC emission whereas precise identification of odorous compounds tends to improve with time.

Purpose: The aim of this study was to determine the potential diagnostic value of regional myocardial adrenergic I-metaiodobenzylguanidine (MIBG) single photon emission computed tomography (SPECT) imaging to identify patients with Lewy body diseases (LBD+). Methods: Sixty-four consecutive patients who underwent cardiac I-MIBG SPECT to differentiate LBD+, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB), from patients without LBD (LBD−) were retrospectively reviewed. A neurologist expert in memory disorders determined the final clinical diagnosis by using international clinical diagnostic criteria. Planar [heart to mediastinum ratio (HMR)] and I-MIBG SPECT[(innervation defect score (IDS)] using the 17-segment left ventricular model (five-point scale) were obtained 4 h after the injection of I-MIBG on a low-energy high-resolution (LEHR) collimator. Receiver-operating characteristic (ROC) analysis was performed to determine the optimal HMR and IDS cut-off values to discriminate LBD+ from LBD−. Results: Of the 64 patients, 45 (70 %) were diagnosed LBD+ (DLB, n = 27; PD, n = 18) and 19 were diagnosed LBD− (5 other dementias, 14 other parkinsonisms). The HMR and IDS of LBD+ were significantly different from those of LBD− (1.30 ± 0.21 vs 1.65 ± 0.26, p < 0.001; 39 ± 28 vs 8 ± 16, p = 0.001). The optimal HMR and IDS cut-off values to discriminate LBD+ ( n = 45) from LBD− ( n = 19) were 1.47 and 6/68, providing a sensitivity and specificity of 82.2 and 84.2 % and 86.7 and 73.7 %, respectively. Conclusion: Regional myocardial adrenergic I-MIBG imaging SPECT has a potential diagnostic value to identify LBD+. [ABSTRACT FROM AUTHOR]
