Objective This study investigated the factors associated with satisfactory early postoperative wound conditions. Method A prospective study was conducted with patients (n=179) submitted to osteosynthesis in general, in a hospital orthopedics service. In the preoperative period, patients underwent laboratory exams and the surgical indications were based on the type of fracture and the patient’s clinical conditions. In the postoperative period, patients were evaluated based on the presence of complications and considering their surgical wounds. Chi-square, Fisher, Mann-Whitney, and Kruskal-Wallis tests were used in the analysis. To identify the factors associated with wound condition, univariate and multiple logistic regression analysis was used. Results In the univariate analysis, each transferring unit reduction increased the chance of satisfactory outcome by 1.1% (p=0.0306; OR= 0.989 (1.011); 95%CI= 0.978;0.999; 1.001;1.023). The presence of SAH increased 2.7 fold the chance of satisfactory outcome (p=0.0424; OR= 2,667; 95%CI= 1,034;6,877). Hip fracture increased 2.6 fold the chance of satisfactory outcome (p=0.0272; OR=2.593; IC95%=1.113; 6.039). And the absence of a compound fracture increased 5.5 fold the chance of satisfactory wound outcome (p=0.0004; OR=5,493; 95%CI=2,132;14,149). In the multiple analysis, patients with non compound fractures were 9.7 times more likely to experience a satisfactory outcome when compared to patients with compound fractures (p=0.0014; OR=9,687; 95%CI= 2,399; 39,125). Conclusion There was an inverse relationship between plasma proteins levels and satisfactory surgical wounds outcome. Only exposure remained associated with wound conditions. Método Foi desenvolvido um estudo prospectivo com pacientes (n=179) submetidos a osteossínteses em geral, em um serviço de ortopedia hospitalar. No período pré-operatório os pacientes foram submetidos a exames laboratoriais e as indicações cirúrgicas foram baseadas no tipo de fratura e condições clínicas. No período pós-operatório, os pacientes foram avaliados conforme a presença de complicações e feridas operatórias. Foram utilizados na análise os testes Qui-quadrado, Fisher, Mann-Whitney, Kruskal-Wallis. Para identificar os fatores associados à condição da ferida, foi utilizada a análise de regressão logística univariada e múltipla. Resultados Na análise univariada, cada unidade a menos de transferrina, aumentou a chance de condição satisfatória em 1.1% (p= 0.0306; OR= 0.989 (1.011); IC95%= 0.978;0.999; 1.001;1.023). A presença de HAS aumentou a chance de condição satisfatória em 2.7 vezes (p=0.0424; OR= 2.667; IC95%= 1.034;6.877). Fratura de quadril aumentou a chance de condição satisfatória em 2.6 vezes (p=0.0272; OR=2.593; IC95%=1.113;6.039). E não apresentar fratura exposta, aumentou a chance de condição satisfatória da ferida em 5.5 vezes (p=0.0004; OR=5.493; IC95%=2.132;14.149). Na análise múltipla, fraturas sem exposição, apresentaram chance 9.7 vezes maior de apresentar condição satisfatória do que insatisfatória; quando comparadas às que apresentaram exposição (p=0.0014; OR=9.687; IC95%= 2,399; 39.125). Conclusão Houve uma relação inversa entre proteínas plasmáticas e presença de feridas operatórias satisfatórias. Apenas a exposição se manteve associada à condição da ferida. Objective The objective of this work was to review papers that deal with nutritional therapy of inpatients with Covid-19 from the beginning of the pandemic. The survey covered the period until the month of September 2020. Methods This is a review of the indexed literature (Scopus, Pubmed and Lilacs), including area consensus. Results The review showed that the initial recommendations were based on generic assumptions and, as specific knowledge was being produced, the recommendations became more targeted. Conclusion In the health crisis generated by Covid-19, in the context of uncertainties related to nutritional care procedures, knowledge and action strategies were generated, coordinated by researchers and health professionals. RESUMO Objetivo O objetivo deste trabalho foi revisar as publicações que trataram da terapia nutricional do paciente hospitalizado com Covid-19 desde o início da pandemia até o mês de setembro de 2020. Métodos Trata-se de uma revisão dos trabalhos indexados nas bases de dados Scopus, Pubmed e Lilacs, incluindo consensos da área. Resultados A revisão mostrou que as recomendações iniciais pautaram-se em pressupostos genéricos e, conforme o conhecimento específico foi sendo produzido, as recomendações passaram a ser mais direcionadas. Conclusão Na crise sanitária gerada pela Covid-19, no contexto de incertezas relacionadas aos procedimentos de atenção nutricional, foram gerados conhecimentos e estratégias de ação, coordenados por pesquisadores e profissionais de saúde.

Objective: to investigate whether the caloric and protein deficits, the diagnosis and the NUTRIC score, behaved as risk factors associated to the survival time. Methods: prospective study with 82 critically ill patients in intensive care unit (ICU), with exclusive enteral nutritional therapy (EENT). We investigated the calorie and protein deficiencies of EENT, the NUTRIC score, and the inflammatory-nutritional index by the C-reactive protein-CRP/albumin. The data were analyzed using the Chi-square, Fisher, Mann-Whitney, Kruskal-Wallis, univariate and multiple Cox regressions and the Kaplan-Meyer method. Results: in the univariate Cox regression, one-year increase in age increased the risk of death by 4.1% (p=0.0009; HR=1.041) and one-day increase with intercurrent events, by 1.8% (p = 0.0485; HR = 1.018). In the multiple Cox regression, the clinical diagnosis (p = 0.0462, HR = 2.091) and the NUTRIC score ≥ 5 (p < 0.0001; HR = 5.740) were the variables that together were associated with the survival time. The critical caloric and protein deficits did not behave as death risk factors in this population. Kaplan-Meier curves showed that the probability of survival in 40 days was 28.1% with clinical diagnosis and 40.2% with surgical diagnosis. The mean survival time with NUTRIC score ≥ 5 was 17.4 days. The probability of survival at 40 days was 72.8% with NUTRIC score < 5 and 6.4% with NUTRIC score ≥ 5. Conclusion: caloric and protein deficits are not risk factors for mortality. Only the diagnosis and the NUTRIC score were considered risk factors associated with the survival time. Resumen Objetivo: investigar si el déficit calórico y proteico, el diagnóstico y la puntuación nutricional se comportaron como factores de riesgo asociados al tiempo de supervivencia. Métodos: estudio prospectivo con 82 pacientes en estado crítico en la Unidad de Cuidados Intensivos (UCI), con terapia nutricional enteral exclusiva (EENT). Investigamos las deficiencias de calorías y proteínas de EENT, el puntuación NUTRIC y el índice inflamatorio-nutricional por la proteína C-reactiva-CRP / albúmina. Los datos se analizaron utilizando las regresiones de Chi-cuadrado, Fisher, Mann-Whitney, Kruskal-Wallis, univariadas y múltiples de Cox y el método de Kaplan-Meyer. Resultados: en la regresión de Cox univariada, el aumento de un año en la edad aumentó el riesgo de muerte en un 4,1% (p = 0,0009; HR = 1,041) y el aumento de un día con eventos intercurrentes, en un 1,8% (p = 0,0485; HR = 1,018). En la regresión múltiple de Cox, el diagnóstico clínico (p = 0,0462, HR = 2,091) y la puntuación nutricional ≥ 5 (p < 0,0001; HR = 5,740) fueron las variables que juntas se asociaron con el tiempo de supervivencia. Los déficits críticos de calorías y proteínas no se comportaron como factores de riesgo de muerte en esta población. Las curvas de Kaplan-Meier mostraron que la probabilidad de supervivencia en 40 días fue del 28,1% con diagnóstico clínico y del 40,2% con diagnóstico quirúrgico. El tiempo medio de supervivencia con puntuación NUTRIC ≥ 5 fue de 17,4 días. La probabilidad de supervivencia a los 40 días fue del 72,8% con una puntuación nutricional < 5 y del 6,4% con una puntuación NUTRIC ≥ 5. Conclusión: los déficits calóricos y proteicos no son factores de riesgo para la mortalidad. Solo el diagnóstico y la puntuación NUTRIC se consideraron factores de riesgo asociados con el tiempo de supervivencia.

BACKGROUND For many years, many studies have reported undesirable outcomes that may occur during the hospital stay of patients diagnosed with malnutrition or even at some nutritional risk. OBJECTIVE To investigate the relationship between nutritional status and clinical outcomes during hospital stay using the multiple correspondence analysis technique. METHODS This cross-sectional study included 600 patients with and without neoplasms. BACKGROUND Older patients are commonly malnourished during hospital stay, and a high prevalence of malnutrition is found in hospitalized patients aged more than 65 years. OBJECTIVE To investigate whether total lymphocyte count is related to other nutritional markers in hospitalized older adults. METHODS Hospitalized older adults (N=131) were recruited for a cross-sectional study. Their nutritional status was assessed by the Nutritional Risk Screening (NRS), anthropometry, and total lymphocyte count. The statistical analyses included the chi-square test, Fisher's exact test, and Mann-Whitney test. Spearman's linear correlation coefficient determined whether total lymphocyte count was correlated with the nutritional markers. Multiple linear regression determined the parameters associated with lymphocyte count. The significance level was set at 5%. RESULTS According to the NRS, 41.2% of the patients were at nutritional risk, and 36% had mild or moderate depletion according to total lymphocyte count. Total lymphocyte count was weakly correlated with mid-upper arm circumference (r=0.20507); triceps skinfold thickness (r=0.29036), and length of hospital stay (r= -0.21518). Total lymphocyte count in different NRS categories differed significantly: older adults who were not at nutritional risk had higher mean and median total lymphocyte count ( P =0.0245). Multiple regression analysis showed that higher lymphocyte counts were associated with higher triceps skinfold thicknesses and no nutritional risk according to the NRS. CONCLUSION Total lymphocyte count was correlated with mid-upper arm circumference, triceps skinfold thickness, and nutritional risk according to the NRS. In multiple regression the combined factors that remained associated with lymphocyte count were NRS and triceps skinfold thickness. Therefore, total lymphocyte count may be considered a nutritional marker. Other studies should confirm these findings. RESUMO CONTEXTO Durante a hospitalização é comum que pacientes idosos se encontrem desnutridos, sendo observada alta prevalência de pacientes idosos hospitalizados acima de 65 anos de idade. OBJETIVO Investigar se a contagem de linfócitos totais apresenta alguma associação com outros marcadores nutricionais em pacientes idosos hospitalizados. MÉTODOS Pacientes idosos hospitalizados (N=131) foram recrutados em um estudo transversal. Investigou-se o estado nutricional por meio do Nutritional Risk Screening (NRS), antropometria e a contagem de linfócitos totais. A análise estatística foi realizada com os testes Qui-quadrado, Fischer e Mann-Whitney. Aplicou-se o coeficiente de correlação linear de Spearman para se investigar a presença de correlação entre a CLT e os marcadores nutricionais e para avaliação dos parâmetros associados à contagem de linfócitos foi utilizada a análise de regressão linear múltipla, com nível de significância de 5%. RESULTADOS O NRS mostrou 41,2% de pacientes com risco nutricional e 36% dos pacientes encontravam-se com depleção leve ou moderada na contagem de linfócitos totais. Houve correlação significativa, de fraca intensidade, entre a contagem de linfócitos totais e a circunferência de braço (r=0,20507); a prega cutânea triciptal (r=0,29036) e o tempo de internação (r= -0,21518). Verificou-se ainda diferença significativa na comparação da contagem de linfócitos totais com as categorias de risco nutricional pelo NRS; a contagem de linfócitos totais foi maior, em média ou mediana, entre os idosos que não apresentaram risco nutricional pelo NRS ( P =0,0245). A análise de regressão múltipla mostrou que a maior contagem de linfócitos foi associada a valores maiores de prega cutânea triciptal e à classificação de sem risco nutricional pelo NRS. CONCLUSÃO A contagem de linfócitos totais apresentou associação com a circunferência de braço, com a prega cutânea triciptal e com o NRS, podendo ser considerada como um marcador nutricional. Na regressão, os fatores que, em conjunto, mantiveram-se associados à contagem de linfócitos foram o NRS e a prega cutânea triciptal. Outras investigações devem ser realizadas para confirmar estes achados.
