8. An analysis of iris pattern as a risk factor for skin cancer development in immunosuppressed renal transplant recipients.

Abstract

Introduction: Renal transplant recipients are at increased risk of keratinocyte skin cancers with a tendency to have multiple, aggressive and difficult to treat tumours. The eye and the skin share the same embryological ectoderm. Iris pattern has recently been reported as a predictive risk factor for skin cancer in non‐immunosuppressed Southern European (Grigore et al., J Eur Acad Dermatol Venereol, 2018, 1662) and Irish populations (Ridge et al., J Eur Acad Dermatol Venereol, 2022, e542). Aims: To analyse if an individual's iris pattern is an independent risk factor for the development of keratinocyte skin cancers in renal transplant recipients. Methods: Iris patterns of 110 renal transplant recipients were evaluated using the Simionescu visual three‐step technique (iris periphery, colarette and iris freckling [Simionescu et al., Ann Res Rev Biol, 2014, 2525]). Established risk factors for skin cancer in transplant patients were recorded as confounding factors. Results: Observational cross‐sectional study including 110 renal transplant population. Thirty‐one participants had skin cancer. In the skin cancer group, iris periphery was blue/grey in 74.3% (p = 0.053, OR 2.5), the colarette was light brown in 57.1% (p < 0.0043) and iris freckles were present in 55%(p = 0.044). Dark brown and blue colarettes were observed in controls. Binary Logistic Regression analysis showed light brown colarette is a significant independent risk factor for skin cancer (OR 4.54, p < 0.02, CI 1.56–10.57). Conclusion: Within this renal transplant population a blue iris periphery, light brown colarette and presence of freckling confers an independent risk for keratinocyte skin cancer. Iris pattern is a useful tool for identification of transplant patients at risk of keratinocyte skin cancer and an easy‐to‐use technique for risk evaluation in this cohort. 18. A Comparison of the Ethical Beliefs of Certified Rehabilitation Counselors and National Certified Counselors

Abstract

Results of a national survey of certified rehabilitation counselors (CRCs) are compared with those obtained by Gibson and Pope (1993) from a national sample of national certified counselors (NCCs) to determine their beliefs about whether particular behaviors are ethical, and the degree of consensus or controversy about these behaviors. Commonalities and differences in the beliefs of these two groups are described. Implications for education and the field of rehabilitation counseling are discussed. The authors conclude that the high degree of similarity between beliefs of CRC and NCC respondents provides empirical support that these counselors share a fundamental set of ethical beliefs.

Abstract

Background: Targeted therapies (TTs) have revolutionised cancer treatment with their enhanced specificity of action. Compared with conventional therapies, TTs are delivered over a longer period and often have unusual symptom profiles. Patient-reported outcome measures such as symptom side-effect lists need to be developed in a time-efficient manner to enable a rapid and full evaluation of new treatments and effective clinical management Objective: The aim of this study was to develop a set of TT-related symptoms and identify the optimal method for developing symptom lists. Patients and Methods: Symptoms from TT treatment in the context of Chronic Myeloid Leukaemia (CML), HER2-positive breast cancer, or Gastrointestinal Stromal Tumours (GIST) were identified through literature reviews, interviews with healthcare professionals (HCPs) and patients, and patient focus groups. The symptom set was then pilot tested in patients across the three cancer diagnoses: The number of items derived from each source (literature, patients, or HCPs) were compared. Results: A total of 316 patients and 86 HCPs from 16 countries participated. An initial set of 209 symptoms was reduced to 61 covering 12 symptom categories. Patient interviews made the greatest contribution to the item set. Conclusions: Symptom lists should be created based on input from patients. The item set described will be applicable to the assessment of new TTs, and in monitoring treatment. © 2020, Springer Nature Switzerland AG.

Abstract

Venous thromboembolism (VTE) is common in cancer patients and is an important cause of morbidity and mortality. The Global Anticoagulant Registry in the FIELD (GARFIELD)-VTE (ClinicalTrials.gov: NCT02155491) is a prospective, observational study of 10,684 patients with objectively diagnosed VTE from 415 sites in 28 countries. We compared baseline characteristics, VTE treatment patterns, and 1-year outcomes (mortality, recurrent VTE and major bleeding) in 1075 patients with active cancer, 674 patients with a history of cancer, and 8935 patients without cancer. Patients with active cancer and history of cancer were older than cancer-free patients, with median ages of 64.8, 68.9, and 58.4 years, respectively. The most common sites of active cancer were lung (14.5%), colorectal (11.0%), breast (10.6%), and gynaecological (10.3%). Active cancer patients had a higher incidence of upper limb and vena cava thrombosis than cancer-free patients (9.0% vs 4.8% and 5.1% vs 1.4%, respectively), and were more likely to receive parenteral anticoagulation as monotherapy than cancer-free patients (57.8% vs 12.1%), and less likely to receive DOACs (14.2% vs 50.6%). Rates of death, recurrent VTE, and major bleeding were higher in active cancer patients than in cancer-free patients, with hazard ratios (95% confidence intervals) of 14.2 (12.1-16.6), 1.6 (1.2-2.0) and 3.8 (2.9-5.0), respectively. VTE was the second most common cause of death in patients with active cancer or history of cancer. In patients with VTE, those with active cancer are at higher risk of death, recurrence, and major bleeding than those without cancer.

46. The Australian Senior Certificates: after 50 years of reforms

Abstract

This article maps current senior secondary certification arrangements in Australia, drawing on data on school completion rates, certificate attainment and post-school destinations, as well as policy documents within and across jurisdictions. It argues that irrespective of the jurisdiction, numerous changes over nearly 50 years to the rules governing the senior certificates have been principally responses to the original and continuing need to prepare young people for university and the more recent need to cater for near universal participation in the senior secondary years. It argues there is no consistent and shared view of the purpose of the senior secondary certificates, no consistent approaches to dealing with disadvantage, and continuing difficulties in meeting the needs of the full range of young people in the senior years, particularly those from regional and remote areas, Indigenous communities and low socio-economic status students. There is also considerable variability in retention rates and rates of attainment of the senior secondary certificates as well as the calculation of the Australian Admissions Tertiary Rank (ATAR) score which was primarily designed for university selection purposes. The certificates also have limited emphasis on capabilities in their design and considerable variation in the manner in which literacy and numeracy minimum standards are defined, set and assessed. Furthermore, there is no consistent approach regarding compulsory subjects or a core curriculum, the design and implementation of VET courses and the evolving role of the ATAR.
