International audience; Background: In France, the first COVID-19-related lockdown (17th March to 10th May 2020) resulted in a major decrease in healthcare service utilization. This raised concerns about the continuity of care for vulnerable patients.Objectives: To identify individual and organizational factors associated with the initiatives taken by French GPs to contact vulnerable patients during the lockdown.Methods: A national observational survey using an online questionnaire was conducted to document French GPs' adaptations to the COVID-19 situation, their individual and organizational characteristics, including practice type (individual, group, multidisciplinary) and size. Data were collected from 7th to 20th May 2020 using mailing lists of GPs from the study partners and GPs who participated in a previous survey. This paper analysed answers to the question exploring whether and how GPs took initiatives to contact vulnerable patients. Responses were categorized in: no initiative; selection of patients to contact with a criteria-based strategy; initiative of contact without criteria-based strategy. Multivariate multinomial logistic regression identified factors associated with each category. Key components of the reported initiatives were described by inductive analysis of verbatim material.Results: Among the 3012 participant GPs (similar to 5.6% of French GPs), 1419 (47.1%) reported initiatives to contact some patients without criteria-based strategy, and 808 (26.8%) with a strategy using various clinical/psychological/social criteria. Women GPs more often declared initiatives of contacts with a criteria-based strategy (OR = 1.41, 95% CI [1.14-1.75]) as well as GPs with more than two patients who died due to COVID-19 in comparison with those having none (OR = 1.84, 95% CI [1.43-2.36]). Teaching GPs more often used criteria-based strategies than the other GPs (OR = 1.94, 95% CI [1.51-2.48]). Compared with those working in small monodisciplinary practice, GPs working alone were less likely to implement criteria-based initiatives of contacts (OR = 0.70, 95% CI [0.51-0.97]), while GPs working in multidisciplinary practice were more likely (OR = 1.94, 95% CI [1.26-2.98] in practices > 20 professionals).Conclusion: French GPs took various initiatives to keep in touch with vulnerable patients, more frequently when working in group practices. These findings confirm the importance of primary care organization to ensure continuity of care for vulnerable people.

BACKGROUND — In the follow-up of homebound, polymorbid patients, health professionals (HP) often use a point-of-care notebook to share information with each other. OBJECTIVE — Analyse how and for what HP use such notebooks and to explore their perceptions regarding the strengths and weaknesses of such written notes for interprofessional collaboration. METHODS — We conducted a mixed-method study among HP caring for homebound elderly patients in Geneva, Switzerland. A sample of notebooks was first analysed descriptively. Then 6 focus groups interviews were conducted with different groups of HP. Focus groups were transcribed verbatim and analysed thematically. RESULTS — The analysis of 11 notebooks revealed that most of the time, the intended recipient of written information was unspecified; content focused mainly on somatic health and medication; explicit interprofessional communication was rare and patients never wrote down any information. There were no explicit care plan goals. Participants of the focus group (31 HP) felt that the notebook embodied the primary care network and was a milestone in the follow-up of homebound patient. Most complained about having to record the information at least twice in the absence of a shared electronic health record. The fact that the notebook was accessible to anyone and was considered the patient’s property influenced how and what professionals wrote in the notebook. Lack of patient contributions to the notebook was a source of concern. CONCLUSIONS — Our results show that fulfilment of notebook’s primary function is suboptimal. In order to enhance written communication, more attention should be paid to defining care plan goals.; CONTEXTE — Les professionnels de la santé (PdS) communiquent souvent entre eux par écrit dans un carnet pour le suivi de patients polymorbides confinés à domicile. OBJECTIFS — Identifier les différences entre ce qui est consigné et ce qui est attendu dans ces carnets, et leur impact sur la collaboration interprofessionnelle. METHODE — Nous avons d’abord réalisé une étude descriptive d’un échantillon de carnets utilisés à Genève (Suisse) ; puis, nous avons conduit une étude qualitative de 6 focus groups de PdS. Nous avons analysé thématiquement leur transcription verbatim. RESULTATS — L’analyse de 11 carnets a révélé qu’aucun destinataire n’était identifié la plupart du temps. Les thèmes principaux relevaient du somatique et des médicaments. La proportion des interactions entre PdS était faible comparativement à la quantité de texte produite ; aucun des patients n’a écrit. Aucun plan de soin explicite n’était consigné. Les focus groups ont permis d’identifier plusieurs thèmes. Les 31 participants trouvaient que le carnet permettait de matérialiser le réseau et qu’il était un repère dans la prise en charge des patients. Pourtant, beaucoup se plaignaient de doublons, en l’absence d’un dossier de soin partagé. Le contenu des messages était influencé par les caractéristiques de ce support (propriété du patient, accessible à tous). L’implication constructive du patient était jugée insuffisante. CONCLUSIONS — Nos résultats ont montré qu’en l’absence de consensus clair et négocié, la fonction première du carnet est sinon dévoyée, du moins sous-optimale. Améliorer la communication interprofessionnelle écrite, centrée sur le patient, passe par la définition de plan de soin partagé.

Afin d'améliorer la couverture vaccinale des enfants en Suisse, le médecin a un rôle à jouer dans la communication auprès du ou des parents hésitants. Pour cela, il doit avoir des connaissances bien référencées et transmettre de la documentation écrite. Négocier un plan de vaccination plus souple, individualisé à l'enfant peut s'avérer parfois nécessaire. Beaucoup d'informations contre la vaccination circulent sur internet et dans les médias. Le médecin doit y être préparé et connaître les principaux arguments des mouvements antivaccins pour pouvoir en discuter avec les parents qui y sont réceptifs. Utiliser des techniques comme l'ajout d'une touche personnelle à son discours ou adapter l'entretien motivationnel à la vaccination aidera le médecin à guider les parents vers un choix informé et éclairé favorable à la vaccination de leur enfant.

Background: Currently in France, participation rate in organised colorectal cancer (CRC) screening remains well below European guidelines, despite general practitioners involvement.Objectives: The overall objective of this thesis was to assess general practitioners practices in organized CRC screening, to understand barriers to screening participation and to assess strategies to improve patient participation to screening.Methods: Our work was based on data from the organised screening programme in the Val-de-Marne district. We conducted a retrospective cohort of 157 979 patients followed by 961 general practitioners, an action research among 21 volunteer general practitioners and a cluster-randomized controlled trial including 144 general practitioners and 20,778 patients.Results: A small part of the variability of the likelihood of participation was due to the heterogeneity among physicians (intraclass correlation coefficient, 5.5%). Screening participation was significantly lower in males (odds ratio [OR], 0.79; 95% CI, 0.78 to 0.91), the youngest age group (50-54 years, OR, 0.61; 95% CI, 0.58 -0.63; 55-59 years, OR, 0.76; 95% CI, 0.73-0.80) and patients living in socioeconomically deprived areas (OR, 0.82; 95% CI, 0.77-0.87).We have identified seven essential requirements for general practitioners when screening for CRC: Be proactive, Be partners in care, Take into consideration the patient's family and friends, Position themselves as the expert, Manage time efficiently, Explain the test procedure and Help carry out the test. We were able to identify techniques used for each requirement.Systematic reminders sent by post to general practitioners with the updated list of eligible patients for screening did not significantly improve patient participation to organised CRC screening after taking clustering into account (multilevel analysis).Conclusions: Targeted actions to improve CRC screening participation should focus on patients younger than 60 years, males, and individuals living in deprived areas. Actions to enhance the influence of general practitioners on patient participation should be directed to the overall population of general practitioners. Patient-centred care and reflective practice should be at the heart of initial and continuing medical education, and guidelines based on practice data should be proposed., Contexte : Actuellement en France, le taux de participation au dépistage organisé du cancer colorectal (CCR) reste nettement inférieur aux recommandations européennes et ce, malgré l’implication des médecins généralistes.Objectifs : L’objectif général de ce travail de thèse était d’évaluer les pratiques des médecins généralistes en matière de dépistage organisé du CCR, de comprendre les freins au dépistage et d’évaluer les stratégies permettant d’améliorer la participation des patients à ce dépistage.Méthode : Nos travaux se sont appuyés sur les données du programme de dépistage organisé dans le Val-de-Marne. Nous avons reconstitué une cohorte historique de 157 979 patients suivis par 961 médecins généralistes, mené une recherche-action auprès de 21 médecins volontaires et réalisé un essai contrôlé randomisé en grappes incluant 144 médecins et 20 778 patients.Résultats : Une faible part de la variabilité de la probabilité de participation était attribuable à l’hétérogénéité entre médecins (coefficient de corrélation intra-classe, 5,5%). La participation au dépistage était moindre chez les hommes (odds ratio [OR], 0,79 ; IC 95%, 0,78–0,91), les jeunes (50–54 ans, OR, 0.61 ; IC 95%, 0.58–0.63 ; 55–59 ans, OR, 0.76 ; IC 95%, 0.73–0.80) ou les résidents des zones les plus défavorisées (OR, 0.82 ; IC 95%, 0.77–0.87).Nous avons identifiés 7 exigences essentielles pour l'activité de dépistage organisé du cancer colorectal par le médecin généraliste : Être proactif, Être un partenaire, Prendre en compte l’entourage, Se positionner comme expert du problème, Gérer le temps de manière efficiente, Expliquer la réalisation du test et Aider la réalisation du test. Pour chacune, nous avons pu identifier des techniques utilisables en situation de pratique clinique.Les rappels systématiques adressés par voie postale aux médecins généralistes comportant la liste mise à jour de leurs patients éligibles au dépistage n'ont pas augmenté de manière significative la participation des patients au dépistage après prise en compte de l’effet grappe (analyse multiniveau).Conclusions : Des actions ciblées vers les patients les plus jeunes, les hommes et les résidents des zones géographiques les plus défavorisées devraient être encouragées afin de réduire les disparités observées et améliorer l’efficacité globale du programme de dépistage. Des actions sur l'ensemble des médecins généralistes doivent être envisagées : mettre l'approche centrée sur le patient et la pratique réflexive au cœur du projet de formation initiale et continue, et proposer des recommandations de pratique basées sur les données issues de la pratique.

Correspondence: julien.foucaud@legs.cnrs-gif.fr; International audience; The invasive ant species Wasmannia auropunctata displays both ecologically dominant and non-dominant populations within its native range. Three factors could theoretically explain the ecological dominance of some native populations of W. auropunctata: (i) its clonal reproductive system, through demographic and ⁄ or adaptive advantages; (ii) its unicolonial social organization, through lower intraspecific and efficient interspecific competition; (iii) the human disturbance of its native range, through the modification of biotic and abiotic environmental conditions. We used microsatellite markers and behavioural tests to uncover the reproductive modes and social organization of dominant and non-dominant native populations in natural and human-modified habitats. Microsatellite and mtDNA data indicated that dominant and non-dominant native populations (supercolonies as determined by aggression tests) of W. auropunctata did not belong to different evolutionary units. We found that the reproductive system and the social organization are neither necessary nor sufficient to explain W. auropunctata ecological dominance. Dominance rather seems to be set off by unknown ecological factors altered by human activities, as all dominant populations were recorded in human-modified habitats. The clonal reproductive system found in some populations of W. auropunctata may however indirectly contribute to its ecological dominance by allowing the species to expand its environmental niche, through the fixation over time of specific combinations of divergent male and female genotypes. Unicoloniality may rather promote the range expansion of already dominant populations than actually trigger ecological dominance. The W. auropunctata model illustrates the strong impact of human disturbance on species’ ecological features and the adaptive potential of clonal reproductive systems.

No English abstract; Les invasions biologiques constituent une menace majeure pour la biodiversité dans les zones ou elles ont lieu. Les travaux que nous développons dans ce mémoire s’intéressent à la prolifération de la petite fourmi de feu Wasmannia auropunctata (Roger), espèce originaire de l’Amérique tropicale qui, parce que l’Homme l’a involontairement transportée, a envahi aujourd’hui une grande partie de la ceinture tropicale du globe. Ce succès, comme celui de plusieurs autres espèces du groupe, est impressionnant. Nous avons étudié les relations intraspécifiques (structure sociale des populations) et interspécifiques (interactions avec les autres espèces de fourmis) de W. auropunctata dans une zone d’introduction (la Nouvelle-Calédonie) et dans sa zone d’origine (le Brésil et la Guyane Française). Nous avons mis en évidence l’unicolonialité de W. auropunctata pour toute la Nouvelle-Calédonie, où elle forme une super-colonie. En anihilant l’agressivité intraspécifique, cette caractéristique lui permet d’atteindre des densités de populations très importantes. La myrmécofaune native de Nouvelle-Calédonie n’offre que peu de résistance à cet envahisseur. Ces observations pourraient être en partie expliquées par la dysharmonie de la faune native, due à la situation insulaire. Par exemple, la myrmécofaune arboricole n’est que très faiblement représentée et les espèces les plus fréquentes ne sont pas des dominantes. La petite fourmi de feu exploite activement les ressources présentes dans cette strate, en particulier le miellat produit par les hémiptères, composante essentielle de son régime alimentaire. L’envahisseur profite d’une opportunité de ressource. Au niveau du sol, les espèces dominantes, appartenant principalement au genre Pheidole, n’adoptent pas de réactions adaptées lors des rencontres avec les ouvrières de W. auropunctata. Seule Pheidole megacephala, une autre espèce introduite peut ponctuellement offrir une résistance significative mais son implantation entraîne également de graves désordres écologiques. Les études menées dans les zones d’origine ont apporté des repères nécessaires à une meilleure compréhension (1) des mécanismes qui prédisposent W. auropunctata à devenir envahissante et (2) des facteurs qui régulent significativement ses populations. Dans sa zone d’origine, W. auropunctata est connue pour être organisée en structures polycaliques et polygynes. Nous avons cependant montré que des populations homogènes pouvaient couvrir des territoires de plusieurs km que ce soit en zones naturelles ou dégradées. Ces caractéristiques peuvent être considérées comme un pas vers l’unicolonialité, prédisposant cette espèce à devenir envahissante. En Guyane, nous avons observé que les autres espèces dominantes appartenant aux genres Pheidole et Solenopsis peuvent se montrer très compétitives et agressives lors de leurs rencontres avec W. auropunctata, leur permettant de défendre efficacement des ressources. Une fourmi légionnaire appartenant au genre Neivamyrmex est un prédateur très efficace. Son action peut être un des mécanismes de régulation de W. auropunctata. Néanmoins, même dans sa zone d’origine, si des perturbations interviennent et réduisent fortement la diversité de la myrmécofaune et que les conditions lui sont favorables, les caractéristiques sociales de W. auropunctata lui permettent de développer d’importantes populations et les situations sont comparables à celles observées dans les zones d’introduction.

The tramp ant Wasmannia auropunctata (Hymenoptera, Formicidae) invaded New Caledonia around 1970. Though it was first supposed to be restricted to disturbed areas, the invader colonized a diversity of native biota. Its occurence was especially noticed in protected areas where the vegetation has a great value for conservation. The invader thrives in sclerophyll forests where it may even saturate the biotope. This raises much concern, as this type of forest is now restricted to relicts and is considered as the most valuable biotope to be preserved in New Caledonia. The invader has been less obvious in the dense humid forests on ultramafic rocks, which sup¬ ported for some time the idea of an ecological resistance to invasion in this forest. But recent studies indicate the steady progress of Wasmannia and its ability to permanently and efficiently colonize these biota. Wasmannia is primarily a litter-dwelling ant and most of its action has been monitored at this level. The domination of the invader on the native myrmecofauna is absolute in localities where it is able to build large populations. II also eliminates a significant part of the arachnofauna and its impact can be observed even on vertebrates like skinks and gekkos. This opportunistic ant also forages in the canopy where its impact on the native fauna is more difficult to assess. Wasmannia auropunctata is a successfull invader because it shares most characters common to the functionnal ant species group known as « tramp species », which are perceived as pests everywhere in the world. In New Caledonia, no native species seems able to compete with Wasmannia and interfere with its colonizing success, as this ant is a very efficient fighter and also is able to adapt its nesting behaviour to adverse situations.
