Many countries have experienced 2 predominant waves of COVID-19-related hospitalizations. Comparing the clinical trajectories of patients hospitalized in separate waves of the pandemic enables further understanding of the evolving epidemiology, pathophysiology, and health care dynamics of the COVID-19 pandemic.In this retrospective cohort study, we analyzed electronic health record (EHR) data from patients with SARS-CoV-2 infections hospitalized in participating health care systems representing 315 hospitals across 6 countries. We compared hospitalization rates, severe COVID-19 risk, and mean laboratory values between patients hospitalized during the first and second waves of the pandemic.Using a federated approach, each participating health care system extracted patient-level clinical data on their first and second wave cohorts and submitted aggregated data to the central site. Data quality control steps were adopted at the central site to correct for implausible values and harmonize units. Statistical analyses were performed by computing individual health care system effect sizes and synthesizing these using random effect meta-analyses to account for heterogeneity. We focused the laboratory analysis on C-reactive protein (CRP), ferritin, fibrinogen, procalcitonin, D-dimer, and creatinine based on their reported associations with severe COVID-19.Data were available for 79,613 patients, of which 32,467 were hospitalized in the first wave and 47,146 in the second wave. The prevalence of male patients and patients aged 50 to 69 years decreased significantly between the first and second waves. Patients hospitalized in the second wave had a 9.9% reduction in the risk of severe COVID-19 compared to patients hospitalized in the first wave (95% CI 8.5%-11.3%). Demographic subgroup analyses indicated that patients aged 26 to 49 years and 50 to 69 years; male and female patients; and black patients had significantly lower risk for severe disease in the second wave than in the first wave. At admission, the mean values of CRP were significantly lower in the second wave than in the first wave. On the seventh hospital day, the mean values of CRP, ferritin, fibrinogen, and procalcitonin were significantly lower in the second wave than in the first wave. In general, countries exhibited variable changes in laboratory testing rates from the first to the second wave. At admission, there was a significantly higher testing rate for D-dimer in France, Germany, and Spain.Patients hospitalized in the second wave were at significantly lower risk for severe COVID-19. This corresponded to mean laboratory values in the second wave that were more likely to be in typical physiological ranges on the seventh hospital day compared to the first wave. Our federated approach demonstrated the feasibility and power of harmonizing heterogeneous EHR data from multiple international health care systems to rapidly conduct large-scale studies to characterize how COVID-19 clinical trajectories evolve.

Professional societies and their conferences provide an important venue for disseminating scientific knowledge. Delivering a keynote for or being named a fellow by an international society is a major recognition. Do such recognitions reflect the composition of the field of bioinformatics? We compiled a list of 412 International Society for Computational Biology honorees (75 fellows and 337 keynote speakers). To assess the composition of senior researchers in the field, we analyze the names and affliations of corresponding authors in leading bioinformatics journals. Comparing to the distribution based on names and affliations of honorees, we looked for disparities in gender, country of affliation, race, and name-origin. Name-origin predictions were created using a neural network trained on 700,000 name-nationality pairs mined from Wikipedia. The proportion of female honorees has kept pace with increasing levels of female authorship, but neither has yet to reach gender parity. However, we noticed a striking geographic disparity where the proportion of honorees with an affliation in the United States was 1.6-fold greater compared to field-specific senior authors. In total, we estimate the U.S. received 85 more honorees than would be expected from randomly selecting honorees from senior authors. Almost half of the excess in U.S. honorees was accounted for by a deficit of 41 honoree slots from China, France, and Italy. Furthermore, within the U.S., we identify racial disparities with an excess of white honorees and a depletion of Asian honorees. This pattern replicated globally, where we find names of East Asian origin have been persistently underrepresented among ISCB honorees. Early indications suggest the ICSB has taken note of our findings by selecting more diverse honorees.

Using a decomposition approach on data collected by the Asian Productivity Organization (APO) as well as World Input-Output data, we show that in most Asian economies the services sector makes the largest contribution to labor productivity. Furthermore, we find evidence of a major reallocation of labor from agriculture directly to services, bypassing the manufacturing sector. This finding challenges the traditional view that countries in their economic development need to have their workforce employed first in manufacturing before switching to services. Lastly, the paper studies how different skill levels contribute to labor productivity growth. We find that high-skilled workers have contributed most to overall labor productivity growth in developing Asia. In services, high-skilled workers have mainly driven labor productivity, indicating that upskilling and training are instrumental in services-led development.

Automated machine learning (AutoML) systems are helpful data science assistants designed to scan data for novel features, select appropriate supervised learning models and optimize their parameters. For this purpose, Tree-based Pipeline Optimization Tool (TPOT) was developed using strongly typed genetic programming to recommend an optimized analysis pipeline for the data scientist’s prediction problem. However, TPOT may reach computational resource limits when working on big data such as whole-genome expression data. We introduce two new features implemented in TPOT that helps increase the system’s scalability: Dataset selector and Template. Dataset selector (DS) provides the option to specify subsets of the features as separate datasets, assuming the signals come from one or more of these specific data subsets. Built in at the beginning of each pipeline structure, DS reduces the computational expense of TPOT to only evaluate on a smaller subset of data rather than the entire dataset. Consequently, DS increases TPOT’s efficiency in application on big data by slicing the dataset into smaller sets of features and allowing genetic programming to select the best subset in the final pipeline. Template enforces type constraints with strongly typed genetic programming and enables the incorporation of DS at the beginning of each pipeline. We show that DS and Template help reduce TPOT computation time and may provide more interpretable results. Our simulations show TPOT-DS significantly outperforms a tuned XGBoost model and standard TPOT implementation. We apply TPOT-DS to real RNA-Seq data from a study of major depressive disorder. Independent of the previous study that identified significant association with depression severity of the enrichment scores of two modules, in an automated fashion, TPOT-DS corroborates that one of the modules is largely predictive of the clinical diagnosis of each individual. Author Summary Big data have recently become prevalent in many fields including meteorology, complex physics simulations, large scale imaging, genomics, biomedical research, environmental research and more. TPOT is a Python Automated Machine Learning (AutoML) tool that uses genetic programming to optimize machine learning pipelines for analyzing biomedical data. However, like other AutoML tools, when analyzing big data, the early implementations of TPOT face the challenges of long runtime, high computational expense as well complex pipeline with low interpretability. Here, we develop two novel features for TPOT, Dataset Selector and Template, that leverage domain knowledge, greatly reduce the computational expense and flexibly extend TPOT’s application to biomedical big data analysis.

This paper examines the surge of imports from the PRC to Viet Nam from 2000 to 2014 in order to evaluate the effects of increased exposure to trade with the PRC on income inequality and household welfare in Viet Nam. Using household level data from the Viet Nam Household Living Standard Survey and combining it with measures of trade exposure, we find that increased imports led to a fall in inequality at the provincial and district level. We distinguish between intermediate and final goods and find similar results. In order to better understand the relative gains and losses across income groups, we apply a quantile regression approach. Our results indicate that increased imports were more often positively correlated with household income for households located in the lower quantiles. In contrast, for households in the upper quantiles the correlation is either negative or less pronounced.
