7. Image processing using the Walsh transform

Abstract

This thesis presents a new algorithm which can be used to register images of the same or different modalities e.g images with multiple channels such as X-rays, temperature, or elevation or simply images of different spectral bands. In particular, a correlation-based scheme is used, but instead of grey values, it correlates numbers formulated by different combinations of the extracted local Walsh coefficients of the images. Each image patch is expanded in terms of Walsh basis functions. Each Walsh basis function can be thought of as measuring a different aspect of local structure, eg horizontal edge, corner, etc. The coefficients of the expansion, therefore, can be thought of as dense local features, estimating at each point the degree of presence of, for example, a horizontal edge, a corner with contrast of a certain type, etc. These coefficients are normalised and used as digits in a chosen number system which allows one to create a unique number for each type of local structure. The choice of the basis of the number system allows one to give different emphasis to different types of local feature (e.g. corners versus edges) and thus the method we present forms a unified framework in terms of which several feature matching methods may be interpreted. The algorithm is compared with wavelet based approaches, using simulated and real images. The images used for the registration experiments are assumed to differ from each other by a rotation and a translation only. Additionally, the method was extended to cope with 3D image sets, while as an add-on, it was also tried in performing image segmentation.

Abstract

Cybersecurity concerns have been at the forefront of regulatory reform in the European Union (EU) recently. One of the outcomes of these reforms is the introduction of certification schemes for information and communication technology (ICT) products, services and processes, as well as for data processing operations concerning personal data. These schemes aim to provide an avenue for consumers to assess the compliance posture of organisations concerning the privacy and security of ICT products, services and processes. They also present manufacturers, providers and data controllers with the opportunity to demonstrate compliance with regulatory requirements through a verifiable third-party assessment. As these certification schemes are being developed, various sectors, including the electrical power and energy sector, will need to access the impact on their operations and plan towards successful implementation. Relying on a doctrinal method, this paper identifies relevant EU legal instruments on data protection and cybersecurity certification and their interpretation in order to examine their potential impact when applying certification schemes within the Electrical Power and Energy System (EPES) domain. The result suggests that the EPES domain employs different technologies and services from diverse areas, which can result in the application of several certification schemes within its environment, including horizontal, technological and sector-specific schemes. This has the potential for creating a complex constellation of implementation models and would require careful design to avoid proliferation and disincentivising of stakeholders. © 2022 by the authors. Abstract

LTX-315 is a first-in-class, 9-mer membranolytic peptide that has shown potent immunomodulatory properties in preclinical models. We conducted a phase I dose-escalating study of intratumoral LTX-315 administration in patients with advanced solid tumors. Thirty-nine patients were enrolled, receiving LTX-315 injections into accessible tumors. The primary objective was to assess the safety and tolerability of this approach, with antitumor and immunomodulatory activity as secondary objectives. Tumor biopsies were collected at baseline and posttreatment for analysis of immunologic parameters. The most common treatment-related grade 1-2 adverse events were vascular disorders including transient hypotension (18 patients, 46%), flushing (11 patients, 28%), and injection site reactions in 38% of patients. The most common grade 3 LTX-315-related toxicities were hypersensitivity or anaphylaxis (4 patients, 10%). Analysis of immune endpoints in serial biopsies indicated that LTX-315 induces necrosis and CD8 T-cell infiltration into the tumor microenvironment. Sequencing of the T-cell receptor repertoire in peripheral blood identified significant expansion of T-cell clones after treatment, of which 49% were present in available tumor biopsies after treatment, suggesting that they were tumor associated. Substantial volume reduction (≥30%) of injected tumors occurred in 29% of the patients, and 86% (12/14 biopsies) had an increase in intralesional CD8 T cells posttreatment. No partial responses by immune-related response criteria were seen, but evidence of abscopal effect was demonstrated following treatment with LTX-315. LTX-315 has an acceptable safety profile, is clinically active, induces changes in the tumor microenvironment and contributes to immune-mediated anticancer activity.

Abstract

Lung cancer remains the leading cause of death due to cancer. We do not have effective tools for the early detection of lung cancer, so patients are usually diagnosed at an advanced stage. However, novel therapies based on molecular pathways (such as those involving the epidermal growth factor receptor, anaplastic lymphoma kinase, serine/threonine-protein kinase B-Raf, proto-oncogene tyrosine-protein kinase ROS, c-Met proto-oncogene protein, and RET proto-oncogene protein) are now commonly used in the treatment of lung cancer. In particular, these therapies are considered as first-line treatments for non-small-cell lung cancer. This manuscript outlines previous research on these targeted treatments, focusing on whether they are effective against eye metastasis.

Abstract

The pneumothorax is an abnormal collection of air or gas in the pleural space that separates the lung from the chest wall. Like pleural effusion where a large abnormal concentration of fluid (>100 mL) is liquid buildup in that space, pneumothorax may interfere with normal breathing. A medical term that it is used is the collapsed lung, although that term may also refer to atelectasis. There are two major types of pneumothorax; there is one that occurs without an apparent cause and in the absence of significant lung disease, while the so called; “secondary” pneumothorax occurs in the presence of existing lung pathology. In a minority of cases, the amount of air in the chest increases markedly when a one-way valve is formed by an area of damaged tissue, leading to a third type of pneumothorax, called “tensioned”.

Abstract

The Heimlich valve is a small one-way valve used for chest drainage that empties into a flexible collection device and prevents return of gases or fluids into the pleural space. The Heimlich valve is less than 13 cm (5 inches) long and facilitates patient ambulation. Currently there are several systems in the market. It can be used in many patients instead of a traditional water seal drainage system. The Heimlich chest drainage valve was developed so that the process of draining the pleural cavity could be accomplished in a safe, relatively simple, and efficient manner. This valve system has replaced the cumbersome underwater drainage bottle system. Moreover; the Heimlich valve system connects to chest tubing and allows fluid and air to pass in one direction only. This system functions in any position, and it does not ever need to be clamped, a regulated suction can be attached to it if necessary. The valve drains into a plastic bag that can be held at any level, allowing the patient undergoing chest drainage to be ambulatory simply by carrying the bag. In the current mini review we will present the Heimlich valve system and method of insertion.

Abstract

Cardiothoracic surgery sternal infections are difficult to treat situations. Until now there are no clear guidelines which or if an antibiotic could be used as prophylactic treatment.We collected retrospectively data from 535 patients from our hospital which underwent cardiothoracic surgery and recorded several biological parameters and technical aspects of the surgery.It was observed that patients to whom vancomycin was administered had less post surgery infection than those to whom begalin was administered. Male who were treated with vancomycin it was observed that they had 1.67 chances to be treated properly than female. Patients which were hospitalized for more than 7 days before surgery had 62.6% higher chances for post surgery infection.It was observed that vancomycin can be used as a prophylactic treatment for cardiothoracic surgeries acting efficiently against sternal wounds.

Abstract

Postoperative pain is a very important issue for several patients. Indifferent of the surgery type or method, pain management is very necessary. The relief from suffering leads to early mobilization, less hospital stay, reduced hospital costs, and increased patient satisfaction. An individual approach should be applied for pain control, rather than a fix dose or drugs. Additionally, medical, psychological, and physical condition, age, level of fear or anxiety, surgical procedure, personal preference, and response to agents given should be taken into account. The major goal in the management of postoperative pain is minimizing the dose of medications to lessen side effects while still providing adequate analgesia. Again a multidisciplinary team approach should be pursued planning and formulating a plan for pain relief, particularly in complicated patients, such as those who have medical comorbidities. These patients might appear increase for analgesia-related complications or side effects.

Abstract

A thoracotomy is an incision into the pleural space of the chest. It is performed by surgeons (or emergency physicians under certain circumstances) to gain access to the thoracic organs, most commonly the heart, the lungs, or the esophagus, or for access to the thoracic aorta or the anterior spine. This surgical procedure is a major surgical maneuver it is the first step in many thoracic surgeries including lobectomy or pneumonectomy for lung cancer and as such requires general anesthesia with endotracheal tube insertion and mechanical ventilation, rigid bronchoscope can be also used if necessary. Thoracotomies are thought to be one of the most difficult surgical incisions to deal with post-operatively, because they are extremely painful and the pain can prevent the patient from breathing effectively, leading to atelectasis or pneumonia. In the current review we will present the steps of this procedure.

Abstract

Novel therapies for lung cancer are being explored nowadays with local therapies being the tip of the arrow. Intratumoral chemotherapy administration and local microwave ablation have been investigated in several studies. It has been previously proposed that lipiodol has the ability to modify the microenvironment matrix. In our current study we investigated this theory in BALBC mice. In total 160 BALBC mice were divided in eight groups: a) control, b) cisplatin, c) microwave, d) microwave and lipiodol, e) cisplatin and lipiodol, f) microwave and cisplatin, g) lipiodol and h) lipiodol, cisplatin and microwave. Lewis lung carcinoma cell lines (106) were injected into the right back leg of each mouse. After the 8th day, when the tumor volume was about 100mm3 the therapy application was initiated, once per week for four weeks. Magnetic resonance imaging was performed for each tumor when a mouse died or when sacrificed if they were still alive by the end of the experiment (8-Canal multifunctional spool; NORAS MRI products, Gmbh, Germany). Imaging and survival revealed efficient tumor apoptosis for the groups b,c,d,e and f. However; severe toxicity was observed in group h and no follow up was available for this group after the second week of therapy administration. Lipiodol in its current form does assist in a more efficient way the distribution of cisplatin, as the microwave apoptotic effect. Future modification of lipiodol might provide a more efficient method of therapy enhancement. Combination of drug and microwave ablation is possible and has an efficient apoptotic effect.

39. Image processing using the Walsh transform

Abstract

This thesis presents a new algorithm which can be used to register images of the same or different modalities e.g images with multiple channels such as X-rays, temperature, or elevation or simply images of different spectral bands. In particular, a correlation-based scheme is used, but instead of grey values, it correlates numbers formulated by different combinations of the extracted local Walsh coefficients of the images. Each image patch is expanded in terms of Walsh basis functions. Each Walsh basis function can be thought of as measuring a different aspect of local structure, eg horizontal edge, corner, etc. The coefficients of the expansion, therefore, can be thought of as dense local features, estimating at each point the degree of presence of, for example, a horizontal edge, a corner with contrast of a certain type, etc. These coefficients are normalised and used as digits in a chosen number system which allows one to create a unique number for each type of local structure. The choice of the basis of the number system allows one to give different emphasis to different types of local feature (e.g. corners versus edges) and thus the method we present forms a unified framework in terms of which several feature matching methods may be interpreted. The algorithm is compared with wavelet based approaches, using simulated and real images. The images used for the registration experiments are assumed to differ from each other by a rotation and a translation only. Additionally, the method was extended to cope with 3D image sets, while as an add-on, it was also tried in performing image segmentation.
