6. Social work and learning disabilities : an exploration of the contribution of social work within a multi-disciplinary team

Abstract

Effective delivery of health and social care requires collaboration between professions. The aim of this research study was to explore one element of that collaboration – the contribution made by social workers to multi-disciplinary professional practice in adult services in England. The study was conducted against the backcloth of the Modernisation agenda for health and social care integration. This approach was first introduced by the New Labour Government (1997-2010) and strengthened by the vision and expectations championed in the policy documents, Valuing People (2001) and Valuing People Now (2009) for adults with learning disabilities. Hermeneutical phenomenology, drawn from Heidegger, underpinned the methodological and philosophical approach which led to an emphasis upon rich description and interpretations of individual lived-worlds. The theoretical position adopted was informed by Wenger’s work on Communities of Practice, which is grounded in the importance of social interactions inherent within multi-disciplinary practice between different professionals. Participants included registered social workers (n=9) and allied health professionals (n=8). Data was generated over a nine month period. Semi-structured interviews were utilised with all professional participants. Data was analysed using Nvivo (10) and an Interpretative Phenomenological Analysis towards the data was adopted. Individual vignettes were presented on behalf of local citizens (n=9) which represented their personal narratives about the value of social work, embedded within this multi-disciplinary context. The key findings highlighted the unwavering commitment from social workers to advocate on behalf of vulnerable adults. This was underpinned by adherence to professional social work values which facilitated their abilities to deliver a plethora of professional services. Allied health professionals substantiated the important contributions of social workers within the multi-disciplinary team, alongside local citizens who confirmed social workers valuable roles as part of their complex networks of support. The implications from this research study suggested that inter-professional education and training could augment collaboration between professions to progress the current health and social care agenda, focused upon integration in England.

10. Family Discipline Practices with Infants at Six Months of Age

Abstract

Objectives: To determine the frequency of 14 discipline strategies used by mothers (n = 564) and fathers (n = 335) in caring for their six-month old infant. Methods: Data on discipline practices were obtained from families participating in an obesity prevention trial (Prevention of Overweight in Infancy). Each parent was asked to indicate the frequency of using 14 different discipline strategies during the previous week with their infant. Associations between the use of these strategies and parity, deprivation, parental ethnicity, depression symptoms and infant sex were investigated. Results: Parents reported using both positive (mean: 8.8 for mothers and 7.9 for partners) and negative (0.7 for mothers and 0.7 for partners) strategies over the past week. At six months, positive strategies such as smiling, praising and distraction were most commonly reported (=87% for both mothers and partners), negative strategies such as smacking, time out and shouting being reported infrequently (=8% for all). Discipline requiring a level of cognitive understanding not developed at six months was reasonably common, including reasoning (17.7% for mothers, 23.4% for partners), negotiation (6.2%, 11.7%) and ignoring (26%, 19%). Multivariate analysis demonstrated that mothers with more than one child had a lower positive discipline index than those with only one child (difference, 95% C.I.: 1.05 (0.57, 1.54). An increase of one point on the 10-point hostile parenting scale was associated with an increase of 0.18 (0.04, 0.31) for the mothers and 0.47 (0.23, 0.70) for partners on the negative strategy index. Conclusions: Discipline strategies emerge early in infancy and are associated with family size and parental hostility.

These immense stressors affect our emotional and behavioral responses, [...], How do we survive, let alone thrive, in health care? The complexity of providing care to patients and families on chronic kidney replacement therapy can be taxing for the most expert and resilient of health care professionals. Further complicating this by a global pandemic can produce feelings of frustration and anxiety that can reduce our effectiveness in the everyday, yet we are compelled to practice competently and professionally. Working on oneself is key to optimizing one's resilience during volatile, uncertain, complex, and ambiguous times. This article introduces the concept of emotional intelligence and provides a brief review of the brain science behind behavioral patterns, highlighting how all health care professionals can engage in self-work to bring our best selves to the every day.
