Abstract

The aim of this research has been to describe the implementation of safe staffing policies in NHS general acute Trusts in England looking at costs and consequences, and examining the factors that have influenced implementation. A mix of qualitative and quantitative methods were used (national survey, analysis of national data, four case studies, realist evaluation) to examine the impact of policies nationally, and explore commonality and variation in local responses to safe staffing policies. The report is based on independent research commissioned and funded by the NIHR Policy Research Programme (“Implementation, Impact and Costs of Policies for Safe Staffing in Acute NHS Trusts”, PR-ST-1115-10017). The views expressed are those of the authors and not necessarily those of the NIHR, the Department of Health and Social Care, ‘arm’s length bodies’ or other government departments.

Abstract

ABSTRACT Aim Design Methods Results Conclusion Implications for the Profession and/or Patient Care Reporting Method Patient or Public Contribution To explore and validate an end of shift survey with a low response burden, practical application and generated evidence of related associations between workload, quality of work and patient care, missed care and job satisfaction.A retrospective cross‐sectional survey of the experiences of nursing staff.Data were collected from 265 nurses who responded to a questionnaire at the end of their shift in 2022. Exploratory factor analysis was undertaken using IBM SPSS v.27 and confirmatory factor analysis was undertaken using IBM AMOS v27. Hypotheses testing was undertaken using IBM SPSS v.27 using multiple regression analyses.All of the hypotheses were supported. There was a negative association between workload and quality of work and job satisfaction. Quality of work was negatively associated with workload and missed care and positively associated with job satisfaction. The association between missed care and job satisfaction was negative.The EOSS is a valid and reliable tool with a low response burden. The tool supports previous research which demonstrated there is a negative relationship between level of workload and shift type with satisfaction, quality of work and potentially nurse retention.In the context of a global nursing shortage nursing leaders must ensure that care we provide is of the highest quality. We must take every action to address high workload to reduce the risk that fundamental care is not sacrificed, job satisfaction is improved and nurses remain in the profession. The EOSS gives nurse leaders a reliable, practical, consistent, applied tool that will better enable associations to be observed between resource configuration, workload and critical impacts on nursing and patient care.We have adhered to the relevant EQUATOR guidelines using the STROBE reporting method.No Patient or Public Contribution. 23. Dignity in the Work Lives of Clinical Nurses

Abstract

This descriptive study examines how clinical nurses understand, experience, and sustain dignity in their work lives. Nursing has embedded dignity, particularly the dignity of others, as a core professional value. However, while the practice of nursing is deeply concerned with the achievement of patient dignity, dignity as a self-regarding professional right is not well articulated. Hodson's (2001) model for dignity at work provided a lens through which to examine the relevant nursing literature. It was revealed that the dignity of nurses as an intrinsic human and worker right has received little explicit attention, and that the significance of this is possibly not sufficiently well understood. A qualitative descriptive approach was used to further investigate the area of nurse dignity. Seven nurses were recruited to participate in facilitated workshops to explore the research question, 'How do clinical nurses understand, experience, and sustain dignity in their work lives'? The data were analysed using directed content analysis and presented as a descriptive summary. Dignity, for the participants, was strongly associated with the worth, value, and meaning that nurses attach to their profession, to the work that they do, and to themselves personally. This was shown to be central to their understanding, experience, and achievement of dignity in their work lives. Each encounter, each moment, was seen to be invested with the potential to maintain, affirm, erode or infringe personal dignity. The nurses perceived nursing to be a meaningful, worthwhile endeavour, but frequently struggled to extract a sense of dignity when working in environments that they perceived as not supporting their agenda of care. Being seen as a respected professional, enjoying daily positive interactions with colleagues and being successful in the act of nursing, had the strongest association with the ability to extract worth, value, and meaning from the work experience.
