Abstract

IMPORTANCE: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. OBJECTIVES: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. DESIGN, SETTING, AND PARTICIPANTS: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. MAIN OUTCOMES AND MEASURES: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. RESULTS: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. 2. High density carbon dispersion fuels program

Abstract

High density carbon dispersion fuels were studied. Promising results were obtained which indicate stable carbon loaded fuels with a minimum of 180,000 Btu per gallon can be made and successfully burned in prototype turbine combustors components. Tests were completed which provide insights to obtaining a better understanding of what types of carbon can be successfully formulated and combusted.

3. Gravitational effects on chemically reacting laminar boundary layer flows over a horizontal flat plate

Abstract

A theoretical study of the chemically reacting laminar boundary layer flow over a horizontal flat plate with gravitationally induced buoyant force is presented. A diffusion flame sheet model was used to describe the combustion process. The effects of gravity on the purely force convection flow can be characterized by a dimensionless coordinate quantity, which is involved in the generation of the governing equations. A numerical solution of the zero and first order governing equations subject to the appropriate physical boundary conditions was obtained. It is shown that the cross stream buoyancy induced body force acts effectively to produce a streamwise pressure gradient in the fluid adjacent to the plate surface. It is concluded that buoyancy plays an important role in boundary layer diffusion flames.

7. Radiative Augmented Combustion.

Abstract

Radiative Augmented Combustion has been identified as a potential technique for extending aircraft operating limits associated with combustion processes. This technique is based on the fact that radiation of selected wavelengths is capable of photodissociating stable molecules, combustion intermediates and other inhibiting species in the flame zone into reactive radicals. Subsequent reactions involving these reactive radicals lead to radiative ignition and combustion enhancement. This program emphasizes research on the interaction between radiation and combustion under flow conditions. Research is divided into two man subjects; ignition and enhancement. The objective of the experimental work is to demonstrate both ignition and enhancement in flow systems. For this purpose, two specially tailored apparatus were designed and fabricated; a plug flow combustor to conduct radiative ignition tests, and a 'pancake' burner to measure combustion enhancement in terms of increase in burning velocity. It is expected that experiments will demonstrate proof of concept by igniting combustible mixtures at conditions where thermal ignition is unreliable, and by increasing the flame speed. Since the role of photochemical reactions within the whole kinetic scheme is currently not completely understood, it is further expected that this research will identify the most effective photodissociative paths. This, in turn, will assist in determining the feasibility of the concept based on the availability of newly developed light sources and their compatibility with the harsh combustion environment., Continuation of work performed under Contract F49620-81-C-0028, Exxon Research and Engineering Co., Linden, NJ.

Abstract

Background: The ultra-Orthodox Jewish community has a unique lifestyle including minimal outdoor activity and intense, prolonged nearby work, beginning at a very young age. Their prevalence of myopia is extremely high. This paper provides a unique insight into the attitudes of this community towards myopia., Methods: Ultra-Orthodox Jewish parents of children who came to the pediatric ophthalmology clinic in one tertiary care and two community centers in ultra-Orthodox-oriented cities were given a questionnaire. Demographic information, along with myopia prevalence in the family, was gathered. In addition, their attitudes and common knowledge regarding myopia were investigated., Results: 161 questioners were collected, mostly completed by mothers (n = 110, 68%). The average number of children per family was 6 (range 1-16). In 148 families (92%) at least one of the parents has myopia. The average parent refraction was - 4.5 diopters (range - 0.5 to 15 diopters). Out of 935 children, 410 (44%) wore glasses. Twelve parents (7%) believe that myopia is a disease and 94 (58%) reported that they are concerned because their child wears glasses. Twenty-four (15%) believe that glasses are a sign of a high education level. Regarding treating myopia progression, 144 (89%) think that myopia progression should be treated, but only 36 (22%) are aware of the available treatments for it., Conclusion: This study examines an insular community with a very high incidence of myopia. In this community most parents think that myopia progression should be treated but most of them are unaware of the currently available treatments.

Abstract

Importance: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale., Objectives: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis., Design, Setting, and Participants: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017., Main Outcomes and Measures: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis., Results: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68])., Conclusions and Relevance: This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs.
