Abstract Restrictive lung disease is a broad term encompassing a number of conditions in which lung volumes are reduced. Dyspnea is a common clinical manifestation of restrictive lung disease and frequently becomes a prominent and disabling symptom that undermines patients' ability to function and engage in activities of daily living (especially in those with more advanced restriction). Effective management of this disabling symptom awaits a better understanding of its underlying physiology. In recent decades, our understanding of the mechanisms of dyspnea in restrictive lung disease has been improved by a small, but significant, body of research. One approach to the study of dyspnea is to identify the major qualitative dimensions of the symptom in an attempt to uncover different underlying neurophysiologic mechanisms. This article will review the existing literature on the intensity and qualitative dimensions of dyspnea during exercise in patients with restrictive lung disease. The main focus will be on interstitial lung disease (ILD), since it is the prototypical restrictive disease.

Physical examination of the thorax is key to the clinical diagnosis of respiratory diseases. Among other examination techniques, palpation evaluates the transmission of high-frequency vibrations produced by vocalizations (tactile fremitus), which helps the physicians to identify abnormalities within the respiratory system. We propose the use of an airborne ultrasound surface motion camera (AUSMC) to quantitatively map the vibrations induced by subject vocalization. This approach could make the examination of vocal fremitus quantifiable, reproducible, and archivable. Massive data collection of vocal fremitus could allow using artificial intelligence algorithms to isolate vibration patterns that could help disease identification. Until now, in contrast, the interpretation of vocal fremitus has been subject to the physician’s experience and remains subjective. In the present work, we demonstrate the capabilities of the AUSMC to measure vocal fremitus thoracic vibration maps on 77 healthy volunteers. We have observed a spatial dependence of vibration maps on vocalization frequency. We observed that the left lung generates fewer surface vibrations than the right one, which was expected according to their respective dimensions. We also discuss the implications of our findings.

The electrification of transport is expected to progressively replace significant shares of light duty mobility, especially in large cities. The European Alternative Fuel Infrastructure Regulation (AFIR) aims to drive the adoption of electric mobility by establishing specific targets for charging point deployment. Innovative charging concepts may complement and accelerate the uptake of this fundamental part of the urban mobility transition. In this paper, one such innovative concept is described and its potential impact is assessed. The core idea involves integrating charging points into existing city tramway infrastructures. Turin’s tramway network is taken as a representative case study. The proposed technical solution encompasses a charging hub powered by four isolated DC/DC converters of 50 kW, directly connected to the DC tramway distribution line. Three of these constitute the heart of a 150 kW charger, while the fourth acts as voltage regulator. This native DC installation greatly simplifies the architecture of the DC chargers. Using a conservative approach, it was estimated that a single recharging station could charge more than 60 vehicles daily. This highly scalable and replicable solution, with the potential for over 100 conversion substations across Italy, would enable the installation of numerous high-power chargers in urban settings. Furthermore, additional benefits could be realized through enhanced recovery of kinetic energy from trams, which is currently dissipated on-board.

BackgroundHyperventilation and inadequate cardiac output (CO) increase are the main causes of exercise limitation in pulmonary hypertension (PH). Intrapulmonary blood flow partitioning between ventilated and unventilated lung zones is unknown. Thoracic impedance cardiography and inert gas rebreathing have been both validated in PH patients for non-invasive measurement of CO and pulmonary blood flow (PBF), respectively. This study sought to evaluate CO behaviour in PH patients during exercise and its partitioning between ventilated and unventilated lung areas, in parallel with ventilation partitioning between ventilated and unventilated lung zones.MethodsEighteen PH patients (group 1 or 4) underwent a cardiopulmonary exercise test (CPET) with a three-step loaded workload protocol. The steps occurred at 0%, 20%, 40%, and 60% of peak workload reached during a preliminary maximum CPET. Ventilatory parameters, arterial blood gases, CO, PBF, and intrapulmonary shunt (calculated as the difference between CO and PBF) were obtained at each step, combining thoracic impedance cardiography and an inert gas rebreathing technique.ResultsDead space ventilation observed throughout the exercise was about 40% of total ventilation. A progressive increase of CO from 4.86 ± 1.24 L/min (rest) to 9.41 ± 2.63 L/min (last step), PBF from 3.81 ± 1.41 L/min to 7.21 ± 2.93 L/min, and intrapulmonary shunt from 1.05 ± 0.96 L/min to 2.21 ± 2.28 L/min was observed. Intrapulmonary shunt was approximately 20% of CO at each exercise step.ConclusionsAlthough the study population was small, the combined non-invasive CO measurement seems a promising tool for deepening our knowledge of lung exercise haemodynamics in PH patients. This technique could be applied in future studies to evaluate PH treatment influences on CO partitioning, since a secondary increase of intrapulmonary shunt is undesirable.

Companies in the manufacturing, service and transport infrastructure sectors will play a key role in the sustainable transition of the energy system implementing innovative technologies at full industrial scale. However, developing long-term investment plans is a challenging task, complicated by the volatility of energy markets and the uncertainties induced in the policy landscape by the recent energy crises. In this respect, mathematical models can support the transition of industrial systems, steering the planning process. In this work, we review and evaluate the suitability of available models to serve this purpose. The availability of modelling tools for local energy planning has already been assessed by several works. However, the tools which are most frequently employed at this scale are generally unable to characterise the evolution of the energy system in the long term, they have a focus on the operational aspects of the system, and are, in many cases, proprietary. By contrast, all the models examined in this review are open source, and the investigated set also features tools able to capture long-term dynamics. Our results provide evidence that several tools could offer valuable insights to the planning process. Nevertheless, a trade-off between the representation of long-term dynamics and the modelling of key aspects of the energy system of a company is, to date, inevitable. The best compromise is offered by a small group of multi-scale tools, able to conjugate a long-term perspective with a detailed modelling of the operational aspects of the system.

The objective of this document was to standardise published cardiopulmonary exercise testing (CPET) protocols for improved interpretation in clinical settings and multicentre research projects. This document: 1) summarises the protocols and procedures used in published studies focusing on incremental CPET in chronic lung conditions; 2) presents standard incremental protocols for CPET on a stationary cycle ergometer and a treadmill; and 3) provides patients' perspectives on CPET obtained through an online survey supported by the European Lung Foundation. We systematically reviewed published studies obtained from EMBASE, Medline, Scopus, Web of Science and the Cochrane Library from inception to January 2017. Of 7914 identified studies, 595 studies with 26 523 subjects were included. The literature supports a test protocol with a resting phase lasting at least 3 min, a 3-min unloaded phase, and an 8- to 12-min incremental phase with work rate increased linearly at least every minute, followed by a recovery phase of at least 2-3 min. Patients responding to the survey (n=295) perceived CPET as highly beneficial for their diagnostic assessment and informed the Task Force consensus. Future research should focus on the individualised estimation of optimal work rate increments across different lung diseases, and the collection of robust normative data.

Abstract Purpose To describe health-related quality of life (HRQoL) and dyspnea of COVID-19, 2 and 12 months after an intensive care unit (ICU) stay. Methods Patients discharged from the ICU between April and June 2020 and subsequently transferred to an inpatient rehabilitation facility were assessed 2 months and 12 months after ICU admission. HRQoL was assessed by the EuroQoL EQ-5D-3L (visual analog scale and time trade-off normalized to the French population algorithm) and dyspnea was assessed by the modified Medical Research Council (mMRC) dyspnea scale. Results We enrolled 94 patients. Median EQ-5D-3L time trade-off was 0.80 (interquartile range, 0.36–0.91) at 2 months and 0.91 (0.52–1.00) at 12 months (P = 0.12). This document reviews 1) the measurement properties of commonly used exercise tests in patients with chronic respiratory diseases and 2) published studies on their utilty and/or evaluation obtained from MEDLINE and Cochrane Library searches between 1990 and March 2015.Exercise tests are reliable and consistently responsive to rehabilitative and pharmacological interventions. Thresholds for clinically important changes in performance are available for several tests. In pulmonary arterial hypertension, the 6-min walk test (6MWT), peak oxygen uptake and ventilation/carbon dioxide output indices appear to be the variables most responsive to vasodilators. While bronchodilators do not always show clinically relevant effects in chronic obstructive pulmonary disease, high-intensity constant work-rate (endurance) tests (CWRET) are considerably more responsive than incremental exercise tests and 6MWTs. High-intensity CWRETs need to be standardised to reduce interindividual variability. Additional physiological information and responsiveness can be obtained from isotime measurements, particularly of inspiratory capacity and dyspnoea. Less evidence is available for the endurance shuttle walk test. Although the incremental shuttle walk test and 6MWT are reliable and less expensive than cardiopulmonary exercise testing, two repetitions are needed at baseline. All exercise tests are safe when recommended precautions are followed, with evidence suggesting that no test is safer than others.

Interstitial lung diseases (ILDs) are diverse parenchymal pulmonary disorders, primarily characterised by alveolar and interstitial inflammation and/or fibrosis, and sharing pathophysiological similarities. Thus, patients generally harbour common respiratory symptoms, lung function abnormalities and modified exercise adaptation. The most usual and disabling complaint is exertional dyspnoea, frequently responsible for premature exercise interruption. Cardiopulmonary exercise testing (CPET) is increasingly used for the clinical assessment of patients with ILD. This is because exercise performance or dyspnoea on exertion cannot reliably be predicted by resting pulmonary function tests. CPET, therefore, provides an accurate evaluation of functional capacity on an individual basis. CPET can unmask anomalies in the integrated functions of the respiratory, cardiovascular, metabolic, peripheral muscle and neurosensory systems in ILDs. CPET uniquely provides an evaluation of all above aspects and can help clinicians shape ILD patient management. Preliminary evidence suggests that CPET may also generate valuable prognostic information in ILDs and can be used to shed light on the presence of associated pulmonary hypertension. This review aims to provide comprehensive and updated evidence concerning the clinical utility of CPET in ILD patients, with particular focus on the physiological and clinical value of ventilatory efficiency (V˙E/V˙CO2).

Cystic fibrosis, due to the absence or abnormal function of the cystic fibrosis transmembrane conductance regulator, is the most common life-limiting autosomal recessive genetic disorder among the Caucasian population. The lungs are particularly affected due to thick and tenacious mucus causing parenchymal anomalies ranging from bronchiectasis, progressive airflow limitation, respiratory infections, lung destruction and ultimately respiratory failure. Despite the remarkable advances in treatment that have greatly improved survival, most patients experience progressive exercise curtailment, with the consequence that a growing number of patients with cystic fibrosis will be referred for exercise-based evaluations in the forthcoming years. Cardiopulmonary exercise testing, in particular, is a useful tool to assess the mechanisms of exercise intolerance in individual patients that may have treatment and prognostic implications. In this review, we will focus on ventilatory efficiency and its clinical and prognostic value in adults with cystic fibrosis.

Coronavirus disease 2019 (COVID-19) is a disease caused by a new coronavirus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-​2), and the predisposing and protecting factors have not been fully elucidated. COVID-19 primarily impacts the respiratory system, and can result in mild illness or serious disease leading to critical illness requiring admission to the intensive care unit due to respiratory failure. After hospital discharge, the more commonly described pulmonary function anomalies are alterations in diffusing capacity and the loss of lung volume. Reduction of inspiratory muscle contraction may also be underestimated. This article will focus on the pathophysiology of pulmonary function anomalies in COVID-19 survivors. We will discuss current advances and provide future directions and also present our perspective on this field.

The freight sector is expected to keep, or even increase, its fundamental role for the major modern economies, and therefore actions to limit the growing pressure on the environment are urgent. The use of electricity is a major option for the decarbonization of transports; in the heavy-duty segment, it can be implemented in different ways: besides full electric-battery powertrains, electricity can be used to supply catenary roads, or can be chemically stored in liquid or gaseous fuels (e-fuels). While the current EU legislation adopts a tailpipe Tank-To-Wheels approach, which results in zero emissions for all direct uses of electricity, a Well-To-Wheels (WTW) method would allow accounting for the potential benefits of using sustainable fuels such as e-fuels. In this article, we have performed a WTW-based comparison and modelling of the options for using electricity to supply heavy-duty vehicles: e-fuels, eLNG, eDiesel, and liquid Hydrogen. Results showed that the direct use of electricity can provide high Greenhouse Gas (GHG) savings, and also in the case of the e-fuels when low-carbon-intensity electricity is used for their production. While most studies exclusively focus on absolute GHG savings potential, considerations of the need for new infrastructures, and the technological maturity of some options, are fundamental to compare the different technologies. In this paper, an assessment of such technological and non-technological barriers has been conducted, in order to compare alternative pathways for the heavy-duty sector. Among the available options, the flexibility of using drop-in, energy-dense liquid fuels represents a clear and substantial immediate advantage for decarbonization. Additionally, the novel approach adopted in this paper allows us to quantify the potential benefits of using e-fuels as chemical storage able to accumulate electricity from the production peaks of variable renewable energies, which would otherwise be wasted due to grid limitations.

Cardiopulmonary exercise testing (CPET) is a frequently used tool in the differential diagnosis of dyspnoea. Ventilatory inefficiency, defined as high minute ventilation (V′E) relative to carbon dioxide output (V′CO2), is a hallmark characteristic of pulmonary vascular diseases, which contributes to exercise intolerance and disability in these patients. The mechanisms of ventilatory inefficiency are multiple and include high physiologic dead space, abnormal chemosensitivity and an altered carbon dioxide (CO2) set-point. A normal V′E/V′CO2 makes a pulmonary vascular disease such as pulmonary arterial hypertension (PAH) or chronic thromboembolic pulmonary hypertension (CTEPH) unlikely. The finding of high V′E/V′CO2 without an alternative explanation should prompt further diagnostic testing to exclude PAH or CTEPH, particularly in patients with risk factors, such as prior venous thromboembolism, systemic sclerosis or a family history of PAH. In patients with established PAH or CTEPH, the V′E/V′CO2 may improve with interventions and is a prognostic marker. However, further studies are needed to clarify the added value of assessing ventilatory inefficiency in the longitudinal follow-up of patients.

COVID-19 is a disease caused by a new coronavirus SARS-CoV-2, primarily impacting the respiratory system. COVID-19 can result in mild illness or serious disease leading to critical illness and requires admission to ICU due to respiratory failure. There is intense discussion around potential factors predisposing to and protecting from COVID-19. The immune response and the abnormal respiratory function with a focus on respiratory function testing in COVID-19 patients will be at the center of this Perspective article of the Frontiers in Physiology Series on “The Tribute of Physiology for the Understanding of COVID-19 Disease.” We will discuss current advances and provide future directions and present also our perspective in this field.

Cardiopulmonary exercise testing (CPET) has long been used as diagnostic tool for cardiac diseases. During recent years CPET has been proven to be additionally useful for 1) distinguishing between normal and abnormal responses to exercise; 2) determining peak oxygen uptake and level of disability; 3) identifying factors contributing to dyspnoea and exercise limitation; 4) differentiating between ventilatory (respiratory mechanics and pulmonary gas exchange), cardiovascular, metabolic and peripheral muscle causes of exercise intolerance; 5) identifying anomalies of ventilatory (respiratory mechanics and pulmonary gas exchange), cardiovascular and metabolic systems, as well as peripheral muscle and psychological disorders; 6) screening for coexistent ischaemic heart disease, peripheral vascular disease and arterial hypoxaemia; 7) assisting in planning individualised exercise training; 8) generating prognostic information; and 9) objectively evaluating the impact of therapeutic interventions. As such, CPET is an essential part of patients' clinical assessment. This article belongs to the special series on the “Ventilatory efficiency and its clinical prognostic value in cardiorespiratory disorders”, addressed to clinicians, physiologists and researchers, and aims at encouraging them to get acquainted with CPET in order to help and orient the clinical decision concerning individual patients.
