For a century, keratins extracted from different sources are being used for medical, cosmetic and textile applications. The excellent bioactivity and physiochemical properties of these protein extracts have recently led to the popularity of a keratin as biomaterial. Like other naturally derived biomaterials, keratins have the potential to form a defined, three-dimensional microstructure that supports cell infiltration, proliferation, and cell-guided tissue formation. In addition, the natural abundance, intrinsic biocompatibility, and mechanical durability of keratins have shown promise in the field of biomaterials in diverse biomedical applications. This mini review summarizes the biological properties, explores in brief the extraction methods and advances of keratin as a biomaterial in various biomedical and dental applications.

Introduction: Titanium (Ti) is widely accepted as a biomaterial for orthopaedic and dental implants, primarily due to its capacity to integrate directly into the bone and its superior corrosion resistance. It has been suggested that titanium–zirconium alloy (TiZr), with 13–17% of zirconium, has better mechanical properties than pure Ti, but there are very few published studies assessing the suitability of TiZr for high-load- bearing implants. This study aimed to compare the mechanical properties and microstructures of TiZr and commercially pure titanium (Ti). Methodology: Pure Ti and TiZr alloy discs were prepared and subjected to characterisation by nanoindentation, electron dispersive spectroscopy (EDS), X-ray diffraction (XRD), and electron backscatter diffraction (EBSD). Results: The TiZr alloy was found to have significantly lower elastic modulus value (p < 0.0001) and greater hardness than Ti (p < 0.05). The EDS results confirmed the presence of Zr (13–17%) in the TiZr alloy, with XRD and EBSD images showing microstructure with the alpha phase similar to commercially available Ti. Conclusion: The lower elastic modulus, higher hardness, presence of alpha phase, and the finer grain size of the TiZr alloy make it more suitable for high-load-bearing implants compared to commercially available Ti and is likely to encourage a positive biological response.

The increasing lifetime of the population on a worldwide scale over the last decades has led to a significant growth in the use of titanium oral implants to replace missing teeth. Longitudinal studies have reported high survival rates of the implants in function, ranging from 90% to 95% over a period up to 20 years.[1],[2] This also includes the increase in revision rate of implant prosthesis after the failure due to various factors. The most prominent and insidious complication around dental implants, emerging at a later stage is peri-implantitis.[3] A recent meta-analysis revealed a weighted mean prevalence of 22% for peri-implantitis.[4] Peri-implantitis is one of the main causes of failure in implant dentistry that affects 14%-30% of the implants.[4],[5] The 2017 World Workshop on the Classification of Periodontal and Peri-Implant Diseases and Conditions stated that peri-implantitis is an inflammatory process from a microbial origin.[6] By definition, it is a complex multifactorial infectious disease and the inflammatory lesion in the surrounding tissues develops as a result of accumulation of bacteria on implant surfaces resulting in a biofilm.[7] Peri-mucositis and Peri-implantitis analogous to gingivitis and periodontitis respectively. They have similar pathogical events but differ in extent and rapidity of tissue destruction. Experimental peri-implantitis induced by sub marginal placement of ligatures using animal models such as dogs and monkeys showed inflammatory lesions, bone loss and implant failures.[8],[9] Though the microbial taxa identified in peri-implantitis is predominantly similar to periodontitis, several studies have indicated the presence of additional strains like Staphylococcus aureus,Staphylococcus epidermidis and Candida spp.[3],[10],[11]Microorganisms colonize implant surfaces and contribute to the biofilm associated infections in a manner similar to teeth.[3],[12]

Reconstituted keratin-hydroxyapatite (K-HA) composites have shown potential as nonload-bearing bone graft substitute material. This in vivo study investigated the bone regeneration response of keratin plus 40% HA composite materials in comparison to collagen counterparts and an unfilled defect site. The implantation site was a noncritical size defect created in the long bones (tibia) of sheep, with observations made at 1, 2, 4, 6, 8, and 12 weeks postimplantation. Porous K-HA materials displayed an excellent biocompatibility similar to collagen counterparts; however, the rate of bone regeneration at K-HA implantation sites was markedly slower than that of the collagen or unfilled defect sites. While collagen materials were undetectable by 4 weeks implantation, K-HA composite remnants were present at 12 weeks. However, there is evidence that K-HA implants participated in the natural remodelling process of bone, with bone regeneration occurring via a creeping substitution mechanism. Observations imply that the rate of bone ingrowth into the K-HA defect site was matched with the rate of K-HA resorption. These results suggest that K-HA materials may offer significant benefits as nonload-bearing bone graft substitutes where it is desirable that the degradation of the scaffolding material be well matched with the rate of bone regeneration. © 2016 Wiley Periodicals, Inc. Millions of people suffer from dental caries, which affects the quality of life of patients of all ages. Current therapy is to remove the diseased tissues and to replace them with inert, synthetic materials that, however, are incapable of replacing the biological function of the lost tissue. Similar to other connective tissue, pulp tissue has the potential to heal; however, to achieve successful pulp–dentine regeneration, a scaffold that mimics the natural extracellular microenvironment is vital. A myriad of polymers have been investigated for this purpose. This chapter will provide an insight into different biomaterials used for fabrication of scaffolds for pulp–dentine regeneration.

The underlying causes of maxillary bone loss during lactation remain poorly understood. We evaluated the impact of lactation on physiological and mechanically-induced alveolar bone remodeling. Nulliparous non-lactating (N-LAC) and 21-day lactating (LAC) mice underwent mechanically-induced bone remodeling by orthodontic tooth movement (OTM). Micro-computed tomography (microCT) was performed in the maxilla, femur and vertebra. Tartrate-resistant-acid phosphatase (TRAP) and Masson's trichrome labelling was performed in the maxillary bone and gene expression was determined in the periodontal ligament. The effect of prolactin on osteoclast (OCL) and osteoblast (OBL) differentiation was also investigated in N-LAC and LAC mice. Lactation increased alveolar bone loss in the maxilla, femur and vertebra, while OTM was enhanced. The number of OCL and OBL was higher in the maxilla of LAC mice. OTM increased OCL in both groups; while OBL was increased only in N-LAC but not in LAC mice, in which cell numbers were already elevated. The alveolar bone loss during lactation was associated with increased expression of receptor activator of nuclear factor-KappaB (RANK), RANK ligand (RANKL), and osteoprotegerin (OPG) in the maxilla. OTM induced the same responses in N-LAC mice, whereas it had no further effect in LAC mice. Lactation enhanced differentiation of OCL and OBL from bone marrow cells, and prolactin recapitulated OCL differentiation in N-LAC mice. Thus, lactation increases physiological maxillary bone remodeling and OTM, and both require activation of RANK/RANKL/OPG system. These findings expand our knowledge of lactation-induced osteopenia and have possible impact on clinical practice regarding orthodontic treatments and dental implants in lactating women.

Background Keratin has shown promising outcomes as a biomaterial due to its inherent bioactivity, biocompatibility and regenerative effects. The effect of keratin on repair and regeneration of dental tissues has never been studied before. Current therapies to treat pulp tissues involve its replacement with inert, synthetic materials that do not have a proper biological function, leading to failure and tooth loss. This study aimed to develop a biocompatible keratin hydrogel (KH) suitable for pulp therapies. Methods Keratins extracted from sheep wool were isolated, quantified and reconstituted to form KH. Different concentrations of keratin gel suitable for dental application were characterized by rheological analysis. The optimized gel based on flow characteristics was studied further for microstructure including porosity, percentage swelling ratio and contact angle measurements, using analytical tools such as scanning electron microscopy (SEM), micro-computed tomography and goniometer. To assess both biocompatibility and pulpal response, KH was implanted into rat upper molar teeth following partial pulpotomy. After 28 days, the tissue sections were analyzed by histological and immunohistochemical methods to identify dentin matrix protein 1 (DMP-1) formation and compared with control (Ca(OH)2-treated) teeth. Results The results of the study demonstrated a viscous and injectable, porous, dimensionally stable, hydrophilic and biocompatible gel that allowed pulp healing to occur by a reparative response, with widespread DMP-1 expression. Conclusions The findings of this study indicate that keratins can be developed as a biomaterial source for alternate biological treatment options for pulp therapies.

This review summarizes the biological response of dentin-pulp complexes to a variety of stimuli and responses to current treatment therapies and reviews the role of tissue engineering and its application in regenerative endodontics. An electronic search was undertaken based on keywords using Medline/PubMed, Embase, Web of Science and Ovid database resources up to March 2012 to identify appropriate articles, supplemented by a manual search using reference lists from relevant articles. Inclusion criteria were mainly based on different combinations of keywords and restricted to articles published in English language only. Biological approaches based on tissue engineering principles were found to offer the possibility of restoring natural tooth vitality, with distinct evidence that regeneration of lost dental tissues is possible. Studies to formulate an ideal restorative material with regenerative properties, however, are still under way. Further research with supporting clinical studies is required to identify the most effective and safe treatment therapy.

The choice of implant surface has a significant influence on osseointegration. Modification of TiZr surface by anodization is reported to have the potential to modulate the osteoblast cell behaviour favouring more rapid bone formation. The aim of this study is to investigate the effect of anodizing the surface of TiZr discs with respect to osseointegration after four weeks implantation in sheep femurs. Titanium (Ti) and TiZr discs were anodized in an electrolyte containing dl-α-glycerophosphate and calcium acetate at 300 V. The surface characteristics were analyzed by scanning electron microscopy, electron dispersive spectroscopy, atomic force microscopy and goniometry. Forty implant discs with thickness of 1.5 and 10 mm diameter (10 of each-titanium, titanium–zirconium, anodized titanium and anodized titanium–zirconium) were placed in the femoral condyles of 10 sheep. Histomorphometric and histologic analysis were performed 4 weeks after implantation. The anodized implants displayed hydrophilic, porous, nano-to-micrometer scale roughened surfaces. Energy dispersive spectroscopy analysis revealed calcium and phosphorous incorporation into the surface of both titanium and titanium–zirconium after anodization. Histologically there was new bone apposition on all implanted discs, slightly more pronounced on anodised discs. The percentage bone-to-implant contact measurements of anodized implants were higher than machined/unmodified implants but there was no significant difference between the two groups with anodized surfaces (P > 0.05, n = 10). The present histomorphometric and histological findings confirm that surface modification of titanium–zirconium by anodization is similar to anodised titanium enhances early osseointegration compared to machined implant surfaces.

Titanium (Ti) and its alloys have been popularly used as implant biomaterial for decades. Recently, titanium-zirconium (TiZr) alloy has been developed as an alternative implant material with improved strength in load bearing areas. Surface modification is one of the key factors to alter the surface properties to hasten osseointegration. Spark anodic oxidation (anodization) is one such method that is reported to enhance the bone formation around implants. This study aims to anodize TiZr and study its surface characteristics and cytocompatibility by cell culture experiments using osteoblast-like cells. Titanium (Ti) and TiZr discs were anodized in an electrolyte containing DL-α-glycerophosphate and calcium acetate (CA) at 300 V. The surface characteristics were analyzed by scanning electron microscopy, electron dispersive spectroscopy, X-ray diffraction (XRD), atomic force microscopy and goniometry. Using osteoblast-like cells viability, proliferation, differentiation and mineralization was assessed. The anodized surfaces demonstrated increased oxygen, entrapped calcium and phosphorous from the electrolyte used. XRD analysis confirmed the presence of anatase in the oxide layer. Average roughness increased and there was a significant decrease in contact angle (P < 0.01) following anodization. The anodized TiZr (aTiZr) surfaces were more nano-porous compared to anodized Ti (aTi). No significant difference was found in the viability of cells, but after 24 h the total number of cells was significantly higher (P < 0.01). Proliferation, alkaline phosphatase activity and calcium deposits were significantly higher on anodized surfaces compared to machined surfaces (P < 0.05, ANOVA). Anodization of TiZr resulted in a more nanoporous and hydrophilic surface than aTi, and osteoblast biocompatibility appeared comparable to aTi.
