Abstract

Abstract Background Retinal diseases, including wet or dry age-related macular degeneration, diabetic macular edema, and diabetic retinopathy (DR), are underdiagnosed and undertreated in the United States. Clinical trials support the effectiveness of anti-vascular endothelial growth factor (anti-VEGF) therapies for several retinal conditions, but real-world data suggest underuse by clinicians, resulting in patients experiencing poorer visual outcomes over time. Continuing education (CE) has demonstrated effectiveness at changing practice behaviors, but more research is needed to understand whether CE can help address diagnostic and treatment gaps. Methods This test and control matched pair analysis examined pre-/post-test knowledge of retinal diseases and guideline-based screening and intervention among 10,786 healthcare practitioners (i.e., retina specialists, ophthalmologists, optometrists, primary care providers, diabetes educators, pharmacists/managed care specialists, and other healthcare providers, such as registered nurses, nurse practitioners, and physician assistants) who participated in a modular, interactive CE initiative. An additional medical claims analysis provided data on practice change, evaluating use of VEGF-A inhibitors among retina specialist and ophthalmologist learners (n = 7,827) pre-/post-education, compared to a matched control group of non-learners. Outcomes were pre-/post-test change in knowledge/competence and clinical change in application of anti-VEGF therapy, as identified by the medical claims analysis. Results Learners significantly improved knowledge/competence scores on early identification and treatment, identifying patients who could benefit from anti-VEGF agents, using guideline-recommended care, recognizing the importance of screening and referral, and recognizing the importance of early detection and care for DR (all P-values = 0.003 to 0.004). Abstract

Safe over the counter (OTC) medication use by older adults (OAs, aged 65+) is difficult to achieve because of age-related physiologic complexities, and millions of OAs in the US who are at risk of experiencing a major adverse drug event linked to an OTC medication. Our interdisciplinary team studies how we can improve OTC safety in OAs by developing human factors-based, community pharmacy interventions. Testing the effectiveness of our interventions on improving OTC safety necessitated the development of a framework that captures the ways OTC medication misuse can occur and designing and implementing a tool that can be used to evaluate for potential misuse, which can be used by researchers and healthcare professionals to understand misuse in OAs. This paper shares the results of our efforts and discusses the implications of our work for other HF practitioners that may be interested applying our approach to their research.

4. Detrital geochronology and lithologic signatures of Weddell Sea Embayment ice streams, Antarctica—Implications for subglacial geology and ice sheet history

Abstract

Tills from moraines adjacent to major ice streams of the Weddell Sea Embayment contain distinct detrital zircon (n = 5304) and K-bearing mineral age populations (n = 323) that, when combined with pebble composition data, can be used to better understand Antarctica's subglacial geology and ice sheet history. Till representing the Institute, Foundation, Academy, Recovery and Slessor Ice Streams each have distinct detrital zircon age populations. Detrital Ar-Ar ages are mostly younger than zircon ages, and distinctive populations include 270–300 Ma (Institute), 170–190 Ma (Foundation), and 1200–1400 Ma (Recovery), which are not easily explained by known outcrops. Pebble fractions of the Foundation and Academy tills are most diverse with up to >40% exotic erratics. The southern side of the Recovery Glacier has fossiliferous limestone erratics. Mixing models created using a nonlinear squares curve-fitting approach were developed to evaluate contributors of zircons to Foundation Ice Stream till. These model results and pebble lithology data both indicate that unexposed (subglacial) bedrock is mixed with exposed rocks to produce the observed till. Notably, the model required limited local Patuxent Formation input to the Foundation till's zircon population. Our data suggest that sandstones underlie the Foundation Ice Stream and Recovery Glacier troughs, which has a bearing on basal ice flow conditions and results in geological controls on ice stream location. This geo- and thermo-chronological characterization of the ice streams will enable ice-rafted debris in Weddell Sea marine sediments to be traced back to its sources and interpreted in terms of ice stream dynamics.

Abstract

ObjectivesThis pilot study examines effectiveness of an innovative pharmacy design change on over-the-counter (OTC) medication misuse in older adults (ages ≥65). Few interventions have attempted to decrease older adult OTC misuse, and none have addressed system barriers. A structural redesign of the pharmacy (the Senior Section™) was conceptualized to increase awareness of higher-risk OTC medications. The Senior Section contains a curated selection of OTC medications (for pain, cough/cold, allergy, sleep) and is close to the prescription department to facilitate pharmacy staff/patient engagement to reduce misuse.MethodsA pre-/post-implementation design was used to recruit 87 older adults from three pharmacies. Using a hypothetical scenario, participants selected an OTC medication, which was compared to their medication list and health conditions, and their reported use was compared against the product labeling. Four misuse outcomes were determined: (1) Drug/Drug, (2) Drug/Disease, (3) Drug/Age, and (4) Drug/Label with five sub-types. Patient characteristics were collected and compiled into a propensity-score matching logistic regression model to estimate their effects on the Senior Section’s association with misuse outcomes at pre-/post-implementation.ResultsPatient characteristic were uniform between pre-/post-implementation and, once entered into a propensity-score matching model, Drug/Disease Misuse significantly lessened over time (z=-2.09, p=0.037). The Senior Section reduced Drug/Drug Misuse, but not significantly. Drug/Label Misuse varied according to the sub-type, with reduced Daily-Dosage (z=-2.42, p=0.016) and Single-Dosage misuse (z=-5.82, p=0.001); however, Timing/Frequency misuse increased (z=2.16, p=0.031).ConclusionsThese nascent outcomes support a well-conceived pharmacy-based OTC aisles redesign as valuable for reducing older adult OTC medication misuse. The Senior Section, when broadly implemented, would create new permanent structures and processes to assist older adults in accessing risk information for confidently selecting safer OTC medications.

Abstract

Project ECHO is a collaborative model of medical education that engages clinicians in a continuous learning system via real-time, interactive videoconferencing. During TeleECHO sessions, expert specialist teams at an academic ‘hub’ are linked with care teams in local communities throughout the world. Each session is case-based, with learners and faculty presenting their patient experiences in a HIPAA compliant manner, promoting collaboration and competence building. As an official ECHO hub, MLG recently completed a large-scale, TeleECHO series on treating to guidelines and managing cardiovascular and renal risk in patients with T2DM, consisting of 47 sessions that reached 1976 healthcare professionals. With the ongoing COVID-19 pandemic, these sessions provided a unique way to facilitate a case-based dialogue among community-based practitioners and a national expert, sharing best practices that to apply to the care of individual patients. MLG recorded each session and used the most informative cases to create an enduring activity that is available for one year online. To further support the translation of education into practice and encourage HCP-patient dialogue, MLG also provided learners access to an online Personalized Poster Portal. Learners could choose a variety of images and design a poster that was shipped to their practice setting. There have been 613 poster orders thus far. The outcomes that MLG has collected regarding this unique program and its point-of-care tools indicate how CME can lead to not only improved knowledge but can also translate to practice behavior. While learners demonstrated significant gains across learning objectives, baseline knowledge of the latest data on the use of GLP-1 receptor agonists and SGLT2inhibitors to reduce cardiovascular events demonstrated a need for such education. Abstract

Purpose To investigate the natural history and genetic associations of drusenoid pigment epithelial detachment (DPED) associated with age-related macular degeneration (AMD). Design Retrospective analysis of a prospective cohort study. Participants Of the 4203 Age-Related Eye Disease Study 2 (AREDS2) participants, 391 eyes (325 participants) had DPED without late AMD at the time of DPED detection. Genetic analyses included 120 white AREDS2 participants and 145 Age-Related Eye Disease Study (AREDS) participants with DPED. Methods Baseline and annual stereoscopic fundus photographs were graded centrally to detect DPED, a well-defined yellow elevated mound of confluent drusen ≥433 μm in diameter, and to evaluate progression rates to late AMD: geographic atrophy (GA) and neovascular (NV)-AMD. Five single nucleotide polymorphisms (CFH [rs10611670], C3 [rs2230199], CFI [rs10033900], C2/CFB [rs114254831], ARMS2 [rs10490924]) and genetic risk score (GRS) group were investigated for association with DPED development. Kaplan–Meier analyses and multivariable proportional hazard regressions were performed. Main Outcome Measures Progression rates to late AMD and decrease of ≥3 lines in visual acuity (VA) from the time of DPED detection; association of rate of DPED development with genotype. Results Mean (standard deviation [SD]) follow-up time from DPED detection was 4.7 (0.9) years. DPED was associated with increased risk of progression to late AMD (hazard ratio [HR], 2.36; 95% confidence interval [CI], 1.98–2.82; P Conclusions This study replicates the results of previous natural history studies of eyes with DPED including the high rates of progression to late AMD and vision loss (regardless of progression to late AMD). The genetic associations are consistent with genes associated with AMD progression.

Abstract

Purpose To analyze the prevalence, incidence, and clinical characteristics of eyes with geographic atrophy (GA) in age-related macular degeneration (AMD), including clinical and genetic factors affecting enlargement. Design Prospective cohort study within a controlled clinical trial. Participants Age-Related Eye Disease Study 2 (AREDS2) participants, aged 50–85 years. Methods Baseline and annual stereoscopic color fundus photographs were evaluated for GA presence and area. Analyses included GA prevalence and incidence rates, Kaplan-Meier rates, mixed-model regression, and multivariable analysis of the square root of GA, area adjusted for covariates, including clinical/imaging characteristics and genotype. Main Outcome Measures (1) Presence or development of GA; (2) change in the square root of GA area over time. Results At baseline, 517 eyes (6.2%) of 411 participants (9.8%) had pre-existing GA (without neovascular AMD), with the following characteristics: 33% central, 67% noncentral; and the following configurations: 36% small, 26% solid/unifocal, 24% multifocal, 9% horseshoe/ring, and 6% indeterminate. Of the remaining 6530 eyes at risk, 1099 eyes (17.3%) of 883 participants developed incident GA without prior neovascular disease during mean follow-up of 4.4 years. The Kaplan-Meier rate of incident GA was 19% of eyes at 5 years. In eyes with incident GA, 4-year risk of subsequent neovascular AMD was 29%. In eyes with incident noncentral GA, 4-year risk of central involvement was 57%. GA enlargement rate (following square root transformation) was similar in eyes with pre-existing GA (0.29 mm/year; 95% confidence interval 0.27–0.30) and incident GA (0.28 mm/year; 0.27–0.30). In the combined group, GA enlargement was significantly faster with noncentrality, multifocality, intermediate baseline size, and bilateral GA (P Conclusions Analyses of AREDS2 data on natural history of GA provide representative data on GA evolution and enlargement. GA enlargement, which was influenced by lesion features, was relentless, resulting in rapid central vision loss. The genetic variants associated with faster enlargement were partially distinct from those associated with risk of incident GA. These findings are relevant to further investigations of GA pathogenesis and clinical trial planning.

Abstract

Women account for over 50% of the workforce in the United States with many working women being of childbearing age. The United States does not provide long paid parental leave, thus mothers who choose to breastfeed are confronted with the reality of combining breastfeeding and returning to work. Return to work is reported to negatively impact breastfeeding exclusivity and duration. While the existing federal law protects some women, not all women have legal support to breastfeed or express milk at work. Exemptions to the federal law include limitations related to the employee's status, classification of employer, total number of employees and the employer's annual revenue. This study aimed to examine existing city-level legislation protecting the rights of women to breastfeed or express milk at their place of employment during the postpartum period.Prospective descriptive study with survey. The national sample includes the three largest cities of each state and the capital city of the United States (Washington, DC) for a total of 151 cities. The data were collected in a tiered approach with three phases: (1) assessment of city website, (2) e-mail to city mayor's office, and (3) telephone follow-up with the city's office.Only 2/151 (1.3%) of cities had specific legislation outlining the protections for all breastfeeding women in the workplace.This research demonstrates a clear need for political action to increase the number of women who have workplace regulations to protect breastfeeding.

Abstract

In 2018, Med Learning Group (MLG) launched the THRIVE initiative, a large-scale, multi-faceted continuing medical education series in glycemic control that incorporates a variety of live and online educational activities/tools for enriched comprehension and engagement. This programming included partners such as ADA chapters, AAFP, Pack Health, Health Resources and Services Administration, Public Health Foundation, and the Hypoglycemia Support Foundation to further enhance content and offer an opportunity for long lasting impact on diabetes care. The series launched 3 national summit meetings with expert and local faculty and 25 grand rounds with chart audits in community-based practices/hospitals throughout the country showing improvements in both knowledge and competence. An online enduring program was created to further extend the impact. THRIVE includes unique and innovative tools to enhance the learning experience, facilitate continuous learning and translation into practice, and encourage patient education and dialogue. These included: App/Mobile website: Features quickly downloadable materials and links to published articles and patient and provider tool kits. iPads: Facilitate Q and A during live sessions and allow participants to engage in the lecture. ADA materials: Provided local patient support information to participants. Chart Audit: Occurred in institutions to identify gaps in care and highlighted change. Personalized Posters: Choose a variety of images to design unique office posters. Health advisors: Pack Health provided patients personalized health coaching. The combination of advanced technology, diverse partnerships, and variety of educational channels ensures a robust delivery and distribution network. CME can advance the conversion of knowledge into practice while also having a visible impact on HCP-patient dialogue. Abstract

Older adults living in rural areas have limited access to geriatrics interprofessional team care. In the Veteran healthcare system, geriatric teams such as geriatricians, nursing professionals, social workers, pharmacists and psychologists, located in urban areas link up with rural clinics to provide geriatric consultation remotely through clinical video telehealth and other means in the project GRECC Connect. Since its inception in 2014, the service has now grown to 16 geriatric teams offering consultation to over 100 clinic sites serving older rural Veterans. GRECC Connect delivered over 2,000 consultations in 2019, meeting complex care needs by identifying and linking geriatric services and management to patients with geriatric syndromes. The network of established geriatric teams, local champions and a shared Electronic Health Record facilitated the spread, while ongoing effort to build and maintain relationships between consultants and local rural provider teams and other community based services are important for ongoing success.

Abstract

Background In 2020, Med Learning Group (MLG) launched an interactive, multi-faceted educational initiative focusing on COVID-19. The innovative education & tools developed for FRONTLINE are publicly available for the entire healthcare community to use. Learners range from a variety of specialties, including infectious disease specialists, pulmonary medicine specialists, emergency room practitioners, advanced practitioners, nurses, & other healthcare professionals to help support them in their effort to optimize care of patients with COVID-19. MLG partnered with Health Resources & Services Administration, Project ECHO, Public Health Foundation & community, VA & academic centers to create a collaborative network with shared goals for education. Methods This initiative seeks to reach over 25,000 learners with innovative educational programs & tools to enhance the learning experience, facilitate continuous learning & support the translation of education into practice, & encourage HCP-patient dialogue: • COVID Community of Care Website/Application • COVID Frontline Update Podcast Series • Virtual/Live ECHO Series with 3D animations of pathophysiology • Enduring ECHO Module with Case Discussions • Quality Improvement Personalized Posters Results By September 2020, we will have the results from pre/posttests, intra-activity Q&A, evaluations, & 60- to 90-day follow-on assessments. We will evaluate learners’ changes in knowledge & competence, & reported practice changes. In addition, MLG will have feedback collected via surveys & interviews on the various point-of-care tools. Based off previous MLG educational initiatives, it is expected that learners will find value in the various tools available in this programming. Conclusion Advanced tools like virtual live learning platforms, mobile websites/apps, 3D animations & podcasts will prove to be an asset to the continuing education of HCPs treating patients with COVID-19. The outcomes are expected to demonstrate the extent to which HCPs have enhanced their ability to identify clinical predictors of disease severity of COVID-19 & apply current treatment guidelines, clinical trial data, & patient-specific factors to the management of patients with COVID-19. Abstract

Introduction The Madison VA Geriatric Research, Education, and Clinical Center (GRECC) provides training to multidisciplinary health professionals at a variety of learner levels. The Wisconsin Geriatric Workforce Enhancement Program (GWEP) is a consortium of five collaborating organizations and many community partnerships. Since 2013, the GRECC and GWEP have collaborated on the Geriatrics Equipped and Ready for Unsupervised Practice (GEAR-UP) project, with a primary goal of designing an adaptable, competency-based learning experience that develops “geriatrics-equipped” inter-professional team members. Within the GEAR-UP project, an interdisciplinary team (IDT) of geriatric educators at the Madison VA implemented a simulation experience to be completed by learners during geriatric training. This allows educators to assess learner competency in 3 core areas of geriatrics: falls, late life depression, and cognitive impairment. Moreover, an online simulation for late life depression, designed to complement the simulated patient assessment has been developed and will launch in the spring of 2019. This poster provides an update on the GEAR-UP Project, and will present representative competencies and curricula that have been adopted to teach geriatric psychiatry to internal medicine interns and other learners, along with updated data on performance of learners on the late life depression and cognitive impairment simulated patient assessments. The GEAR-UP project highlights the need for broad health professions education on late life depression and cognitive impairment, and provides innovative ideas on ways to incorporate such education into trainee curricula. Methods In 2014, the IDT team developed curricula for a one-month geriatric rotation for internal medicine interns which incorporated standardized in-person patient simulations (objective clinical skills examinations, or OSCEs) in the three content areas. These simulations were tested in collaboration with the University of Wisconsin Health Sciences Learning Center (HSLC) Clinical Training and Assessment Center (CTAC) site in March 2014, and have run monthly to date. The simulation experiences were designed for both instruction and evaluation, and are strategically placed near the mid-point of the learners’ geriatric rotations to heighten their learning experience in the core geriatric principles. This provides a formative assessment in these topics to highlight areas for improvement for the remainder of the geriatric rotation. Assessment scores are maintained in a current database that can be used for further assessment of learner performance against their peers. Here we present i) specific competencies and sample curricula developed to teach geriatric psychiatry; ii) data from academic years 2014 to present for the late life depression and cognitive impairment simulations (as the assessment points varied from year to year in an effort to optimize the experience for learners, only common key, evidence-based evaluation points were examined in the comparison across years); and iii) portions of the online LLD simulation. Results There were a total of 123 learners from academic years 2014 through present. The distribution of learner disciplines across all academic years is shown in Figure 1. The largest group of learners were general medicine interns (including transitional year interns). Geriatric scholars represent VA providers already in practice who participate in a one-week immersion experience with the GRECC, and come from a variety of disciplines including social workers, nurse practitioners, pharmacists in clinical practice, and psychologists. PharmD residents are pharmacists in their first year of post-graduate clinical training specific to the nationwide VA system. Medical students represent MS4 students completing geriatric medicine sub-internships. The fellows who completed the simulations are largely geriatric medicine fellows. Learner performance broken down by learner discipline is shown in Figure 2, with geriatric medicine fellows demonstrating the highest performance on both simulations, and medical students the lowest. Currently, we are working on analyzing the data for individual performance items. We anticipate having this analysis done by time of poster presentation if selected. Conclusions There were observed differences in performance among the disciplines and levels of training. Performance on the late life depression assessment was overall higher than on the cognitive impairment assessment among most learner groups, with the exception of the medical student group. The comparative performance of the learners appear congruent with their level of training, with the exception of geriatric scholars. One explanation for the comparatively low performance of the geriatric scholars is that through their years of practice they have likely grown unfamiliar with the OSCE format. Possible broad explanations for variations in learner performance include discipline specific biases, differences in training programs, time out of core training and differences in content exposure related to time allocated for geriatric training in different disciplines. Several images are included from the online computer-based simulation for late life depression, to illustrate the interface. Rather than a costly OSCE with standardized patients, the computer simulation is customizable to learner discipline and level of training, and includes options for non-prescribers, basic prescribers, and advanced prescribers. Although the initial investment is considerable, this will be a tool that can be utilized broadly across disciplines, training sites, and even institutions. This research was funded by William S. Abstract

Background Older adults are the largest consumers of over the counter (OTC) medications. Of the older adults who are at risk of a major adverse drug event, more than 50% of these events involve an OTC medication. Objective To explore how older adults select and hypothetically use OTC medications and if the selected medications would be considered safe for use. Methods Walking interviews were conducted with 20 community-dwelling older adults in a community pharmacy. Each participant selected an OTC medication for a hypothetical pain and sleep scenario. Data were analyzed for four types of misuse: drug–drug interaction, drug–disease interaction, drug–age interaction, and excess usage. Results At least one instance of potential misuse was found in 95% of participants. For sleep medications, drug–drug interactions and drug–age interactions were more common, affecting 50% and 65% of participants respectively. The most common type of misuse noted in the pain products selected was that of drug–drug interaction, with a total of 39 occurrences, affecting 60% of the participants. Conclusions OTC misuse is common among older adults, and it is important for older adults to seek out resources, such as a pharmacist, to help them make safe OTC decisions.

Abstract

To evaluate the association of subretinal hyperreflective material (SHRM) with visual acuity (VA), geographic atrophy (GA), and scar in the Comparison of Age-Related Macular Degeneration Treatments Trials (CATT).Prospective cohort study within a randomized clinical trial.The 1185 CATT participants.Masked readers graded scar and GA on fundus photography and fluorescein angiography and graded SHRM on time-domain and spectral-domain (SD) optical coherence tomography (OCT) throughout 104 weeks. Measurements of SHRM height and width in the fovea, within the center 1 mm(2), or outside the center 1mm(2) were obtained on SD OCT images at 56 (n = 76) and 104 (n = 66) weeks.Presence of SHRM, as well as location and size, and associations with VA, scar, and GA.Among CATT participants, the percentage with SHRM at enrollment was 77%, decreasing to 68% at 4 weeks after treatment and to 54% at 104 weeks. At 104 weeks, scar was present more often in eyes with persistent SHRM than in eyes with SHRM that resolved (64% vs. 31%; P0.0001). Among eyes with detailed evaluation of SHRM at weeks 56 (n = 76) and 104 (n = 66), mean VA letter score was 73.5 (standard error [SE], 2.8), 73.1 (SE, 3.4), 65.3 (SE, 3.5), and 63.9 (SE, 3.7) when SHRM was absent, present outside the central 1 mm(2), present within the central 1 mm(2) but not the foveal center, or present at the foveal center (P = 0.02), respectively. When SHRM was present, the median maximum height under the fovea, within the central 1 mm(2) including the fovea and anywhere within the scan, was 86 μm, 120 μm, and 122 μm, respectively. Visual acuity was decreased with greater SHRM height and width (P0.05).In eyes with neovascular age-related macular degeneration (AMD), SHRM is common and often persists after anti-vascular endothelial growth factor treatment. At 2 years, eyes with scar were more likely to have SHRM than other eyes. Greater SHRM dimensions were associated with worse VA. In eyes with neovascular AMD, SHRM is an important morphologic biomarker.

Abstract

BACKGROUND There is increasing interest in the possible role of new fluoroquinolone antibiotics for treatment of tuberculosis, but widespread use of fluoroquinolones for treatment of other bacterial infections may select for resistant strains of Mycobacterium tuberculosis. METHODS We evaluated fluoroquinolone susceptibility using the proportion method (critical ciprofloxacin concentration for susceptibility testing, 2.0 mu g/mL) in isolates obtained from patients enrolled in Tuberculosis Trial Consortium clinical trials during the period of 1995-2001 and in a referral sample of isolates sent to the Centers for Disease Control and Prevention (Atlanta, GA) during the period of 1996-2000 for additional testing, often because of drug resistance. RESULTS Of the 1373 isolates from the clinical trials, 1324 (96%) were susceptible to isoniazid and rifampin; 2 (0.15%) of these isolates were also resistant to ciprofloxacin. Of the 1852 isolates from the referral sample, 603 (32.6%) were resistant to isoniazid and rifampin (i.e., multidrug resistant), 849 (45.7%) were resistant to >or=1 first-line drug but were not resistant to both isoniazid and rifampin, and 400 (21.6%) were susceptible to all first-line agents. Ciprofloxacin resistance was found in 33 (1.8%) of the referral-sample isolates. Most ciprofloxacin-resistant isolates (25 [75.8%]) were resistant to isoniazid and rifampin. CONCLUSIONS Despite widespread use of fluoroquinolones for treatment of common bacterial infections, resistance among clinical isolates of M. tuberculosis in the United States and Canada remains rare, occurring primarily among multidrug-resistant strains.

Abstract

To compare baseline characteristics, treatment frequency, visual acuity (VA), and morphologic outcomes of eyes with50% of the lesion composed of blood (B50 group) versus all other eyes (Other group) enrolled in the Comparison of Age-Related Macular Degeneration Treatments Trials (CATT).Prospective cohort study within a multicenter randomized clinical trial.CATT patients with neovascular age-related macular degeneration (AMD).Treatment for the study eye was assigned randomly to either ranibizumab or bevacizumab and to 3 different dosing regimens over a 2-year period. Reading center graders evaluated baseline and follow-up morphology in color fundus photographs, fluorescein angiography (FA), and optical coherence tomography (OCT). Masked examiners tested VA.Morphologic features and VA at 1 and 2 years.The B50 group consisted of 84 of 1185 (7.1%) patients enrolled in CATT. Baseline lesion characteristics differed between groups. In the B50 group, choroidal neovascularization size was smaller (0.73 vs 1.83 disc areas [DA]; P0.001), total lesion size was greater (4.55 vs 2.31 DA; P0.001), total retinal thickness was greater (524 vs 455 μm; P = 0.02), and mean VA was worse (56.0 vs 60.9 letters; P = 0.002). Increases in mean VA were similar in the B50 and Other groups at 1 year (+9.3 vs +7.2 letters; P = 0.22) and at 2 years (9.0 vs 6.1 letters; P = 0.17). Eyes treated PRN received a similar number of injections in the 2 groups (12.2 vs 13.4; P = 0.27). Mean lesion size in the B50 group decreased by 1.2 DA at both 1 and 2 years (primarily owing to resolution of hemorrhage) and increased in the Other group by 0.33 DA at 1 year and 0.91 DA at 2 years (P 0.001). Leakage on FA and fluid on OCT were similar between groups at 1 and 2 years.In CATT, the B50 group had a visual prognosis similar to the Other group. Lesion size decreased markedly through 2 years. Eyes like those enrolled in CATT with neovascular AMD lesions composed of50% blood can be managed similarly to those with less or no blood.
