Abstract Background Stroke increases subsequent dementia risk yet there are no specific post-stroke therapies to protect cognition. Cardiorespiratory exercise is recommended for secondary prevention of stroke and may be neuroprotective. The Post Ischaemic Stroke Cardiovascular Exercise Study (PISCES) aims to reduce post-stroke secondary neurodegeneration and cognitive decline. During the pandemic, we pivoted to a ZOom Delivered Intervention Against Cognitive decline (ZODIAC) protocol, reducing pandemic-amplified barriers to exercise. Methods We present pandemic adaptions for a multicentre phase IIb assessor-blinded randomised controlled trial of ischaemic stroke survivors testing the efficacy and feasibility of an 8-week home-based exercise intervention delivered at 2 months post-stroke. We compare cardiorespiratory exercise (intervention arm) versus balance and stretching (active control arm). Participants are assessed with magnetic resonance imaging (MRI), fitness, blood, microbiome, and neuropsychological tests at three study visits: before and after the exercise intervention and at 12 months. Modifications to the original protocol include pre-exercise safety home visits, commercial delivery of exercise equipment to facilitate assessor blinding, and reconsideration of statistical plan to allow pooling of the studies. We have reduced in-person study visits from 27 to 3. Primary outcome remains between-group (intervention versus control) difference in brain volume change; secondary outcome is between-group difference in global cognitive ability to allow remote administration of a validated cognitive scale. Discussion Remotely delivered exercise interventions reduce participant burden and may reduce barriers to recruitment. A decrease in the number of in-person study visits can be supported by greater information capture via self-reported questionnaires and phone surveys. Abstract Background Treatment decisions in prostate cancer (PCa) rely on disease stratification between localised and metastatic stages, but current imaging staging technologies are not sensitive to micro-metastatic disease. Circulating tumour cells (CTCs) status is a promising tool in this regard. The Parsortix® CTC isolation system employs an epitope-independent approach based on cell size and deformability to increase the capture rate of CTCs. Here, we present a protocol for prospective evaluation of this method to predict post radical prostatectomy (RP) PCa cancer recurrence. Methods We plan to recruit 294 patients diagnosed with unfavourable intermediate, to high and very high-risk localised PCa. Exclusion criteria include synchronous cancer diagnosis or prior PCa treatment, including hormone therapy. RP is performed according to the standard of care. Two blood samples (20 ml) are collected before and again 3-months after RP. The clinical team are blinded to CTC results and the laboratory researchers are blinded to clinical information. Treatment failure is defined as a PSA ≥ 0.2 mg/ml, start of salvage treatment or imaging-proven metastatic lesions. The CTC analysis entails enumeration and RNA analysis of gene expression in captured CTCs. The primary outcome is the accuracy of CTC status to predict post-RP treatment failure at 4.5 years. Observed sensitivity, positive and negative predictive values will be reported. Specificity will be presented over time. Discussion CTC status may reflect the true potential for PCa metastasis and may predict clinical outcomes better than the current PCa progression risk grading systems. Therefore establishing a robust biomarker for predicting treatment failure in localized high-risk PCa would significantly enhance guidance in treatment decision-making, optimizing cure rates while minimizing unnecessary harm from overtreatment. En este artículo se analizan las narrativas de autoorganización de la población productora de milpa intercalada con árboles frutales (MIAF) en la región mazahua al noroeste del Estado de México. Se llevó a cabo un estudio cualitativo de tipo biográfico, mediante entrevistas a 13 productoras/es. Las y los productores MIAF fueron impulsados por actores externos desde 2018 y se les proporcionó semilla, material para la siembra y capacitaciones. Posteriormente, se autoorganizaron de tres formas: acción colectiva, interacción entre productoras/es y dependencia de recursos propios. Como limitantes están los costos en la producción de alimentos y las políticas públicas. Encontramos que las narrativas de autoorganización abonan a la discusión sobre la resiliencia socioecológica. Es importante que las y los productores MIAF potencien su acción colectiva para hacer frente a las adversidades, principalmente a la sequía. Tanto a nivel familiar como comunitario se pueden impulsar prácticas sostenibles destinadas a desarrollar la resiliencia.

Introduction Head and neck cancer is the eighth most common cancer in the UK. Current standard of care treatment for patients with recurrent/metastatic squamous cell head and neck carcinoma (HNSCC) is platinum-based chemotherapy combined with the anti-epidermal growth factor receptor (anti-EGFR) monoclonal antibody, cetuximab. However, most patients will have poor median overall survival (OS) of 6–9 months despite treatment. HNSCC tumours exhibit an immune landscape poised to respond to immunotherapeutic approaches, with most tumours expressing the immunosuppressive receptor programmed death-ligand 1 (PD-L1). We undertook the current study to determine the safety and efficacy of avelumab, a monoclonal antibody targeting the interaction between PD-L1 and its receptor on cytotoxic T-cells, in combination with cetuximab.Methods and analysis This is a multi-centre, single-arm dose de-escalation phase II safety and efficacy study of avelumab combined with cetuximab; the study was to progress to a randomised phase II trial, however, the study will now complete after the safety run-in component. Up to 16 participants with histologically/cytologically recurrent/metastatic squamous cell carcinoma (including HNSCC) who have not received cetuximab previously will be recruited. All patients will receive 10 mg/kg avelumab and cetuximab (500, 400 or 300 mg/m2 depending on the cohort open at time of registration) on days 1 and 15 of 4-week cycles for up to 1 year, (avelumab not given cycle 1 day 1). A modified continual reassessment method will be used to determine dose de-escalation. The primary objective is to establish the safety of the combination and to determine the optimum dose of cetuximab. Secondary objectives include assessing evidence of antitumour activity by evaluating response rates and disease control rates at 6 and 12 months as well as progression-free and OS.Ethics and dissemination Approval granted by City and East REC (18/LO/0021). Findings will be published in peer-reviewed journals and disseminated at conferences.Trial registration number NCT03494322.

Port sediments are often contaminated with metals and organic compounds from anthropogenic sources. Remobilization of sediment during a planned expansion of Port Everglades near Fort Lauderdale, Florida (USA) has the potential to harm adjacent benthic communities, including coral reefs. Twelve sediment cores were collected from four Port Everglades sites and a control site; surface sediment was collected at two nearby coral reef sites. Sediment cores, sampled every 5 cm, were analyzed for 14 heavy metals using inductively coupled plasma-mass spectrometry. Results for all three locations yielded concentration ranges (µg/g): As (0.607–223), Cd (n/d–0.916), Cr (0.155–56.8), Co (0.0238–7.40), Cu (0.004–215), Pb (0.0169–73.8), Mn (1.61–204), Hg (n/d–0.736), Mn (1.61–204), Ni (0.232–29.3), Se (n/d–4.79), Sn (n/d–140), V (0.160–176), and Zn (0.112–603), where n/d = non-detected. The geo-accumulation index shows moderate-to-strong contamination of As and Mo in port sediments, and potential ecological risk indicates moderate-to-significantly high overall metal contamination. All four port sites have sediment core subsamples with As concentrations above both threshold effect level (TEL, 7.24 µg/g) and probable effect level (PEL, 41.6 µg/g), while Mo geometric mean concentrations exceed the background continental crust level (1.5 µg/g) threshold. Control site sediments exceed TEL for As, while the reef sites has low to no overall heavy metal contamination. Results of this study indicate there is a moderate to high overall ecological risk from remobilized sediment due to metal contamination. Due to an imminent dredging at Port Everglades, this could have the potential to harm the threatened adjacent coral communities and surrounding protected habitats.

ABSTRACT Cajal bodies (CBs) are major sub-nuclear structures in most eucaryotic cells. In human adenovirus 5 (Ad5) infection, CBs are reorganized into microfoci in the late phase of infection. Here we show that many CB protein components (p80-coilin, SMN-1, and WRAP53) remained stable throughout most of the infectious cycle of Ad type 5 (Ad5) in human A549 epithelial cells, even when CBs were reorganized into microfoci. Reduction of p80-coilin expression by RNA interference resulted in significant reductions in the levels of early (E1A, E2A-DBP), intermediate (pIX and IVa2), and late (L1-IIIa, L2-penton base, L3-hexon, L4-100K, and L5-fiber) proteins in Ad5-infected A549 cells. Depletion of p80-coilin did not significantly alter the total cellular levels of the corresponding Ad cytoplasmic mRNAs (with the exception of E1A 12S and 13S and pIX mRNA) or the production of the Ad5 pre-mRNAs tested (E1A, E2A-DBP, IVa2, or late pre-mRNAs containing the tripartite leader). However, the ratio of viral cytoplasmic to nuclear-spliced Ad RNAs was reduced in p80-coilin-depleted, Ad5-infected cells compared to control-infected cells. Immunofluorescent staining of Ad5-infected cells revealed co-localization of p80-coilin with areas of immunoreactivity defined by a polyclonal antibody that recognized the L4-22K and L4-33K proteins in a fraction of microfoci. Immunoprecipitation analysis showed that only the L4-22K protein formed a stable complex with p80-coilin in Ad5-infected cells and in cells co-transfected with plasmids encoding p80-coilin and either the L4-22K or L4-33K protein. p80-coilin therefore plays an important role in Ad replication by complex formation with L4-22K and facilitating nuclear export of Ad mRNAs. IMPORTANCE The architecture of sub-nuclear structures of eucaryotic cells is often changed during the infectious cycle of many animal and plant viruses. Cajal bodies (CBs) form a major sub-nuclear structure whose functions may include the regulation of cellular RNA metabolism. During the lifecycle of human adenovirus 5 (Ad5), CBs are reorganized from their spherical-like structure into smaller clusters termed microfoci. The mechanism of this reorganization and its significance for virus replication has yet to be established. Here we show that the major CB protein, p80-coilin, facilitates the nuclear export of Ad5 transcripts. Depletion of p80-coilin by RNA interference led to lowered levels of viral proteins and infectious virus. p80-coilin was found to form a complex with the viral L4-22K protein in Ad5-infected cells and in some reorganized microfoci. These findings assign a new role for p80-coilin as a potential regulator of infection by a human DNA virus.

ABSTRACT The DENV envelope (E) and pre-membrane (prM) glycoproteins are primary targets of serologic immunity after infection and vaccination. Of these, serotype-specific (TS) antibodies typically target E domains, while serotype cross-reactive (CR) antibodies typically the target prM protein and conserved E regions. To identify and quantify E-domain TS neutralizing antibody responses in polyclonal sera, we developed a panel of chimeric DENV4/2 viruses that incorporate DENV2 envelope domain I, II, and III (DENV4/2-EDI, EDII, EDIII) into the DENV4 E glycoprotein. Chimeric DENV4/2 viruses were recovered, replicated efficiently, and displayed similar maturation states as parental viruses. The recovery of viable DENV4/2-EDII recombinants required the inclusion of chimeric DENV4/2 prM that maintained critical interactions with chimeric E. To assess structural integrity and epitope display of chimeric viruses, we examined neutralization of mature virions by monoclonal antibodies (mAbs) and heterotypic polyclonal sera. The ED-chimeric virions preserved epitopes of TS and envelope-dimer-epitope CR mAbs and had similar sensitivity to CR polyclonal responses as parental strains. Primary sera from natural infection and human challenge target a region centered on EDIII and secondarily target EDII and EDI. Sera from natural infection had a unique neutralization pattern compared to sera from human challenge, which included greater frequency and higher titer of responses against DENV EDII. In summary, DENV4/2 E recombinant viruses delineate the subdomain targets of TS antibodies after vaccination and primary infection, which may provide new correlates of protection or identify epitopes of neutralizing monoclonal antibodies. IMPORTANCE The four dengue virus (DENV) serotypes infect several hundred million people each year. Although primary infection is generally mild, subsequent infection by differing serotypes increases the risk for symptomatic disease ranging from fever to life-threatening shock. Despite the availability of licensed vaccines, a comprehensive understanding of antibodies that target the viral envelope protein and protect from infection remains incomplete. In this manuscript, we develop a panel of recombinant viruses that graft each envelope domain of DENV2 onto the DENV4 envelope glycoprotein, revealing protein interactions important for virus viability. Furthermore, we map neutralizing antibody responses after primary DENV2 natural infection and a human challenge model to distinct domains on the viral envelope protein. The panel of recombinant viruses provides a new tool for dissecting the E domain-specific targeting of protective antibody responses, informing future DENV vaccine design.

Here, the authors report a live chimeric DENV2/4 EDII virus, encoding DENV2 and DENV4 neutralizing epitopes, that replicates efficiently in primates and simultaneously elicits neutralizing DENV2 and DENV4 type-specific antibodies, providing domain-specific diagnostic reagents and simplified vaccine strategies.

Purpose: At the heart of the paradigm shift in approach to patient care from paternalism toward shared decision-making lies the international “What Matters To You?” (WMTY) movement. However, WMTY principles are not frequently applied to the critical care setting. The aim of this quality improvement project work was to design and integrate a tool for all patients admitted to the intensive care unit (ICU) that helped answer WMTY. Methods: Using Plan-Do-Study-Act (PDSA) methodology across 8 cycles, a multidisciplinary team designed and integrated a bedside poster into the ICU. Quantitative and qualitative data were collected via a bedside audit process on a regular basis during each of the study phases comprising PDSA cycles. Results: Project results confirmed that the introduction of this poster/tool, alongside resource- and staff engagement-focused interventions, enabled the ICU to offer more than 50% of patients a WMTY conversation, as compared to zero at the start of the project. Consistently, 100% of staff surveyed (n = 46 over all cycles) felt the posters were a useful addition to the ICU and confirmed they learned something new about their patients that they didn’t know already. Conclusions: This novel poster design successfully summarized patients’ responses to the question “What matters to you?” for ICU staff and would be transferable to other ICUs.

Soft tissue sarcomas are pleiotropic tumors of mesenchymal cell origin. These tumors are rare in humans but common in veterinary practice, where they comprise up to 15% of canine skin and subcutaneous cancers. Because they present similar morphologies, primary sites, and growth characteristics, they are treated similarly, generally by surgical resection followed by radiation therapy. Previous studies have examined a variety of genetic changes as potential drivers of tumorigenesis and progression in soft tissue sarcomas as well as their use as markers for soft tissue sarcoma subtypes. However, few studies employing next generation sequencing approaches have been published. Here, we have examined gene expression patterns in canine soft tissue sarcomas using RNA-seq analysis of samples obtained from archived formalin-fixed and paraffin-embedded tumors. We provide a computational framework for using resulting data to categorize tumors, perform cross species comparisons and identify genetic changes associated with tumorigenesis. Functional overrepresentation analysis of differentially expressed genes further implicate both common and tumor-type specific transcription factors as potential mediators of tumorigenesis and aggression. Implications for tumor-type specific therapies are discussed. Our results illustrate the potential utility of this approach for the discovery of new therapeutic approaches to the management of canine soft tissue sarcomas and support the view that both common and tumor-type specific mechanisms drive the development of these tumors.

At our university based high throughput screening program, we test all members of our community weekly using RT-qPCR. RT-qPCR cycle threshold (CT) values are inversely proportional to the amount of viral RNA in a sample and are a proxy for viral load. We hypothesized that CT values would be higher, and thus the viral loads at the time of diagnosis would be lower, in individuals who were infected with the virus but remained asymptomatic throughout the course of the infection. We collected the N1 and N2 target gene CT values from 1633 SARS-CoV-2 positive RT-qPCR tests of individuals sampled between August 7, 2020, and March 18, 2021, at the BU Clinical Testing Laboratory. We matched this data with symptom reporting data from our clinical team. We found that asymptomatic patients had CT values significantly higher than symptomatic individuals on the day of diagnosis. Symptoms were followed by the clinical team for 10 days post the first positive test. Within the entire population, 78.1% experienced at least one symptom during surveillance by the clinical team (n = 1276/1633). Of those experiencing symptoms, the most common symptoms were nasal congestion (73%, n = 932/1276), cough (60.0%, n = 761/1276), fatigue (59.0%, n = 753/1276), and sore throat (53.1%, n = 678/1276). The least common symptoms were diarrhea (12.5%, n = 160/1276), dyspnea on exertion (DOE) (6.9%, n = 88/1276), foot or skin changes (including rash) (4.2%, n = 53/1276), and vomiting (2.1%, n = 27/1276). Presymptomatic individuals, those who were not symptomatic on the day of diagnosis but became symptomatic over the following 10 days, had CT values higher for both N1 (median = 27.1, IQR 20.2-32.9) and N2 (median = 26.6, IQR 20.1-32.8) than the symptomatic group N1 (median = 21.8, IQR 17.2-29.4) and N2 (median = 21.4, IQR 17.3-28.9) but lower than the asymptomatic group N1 (median = 29.9, IQR 23.6-35.5) and N2 (median = 30.0, IQR 23.1-35.7). This study supports the hypothesis that viral load in the anterior nares on the day of diagnosis is a measure of disease intensity at that time.

Summary: Background: Detection of dengue virus antibodies is important for understanding future dengue virus risk and for prevaccination screening. We aimed to evaluate the performance of a dengue IgG indirect ELISA in determining dengue seroprevalence in a cohort of children in the Philippines, using a focus reduction neutralisation test (FRNT) as the reference test. Methods: In this prospective population-based cohort study, we enrolled healthy children residing in Bogo or Balamban, Cebu, Philippines, who were to be aged 9–14 years at the time of a mass dengue vaccination campaign. Sera were collected from participants and batch tested by indirect IgG ELISA and FRNT. The primary endpoint was dengue seroprevalence in the cohort, detected by ELISA, and validated by that detected by reference FRNT. This study is registered with ClinicalTrials.gov, NCT03465254. Findings: We collected 2996 serum samples between May 2, and June 2, 2017, and we tested each sample with IgG ELISA. Using 1961 samples (65·5%) that were tested with FRNT, and 1035 samples (34·5%) with imputed results, we found that 320 (10·7%) of 2996 children were dengue naive and 2676 (89·3%) were seropositive for previous dengue virus infection. Based on the 1961 non-imputed FRNT results classified as dengue seronegative or seropositive, the ELISA (with a 0·9 index value cutoff) showed 95·2% sensitivity, 93·4% specificity, 6·6% false positivity, and 4·8% false negativity. However, sensitivity of the ELISA was poor (77·1%) among children with immunity to just one dengue virus serotype. Of the 11 sera that were false positive with ELISA, seven samples (63·6%) were seropositive for Zika virus or Japanese encephalitis virus with FRNT. Interpretation: Most children (89·3%) assessed in our study and eligible to participate in the mass dengue vaccination campaign were seropositive for previous dengue virus infection. Compared with FRNT, ELISA had high sensitivity and specificity (>90%), but the false-negative and false-positive rates makes the test suboptimal for prevaccination screening. Individuals who are falsely identified as seropositive by dengue IgG ELISA and then vaccinated might be at risk of developing severe disease during a subsequent exposure to wild-type dengue virus. Those with a monotypic profile would benefit the most from vaccination, but the sensitivity of the IgG ELISA was much lower in this group than in those with a multitypic profile. Introduction Mesothelioma remains a lethal cancer. To date, systemic therapy with pemetrexed and a platinum drug remains the only licensed standard of care. As the median survival for patients with mesothelioma is 12.1 months, surgery is an important consideration to improve survival and/or quality of life. Currently, only two surgical trials have been performed which found that neither extensive (extra-pleural pneumonectomy) or limited (partial pleurectomy) surgery improved survival (although there was some evidence of improved quality of life). Therefore, clinicians are now looking to evaluate pleurectomy decortication, the only radical treatment option left.Methods and analysis The MARS 2 study is a UK multicentre open parallel group randomised controlled trial comparing the effectiveness and cost-effectiveness of surgery—(extended) pleurectomy decortication—versus no surgery for the treatment of pleural mesothelioma. The study will test the hypothesis that surgery and chemotherapy is superior to chemotherapy alone with respect to overall survival. Secondary outcomes include health-related quality of life, progression-free survival, measures of safety (adverse events) and resource use to 2 years. The QuinteT Recruitment Intervention is integrated into the trial to optimise recruitment.Ethics and dissemination Research ethics approval was granted by London – Camberwell St. Giles Research Ethics Committee (reference 13/LO/1481) on 7 November 2013. Los huertos familiares son relevantes para la conservación de la biodiversidad, la seguridad alimentaria y la dotación de servicios ecosistémicos. El objetivo de este artículo es analizar la diversidad biocultural en los huertos familiares de San Andrés Nicolás Bravo. Se estudiaron 98 huertos mediante entrevistas a sus dueños sobre su uso y manejo; se identificó la familia, género y especie de las plantas y frutos cultivados; se calculó su abundancia, riqueza, uso e índice de importancia cultural, y se realizó un análisis de sus componentes principales y de clúster para conocer la diferencia entre los huertos. Se identificaron 48 especies con siete usos, donde destacan el alimenticio, medicinal y el destinado a la venta. Además del conocimiento asociado con las diversas formas de aprovechamiento de los frutos, destaca el valor sentimental atribuido a las plantas y su uso ritual. Las especies más abundantes fueron las de mayor valor cultural, por lo que su multifuncionalidad influye en la riqueza de los huertos y muestra la bioculturalidad de las especies frutales. Se concluye que hay diversidad entre los huertos en cuanto a riqueza, forma biológica, abundancia y superficie dedicada a las especies frutales.

Little is known about the contribution of virus-specific and cross-reacting antibodies (Abs) or the cellular immune response generated by a primary dengue (DENV) infection on the course of a secondary zika (ZIKV) infection in vivo. Here we show that the length of time between DENV/ZIKV infections has a qualitative impact on controlling early ZIKV replication. Depletion of DENV2-specific Abs in sera confirmed that those type-specific Abs do not contribute to ZIKV control. We show that the magnitude and durability of the neutralizing antibodies (nAbs) induced by a secondary ZIKV infection is modest compared to the response induced after a secondary heterologous DENV infection. Our in vivo results are showing a complex interplay between the cellular and innate immune responses characterized by a high frequency of plasmacytoid dendritic cells (pDC) correlating with an increase in the frequency of DENV antigen specific T cells and a significant control of ZIKV replication which is time dependent. Taken together, our results suggest that early after ZIKV infection other mechanisms such as the innate and cellular immune responses may play a predominant role in controlling ZIKV replication. Regardless of the time elapsed between infections there was no evidence of in vivo antibody-dependent enhancement (ADE) of ZIKV by DENV immunity. These findings have pivotal implications while interpreting ZIKV pathogenesis in flavivirus-experimented populations, diagnostic results interpretation and vaccine designs and schedules among others.

Abstract Background The 4 dengue serotypes (DENV) are mosquito-borne pathogens that are associated with severe hemorrhagic disease. DENV particles have a lipid bilayer envelope that anchors two membrane glycoproteins prM and E. Two E-protein monomers form head-to-tail homodimers and three E-dimers align to form “rafts” that cover the viral surface. Some human antibodies that strongly neutralize DENV bind to quaternary structure epitopes displayed on E protein dimers or higher order structures forming the infectious virus. Expression of prM and E in cell culture leads to the formation of DENV virus-like particles (VLPs) which are smaller than wildtype virus particles and replication defective due to the absence of a viral genome. There is no data available that describes the antigenic landscape on the surface of flavivirus VLPs in comparison to the better studied infectious virion. Methods A large panel of well characterized antibodies that recognize epitope of ranging complexity were used in biochemical analytics to obtain a comparative antigenic surface view of VLPs in respect to virus particles. DENV patient serum depletions were performed the show the potential of VLPs in serological diagnostics. Results VLPs were confirmed to be heterogeneous in size morphology and maturation state. Yet, we show that many highly conformational and quaternary structure-dependent antibody epitopes found on virus particles are efficiently displayed on DENV1–4 VLP surfaces as well. Additionally, DENV VLPs can efficiently be used as antigens to deplete DENV patient sera from serotype specific antibody populations. Conclusions This study aids in further understanding epitopic landscape of DENV VLPs and presents a comparative antigenic surface view of VLPs in respect to virus particles. We propose the use VLPs as a safe and practical alternative to infectious virus as a vaccine and diagnostic antigen.

Fir forests (Abies religiosa) in central Mexico have been subjected to strong human pressures and their richness and the geographic affinity of their flora in some regions have been poorly analyzed. Through field trips and collections of botanical specimens, the floristic composition and patterns of current geographical distribution of families, genera and species were determined of a fir forest located in the Transmexican Volcanic Belt. We obtained a list of 94 species grouped in 74 genera and 42 families. 67 % of the flora is grouped in 13 families of which Asteraceae family stands out for gather the largest number of species (24.5 %) and genera (22 %). The floristic similarity between the study area and neighboring forests increases with geographical proximity. 60 % of the families have a widespread distribution, while 50 % of the genera are exclusive of the Americas, distributed mainly from Mexico to Central America. At the species level 95.8 % have American distribution, of which 44.2 % are found only in Mexico and 36.8 % from Mexico to Central America. Among the tree species 61.4 % elements are from Mexico to Central America, while the shrubs and herbs are mainly endemic species to Mexico (60 % and 43.6 % respectively). The high proportion of endemic species in the analyzed fir forest; require continuous monitoring to ensure the generation of new alternatives for its conservation.

. Los huertos familiares son agroecosistemas heterogéneos en estructura y manejo, en ellos se mantienen procesos de conservación in situ de la biodiversidad. La ora en los huertos es variada, su estudio contribuye a comprender las especies que coexisten y se conservan, además de entender la estructura, importancia, y funciones para las familias de las comunidades rurales. El estudio se enfocó al estrato arbóreo del agroecosistema huerto familiar (AHF) en San Nicolás, Malinalco, México, por tener especies clave en la estructura de los agroecosistemas y por su importancia social, cultural y económica. Se estudiaron 14 agroecosistemas, ubicados en la zona de transición ecológica del Estado de México. El estudio se llevó a cabo de marzo de 2011 a abril de 2012, por medio de entrevistas no estructuradas a los propietarios de los huertos. El objetivo fue realizar un análisis del estrato arbóreo sobre la diversidad, estructura, función y análisis biogeográco. Los agroecosistemas poseen una diversidad de 36 especies, 26 géneros y 18 familias de plantas. La especie más frecuente fue Persea americana Mill. var. Hass presente en el 100 % de los agroecosistemas. De las especies encontradas, 52.7 % presentan una distribución natural en México, y las otras son introducidas. El aprovechamiento del estrato arbóreo, es con nes de autoconsumo, principalmente como fruta. La diversidad orística de las especies arbóreas es alta, lo que permite ubicar a estos agroecosistemas como importantes sitios de conservación de germoplasma in situ

Existe interés por las prácticas curativas de la medicina tradicional como forma de curación/ sanación alterna y complementaria. El objetivo del artículo es documentar las prácticas curativas y las plantas medicinales que actualmente se utilizan en una comunidad rural del Estado de México. Desde la etnomedicina, se abordan las explicaciones cognitivas de los sujetos que participan del binomio salud-enfermedad. Se utilizó el método etnográfico para obtener y sistematizar información sobre las percepciones acerca de las prácticas curativas y plantas medicinales. Las prácticas son el resultado del conocimiento acumulado sobre el ambiente y se llevan a cabo por el interés de ayudar, más que por beneficio económico. Se concluye que la herbolaria es un indicativo de: 1) la pervivencia de la cosmovisión mesoamericana para tratar enfermedades de cuerpo y espíritu; 2) se privilegia una atención integral del paciente y 3) el ambiente físico que rodea a las personas es un botiquín bio-cultural.

BackgroundBiomarkers of monocyte activation (soluble CD14 [sCD14]), inflammation (interleukin-6 [IL-6]), and altered coagulation (D-dimer) are associated with increased mortality risk in people with HIV. The objective of the Russia Alcohol Research Collaboration on HIV/AIDS (ARCH) study was to evaluate the association between heavy alcohol use and inflammatory biomarkers over time.MethodsThe study sought antiretroviral therapy naive participants with HIV (n = 350) and assessed them at baseline, 12 and 24 months. Linear mixed effects models were used to determine whether heavy drinking (self-report augmented by phosphatidylethanol [PEth], an alcohol biomarker) was longitudinally associated with IL-6, sCD14 and D-dimer adjusting for potential confounders (e.g., demographics, HIV factors, comorbid conditions).ResultsParticipants' baseline characteristics were as follows: 71% male; mean age of 34 years; 87% self-reported hepatitis C; and 86% current smokers. Mean log10 (HIV RNA) was 4.3 copies/mL. Heavy alcohol use, based on National Institute of Alcohol Abuse and Alcoholism risky drinking criteria and PEth (versus non-heavy alcohol use) was associated with higher sCD14 (adjusted mean difference 125 ng/mL [95% CI: 42, 209]), IL-6 (ratio of means 1.35 [95% CI: 1.17, 1.55] pg/mL), and D-dimer (ratio of means 1.20 [95% CI: 1.06, 1.37] ug/mL) across the two-year follow-up.ConclusionAmong HIV+ adults, current heavy alcohol use is associated with higher sCD14, IL-6 and D-dimer over time. Since these biomarkers are associated with mortality, interventions to mitigate effects of heavy drinking on these immune processes merit consideration.

Se determinaron aspectos sobre el hábitat, uso y función de los recursos vegetales utilizados en la medicina tradicional, en la comunidad de San Nicolás, municipio de Malinalco, ubicada al sur del Estado de México. De enero de 2010 a enero de 2011 se aplicaron entrevistas a 30 hombres y mujeres, de entre 20 y 60 años de edad. La observación directa y participativa permitió verifi car la función y manejo de las plantas. Se registraron 165 especies correspondientes a 147 géneros y 70 familias de plantas vasculares. Las especies utilizadas en la medicina tradicional se agruparon en 13 clases o categorías de acuerdo con el tipo de enfermedad en que se utilizan. Las categorías de plantas medicinales que mostraron las mayores frecuencias de utilización se relacionan con el tratamiento de enfermedades del aparato digestivo, aparato respiratorio, enfermedades asociadas con la nutrición y sistema endócrino, además de las referidas como síndromes de filiación cultural.Las plantas medicinales se presentaron en dos diferentes hábitat; 45.4% son especies pertenecientes a la flora viaria y el 54.5% prosperan en agroecosistemas familiares, llamados localmente huertos o solares. Aproximadamente un tercio de las especies eran cultivadas y el resto silvestres. Tres cuartos de las especies presentan una distribución natural en América, y las demás han sido introducidas de ultramar. Los datos muestran que el mayor número de especies utilizadas para fines medicinales se encuentran en los huertos. Esto implica nuevos retos debido a que los huertos familiares tradicionales están siendo utilizados para fines diversos, como la construcción de viviendas o establecimientos comerciales, ocasionando pérdida de germoplasma in situ y en consecuencia pérdida de la biodiversidad en este territorio del subtrópico mexicano.

Rapid diagnosis of pulmonary tuberculosis (TB) is critical for timely initiation of treatment and interruption of transmission. Yet, despite recent advances, many patients remain undiagnosed. Culture, usually considered the most sensitive diagnostic method, is sub-optimal for paucibacillary disease.We evaluated the Totally Optimized PCR (TOP) TB assay, a new molecular test that we hypothesize is more sensitive than culture. After pre-clinical studies, we estimated TOP's per-patient sensitivity and specificity in a convenience sample of 261 HIV-infected pulmonary TB suspects enrolled into a TB diagnostic study in Mbarara, Uganda against MGIT culture, Xpert MTB/RIF and a composite reference standard. We validated results with a confirmatory PCR used for sequencing M. tuberculosis.Using culture as reference, TOP had 100% sensitivity but 35% specificity. Against a composite reference standard, the sensitivity of culture (27%) and Xpert MTB/RIF (27%) was lower than TOP (99%), with similar specificity (100%, 98% and 87%, respectively). This study is aimed to evaluate the traditional knowledge applied at agroecosystems of familiar orchards in San Nicolas, at southwest of the State of Mexico, in the Subtropical Central Highlands of Mexico generated by the Balsas River Depression; the selection of properties and informers was by means of intention. Were analyzed fourteen systems between march 2011 and april 2012. Was elaborated the geographic, phyto geographic and agro ecological characterizations; the geographic characterization included physical, biotic and socio cultural aspects, mean by photo interpretation, localization through global positional systems, analysis of social statistics, in field observation about cultural outlines and elaboration of digital cartography. Related to phyto geographic characterization, was realized a floristically inventory, that included from de approach of Ethnobotanics the management and functionality of vegetal resources employed at the familiar orchards agroecosystems, based on the local inhabitants traditional knowledge; were realized collections of wild and cultivated botanical specimens. The agro ecological characterization was elaborated mean by in field work and interviews realized to the terrains owners, the obtained information was processed by means of data bases. The theoretical basis of the study was sustained on Agroecology, Cultural Ecology and Ethno botanical. These allows to evaluate the ecological importance, and to determinate the cultural and socioeconomic impacts of the familiar agroecosystems from a systemic point of view.

Rhizosphere bacterial communities have become a major focal point of research in recent years, especially regarding how they affect plants and vice versa (Philippot et al., 2013). Changes in microbial density and diversity within the rhizosphere occur in a spatial temporal manner. The soil zone closest to the plant roots has the most density and diversity of microbes (Clark, 1940). The lack of methods to consistently isolate rhizosphere samples in a spatially defined manner is a major bottleneck in rhizosphere microbiology. We hypothesized that microbes with increasing affinities to and distance from the plant root can be isolated using increasing strengths of physical disruption. Sonication is an excellent choice due to the ability to gently remove rhizosphere soil and bacterial biofilms without damaging plant roots (Doi T et al., 2007; Bulgarelli et al., 2012; Lundberg et al., 2012). In addition, simply increasing the time of sonication can increase the amount of physical force. We used such an approach to consistently isolate microbial communities with different affinities to the soybean roots (White et al., 2014). This article describes the use of successive sonication to isolate distal, middle, and proximal soil from the rhizosphere of soybean roots.
