Background This study tested whether the dissonance-based Body Project eating disorder prevention program reduced onset of subthreshold/threshold anorexia nervosa (AN), bulimia nervosa (BN), binge eating disorder (BED), and purging disorder (PD) over long-term follow-up. Methods Data were combined from three prevention trials that targeted young women at high-risk for eating disorders (N = 1092; M age = 19.3). Participants were randomized to Body Project groups led by peer educators or expressive writing/educational controls and completed masked diagnostic interviews over 2- to 4-year follow-ups. Logistic regressions tested whether onset of each eating disorder over follow-up differed between Body Project and control participants. Results Peer-led Body Project groups produced a 46% reduction in onset of subthreshold/threshold BN and a 62% reduction in onset of PD relative to controls over follow-up. Rates of onset of subthreshold/threshold AN and BED did not significantly differ between peer-led Body Project participants and control participants. Conclusions Results support the dissemination of the peer-led Body Project for reducing future onset of BN and PD. This study and recent research suggest that thin-ideal internalization, the risk factor for eating disorders targeted in the Body Project, may be more relevant for predicting onset of BN and PD compared to AN and BED. Findings support the development of a version of the Body Project aimed to reduce risk factors that have predicted future onset of all four types of eating disorders (e.g. overvaluation of weight/shape, fear of weight gain), which may more effectively prevent all eating disorder types.

BACKGROUND Most individuals with eating disorders (EDs) do not receive treatment. Leveraging digital tools such as chatbots may be a promising method to mobilize mental health care utilization following ED screening. OBJECTIVE This study examined rates and predictors of uptake of a chatbot aimed to promote mental health services use among adults who screened positive for clinical/subclinical EDs on an online ED screen. METHODS Following screening, eligible individuals (≥18 years, screened positive for a clinical/subclinical ED, not in treatment) were shown the study opportunity. Participants who enrolled were instructed to initiate an SMS conversation with the chatbot. Chi-square tests and logistic regressions explored differences in demographics, ED symptoms, suicidality, and probable ED diagnoses between those who enrolled and engaged with the chatbot versus those who did not. RESULTS Of 6,747 respondents shown the study opportunity, 205 (3.0%) enrolled, of whom 185 (90.2%) subsequently used the chatbot. Chatbot uptake was more common among individuals aged ≥25 versus young adults and less common among individuals engaging in regular dietary restriction. No other differences across demographics or ED presentations were observed. CONCLUSIONS Overall chatbot uptake was low, reflecting a need for additional strategies to promote help-seeking behaviors. However, chatbot uptake was high among those that enrolled and equal across most demographics or ED symptoms. Differences by age and dietary restriction in chatbot uptake indicate a need to better engage those who are younger or report restriction.

PURPOSE OF REVIEW The aim of this review is to understand why and how to improve the detection rate of the fetal anomalies in the first trimester. RECENT FINDINGS The role of the first trimester is evolving from aneuploid and basic anatomical screening to a necessary tool to identifying early fetal malformations. In the past 20 years, several studies were conducted to assess the detection rate of anomalies in the first trimester, and despite the improvement in the diagnoses of spina bifida and cleft lip, many anomalies detectable in the first trimester are still being diagnosed in the second trimester scan. Analyzing the factors that improve the detection rate can show the reason why, even in recent years, only 50% of the anomalies are diagnosed in the first trimester. SUMMARY Thanks to the improvement in the ultrasound technology, the fetus can be thoroughly studied even in early gestation and it is, therefore, indispensable to increase the detection rate of the anomalies diagnosable from the first trimester. The sonographer's utmost attention should be applied to all pregnancies, not only in high-risk patients, and new protocols based on the mid trimester scan are needed to spread the concept of first trimester anatomical scan.

Unfortunately, patients may face barriers to receiving treatment, such as stereotypes surrounding EDs, denial of illness severity, lack of motivation for treatment, and lack of knowledge about treatment resources. Barriers such as these result in a large treatment gap: only 20% of those with EDs will ever receive treatment. Digital tools like chatbots show potential to disseminate mental health–related interventions to large populations while offering a user-friendly, cost-effective, accessible, and anonymous means of tackling patient concerns. OBJECTIVE This study developed and evaluated the usability of a chatbot designed for pairing with online ED screening. The tool aimed to promote mental health service utilization by improving motivation for treatment and self-efficacy among individuals with EDs. METHODS A chatbot prototype, Alex, was designed using decision trees and theoretically informed components: psychoeducation, motivational interviewing, personalized recommendations, and repeated administration. Usability testing was conducted over 4 iterative cycles, with user feedback informing refinements to the next iteration. Postintervention, participants (N=21) completed the System Usability Scale (SUS), the Usefulness, Satisfaction, and Ease of Use Questionnaire (USE), and a semistructured interview. This process aimed to create an optimized chatbot by the final cycle for use in a randomized trial. RESULTS Interview feedback detailed chatbot aspects participants enjoyed and aspects necessitating improvement. Feedback converged on four themes: user experience, chatbot qualities, chatbot content, and ease of use. Following refinements, users described Alex as humanlike, supportive, and encouraging. Content was perceived as novel and personally relevant. USE scores across domains were generally above average (~5 out of 7), and SUS scores indicated “good” to “excellent” usability across cycles, with the final iteration receiving the highest average SUS score. CONCLUSIONS Overall, participants responded well in interactions with Alex, including the initial version. Refinements between cycles further improved user experiences. This study provides preliminary evidence of the feasibility and acceptance of a chatbot designed to promote motivation for and use of services among individuals with EDs. Alex is the first chatbot designed for pairing with an ED or other mental health–related online screen, with the goal of ultimately increasing service utilization.

Acceptance-based behavioral treatment (ABT) supplements standard behavioral treatments (BT) for obesity by teaching skills to accept the discomfort inherent to weight control behaviors and prioritize long-term, values-based goals. Grit, the ability to persevere in goal pursuit, overlaps conceptually with ABT principles and may predict outcomes in ABT. During a randomized controlled trial comparing three weight loss interventions (BT, BT with an emphasis on physical activity [BT+PA], ABT with an emphasis on physical activity [ABT+PA]), this study examined if grit predicted weight loss, intervention engagement (session attendance and dietary self-monitoring), and perceived intervention effectiveness, and whether intervention condition moderated these relationships. Participants (N=309) with overweight/obesity enrolled in an 18-month weight loss intervention completed the Short Grit Scale at baseline. Weight and PA were measured at baseline, during the intervention (12 and 18 months), and at follow-up (24 and 36 months). Session attendance and dietary self-monitoring were assessed throughout the intervention, and perceived intervention effectiveness at end-of-intervention. The relation of grit to several outcomes depended on condition. In ABT+PA, but not BT or BT+PA, lower grit related to higher weight loss at 12 and 24 months, session attendance, and perceived intervention effectiveness. Grit was not related to PA or dietary self-monitoring in any condition. ABT’s focus on building skills to facilitate long-term goal pursuit may be unique and beneficial to those with lower grit. Those with higher grit may already possess ABT-consistent skills and benefit less from ABT. Research on trait-level characteristics like grit in relation to weight must be cautious not to reinforce weight bias; rather, this work suggests that an evidence-based intervention (ABT) may be well-suited to those with lower grit levels seeking weight loss.

Abnormal fetal tongue size is a phenotypic feature of various syndromes including Beckwith-Wiedemann, Pierre-Robin, oromandibular limb hypoplasia, chromosomal aberrations, etc. Current data regarding normal fetal tongue size are limited. Hence, micro/macroglossia are subjectively determined. The aim of the study was to construct a contemporary fetal tongue nomogram and to assess its clinical contribution.A prospective cross-sectional study was performed in well dated, low risk, singleton pregnancies. Fetal tongues were measured by 5 trained sonographers. Highest quality images were selected. Intra- and interobserver variability was assessed. Tongue length, width, area, and circumference 1st to 99th centiles were calculated for each gestational week. Based on the normal tongue size charts, we created a Tongue Centile Calculator.Over 18 months, 664 tongue measurements were performed. A cubic polynomial regression model best described the correlation between tongue size and gestational age. The correlation coefficient (rWe present novel fetal tongue size charts from 13 to 40 weeks of gestation. Clinical application of these nomograms may be beneficial in the prenatal diagnosis of syndromes or malformations associated with abnormal fetal tongue size.

OBJECTIVE Test the hypothesis that the efficacy of a dissonance-based obesity/eating disorder prevention program, Project Health, could be enhanced by implementing it in single-sex groups and adding food response inhibition and attention training. METHOD Using a 2 × 2 factorial design, young adults (N = 261; Mage = 19.3, 79% female; 64% White) were randomized to (a) single-sex or (b) mixed-sex groups that completed food response inhibition and attention training or (c) single-sex or (d) mixed-sex groups that completed generic response inhibition and attention training with nonfood images. Preregistered primary outcomes (body fat, Body Mass Index [BMI]), eating disorder symptoms and other outcomes were assessed at pretest and posttest. RESULTS For one preregistered primary outcome, body fat loss, there was a significant interaction between the two manipulated factors (d = -.28), as well as significant main effects for sex composition of groups (d = -.18) and food response inhibition and attention training (d = -0.17), with the largest body fat loss occurring for single-sex groups implemented with food response inhibition and attention training. Although the two manipulated factors did not significantly affect the other outcomes (including BMI, the other preregistered primary outcome), there was a significant reduction in eating disorder symptoms across the conditions (within participant d = -.78), converging with prior evidence that Project Health produced larger reductions in symptoms than educational control participants. CONCLUSIONS Results suggest that implementing Project Health in single-sex groups with food response inhibition and attention training produced the largest body fat loss effects, as well as significant reductions in eating disorder symptoms, suggesting that efforts to disseminate this prevention program are warranted. The importance of the pelvis, as a relevant factor in the birth process, dates back to the beginning of the eighteenth century. In 1701, the Dutch obstetrician and orthopedic Hendrik van Deventer (Fig. 2.1) published a handbook for midwives, in which he described with precision the pelvis. He distinguished between three types of pelvis responsible for difficult births: too wide, too small, and too narrow [1].

Objective Consistent self-monitoring of dietary intake, weight, and physical activity predicts better outcomes during behavioral weight loss, but the factors that influence self-monitoring adherence are not well understood. This study attempted to fill gaps in the existing literature by examining whether pre-treatment eating behaviors predict adherence to digital self-monitoring during a behavioral weight loss program. Method Participants (N = 77) reported on binge eating, uncontrolled eating, and emotional eating at baseline, and were instructed to self-monitor their food intake, weight, and physical activity using digital devices (food logging app, Fitbit, and wireless “smart” scale) throughout the 12-week treatment. Adherence to self-monitoring was assessed using data captured from these devices. Results Greater baseline binge eating severity predicted greater adherence to self-monitoring of weight (ρ = 0.25, p = .03) and eating (ρ = 0.25, p = .03), but not self-monitoring of physical activity. Uncontrolled eating and emotional eating did not significantly predict self-monitoring adherence. Conclusions In contrast to previous research, this study found that participants with greater pre-treatment binge eating severity had better adherence to self-monitoring of eating, and for the first time established a relationship between binge eating severity and digital self-monitoring of weight in behavioral weight loss. Individuals with greater pre-treatment binge eating may exhibit characteristics, such as motivation or rigidity, that are beneficial during the initial period of weight loss. Future studies should determine if there are features of analogue versus digital self-monitoring that may explain this pattern of findings, and examine these associations longitudinally.

Weight suppression (WS) predicts future weight gain and increases in eating disorder symptoms in community and clinical samples but has received minimal attention in obesity and eating disorder prevention programs. In a sample of emerging adults (N = 364) in a randomized controlled trial evaluating two obesity and eating disorder prevention interventions versus a control condition, this study aimed to replicate the findings that WS and its interaction with baseline BMI predict increases in weight and eating disorder symptoms and test a novel hypothesis that WS would moderate the effects of the interventions on change in weight and eating disorder symptoms. Participants completed assessments at baseline, post-intervention, 6-, 12-, and 24-months. WS was calculated as the difference between highest lifetime weight and baseline weight. WS interacted with baseline BMI to predict greater weight gain over 24-months, such that those with high WS and lower baseline BMI gained weight most rapidly. WS did not predict eating disorder symptom change and did not moderate the effects of the prevention programs. Given that individuals with WS are at increased risk for weight gain, expressly targeting this high-risk population with evidence-based obesity prevention programs may be useful. INTRODUCTION The purpose of the study was to evaluate the differences in individual histopathologic placental lesions in pregnancies complicated by early-onset (

e20027 Background: Compared to a baseline normative population, patients with multiple myeloma (MM) have decreased physical function[1]. Studies have revealed that sarcopenia[2, 3], decreased performance status[4], and poor performance on biometric testing[3, 5] predicts worse survival in patients with advanced, incurable malignancies. Physical activity (PA) has been shown to mitigate these effects and in some cases improve cancer specific survival [6-9]. However, there is a need for further research in this area for patients with hematologic malignancies. Here in we describe the results of a pilot study to assess a physical activity intervention in elderly patients with newly diagnosed multiple myeloma. Methods: We designed a single arm, feasibility study utilizing physical and behavioral therapy interventions to help participants adhere to a prescribed exercise program. Eligible patients (age ≥65 years old) were recruited from the multiple myeloma treatment clinic. Baseline, 3-month and 6-month assessments occurred. The intervention included both text messaging and video visits with behavioral coaches and physical therapists to monitor activity levels, review and modify exercise plans, , and enhance motivation and self-efficacy. Our primary aim was to establish the feasibility of recruitment, enrollment, and adherence of the exercise intervention. Our secondary objective was to evaluate changes in fitness (6 minute walk test, grip strength, and the Short Physical Performance Battery). Results: Between 11/18/2019-03/04/2020, 9 participants enrolled on the study (goal of 20). New enrollment was halted due to the COVID-19 pandemic. Median age of the participants was 70 (range 65-86). Six (67%) were female. Eight participants (89%) completed the 6-month intervention. Participants completed 97.8% of the scheduled visits. At 3 and 6 months there was a mean change of moderate/vigorous activity of 26.0% (SD 89.6%) and -0.5% (SD 56.4%) respectively. At 3 and 6 months there was a mean change in step count per day of -5.1% (SD 25.3%) and -1.3% (SD 32.2%) respectively. Overall fitness was preserved over the 6-month period. Conclusions: Prior to COVID-19 pandemic, the study was on track to meet feasibility endpoints of recruitment, enrollment, and retention. The primary endpoint of increasing physical activity was not met. However, adults in our region experienced a 20-30% reduction in physical activity during the pandemic [10]. In the target population, reductions in the absence of intervention may have been even greater, given their pronounced vulnerability to COVID-19 complications. Thus, the true benefits of the intervention may have been difficult to detect during this period. Lack of a control arm is a limiting factor in making further conclusions about the potential benefits of our intervention.
