A combination labial infiltration (1.8 mL) plus lingual infiltration (1.8 mL) of 4% articaine with 1:100,000 epinephrine in the mandibular lateral incisor was found superior to a labial infiltration of 1.8 mL of the same solution. However, it is not known whether the volume or the location had the greatest effect. Therefore, the purpose of this prospective, randomized crossover study was to determine the anesthetic efficacy of a labial infiltration of a 3.6 mL volume of 4% articaine with 1:100,000 epinephrine compared with labial infiltration (1.8 mL) plus lingual infiltration (1.8 mL) of 4% articaine with 1:100,000 epinephrine in the mandibular lateral incisor. One hundred subjects randomly received 2 sets of injections, using 4% articaine with 1:100,000 epinephrine, consisting of labial and lingual infiltrations of 1.8 mL (3.6 mL total) and 2 labial infiltrations of 1.8 mL (3.6 mL total) of the mandibular lateral incisor in 2 separate appointments. Electric pulp testing was used to determine anesthetic success (highest 80/80 reading). The data were analyzed statistically. The labial and lingual combination exhibited a significantly higher anesthetic success rate (97%) when compared with the 2 labial infiltrations (74%) and had significantly higher 80/80s readings from 1 minute to 58 minutes. Within the limitations of this clinical study, a combination labial plus lingual infiltration using a 3.6-mL volume of 4% articaine with 1:100,000 epinephrine significantly increased pulpal anesthetic success for the mandibular lateral incisor when compared with a labial infiltration using a 3.6-mL volume of articaine. Therefore, location of the infiltrations was more important than volume. [ABSTRACT FROM AUTHOR]

Dentigerous cysts involves erupted or developing teeth in decreasing order of frequency as mandibular third molars, the maxillary canines, the maxillary third molars with rare involvement of the central incisors & are usually asymptomatic but becomes symptomatic with symptoms such as swelling, mild sensitivity, tooth mobility and displacement when it reaches size > 2 cm in diameter. Dentigerous cysts are seen most commonly in 20–30 years age group with relative low frequency in children with proportion of 4–9% of these cysts occur in the first 10 years after birth. Hospital based study conducted in the Department of ENT at Ashwini Rural medical college & Hospital, Solapur in which we have reported a series of three cases of dentigerous cyst in paediatric patients. Dentigerous Cysts in our study were associated with maxillary permanent lateral incisor, mandibular permanent premolar, Mandibular second molar tooth in pediatric age group. Provisional diagnosis of Dentigerous cyst based on clinical, radiological & biochemical study but Histopathological examination confirms the diagnosis. Dental orthopentamogram aids in diagnosing and localizing the Dentigerous cyst and ct face in all views helps to see bony effects and complications due to expansion of Dentigerous cyst. Enucleation of the cystic lesion followed by extraction of the offending teeth is a suitable treatment modality for Dentigerous cyst associated with maxilla and mandible giving adequate balance between complete removal of cyst & giving chance for eruption of unerupted teeth. Palatogingival grooves (PGGs) are developmental anomalies well known for causing endodontic and periodontal diseases to the teeth that have no evidence of caries or trauma. It is a rare developmental defect that frequently exhibits the palatal aspect of maxillary anterior teeth. The aim of this case report is to diagnose PGG using cone-beam computed tomography (CBCT) and manage it with an interdisciplinary approach by endodontic and periodontic intervention. A 20-year-old healthy female patient came with a chief complaint of foul smell and discharge of pus from the gum region. During clinical examination, a sinus opening and deep periodontal pocket were noticed with respect to the maxillary left lateral incisor. The tooth showed mild tenderness to percussion and a negative response to thermal and electric pulp testing. The intraoral periapical radiograph showed a vertical radiolucent line with periapical radiolucency, giving suspicion of PGG, which was confirmed by CBCT investigation. The diagnosis of pulp necrosis with chronic periapical abscess was made. After root canal treatment, surgical intervention was done and sealed the groove with Biodentine. This case report shows the successful outcome of a tooth associated with PGG after a 9-month follow-up. The tooth associated with this anomaly can be correctly diagnosed and planned with CBCT, and successfully treated using an interdisciplinary approach with a bioactive restorative material. Kata kunci: perawatan endodontik satu kunjungan; insisivus sentral; insisivus lateral; pulpitis ireversibel; periodontitis apikalis ABSTRACT Introduction: Consideration in determining the number of visits for root canal treatment (single-visit or multiple visits) has been discussed among clinicians for a considerable time. Using the appropriate armamentarium and technique allows one-visit root canal treatment. The purpose of this case report is to describe one-visit root canal treatment of two incisors as prosthetic preparation. Case report: A 28-year-old female came with a referral from the Department of Prosthodontics Universitas Padjadjaran Dental Hospital for root canal treatment for prosthetics preparation. Tooth 21 and 22 were diagnosed as symptomatic irreversible pulpitis with symptomatic apical periodontitis. Root canal treatment was performed on both teeth respectively in one visit. The patient was referred back to Prostodontist for definitive restoration. Conclusion: One-visit root canal treatment can be selected as a treatment plan for prosthetic preparation of teeth that need endodontic treatment with aesthetic involvement because of the lower risk of infection in a cleaned and shaped root canal. Fewer appointments and prosthetic restoration can be made sooner considering various aspects such as operator, patient, armamentarium, and difficulty level of the case. Dens invaginatus (DI) often causes dysmorphic changes in both the crown and root. This case report presents a complicated type 3a DI in a maxillary lateral incisor with labial talon cusp and root bifurcation. Although lingual talon cusp is common in maxillary lateral incisor, labial talon cusp is rare. An auxiliary palatal root in maxillary lateral incisor is also unusual. No such case involving all three variations has been described in the literature. The DI was classified as type B4 according to Schulze and Brand, as it involved division of pulp and root. DI was managed by orthograde MTA, radisectomy and periodontal regeneration was done for the palatal root. Labial talon associated with DI and can lead to early periodontal/pulpal involvement. Type 3 DI can affect the root with marked dilatation and division. Additional palatal root should be carefully detected in type 3a DI and managed with the aid of CBCT. [ABSTRACT FROM AUTHOR]

The odontogenic tissue surrounding an impacted tooth is a harbinger of a wide variety of odontogenic cysts and tumors. As these hamartomatous and neoplastic aberrations can occur at any stage of odontogenesis, therefore combined features of odontogenic tumors with epithelial and mesenchymal components can develop within the odontogenic cysts. One of the odontogenic cysts is the dentigerous cyst (DC) which is not only the most common developmental odontogenic cyst arising from the pericoronal tissue but is of great clinical significance owing to its neoplastic potential and association with other rare odontogenic tumors. We herein describe an unusual case of adenomatoid odontogenic tumor which developed secondary to a DC in a 22-year-old female and created a diagnostic conundrum. The approach to such a case, differential diagnosis, and review of the literature is also presented.

Transpositions are rare anomalies where there is an exchange of positions between two adjacent teeth. Treatment, ideally involves alignment of teeth, consideration for aesthetic, social and functional purposes. This article/case report describes transposition in two sisters of age 11 and 14 year and discusses the treatment possibilities for aesthetic and functional needs. The treatment considerations encompass comprehensive caries management: restorative and endodontic treatments including a gutta percha obturation of a pulpally involved primary molar for delaying the resorption, preventive care and regular follow-ups until definitive treatment. CLEFT lip , *DENTITION , *SUPERNUMERARY teeth , *CLEFT palate , *LOGISTIC regression analysis , *INCISORS - Abstract
Objectives: Here, we retrospectively investigated cases of bilateral oral clefts (OCs) to determine the clinical relevance of detailed distinction of incomplete cleft lip subphenotypes, based on morphological severity of the cleft, within the categories cleft lip with or without alveolus (CL ± A) and cleft lip, alveolus, and palate (CLAP). We further assessed possible associations between CL subphenotypes (complete vs different incomplete types) and different dentition patterns of the lateral incisor. Materials and methods: Our analysis included 151 non-syndromic Caucasian bilateral OC-patients (8–20 years old) from the Dutch Association for Cleft Palate and Craniofacial Anomalies registry. Six different deciduous and permanent lateral incisor patterns were distinguished: normal position (z/Z), supernumerary lateral incisor (n/N), presence in the anterior (x/X) or posterior (y/Y) segment of the cleft, one in each cleft segment (xy/XY), and agenesis (ab/AB). Logistic regression was performed to show the associations between the CL subphenotypes and dentition patterns of the lateral incisor. Results: One hundred three had complete, while 48 had incomplete CLs. Patterns z/Z and n/N were associated with a submucous/vermillion notch, incomplete CL, and intact alveolus. Patterns x/X, y/Y, and xy/XY were most common in patients with two-thirds to subtotal CL and complete CL. The most severe pattern, ab/AB, was most commonly associated with complete CL. Conclusions: Based on the morphological severity of the CLs, it can be stated that the more severe the CL in bilateral CL ± A and CLAP, the more severe the abnormal pattern of the dentition. Clinical relevance: Further distinction of incomplete cleft lip subphenotypes (submucous/vermillion notch, one-third to two-thirds CL, two-thirds to subtotal CL) in bilateral CL ± A and CLAP has clinical relevance. Fusion is a congenital disturbance affecting primary dentition mostly during morpho differentiation of the primary tooth germs. Fusion leads to the union of two or more primary teeth by the enamel and dentin while the pulp and roots remain separate. These abnormalities may be unilateral or bilateral. Prompt diagnosis and a treatment plan in such anomalies may help to overcome problems concerning aesthetics, caries susceptibility and space management. This report describes a case of unilateral fusion of the primary mandibular lateral incisor and canine and aims to evaluate any associated pathology.

ABSTRACT Introduction: Endodontic treatment failure generally results from inadequate obturation and leakage at the final restoration. This failure can be relieved by re-root canal treatment. Such treatment in geriatric patients requires various management considerations according to the patient’s physiological and systemic conditions. This case report was aimed to describe the non-surgical root canal re-treatment of maxillary right lateral incisor in a geriatric patient. Case report: A 69-years-old woman came to the Conservative Dentistry Specialist Outpatient Unit of the Universitas Padjadjaran Dental Hospital (RSGM Unpad) to repair a broken maxillary right front tooth fill and to treat the tooth due to the uncomfortable feeling each time the mastication was performed. The tooth was treated with root canals and tooth-coloured fillings about four years ago. Intraoral examination revealed positive percussion of the maxillary right lateral incisor. The periapical radiograph showed inadequate root canal filling, dilation of the periodontal membrane in the third apical, and severed lamina dura in the third apical. The treatment plan was non-surgical root canal re-treatment. The treatment took place in six short sessions, with the steps of gutta-percha collection using a Hedstrom file, the root canal preparation with a rotary instrument, and the root canal’s obturation with gutta-percha plus resin sealer and final restoration of a composite direct veneer. Conclusion: Non-surgical root canal treatment of the upper right lateral incisor was successfully performed as indicated by the absence of the patient’s complaints, improvement in the radiographic image, and the teeth’ normal function. The treatment's success is influenced by a proper treatment plan considering the aetiological factors of endodontic treatment failure, the physiological changes, and pathological conditions present in the patient. Keywords: Root canal re-treatment, lateral incisor, geriatric patient.

This study aimed to identify the location of root resorption in relation to an impacted maxillary canine and grade its severity using cone-beam computed tomography (CBCT) in the Saudi Arabian population. Materials and Methods: CBCT scans of 169 patients with maxillary canine impaction were evaluated. The location and the severity of root resorption of the affected tooth in relation to the impacted maxillary canine were recorded for each patient. Results: a total 204 impacted maxillary canines caused root resorption in 218 adjacent teeth. Maximum root resorption was present in 63.3% of the apical one-third of the root and 37.6% of the palatal surfaces. There was mild root resorption in 55%, moderate in 10% and severe in 35%. There was no statistically significant difference between the gender, age, type of impaction, side of impaction, and the number, location, or degree of root resorption. Multiple logistic regression models showed significant association (p = 0.024) between gender, type of impaction, and root resorption. A significant correlation was found between the level and the surface of the root resorption (p = 0.018). Conclusion: In the Saudi population, apical one-third root levels and palatal surfaces were primarily involved in root resorption caused by impacted canines. The females with bilateral canine impaction were more likely affected by root resorption. [ABSTRACT FROM AUTHOR]

Talon cusp is a rare dental anomaly in which a cusp-like structure projects from the cingulum area or cementoenamel junction of the maxillary or mandibular anterior teeth. This unusual cusp resembles the talon of an eagle. It can be found in both the primary and permanent dentitions. A comprehensive literature study reveals that only 37 examples of talon cusps in the primary dentition have been described, with only 3 cases reporting this anomaly on the primary maxillary lateral incisor. We present a male patient with cleft lip and palate having primary lateral incisor talon cusp. The clinical implications of this anomaly, as well as the dispute over its etiology, are reviewed. - Abstract
Knowledge of dental anomalies and variations is a prerequisite for having a better prognosis after endodontic treatment. In this case report, a patient having birooted maxillary lateral incisor with periapical pathosis has been discussed. Due to lack of magnification and identification of two roots in intraoral periapical radiograph, cone-beam computed tomography was advised which help in diagnosis and location of the accessory canal of maxillary lateral incisor. With proper diagnosis and standard endodontic protocol, successful outcome of endodontic treatment can be achieved. [ABSTRACT FROM AUTHOR]

Palatoradicular grooves are usually found on the palatal or lateral roots of maxillary central and lateral incisors. Since clinical identification of these grooves are inaccessible and arduous in routine oral hygiene practices and are susceptible alcoves for microorganism habituation and plaque accumulation, it may result in acute to severe periodontitis and, if untreated, periapical pathosis also. This paper discusses about a female patient who reported pus discharge in left upper lateral incisor. Based on history, clinical examination and IOPA (intra-oral periapical radiograph), a deep pocket of about 10–14 mm was noticed in the left upper lateral incisor. A timely investigation was made and was treated surgically with advanced modified procedures which include odontoplasty and restoration of the defect with Platelet Rich Fibrin. After follow up of 6 months, the patient reported no signs of disease progression, had good oral hygiene and the tooth remained to be vital.

Objectives: In this retrospective study, the frequency and severity of ectopic eruption in the children's permanent canines, changes between the impacted canine parameters on orthopantomography (OPG) taken for routine follow-up were demonstrated. Also, it was aimed to reveal the differences between OPG and cone beam computed tomography (CBCT). Methods: Ten thousand (5020 girls, 4980 boys) pediatric patient, were retrospectively investigated for the presence of impacted canine radiographically. OPG and CBCT images of 343 pediatric patients (207 girls, 136 boys) detected impacted canines (n = 432) were evaluated in four steps that the angular and linear measurement, position and status relationships with adjacent anatomical structures were investigated. Results: The incidence of impacted canine in children was 3.43% (4.02% girls, 2.73% boys). The percentage of adjacent lateral incisor' resorption was 33%. Although the presence of resorption in lateral incisors in contact with a canine was found to be statistically significant (p = 0.02), there was no statistically significant relationship the follicle size and localization of canine in the transverse plane. When images were compared, it was found that there was a statistically significant difference between CBCT and OPG about the parameter of canine angulation to the midline and parameter of canine angulation to the lateral incisor. Conclusions: Pediatric dentists should prefer OPG as the first choice for radiographic imaging for the diagnosis of impacted canine at an early age. Follow-up OPGs should be carefully studied with advancing age. The CBCT should be preferred to prevent complications, to view its relationship with adjacent anatomical structures and canine localization in detail. [ABSTRACT FROM AUTHOR]

Dens in dente is known as a growth disorder caused by inversion of the coronary tissue into the pulp chamber prior to mineralization; it can be constrained to the crown or extent the root and influence the periapical area. The condition seen in radiographic image like a tooth within a tooth. Although it happens rarely, it is the most frequent malformation detected in human dentition. It can be specially observed in maxillary lateral and then central incisors. Because of the complexity as well as strange anatomy, the tooth has the potential to show pulp necrosis and open apex, which may bring about some challenges to manage it. An early detection of the malformation is critical for better handling and makes treatment plan more conservative like restorative procedures. We describe the management of an uncommon case of type III dens in dente diagnosed using cone-beam CT scan in a maxillary lateral incisor, having a chronic periapical lesion, buccal sinus tract as well as palatal expansion healed successfully after treating with non-surgical root canal procedure.

[ABSTRACT FROM AUTHOR]

- Abstract
Aim: The aim of the present research was to assess the mesiodistal angulation of the maxillary anterior teeth utilizing Image J computer software, a Profile projector, and a Custom-made jig., Materials and Methods: A total of 34 subjects (17 males and 17 females) were chosen from a group of 18-30 years old with bilateral Angle Class I molars and canine relationships. One manual approach (Custom-made jig) and two digital methods (J computer software, a Profile projector) were used to record the mesiodistal angulation in incisal view. The individuals had alginate impressions made, and a facebow was used to capture the maxilla's spatial relationship with the cranium. The articulated cast with the help of mounting ring moved to the specially customized jig, then the angulations was measured in the incisal view after the casts were placed in a semi-adjustable articulator. Data were recorded and statistically analyzed., Results: The mesiodistal angulation in the incisal view via three methods between the 17 males and 17 females has statistically significant different. Although the mesiodistal angulation for maxillary lateral incisor and canine did not show any statistically significant difference, the maximum and minimum values obtained were always greater in males in comparison with the females. This indicates that the positions of six maxillary anterior teeth in the males resulted in the creation of upward sweep of incisal edges of central and lateral incisors which was also referred to as "smiling line" producing masculine surface anatomy more squared and vigorous while feminine surface anatomy being more rounded, soft, and pleasant. There was no statistically significant difference between the right and left sides, indicating bilateral arch symmetry and the symmetrical place of the right teeth compared with the left side's corresponding teeth., Conclusion: On conclusion, according to the current study's findings, all three approaches can measure the mesiodistal angulations of maxillary anterior teeth in incisal view with clinically acceptable accuracy. The digital methods, which included using the Image J computer software and the profile projector, achieved more accurate results than the manual method., Clinical Significance: The outcomes of this study's mesiodistal angulations can be used as a reference for placing teeth in both fully and partially edentulous conditions. This study contributes to a better understanding of the importance of achieving the ideal occlusion in the Indian population by placing the maxillary anterior teeth at the proper mesiodistal angulation. Background: Bolton’s two main ratios describing the proportional size of upper and lower teeth, could contribute to estimating the excess or deficiency of tooth size necessary to obtain an ideal occlusion. However, the mean Bolton values are not the same among different societies. Determining the prevalence of tooth size deviations from population-specific Bolton indices might help local orthodontists to have a more concise treatment plan. Objective: The study aimed to define the prevalence of clinically significant tooth size discrepancies (TSD) in an Iranian population and to evaluate the influence of lateral incisors’ size on this discrepancy. Methods: This cross-sectional study was conducted on study casts of orthodontic patients attending Imam Reza Dental Clinic from September 2008 to December 2016. The sample comprised of 150 randomly selected pretreatment study casts (64 males and 86 females from 17 to 28). The mesiodistal diameter of all permanent teeth from the first molar on the right to the first molar on the left was measured using 2 similar digital calipers, and Bolton analysis was calculated. Subjective visual estimation of Bolton discrepancy was also performed. SPSS v18.0, Wilcoxon signed ranks test, Pearson correlation and Receiver Operating Characteristic (ROC) curve analysis were used for statistical analysis. A p

Dens invaginatus is a developmental anomaly resulting from an invagination in the surface of a tooth crown before its calcification. It involves more commonly maxillary anterior teeth. Dens invaginatus with supernumerary teeth and double dens invagintus were also reported. The management of dens invaginatus includes simple prophylactic restoration to conventional endodontic treatment or extraction, depending on the type of invagination, function, esthetics, and morphology of the root canal. Extraction is indicated as a last choice of treatment in cases of failure of root canal treatment and in supernumerary teeth associated with dens invaginatus. In this paper, we have reported a series of cases of dens invagintus.

Dens invaginatus (DI or dens in dente) and Talons cusp or dens evaginatus are relatively rare developmental anomalies which affect the shape of teeth. DI is a deep surface invagination of the crown or root which is commonly detected on routine radiographic examination. Talon cusp is an additional cusp that projects predominately from the lingual surface of anterior teeth. These anomalies most commonly may be observed in the maxillary lateral tooth. Concurrently, the coexistence of these two anomalies in a single tooth is considered extremely rare. However, still in literature, there are a few reports cases available. This article is a case report of Talons cusp with DI involving maxillary lateral incisors in a 16-year-old patient.

Based on history, clinical examination and IOPA (intra-oral periapical radiograph), a deep pocket of about 10-14 mm was noticed in the left upper lateral incisor. A timely investigation was made and was treated surgically with advanced modified procedures which include odontoplasty and restoration of the defect with Platelet Rich Fibrin. After follow up of 6 months, the patient reported no signs of disease progression, had good oral hygiene and the tooth remained to be vital. [ABSTRACT FROM AUTHOR]

The compromised tooth structure after endodontic treatment necessitates the use of post and core for structural durability. Management of the flared root canal is still a tough obstacle for the clinicians. Reinforcing the root canal space with the customised anatomic post thus regains the lost structure within the canal and aids in establishing a great strength and stability to the tooth structure. The report of this case scenario aims on clinical technique of fabricating anatomic post with glass - fibre posts and composite resin on a central and lateral incisor.

Fusion is a developmental anomaly which occurs due to a union of one or more adjacent teeth during morphodifferentiation of the dental germs. The prevalence of tooth fusion is estimated at 0.5%–2.5% in the primary dentition. These anomalies may be unilateral or bilateral and may affect either dentition although the deciduous teeth are more commonly affected. Early diagnosis of such condition is important because it may cause clinical problems, such as esthetic concerns, and caries. This report describes a case of unilateral fusion of the primary mandibular lateral incisor and canine and aims to evaluate any associated pathology.

OBJECTIVE: To determine the frequency of congenitally missing maxillary lateral incisors (LIs) and to find out its variability in relation to gender. MATERIALS AND METHODS: A retrospective study was carried out between January 2017 and December 2017. Orthopantomographs (OPGs) and lateral cephalographs record search of all orthodontic adolescent patients aged 12 to 18 years were taken from the archival records of the department. Orthopantomographs helped to diagnose the presence of unilateral/bilateral maxillary lateral incisors while the ANB angle was calculated from the lateral cephalographs to divide the subjects into various skeletal malocclusions. STATISTICAL ANALYSIS USED: The statistical analysis was done using the Statistical Package for Social Sciences (SPSS version 17.0). The frequencies were compared with the help of the Chi-square test.
