- Abstract
Background: The European Association of Urology (EAU) recommends discussing upfront radical cystectomy for all patients with very high risk (VHR) non-muscle-invasive bladder carcinoma (NMIBC), but the role of bacillus Calmette-Guérin (BCG) treatment remains controversial., Objective: To analyze oncological outcomes in VHR NMIBC patients (EAU risk groups) treated with adequate BCG., Design, Setting, and Participants: A multi-institutional retrospective study involving patients with VHR NMIBC who received adequate BCG therapy from 2007 to 2020 was conducted., Outcome Measurements and Statistical Analysis: A survival analysis estimated recurrence-free survival (RFS), progression-free survival (PFS), and the cumulative incidence of cancer-specific mortality (CSM) after accounting for other causes of mortality as competing risk events and of the overall mortality (OM). Conditional survival probabilities for 0-4 yr without events were computed. Cox regression assessed the predictors of oncological outcomes., Results and Limitation: A total of 640 patients, with a median 47 (32-67) mo follow-up for event-free individuals, were analyzed. High-grade RFS and PFS at 5 yr were 53% (49-57%) and 78% (74-82%), respectively. The cumulative incidence of CSM and OM at 5 yr was 13% (10-16%) and 16% (13-19%), respectively. Conditional RFS, PFS, overall survival, and cancer-specific survival at 4 yr were 91%, 96%, 87%, and 94%, respectively. Cox regression identified tumor grade (hazard ratio [HR]: 1.54; 1.1-2) and size (HR: 1.3; 1.1-1.7) as RFS predictors. Tumor multiplicity predicted RFS (HR: 1.6; 1.3-2), PFS (HR: 2; 1.2-3.3), and CSM (HR: 2; 1.2-3.2), while age predicted OM (HR: 1.48; 1.1-2)., Conclusions: Patients with VHR NMIBC who receive adequate BCG therapy have a more favorable prognosis than predicted by EAU risk groups, especially among those with a sustained response, in whom continuing maintenance therapy emerges as a viable alternative to radical cystectomy., Patient Summary: Our research shows that a sustained response to bacillus Calmette-Guérin in patients can lead to favorable outcomes, serving as a viable alternative to cystectomy for select cases., (Copyright © 2024 European Association of Urology. - Abstract
Objective: Retrospective assessment of periapical health in relation to sex, age and quality of root-canal treatment among adult patients of the Institute of Dentistry in Lódź. Orthopantomograms of 439 subjects were assessed considering the prevalence of endodontic treatment, technical quality of root-canal fillings and the state of periapical tissues. The assessment of orthopantomograms of 174 (39.6%) men and 265 (60.4%) women was carried out. A significant increase (p=0.016) in the mean number of endodontically treated teeth according to age (range from 1.4 in subjects under the age of 20 to 3.4 in those aged 70 years and over) and the mean number of teeth with periapical pathosis (from 0.9 to 2.4, respectively) (p=0.020) was observed. Of 10054 radiographically examined teeth 9.7% was treated endodontically, whereas, 5.5% exhibited periapical pathosis. The percentage of teeth with periapical lesions was significantly higher (p=0.015) in men (6.2%) as compared to women (5.0%). Periapical radiolucency occurred significantly more often (p=0.0001) in endodontically treated teeth (36.4% of cases) than in those with untreated root canals (2.1%). Teeth with inadequately filled root canals significantly more often revealed the presence of periapical pathosis (49.6%) in comparison to teeth with adequately filled root canals (4.2%)., Conclusions: The prevalence of endodontic treatment and apical periodontitis increases with age. The prevalence of apical periodontitis depends on sex - periapical lesions were more often found in men. The quality of endodontic treatment significantly affects periapical health.

- Abstract
Background/aims: One of the consequences of inflammatory diseases and neoplastic transformation is disregulation in metabolism of collagen and its interaction with cell surface integrin receptors. Although extracellular collagenases initiate the breakdown of tissue collagen the final step of collagen degradation is mediated by prolidase (E.C.3.4.13.9). We investigated whether prolidase activity may reflect disturbances of collagen metabolism in human pancreatitis and pancreatic cancer., Methodology: Ten human samples of advanced chronic pancreatitis and pancreatic cancer and ten samples of normal pancreas were compared in respect to prolidase activity and expression (western immunoblot), collagen content (hydroxyproline determination), collagenolytic activity (zymography) and beta 1 integrin subunit expression (western immunoblot)., Results: In pancreatitis tissue, an increase in collagen content, collagenolytic activity and beta 1 integrin expression were accompanied by significant decrease (about 2-fold) in prolidase activity and marked decrease in its expression, compared to normal pancreas. Pancreatic cancer tissue, compared to normal pancreas showed insignificant increase in collagen content, dramatic increase of collagenolytic activity and undetectable beta 1 integrin expression. These phenomena were accompanied by over 10-fold decrease in prolidase activity and marked decrease in prolidase expression., Conclusions: The results suggest that prolidase activity may reflect the extent of disturbances in tissue collagen metabolism during pancreatic diseases and the level of its activity may serve as a marker of chronic pancreatitis and pancreatic cancer.

- Abstract
Surgical treatment of goitre in children and teenagers under 18 is relatively rare and undertaken for strict therapeutic indications. It still arouses much controversy among surgeons, being loaded with a high percentage of complications, usually distant ones. The aim of the study was to analyse the surgical treatment of various goitre types under the age of 18 basing on 65 operations performed in the years 1994-1998. Operations for non-toxic and hyperactive nodular goitre, including Graves-Basedow disease, were predominant. Five cases of thyroid carcinoma were found. No serious complications were observed during and after the surgery. Our own material and the 5-year observation reveal that surgical treatment of thyroid diseases in children and adolescents carried out for definite indications by an experienced operating team is successful and causes only few complications.

- Abstract
The aim of the study was presentation of our experiences in the diagnostics and treatment of pancreatic carcinoma in confrontation with current opinions. Between 1983 and April 1998 308 patients (198 males and 110 females with mean age 61 years) were treated at our department. In 211 cases tumor was primarily localized in the head and in 97 cases (31.5%) in the body or tail of pancreas. The diagnosis was determined on the basis of clinical symptoms and laboratory investigations. Diagnostic accuracy of performed investigations was as following. Ultrasonography-86%, CT-scan-95%, and ERCP-94%. Ultrasonography or CT guided biopsy and serodiagnostics improved detectability of resectionable carcinoma in the last past years. Among 308 patients, 292 underwent surgery. Since 1997 an intraoperative ultrasonography and cholangioscopy of common bile duct and Virsung were performed as a routine. The procedures depended on the localization and the grade of advance of the tumors. UICC classification of pancreatic tumors, pTNM (4th edition 1987) it was used. Only in 25 patients (8%) (tumors pT1a-bN0M0) it was possible to perform radical operation (resection of the pancreas). However in 172 patients (56%) only palliative procedures were done (pT2N1M0). In 95 patients (31%) only laparotomy and biopsy were performed (tumors pT2N1M1). The study shows that although diagnostic methods are improved, the tumors of the pancreas are diagnosed in stages making unable the radical procedures. Only about 10% of carcinomas are resectable. It is caused by with non-characteristic picture of the disease in its early stage.

- Abstract
The authors present their own experience in diagnosing and surgical treatment of adrenal tumors. The group consisted of 52 patients with adrenal tumors treated surgically in our Department in 1980-1996. There were 37 women (71%) and 15 men (29%); 22 with Cushing syndrome, 14 with pheochromocytoma, 4 with Conn's syndrome, 1 with virilizing tumor, 1 with adenocarcinoma, 1 with adrenocortical carcinoma, 1 with ganglioneuroblastoma 2 with adenomas, 6 with adrenal cyst. Diagnosis pre- and intraoperative was verified with histological examination. We suggest that the ultrasonography and computed tomography are the most useful methods in determining of tumor localisation. The authors prefer intraperitoneal approach, enabling evaluation of the contralateral adrenal gland and possible localisation of extraadrenal foci. Good results were obtained in 51 patients (98%).
