Enhanced recovery after surgery (ERAS) pathways are now implemented worldwide with strong evidence that adhesion to such protocol reduces medical complications, costs and hospital stay. This concept has been applied for pancreatic surgery since the first published guidelines in 2012. This study presents the updated ERAS recommendations for pancreatoduodenectomy (PD) based on the best available evidence and on expert consensus. A systematic literature search was conducted in three databases (Embase, Medline Ovid and Cochrane Library Wiley) for the 27 developed ERAS items. Quality of randomized trials was assessed using the Consolidated Standards of Reporting Trials statement checklist. The level of evidence for each item was determined using the Grading of Recommendations Assessment Development and Evaluation system. The Delphi method was used to validate the final recommendations. A total of 314 articles were included in the systematic review. Consensus among experts was reached after three rounds. A well-implemented ERAS protocol with good compliance is associated with a reduction in medical complications and length of hospital stay. The highest level of evidence was available for five items: avoiding hypothermia, use of wound catheters as an alternative to epidural analgesia, antimicrobial and thromboprophylaxis protocols and preoperative nutritional interventions for patients with severe weight loss (> 15%). The current updated ERAS recommendations for PD are based on the best available evidence and processed by the Delphi method. Prospective studies of high quality are encouraged to confirm the benefit of current updated recommendations.

32. Features of steady magnetospheric convection

Abstract

The large-scale patterns of ionospheric convection and particle precipitation are described during two intervals of steady magnetospheric convection (SMC) on November 24, 1981. The unique data set used in the analysis includes recordings from the worldwide network of magnetometers and all-sky cameras, global auroral images from the Dynamics Explorer (DE) 1 spacecraft, and particle precipitation data from low-altitude National Oceanic and Atmospheric Administration (NOAA) 6 and NOAA 7 spacecraft. The data show that intense magnetospheric convection continued during more than 10 hours under the steady southward interplanetary magnetic field without any distinct substorm signatures. All data sets available confirmed the stable character of the large-scale magnetospheric configuration during this period. In particular, the magnetic flux threading the polar cap was stable (within 10%) during 3.5 hours of continued DE 1 observations. The dayside cusp was located at an unusually low latitude (70 deg CGL). The nightside auroral pattern consisted of two distinct regions. The diffuse aurora in the equatorward half of the expanded (10 deg wide) auroral oval was well-separated from the bright, active auroral forms found in the vicinity of the poleward boundary of the oval. The twin-vortex convection pattern had no signature of the Harang discontinuity; its nightside 'convection throat' was spatially coincident with the poleward active auroras. This region of the auroral oval was identified as the primary site of the short-lived transient activations during the SMC intervals. The energetic particle observations show that the auroral precipitation up to its high-latitude limit is on closed field lines and that particle acceleration up to greater than 30-keV energy starts close to this limit. The isotropic boundaries of the greater than 30-keV protons and electrons were found close to each other, separating regions of discrete and diffuse precipitation. This suggests that these precipitation types originate on the very taillike and very dipolelike field lines, respectively.

Abstract

MOTIVATION: The fact that the multiple sequence alignment problem is of high complexity has led to many different heuristic algorithms attempting to find a solution in what would be considered a reasonable amount of computation time and space. Very few of these heuristics produce results that are guaranteed always to lie within a certain distance of an optimal solution (given a measure of quality, e.g. parsimony). Most practical heuristics cannot guarantee this, but nevertheless perform well for certain cases. An alignment, obtained with one of these heuristics and with a bad overall score, is not unusable though, it might contain important information on how substrings should be aligned. This paper presents a method that extracts qualitatively good sub-alignments from a set of multiple alignments and combines these into a new, often improved alignment. The algorithm is implemented as a variant of the traditional dynamic programming technique. RESULTS: An implementation of ComAlign (the algorithm that combines multiple alignments) has been run on several sets of artificially generated sequences and a set of 5S RNA sequences. To assess the quality of the alignments obtained, the results have been compared with the output of MSA 2.1 (Gupta et al., Proceedings of the Sixth Annual Symposium on Combinatorial Pattern Matching, 1995; Kececioglu et al., http://www.techfak.uni-bielefeld. de/bcd/Lectures/kececioglu.html, 1995). In all cases, ComAlign was able to produce a solution with a score comparable to the solution obtained by MSA. The results also show that ComAlign actually does combine parts from different alignments and not just select the best of them. It has recently been suggested that the solar irradiance has varied in phase with the 80- to 90-year period represented by the envelope of the 11-year sunspot cycle and that this variation is causing a significant part of the changes in the global temperature. This interpretation has been criticized for statistical reasons and because there are no observations that indicate significant changes in the solar irradiance. A set of data that supports the suggestion of a direct influence of solar activity on global climate is the variation of the solar cycle length. Data were retrieved from standard databases and personal archives. Evidence and recommendations were classified according to the GRADE system and reached through consensus in the group. The quality of evidence was rated "high", "moderate", "low" or "very low". Recommendations were graded as "strong" or "weak". RESULTS: Comprehensive guidelines are presented. Available evidence is summarised and recommendations given for 27 care items. The quality of evidence varies substantially and further research is needed for many issues to improve the strength of evidence and grade of recommendations. CONCLUSIONS: The present evidence-based guidelines provide the necessary platform upon which to base a unified protocol for perioperative care for pancreaticoduodenectomy. A unified protocol allows for comparison between centres and across national borders. It facilitates multi-institutional prospective cohort registries and adequately powered randomised trials.

Contains fulltext : 108907.pdf (publisher's version ) (Open Access), BACKGROUND: The use of lLaparoscopic liver resection in terms of time to functional recovery, length of hospital stay (LOS), long-term abdominal wall hernias, costs and quality of life (QOL) has never been studied in a randomised controlled trial. Therefore, this is the subject of the international multicentre randomised controlled ORANGE II trial. METHODS: Patients eligible for left lateral sectionectomy (LLS) of the liver will be recruited and randomised at the outpatient clinic. All randomised patients will undergo surgery in the setting of an ERAS programme. The experimental design produces two randomised arms (open and laparoscopic LLS) and a prospective registry. The prospective registry will be based on patients that cannot be randomised because of the explicit treatment preference of the patient or surgeon, or because of ineligibility (not meeting the in- and exclusion criteria) for randomisation in this trial. Therefore, all non-randomised patients undergoing LLS will be approached to participate in the prospective registry, thereby allowing acquisition of an uninterrupted prospective series of patients. The primary endpoint of the ORANGE II trial is time to functional recovery. Secondary endpoints are postoperative LOS, percentage readmission, (liver-specific) morbidity, QOL, body image and cosmetic result, hospital and societal costs over 1 year, and long-term incidence of incisional hernias. It will be assumed that in patients undergoing laparoscopic LLS, length of hospital stay can be reduced by two days. A sample size of 55 patients in each randomisation arm has been calculated to detect a 2-day reduction in LOS (90% power and alpha = 0.05 (two-tailed)).The ORANGE II trial is a multicenter randomised controlled trial that will provide evidence on the merits of laparoscopic surgery in patients undergoing LLS within an enhanced recovery ERAS programme.
