40. Quantitative analysis of (-)-N-11C-propyl-norapomorphine in vivo binding in nonhuman primates

Abstract

(-)-N-11C-propyl-norapomorphine (11C-NPA) is a new dopamine agonist PET radiotracer that holds potential for imaging the high-affinity states of dopamine D2-like receptors in the living brain. The goal of this study was to develop and evaluate analytic strategies to derive in vivo 11C-NPA binding parameters. Methods: Two baboons were scanned 4 times after 11C-NPA injections. The metabolite-corrected arterial input functions were measured. Regional brain time-activity curves were analyzed with kinetic and graphical analyses, using the arterial time-activity curve as the input function. Data were also analyzed with the simplified reference-tissue model (SRTM) and graphical analysis with reference-region input. Results: 11C-NPA exhibited moderately fast metabolism, with 31% ± 5% of arterial plasma concentration corresponding to the parent compound at 40 min after injection. Plasma clearance was 29 ± 1 L/h, and plasma free fraction (f1) was 5% ± 1%. For kinetic analysis, a 1-tissue compartment model (1TCM) provided a good fit to the data and more robust derivations of the tissue distribution volumes (VT, in mL/g) than a 2-tissue compartment model (2TCM). Using 1TCM, VTs in the cerebellum and striatum were 3.4 ± 0.4 and 7.5 ± 2 mL/g, respectively, which led to estimates of striatal binding potential (BP) of 4.0 ± 1.1 mL/g and striatal equilibrium specific-to-nonspecific partition coefficient (V3″) of 1.2 ± 0.2. VT values derived with graphical analysis were well correlated with but slightly lower than VT values derived with kinetic analysis. V3″ values derived with SRTM were well correlated with but slightly higher than V3″ values derived with kinetic analysis. Using any method, a significant difference was detected in BP and V3″ values between the 2 animals. It was determined that 30 min of scanning data were sufficient to derive V3″ values using kinetic, graphical (arterial input and reference-region input), and SRTM analyses. Conclusion: This study indicates that 11C-NPA is a suitable PET tracer to quantify the agonist high-affinity sites of D2-like receptors.

43. PET imaging of dopamine-D2 receptor internalization in schizophrenia

Abstract

Recent genetic, molecular and post-mortem studies suggest impaired dopamine (DA)-D2 receptor (D2R) trafficking in patients with schizophrenia (SZ). Imaging and preclinical studies have shown agonist-induced D2R internalization can be imaged with positron emission tomography (PET) using D2R radiotracers combined with psychostimulant challenge. This is feasible if radiotracer binding is measured when postchallenge DA levels have returned to baseline, following the initial competition phase between DA and radiotracer for binding to D2R. Here we used ‘late-phase' imaging after challenge to test the hypothesis that impaired D2R internalization in SZ leads to blunted late-phase displacement, or a faster return to baseline, in patients compared with healthy controls (HCs). We imaged 10 patients with SZ and 9 HCs with PET and [11C]raclopride at baseline and two times (3–5 and 6–10 h) following 0.5 mg kg−1dextroamphetamine. We measured binding potential relative to non-displaceable compartment (BPND) and derived percent reduction from baseline (ΔBPND) for each postamphetamine scan. To test the hypothesis that time course of return of striatal BPNDto baseline differed between SZ and HCs, we implemented a linear model with ΔBPNDas dependent variable, time after amphetamine as repeated measure and time after amphetamine and diagnostic group as fixed effects. Neither diagnostic group nor interaction of diagnostic group-by-time after amphetamine significantly affected striatal ΔBPND(F=1.38, P=0.26; F=0.51, P=0.61). These results show similar pattern of return of BPNDto baseline as a function of time in patients with SZ and HC, suggesting that striatal D2R internalization as measured by our imaging paradigm is normal in patients with SZ.

Abstract

Iodine-123-beta-CIT is a SPECT radioligand for dopamine and 5-HT transporters with potential use in Parkinson's disease, schizophrenia and cocaine addiction studies. At present, preparation of no-carrier-added (NCA) [123I] beta-CIT is achieved by iododestannylation of a trialkylstannyl precursor with sodium [123I]iodide in the presence of oxidizing agent, followed by preparative HPLC. The purpose of this study was to develop a faster and simpler method for the routine preparation of this radiopharmaceutical.Purification of the labeled compound was accomplished by solid phase extraction (SPE) with a C-18 Sep-Pak Light cartridge, which removed unreacted iodide, reaction reagents, polar side products and tributylstannyl precursor. The tributylstannyl precursor was preferred as starting material over the trimethylstannyl precursor due to its higher lipophilicity, allowing better separation of the labeled product and precursor. A TLC method was developed to assess the radiochemical purity of the final product.The method produced [123I] beta-CIT in high radiochemical yields (75% +/- 4%), with high radiochemical purity (or = 98%) and specific activity (67000 Ci/mmole), in 1.5 hr. The final formulation was sterile and pyrogen free.The results obtained by solid phase extraction are consistent with those obtained by the HPLC method; with the advantage that the SPE method does not require solvent extraction, evaporation under reduced pressure or HPLC purification.
