The use of introspection is unavoidable even in psychological research on animals. Difficulties may arise from the absence of reliable introspective data; it is suggested that this could be the case for the so-called “reward system”. In other circumstances confusion comes from using introspection without being aware of it: speaking and thinking have for many years been considered as the same operation, but the reason for this belief is not clearly documented and it may be suspected of being only introspection. Amongst other evidence, a careful and conscious use of introspection demonstrated by auto-analyses of former aphasic patients strongly suggests that language and thought are quite distinct entities.

Two clinico-pathological cases of Kearns-Sayre syndrome are reported. In both cases the typical triad (progressive external ophthalmoplegia, heart block, retinitis pigmentosa) was present and spongiosis was the main pathological finding. In one case there was also a marked capillary proliferation, significance of which is discussed. A deletion of the mitochondrial DNA was found in the muscle, spinal cord and brain of this last case.

Three patients showed dramatic psychic akinesia after recovery from toxic encephalopathy. They had no or only mild motor disorders. The spontaneous psychic akinesia was reversible when the patient was stimulated, as if there was a loss of self psychic activation. Intellectual capacities were normal. Two patients had stereotyped behaviours resembling compulsions. In all patients CT cans showed bilateral lesions in the basal ganglia, mainly within the globus pallidus. [ABSTRACT FROM AUTHOR]

Motor neglect is characterised by an underutilisation of one side, without defects of strength, reflexes or sensibility. Twenty cases of frontal, parietal and thalamic lesions causing motor neglect, but all without sensory neglect, are reported. It is proposed that the cerebral structures involved in motor neglect are the same as those for sensory neglect and for the preparation of movement. As in sensory neglect, the multiplicity of the structures concerned suggests that this interconnection is necessary to maintain a sufficient level of activity. Predominance of left sided neglect by right sided lesions suggests that the left hemisphere is dominant for deliberate activity; hemispheric dominance could be applied to sensory neglect where conscious awareness would play the role of deliberate activity. A pattern of ultrasonic anomalies diagnostic of dissection is described, including association of a localized increase in diameter of the artery with hemodynamic signs of stenosis or occlusion at the same level and decreased pulsatility and presence of intravascular echoes in the enlarged segment.

Vertebral arteries were studied by Duplex scanning in 50 normal subjects. Pretransverse and C6-C5, C5-C4 intertransverse segments were visualized in all cases on both sides; segment C4-C3 was visualized in 100% of the cases on the right side and in 90% on the left; ostium was obtained in 94% of the cases on the right and in 60% on the left. The left vertebral artery was dominant in 48% of the cases while the right vertebral artery was dominant in 14%. Three vertebral arteries were hypoplasic. Duplex scanning was thus found to be an easily performed noninvasive method to study morphological and hemodynamic characteristics of vertebral arteries from their origin to the C4-C3 level.

A prolactin-secreting pituitary adenoma was removed trans-sphenoidally from a 37 years old man with gigantism (218 cm). Serum levels of prolactin (PRL) were elevated pre-operatively and decreased after administration of l-Dopa with no increase after TRH as is usually observed in PRL-secreting adenomas. Growth hormone (GH) and somatomedin serum levels were normal with no modification of GH after insulin hypoglycaemia, oral glucose loading or l-Dopa. Morphological examination of the tumour demonstrated the presence of lactotrophs by light and electron microscopy and by immunofluorescense staining. No somatotrophs were found. In this unique case, the relationship between a PRL-secreting adenoma and gigantism is discussed.Acromegaly-gigantism or gigantism with documented hormonal and morphological studies has rarely ben reported in the recent literature. The prevalence of internal carotid artery stenosis was studied with continuous wave Doppler and Duplex scanning in 526 subjects aged 45-84 years of age. They were devoid of any cerebrovascular symptoms or signs and were not referred to us because of risk factors or manifestations of atherosclerosis. The prevalence of stenosis, whatever the grade, was low in both sexes aged under 65 years: 2.4% in males aged 55-64 years. It increased sharply with age, reaching 30.3% for stenosis of less than 50% and 6.1% for stenosis of greater than 50% in males aged 75-84 years. The prevalence of minimal lesion (plaque with less than 15% diameter reduction) was high: 32.1% in males aged 45-54 years and 48.5% in the 75- to 84-year age range.

A 59-year-old patient presented with compulsive behaviors and lasting apathy after carbon monoxide intoxication. The apathy could be overcome by external stimulation (self-activation deficit). There was severe neuronal loss bilaterally in the anterior part of the pallidum and in the substantia nigra, pars reticulata. This first clinico-pathological case of a self-activation deficit illustrates the dissociation between motor and behavioral symptoms in lesions of the pallido-nigral complex, with the behavioral symptoms being related to lesions of the substantia nigra, pars reticulata and of the anterior part of the pallidum.

The debate on hysteria at the "Société de Neurologie" in 1908 signed the official death certificate for Charcot's hysteria, which even in his day had started to come under attack. The article by Babinski in 1901 had delivered the "coup de grâce". The debate paints an astonishing picture of the medical world of the day, and also of hysteria, which would never again present the spectacular clinical picture seen up to that point. Babinski, dominating the debate with his strong personality, prevented a discussion on the mental basis for hysteria, requested by several participants, in favour of pithiatism, which in his view offered an acceptable definition of hysteria. It is surprising that more was not made of the contradiction in terms in the expression "auto-suggestion", and of the fact that Babinski was begging the question when he asserted that it could not be asserted that a patient had been subject to suggestion! This debate effectively banished hysteria from the columns of the neurological press, whose pages it had tended to overburden. It cannot however be blamed for not having made a positive contribution to our understanding of this neurosis which, even today, remains enigmatic. It does our Society credit to have ruled out "for ethical reasons" the hypothesis of simulation.

- Abstract
The affective state of the patients suffering from loss of psychic autoactivation (LPA) is preserved and is dampered as exhibited by a subtle examination, but depends on the same activation by the surrounding people as cognitive thought: it fades if the patient is not stimulated. This affectivity disorder cannot be responsible for LPA, but rather is part of its expression. The empty mind, characteristic of LPA, is not incompatible with the painful mood of the depression contrarily to our intuitive notion. In other words, the mood (or thymia) of patients with LPA is not neutral or null as suggested by the term "athymia". The term "anhormia" would describe rather well part but only a part of the disorder, the loss of drive. Furthermore, the term "self activation" has the advantage of highlighting, by contrast, the preserved heteroactivation, an essential part of the clinical picture. The heuristic value of this distinction is illustrated by the division it suggests between the so called "will" or "motivation" and of the circuitry supporting the two components. The term of LPA leads also to the description of a new state of conscious awareness, a consciousness devoid of any content.

Two patient with familial cavernous angiomatosis presenting with long lasting variable epilepsy with a poor therapeutic response and variable neurologic impairments are presented here. One of the numerous cavernous angiomas was resected in one case. This last patient remains asymptomatic. Familial cerebral cavernous angiomas are often numerous and disseminated in the brain, therefore clinical manifestations are very polymorphous. Moreover the course of these lesions is variable. Therefore MRI should be performed to every patient presenting with poorly understood neurological symptoms, focal or generalized epileptic seizures or absence in order to look for potentially imputable brain lesions. A reliable genetic marker might be helpful for diagnosis of this disease with a variable penetrance and autosomal dominant inheritance. Then a neurosurgical treatment should be carefully discussed if lesions are accessible and medications are poorly efficient with recurrent neurologic impairments or epilepsy.

In addition to clinical and neuropathological similarities between Alzheimer's disease and Down syndrome there are genetic and biochemical data which suggest common disease mechanism. Using an in vitro assay examining variations of the mitotic index in the presence or absence of various inhibitors or metabolites of purine and/or pyrimidine synthesis, we studied 19 Alzheimer disease patients and 16 patients with both Down syndrome and Alzheimer type dementia. A highly significant decrease in mitotic index in the presence of exogenous glutamine was noted in patients presenting an Alzheimer type dementia with or without associated Down syndrome. These findings suggest that glutamine sensitivity or some dysregulation of the glutamine/glutamate pathway may play a role in the pathogenesis of Alzheimer's disease. If these findings are confirmed, they would have important implications in the development of preventive strategies.

Objective: To study the effect of the levorotatory form of 5-hydroxytryptophan on the cerebellar symptoms of Friedreich's ataxia., Design: Cooperative double-blind study of the levorotatory form of 5-hydroxytryptophan vs placebo., Setting: Twelve centers in research hospitals., Patients: Twenty-six patients were included; 19 completed the study (mean +/- SD age of patients, 25.9 +/- 8.1 years). Of these 19 patients, eight were treated with placebo and 11 were treated with the drug., Main Outcome Measures: A semiquantitative scale for kinetic and static ("postural") cerebellar functions and quantitative measurements of time in standard tests that evaluated stance, speech, writing, and drawing., Results: In the active treatment group, a significant decrease of the kinetic score was observed (P = .03), indicating an improvement in coordination., Conclusions: These results demonstrated that the levorotatory form of 5-hydroxytryptophan is able to modify significantly the cerebellar symptoms in patients with Friedreich's ataxia. However, the effect is only partial and not clinically major.

We evaluated the efficacy, development of adverse effects, and possible correlation between the plasma concentration of carbamazepine (CBZ) and its major metabolite, carbamazepine-10,11-epoxide (CBZ-E), in a group of epileptic patients in whom selective increases in CBZ doses were made. Eighteen patients with refractory partial epilepsy participated in an open trial. Five were on monotherapy and 13 on polytherapy. All the patients were on CBZ before the trial and had plasma levels within the therapeutic range (17-42 mumol/L). After a baseline period, CBZ doses were progressively increased either to reach a 50% reduction in seizure frequency for 2 months or until side effects appeared. Thirty-nine percent of the patients had a 50% decline in seizure frequency, but only 17% improved for > 6 months. Mild or moderate side effects were observed in 78% of the patients. Side effects were correlated with CBZ plasma levels but not with CBZ-E plasma levels. Correlation between CBZ and CBZ-E plasma levels were found in the monotherapy group, but not in the polytherapy group. Our results confirm that higher doses of CBZ can successfully be used in some patients with refractory partial epilepsy. Furthermore, the plasma level of CBZ-E does not seem to be a useful indicator of toxicity in CBZ-treated ambulatory epileptic patients.

Two complementary models of brain organization are nowadays offered: one anatomical and functional, the other computational. The first model implies a subdivision of brain functions in order to find the corresponding brain areas. It is easy to show that this cutting up can only be artificial and is influenced by some kind of "folk psychology", thereby raising suspicion on the "top down" method. On the other hand, it seems impossible to do without any psychological and even any subjective reference and work only according to the bottom up way. It must therefore be concluded that our subdivisions of brain functions must always be regarded as provisional as long as the two methods do not exactly overlap. The computational model uses either the conventional method or the parallel distributed processing. It can be shown that the conventional language utilized impoverishes the normal language and even more thought and is therefore unable to yield more than restricted results. It is suggested that the main advantage of parallel distributed processing is precisely to erase the formalization of language and it is then possible to expect from it much more interesting models. Although it seems possible to improve continuously our understanding of the functional conditions required for consciousness to appear, there seems to be theoretical limits to a complete scientific explanation of self awareness.

CT scan and magnetic resonance imaging in patients with bilateral lesions of the lenticular nuclei, especially the pallidum, have demonstrated a relationship between these lesions and obsessive-compulsive behaviours. Typical behaviour may satisfy the DSM III criteria for obsessive-compulsive disorders, but in some patients signs are limited to elementary stereotyped movements or thoughts, either together or separately. The same patient may exhibit several kinds of behaviour. The range of disorders suggests that circuits running through the striatum and pallidum participate in regulatory and unification processes of sensori-motor and mental systems. Neuropsychological and metabolic tests suggest that frontal circuits involving the pallidum are especially important in this process. Similar disorders may be observed in patients with frontal or bilateral lesions of the caudate. Obsessive-compulsive behaviour is often associated with a loss of psychic self-activation, but there are exceptions. This evidence suggests a similarity between obsessions related to demonstrated lesions of the basal ganglia and other diseases displaying obsessive-compulsive behaviour, including Gilles de la Tourette disease and the former obsessive neurosis, currently termed obsessive-compulsive trouble. As after von Economo encephalitis, these pathological and clinical features should be considered in the current interpretation of the mechanisms of obsession-compulsion. It appears to be easier to apply Charcot's intuitions about neuroses to obsessive-compulsive behaviour than to hysteria.

In normal subjects, the application of heterotopic painful stimuli induces simultaneous and parallel decrease in the sensation of pain and of the spinal nociceptive flexion (RIII) reflex evoked by electrical stimulation of the sural nerve. This inhibition of the RIII reflex is absent in tetraplegic patient with clinically complete spinal cord transections and can be triggered only from the analgesic hand in patients with Wallenberg's syndrome. These findings suggest that the inhibitory phenomena observed in normal subjects are likely to be examples of diffuse noxious inhibitory controls (DNICs), being sustained by a loop involving supraspinal structures, the ascending part of which is localized in the spinoreticular tract. We now report an exceptional case of a patient with Brown-Séquard syndrome due to a 4-year-old spinal cord lesion (left side, T-6 level) produced by a knife-wound in the back. Nociceptive flexion (RIII) reflexes elicited by stimulation of cutaneous afferents in the ulnar and sural nerves were studied in the upper and lower limbs by recording from the biceps brachialis and biceps femoris muscles, respectively. For each limb, the RIII reflex threshold was determined.
