Abstract

This paper develops and discusses a residual-based a posteriori error estimate and a space--time adaptive algorithm for solving parabolic surface partial differential equations on closed stationary surfaces. The full discretization uses the surface finite element method in space and the backward Euler method in time. The proposed error indicator bounds the error quantities globally in space from above and below, and globally in time from above and locally from below. A space--time adaptive algorithm is proposed using the derived error indicator. Numerical experiments illustrate and complement the theory., Comment: 18 pages, 4 figures

6. Enhanced Cycle Life and Expanded Voltage Window in Aqueous Proton Full Cells Using TiO2Negative Electrodes with Cationic Vacancies

Abstract

Aqueous batteries face the challenge of limited energy density due to parasitic gas production from hydrogen and oxygen evolution reactions, particularly at the negative electrode. This study investigates the electrochemical properties and mechanisms of proton intercalation in anatase TiO2featuring vacancies (Vac-TiO2), stabilized via a low-temperature sol–gel process. XRD refinement analysis, supported by thermal analysis, estimated 17% cationic vacancies, while 1H MAS NMR spectroscopy revealed stabilization of these vacancies by OH groups. The presence of cationic vacancies led to changes in the oxide anion sublattice, which accommodate proton insertion. Electrochemical assessments in acetate buffer electrolyte demonstrated Vac-TiO2’s ability to delay the hydrogen evolution reaction and enhance proton capacity, validated by pH-dependent studies, DFT calculations, and kinetic analyses. Notably, the occurrence of undercoordinated oxide anions was shown to induce the insertion of H+at higher potential values, and the insertion mechanism was suggested to occur via a solid-solution mechanism. Owing to these features, Vac-TiO2exhibited superior cyclability and performance compared to pure anatase TiO2, highlighting its potential for sustainable proton intercalation processes. In half-cell configurations, Vac-TiO2showed a high Coulombic efficiency (CE exceeding 90% after 48 cycles), while full cells (MnO2||Vac-TiO2) demonstrated an excellent cycling stability (CE exceeding 95.4% over 1000 cycles), high power density (10.5 kW·kg–1vs 6.2 kW·kg–1), and improved self-discharge. This study paves the way for innovative approaches to improving proton intercalation materials, positioning Vac-TiO2as a viable candidate for next-generation energy storage solutions.

7. Association Between Aortic Wall Thrombus and Thromboembolic Events After Transfemoral Transcatheter Aortic Valve Replacement.

Abstract

Thromboembolic events, particularly strokes, remain a major complication of transcatheter aortic valve replacement (TAVR). Embolic protection devices have failed to show significant clinical benefit in large randomized clinical trials. Aortic wall thrombus (AWT) is often observed on multidetector computed tomography during TAVR work-up, but its prognostic significance is uncertain. This study sought to evaluate the association between the presence of AWT and the incidence of thromboembolic outcomes in patients undergoing transfemoral (TF) TAVR for severe aortic stenosis. This was a prospective cohort study of consecutive patients who underwent TF TAVR for severe aortic stenosis between January 2011 and April 2022. A dedicated scale (range: 0-10) was qualitatively used to assess AWT. The primary outcome was a composite of procedural thromboembolic events defined as ischemic stroke, blue toe syndrome, bowel ischemia, or other solid organ infarction. The secondary endpoints were ischemic strokes and procedural death. Of the 641 patients included, severe AWT (score ≥8) was identified in 73 (11.4%). The presence of severe AWT was strongly associated with an increase in the primary outcome (OR: 8.48; 95% CI: 3.36-21.40; P < 0.001). This relationship persisted following multivariable analysis, which adjusted for comorbidities and procedural characteristics. The presence of severe AWT was also found to be associated with an increased incidence of stroke and procedural death (OR: 5.66; 95% CI: 2.00-15.30; P = 0.002 and OR: 4.66; 95% CI: 1.80-11.30; P = 0.002, respectively). The presence of severe AWT on preprocedural multidetector computed tomography is strongly associated with thromboembolic complications including stroke and mortality after TF TAVR. Abstract

Endovascular renal denervation reduces blood pressure in patients with mild-to-moderate hypertension, but its efficacy in patients with true resistant hypertension has not been shown. We aimed to assess the efficacy and safety of endovascular ultrasound renal denervation in patients with hypertension resistant to three or more antihypertensive medications.

FindingsIn this nonprespecified, post hoc analysis of the GLOBAL LEADERS randomized clinical trial, beyond 1 month after percutaneous coronary intervention in acute coronary syndrome, aspirin was associated with increased bleeding risk and appeared not to add to the benefit of ticagrelor on ischemic events. MeaningThe findings of this hypothesis-generating analysis pave the way for further trials evaluating aspirin-free antiplatelet strategies after percutaneous coronary intervention. ImportanceThe role of aspirin as part of antiplatelet regimens in acute coronary syndromes (ACS) needs to be clarified in the context of newer potent P2Y12 antagonists. ObjectiveTo evaluate the benefit and risks of aspirin in addition to ticagrelor among patients with ACS beyond 1 month after percutaneous coronary intervention (PCI). Design, Setting, and ParticipantsThis is a nonprespecified, post hoc analysis of GLOBAL LEADERS, a randomized, open-label superiority trial comparing 2 antiplatelet treatment strategies after PCI. The trial included 130 secondary/tertiary care hospitals in different countries, with 15991 unselected patients with stable coronary artery disease or ACS undergoing PCI. Patients had outpatient visits at 1, 3, 6, 12, 18, and 24 months after index procedure. InterventionsThe experimental group received aspirin plus ticagrelor for 1 month followed by 23-month ticagrelor monotherapy; the reference group received aspirin plus either clopidogrel (stable coronary artery disease) or ticagrelor (ACS) for 12 months, followed by 12-month aspirin monotherapy. In this analysis, we examined the clinical outcomes occurring between 31 days and 365 days after randomization, specifically in patients with ACS who, within this time frame, were assigned to receive either ticagrelor alone or ticagrelor and aspirin. Main Outcomes and MeasuresThe primary outcome was the composite of all-cause death or new Q-wave myocardial infarction. ResultsOf 15968 participants, there were 7487 patients with ACS enrolled; 3750 patients were assigned to the experimental group and 3737 patients to the reference group. Between 31 and 365 days after randomization, the primary outcome occurred in 55 patients (1.5%) in the experimental group and in 75 patients (2.0%) in the reference group (hazard ratio [HR], 0.73; 95% CI, 0.51-1.03; P=.07); investigator-reported Bleeding Academic Research Consortium-defined bleeding type 3 or 5 occurred in 28 patients (0.8%) in the experimental group and in 54 patients (1.5%) in the reference arm (HR, 0.52; 95% CI, 0.33-0.81; P=.004). Conclusions and RelevanceBetween 1 month and 12 months after PCI in ACS, aspirin was associated with increased bleeding risk and appeared not to add to the benefit of ticagrelor on ischemic events. These findings should be interpreted as exploratory and hypothesis generating; however, they pave the way for further trials evaluating aspirin-free antiplatelet strategies after PCI.
