Abstract

BackgroundTreatment of multidrug-resistant tuberculosis requires long-term therapy with a combination of multiple second-line drugs. These drugs are associated with numerous adverse events that can cause severe morbidity, such as deafness, and in some instances can lead to death. Our aim was to estimate the absolute and relative frequency of adverse events associated with different tuberculosis drugs to provide useful information for clinicians and tuberculosis programmes in selecting optimal treatment regimens.MethodsWe did a meta-analysis using individual-level patient data that were obtained from studies that reported adverse events that resulted in permanent discontinuation of anti-tuberculosis medications. We used a database created for our previous meta-analysis of multidrug-resistant tuberculosis treatment and outcomes, for which we did a systematic review of literature published between Jan 1, 2009, and Aug 31, 2015 (updated April 15, 2016), and requested individual patient-level information from authors. We also considered for this analysis studies contributing patient-level data in response to a public call made by WHO in 2018. Meta-analysis for proportions and arm-based network meta-analysis were done to estimate the incidence of adverse events for each tuberculosis drug.Findings58 studies were identified, including 50 studies from the updated individual patient data meta-analysis for multidrug-resistant tuberculosis treatment. 35 of these studies, with 9178 patients, were included in our analysis. Using meta-analysis of proportions, drugs with low risks of adverse event occurrence leading to permanent discontinuation included levofloxacin (1·3% [95% CI 0·3-5·0]), moxifloxacin (2·9% [1·6-5·0]), bedaquiline (1·7% [0·7-4·2]), and clofazimine (1·6% [0·5-5·3]). Relatively high incidence of adverse events leading to permanent discontinuation was seen with three second-line injectable drugs (amikacin: 10·2% [6·3-16·0]; kanamycin: 7·5% [4·6-11·9]; capreomycin: 8·2% [6·3-10·7]), aminosalicylic acid (11·6% [7·1-18·3]), and linezolid (14·1% [9·9-19·6]). Risk of bias in selection of studies was judged to be low because there were no important differences between included and excluded studies. Variability between studies was significant for most outcomes analysed.InterpretationFluoroquinolones, clofazimine, and bedaquiline had the lowest incidence of adverse events leading to permanent drug discontinuation, whereas second-line injectable drugs, aminosalicylic acid, and linezolid had the highest incidence. These results suggest that close monitoring of adverse events is important for patients being treated for multidrug-resistant tuberculosis. Abstract

BackgroundTreatment outcomes for multidrug-resistant tuberculosis remain poor. We aimed to estimate the association of treatment success and death with the use of individual drugs, and the optimal number and duration of treatment with those drugs in patients with multidrug-resistant tuberculosis.MethodsIn this individual patient data meta-analysis, we searched MEDLINE, Embase, and the Cochrane Library to identify potentially eligible observational and experimental studies published between Jan 1, 2009, and April 30, 2016. We also searched reference lists from all systematic reviews of treatment of multidrug-resistant tuberculosis published since 2009. To be eligible, studies had to report original results, with end of treatment outcomes (treatment completion [success], failure, or relapse) in cohorts of at least 25 adults (aged >18 years). We used anonymised individual patient data from eligible studies, provided by study investigators, regarding clinical characteristics, treatment, and outcomes. Using propensity score-matched generalised mixed effects logistic, or linear regression, we calculated adjusted odds ratios and adjusted risk differences for success or death during treatment, for specific drugs currently used to treat multidrug-resistant tuberculosis, as well as the number of drugs used and treatment duration.FindingsOf 12 030 patients from 25 countries in 50 studies, 7346 (61%) had treatment success, 1017 (8%) had failure or relapse, and 1729 (14%) died. Compared with failure or relapse, treatment success was positively associated with the use of linezolid (adjusted risk difference 0·15, 95% CI 0·11 to 0·18), levofloxacin (0·15, 0·13 to 0·18), carbapenems (0·14, 0·06 to 0·21), moxifloxacin (0·11, 0·08 to 0·14), bedaquiline (0·10, 0·05 to 0·14), and clofazimine (0·06, 0·01 to 0·10). There was a significant association between reduced mortality and use of linezolid (-0·20, -0·23 to -0·16), levofloxacin (-0·06, -0·09 to -0·04), moxifloxacin (-0·07, -0·10 to -0·04), or bedaquiline (-0·14, -0·19 to -0·10). Compared with regimens without any injectable drug, amikacin provided modest benefits, but kanamycin and capreomycin were associated with worse outcomes. The remaining drugs were associated with slight or no improvements in outcomes. Treatment outcomes were significantly worse for most drugs if they were used despite in-vitro resistance. The optimal number of effective drugs seemed to be five in the initial phase, and four in the continuation phase. In these adjusted analyses, heterogeneity, based on a simulated I2 method, was high for approximately half the estimates for specific drugs, although relatively low for number of drugs and durations analyses.InterpretationAlthough inferences are limited by the observational nature of these data, treatment outcomes were significantly better with use of linezolid, later generation fluoroquinolones, bedaquiline, clofazimine, and carbapenems for treatment of multidrug-resistant tuberculosis. 27. Gaps in the Diagnosis and Treatment of Drug-resistant Tuberculosis in Mexico

Abstract

Objective: The study aimed to determine the critical gaps in the care of drug-resistant tuberculosis to implement strategies for its elimination.Material and Methods: Cases with various resistance profiles that were presented to the National Advisory Group for Drug Resistance during the years 2019-2022 have been described.Results and Discussion: 104 cases were included in this study. 41% of the patients had diabetes (41%) as a comorbidity. There have been observed delays in the reporting of rapid molecular tests; with the Xpert MTB/RIF, delays of 6.33 days have been reported. There have also been observed delays in the request for phenotypic drug susceptibility tests; from the time of initial diagnosis, patients have been found to receive their first drug susceptibility test, on average, after 20 months. The bacteriological follow-up during the treatment with monthly culture has been found to only be carried out in 8% of the cases. In the contact investigation, only 16% of the listed contacts had been studied for infection or active disease.Conclusion: The elimination of these gaps requires the decentralization of susceptibility tests for fluoroquinolones, shortening the times between the diagnosis of drug resistance and the start of treatment, a close clinical and bacteriological follow-up, and an exhaustive investigation of contacts.

Abstract

Background: Few studies have focused on risk factors for early readmission in patients with tuberculosis. Objective: Determine what are the risk factors for the early readmission of patients with tuberculosis at General Tijuana Hospital, Mexico. Material and methods: All patients aged ≥ 18 years who were admitted with the confirmed diagnosis of pulmonary tuberculosis were prospectively included. Information was obtained on demographic, socioeconomic variables, previous hospitalizations, clinical data, and laboratory and radiographic studies. Results: One hundred and thirty-four patients with tuberculosis were included, and 24 of them (17.9%) corresponded to early hospital readmissions. The interval between initial admission and readmission was 1.45 ± 0.183 months. The readmission group had used illicit drugs for more years (11.3 ± 13.9 years) than the new cases group (8.01 ± 8.25 years; p = 0.03). Forty percent of the patients who were readmitted did not go to their referral health unit after their initial hospital discharge. The reasons argued by the patients included, among others, not having received information regarding their illness during hospitalization, the abuse of illegal substances and the perception of hostility by health personnel. Discussion: One out of every five patients admitted for tuberculosis will be readmitted after only six weeks of initial discharge. Sociocultural factors (addictions, comorbidities, poverty) and of the health system (limited hours of medical care, accessibility) contribute to this phenomenon. Conclusions: Loss of follow-up after hospital discharge is common in most settings and contributes to an increase in morbidity and mortality, and transmission of infection in the community.

Abstract

Objetivo. Determinar la prevalencia de infección tuberculosa en escolares de la ciudad de Tijuana, México. Material y métodos. Se seleccionó aleatoriamente una muestra a partir del registro escolar municipal y se incluyeron 1 131 estudiantes de primaria y secundaria. A todos se les aplicó una dosis de tuberculina 5TU (Mantoux). Los sujetos con induración > o = 10 mm se clasificaron como reactores positivos. Resultados. El estudio demostró que entre los sujetos participantes la prevalencia global de reactores positivos fue de 57%. La proporción de reactores positivos fue significativamente mayor en el grupo de sujetos inmunizados con BCG que en el grupo de no inmunizados (59.7 vs 45.6%; p < 0.001). No se encontró una correlación significativa entre el diámetro de la induración y la edad de aplicación de la BCG. Conclusiones. Estos hallazgos demuestran que la prevalencia de infección tuberculosa es extremadamente alta en la ciudad de Tijuana y tiene importantes implicaciones si se considera como una evidencia básica en el proceso para el control de la tuberculosis en la región.

Abstract

Clinical isolates with discordant phenotypic and genotypic results were submitted to DNA sequencing to identify which were genuinely resistant to rifampin and determine the frequency of silent and disputed mutations in our region. We present the retrospective analysis of all the culture-proven TB cases tested with the Xpert®MTB/RIF assay at the Tuberculosis Clinic and Laboratory of the Tijuana General Hospital, Mexico. Clinical isolates showing a discrepancy between phenotypic and molecular tests were analyzed by DNA sequencing. Thirteen isolates tested as rifampin susceptible on the MGIT system were rifampin-resistant according to Xpert®MTB/RIF assay. DNA sequencing showed that seven (53.8%) isolates had a silent (P514P) mutation; three isolates showed different missense (L511P, D516Y, and S531L) mutations. Three isolates showed no mutations. The existence of heteroresistance and silent or disputed mutations warrants that all rifampin-resistance cases diagnosed with the Xpert®MTB/RIF should be referred to specialized centers for DNA sequencing.
