Abstract

Indirect Drive Inertial Confinement Fusion Experiments on the National Ignition Facility (NIF) have achieved a burning plasma state with neutron yields exceeding 170 kJ, roughly 3 times the prior record and a necessary stage for igniting plasmas. The results are achieved despite multiple sources of degradations that lead to high variability in performance. Results shown here, for the first time, include an empirical correction factor for mode-2 asymmetry in the burning plasma regime in addition to previously determined corrections for radiative mix and mode-1. Analysis shows that including these three corrections alone accounts for the measured fusion performance variability in the two highest performing experimental campaigns on the NIF to within error. Here we quantify the performance sensitivity to mode-2 symmetry in the burning plasma regime and apply the results, in the form of an empirical correction to a 1D performance model. Furthermore, we find the sensitivity to mode-2 determined through a series of integrated 2D radiation hydrodynamic simulations to be consistent with the experimentally determined sensitivity only when including alpha-heating.

Abstract

We describe the curation, annotation methodology, and characteristics of the dataset used in an artificial intelligence challenge for detection and localization of COVID-19 on chest radiographs. The chest radiographs were annotated by an international group of radiologists into four mutually exclusive categories, including typical, indeterminate, and atypical appearance for COVID-19, or negative for pneumonia, adapted from previously published guidelines, and bounding boxes were placed on airspace opacities. This dataset and respective annotations are available to researchers for academic and noncommercial use.

Abstract

An experimental program is currently underway at the National Ignition Facility (NIF) to compress deuterium and tritium (DT) fuel to densities and temperatures sufficient to achieve fusion and energy gain. The primary approach being investigated is indirect drive inertial confinement fusion (ICF), where a high-Z radiation cavity (a hohlraum) is heated by lasers, converting the incident energy into x-ray radiation which in turn drives the DT fuel filled capsule causing it to implode. Previous experiments reported DT fuel gain exceeding unity [O.A. Hurricane et al., Nature 506, 343 (2014)] and then exceeding the kinetic energy of the imploding fuel [S. Le Pape et al., Phys. Rev. Lett. 120, 245003 (2018)]. We report on recent experiments that have achieved record fusion neutron yields on NIF, greater than 100 kJ with momentary fusion powers exceeding 1PW, and have for the first time entered the burning plasma regime where fusion alpha-heating of the fuel exceeds the energy delivered to the fuel via compression. This was accomplished by increasing the size of the high-density carbon (HDC) capsule, increasing energy coupling, while controlling symmetry and implosion design parameters. Two tactics were successful in controlling the radiation flux symmetry and therefore the implosion symmetry: transferring energy between laser cones via plasma waves, and changing the shape of the hohlraum. In conducting these experiments, we controlled for known sources of degradation. Herein we show how these experiments were performed to produce record performance, and demonstrate the data fidelity leading us to conclude that these shots have entered the burning plasma regime.

12. Dynamical disentangling and cooling of atoms in bilayer optical lattices

Abstract

We show how experimentally available bilayer lattice systems can be used to prepare quantum many-body states with exceptionally low entropy in one layer, by dynamically disentangling the two layers. This disentangling operation moves one layer - subsystem $A$ - into a regime where excitations in $A$ develop a single-particle gap. As a result, this operation maps directly to cooling for subsystem $A$, with entropy being shuttled to the other layer. For both bosonic and fermionic atoms, we study the dynamics of this process, and show that disentangling can be realised cleanly in ongoing optical lattice experiments. The corresponding entanglement entropies are directly measurable with quantum gas microscopes, and as a tool for producing lower-entropy states, this technique opens a range of applications beginning with simplifying production of anti-ferromagnetically ordered states of fermions., Comment: 4 pages + supplementary material, 4 figures

Insufficient microbiological specimens obtained by superficial swabbing may only detect bacteria on the surface, but pathogens that are localized in the deep tissue may be missed. The aim of this study was to analyze deep sternal wound infection (DSWI) samples by conventional microbiological procedures and fluorescence in situ hybridization (FISH) in order to discuss a diagnostic benefit of the culture-independent methods and to map spatial organization of pathogens and microbial biofilms in the wounds.Methods: Samples from 12 patients were collected and analyzed using classic microbiological culture and FISH in combination with molecular nucleic acid amplification techniques (FISHseq). Frequency of and the time to occurrence of a DSWI was recorded, previous operative interventions, complications, as well as individual risk factors and the microbiologic results were documented.Results: Tissue samples were taken from 12 patients suffering from DSWI. Classical microbiological culture resulted in the growth of microorganisms in the specimens of five patients (42%), including bacteria and in one case Candida. FISHseq gave additional diagnostic information in five cases (41%) and confirmed culture results in seven cases (59%).Conclusion: Microbial biofilms are not always present in DSWI wounds, but microorganisms are distributed in a “patchy” pattern in the tissue. Therefore, a deep excision of the wound has to be performed to control the infection. We recommend to analyze at least two wound samples from different locations by culture and in difficult to interpret cases, additional molecular biological analysis by FISHseq.Keywords: biofilm, FISH, mediastinitis, microorganisms, DSWI

17. On the psycho-emotional deficitisation of workers in the age of cognitive enhancement

Abstract

Despite being the subject of public and scholarly debates for some time, the topic of cognitive enhancement remains theoretically under-developed in organisation studies. This is because the ‘dots’ still have to be ‘connected’ between macro-level phenomena (here, the therapeutic ethos and cognitive capitalism), and micro-level phenomena (in this case, cognitive abilities). In this essay, we use Fromm’s notion of social character to theorise dialectally about the interaction between these macro and micro-level phenomena. Doing so enables us to examine how the macro/micro interaction fosters to adoption of cognitive enhancement in the context of work, and what kinds of consequences might emerge from this. We propose the psycho-emotional deficitisation of workers as a central consequence of the aforementioned interaction, and define it as an internalised version of external ideals of what it means to be a productive worker under cognitive capitalism, which over time generates and reinforces the affective experience of being deficient. Our theorising around socially patterned defects of a cognitive kind has crucial ramification for our understanding of technology-mediated affective control at work and how human–technology interactions shape the subjectivities of workers towards greater self-inferiorisation vis-à-vis the perceived superiority of technology. We close by foreshadowing avenues for future research.

Abstract

Long-range order in quantum many-body systems is usually associated with equilibrium situations. Here, we experimentally investigate the quasicondensation of strongly-interacting bosons at finite momenta in a far-from-equilibrium case. We prepare an inhomogeneous initial state consisting of one-dimensional Mott insulators in the center of otherwise empty one-dimensional chains in an optical lattice with a lattice constant $d$. After suddenly quenching the trapping potential to zero, we observe the onset of coherence in spontaneously forming quasicondensates in the lattice. Although survival is increased, mastectomies can adversely affect a patient physically, psychologically, and psychosexually. High health-related quality of life (HRQoL) is often achieved after simultaneous breast reconstruction (BR) following BRRM; however, data on the pre- and postoperative results of HRQoL are lacking. Therefore, we investigated the quality of life, esthetic outcome, and patient well-being after BRRM and simultaneous implant-based BR.Patients and Methods: Of the 35 patients who underwent skin-sparing or nipple-sparing mastectomy between May 2012 and December 2017 at a university hospital, only 22 completed the evaluation. Baseline data and data on previous operations and operation techniques were retrieved from the patient’s charts. BREAST-Q and short form-36 health survey (SF-36) questionnaires were used to evaluate patient satisfaction and HRQoL.Results: SF-36 analysis showed a significantly higher score for pain (p=0.043) in our population than in the general female population. Comparing the pre- and postoperative BREAST-Q results, a significant decrease in the physical well-being of the chest (p=0.0179) and a slight improvement in breast satisfaction were observed (p=0.3266). All patients were well-satisfied with the postoperative outcome, reconstruction, and perioperative surgeon care.Conclusion: Bilateral mastectomy with simultaneous BR using pre-pectoral implants is associated with an HRQoL similar to that of the healthy population. Therefore, this study evaluated free flap reconstruction of the extremities in older patients and compared the outcomes to those from younger patients who underwent similar procedures during the same period.Patients and Methods: This single-center retrospective study used a case-control design to compare older and younger patients who underwent free flap reconstruction of soft tissue defects in the extremities. One-to-one matching was performed for older patients (≥ 65 years) and younger patients (≤ 64 years) according to indication, flap recipient site, and flap type. The parameters of interest were clinico-demographic characteristics, flap type, defect location, indication for free flap reconstruction, number of venous anastomoses, and postoperative complications (flap loss, infection, and wound healing disorders).Results: The study included 48 older patients and 133 younger patients, with a mean follow-up of 12 months after discharge. The free flap reconstruction was performed at a mean interval of 19.8± 22.8 days (range: 0– 88 days). The 1:1 matching created 38 pairs of patients, which revealed no significant differences in the rates of flap necrosis and flap failure.Conclusion: This study failed to detect a significant age-related difference in the flap necrosis rate after free flap reconstruction of extremity defects. Abstract

The inertial confinement fusion program relies upon detailed simulations with inertial confinement fusion (ICF) codes to design targets and to interpret the experimental results. These simulations treat as much physics from essential principles as is practical, including laser deposition, cross beam energy transfer, x-ray production and transport, nonlocal thermal equilibrium kinetics, thermal transport, hydrodynamic instabilities, thermonuclear burn, and transport of reaction products. Improvements in radiation hydrodynamic code capabilities and vast increases in computing power have enabled more realistic, accurate 3D simulations that treat all known asymmetry sources. We describe how numerical simulations helped to guide the program, assess the impediments to breakeven, and optimize every aspect of target design. A preshot simulation of the first National Ignition Facility experiment that surpassed breakeven predicted an increased yield that matches the experimental result, within the preshot predicted uncertainty, with a target gain of 1.5. We will cover the key developments in Lawrence Livermore National Laboratory ICF codes that enabled these simulations and give specific examples of how they helped to guide the program. Abstract

We investigate the expansion of bosons and fermions in a homogeneous lattice after a sudden removal of the trapping potential using exact numerical methods. As a main result, we show that in one dimension, both bosonic and fermionic Mott insulators expand with the same velocity, irrespective of the interaction strength, provided the expansion starts from the ground state of the trapped gas. Furthermore, their density profiles become identical during the expansion: The asymptotic density dynamics is identical to that of initially localized, non-interacting particles, and the asymptotic velocity distribution is flat. The expansion velocity for initial correlated Mott insulating states is therefore independent of the interaction strength and particle statistics. Interestingly, this non-equilibrium dynamics is sensitive to the interaction driven quantum phase transition in the Bose-Hubbard model: While being constant in the Mott phase, the expansion velocity decreases in the superfluid phase and vanishes for large systems in the non-interacting limit. These results are compared to the set-up of a recent experiment [PRL 110, 205301 (2013)], where the trap opening was combined with an interaction quench from infinitely strong interactions to finite values. We carry out an analogous analysis for a two-component Fermi gas, with similar observations. In addition, we study the effect of breaking the integrability of hard-core bosons in different ways: While the fast ballistic expansion from the ground state of Mott insulators in one dimension remains unchanged for finite interactions, we observe strong deviations from this behavior on a two-leg ladder even in the hard-core case. We revisit the problem of the spin Drude weight D of the integrable spin-1/2 XXZ chain using two complementrary approaches, exact diagonalization (ED) and the time-dependent density-matrix renormalization group (tDMRG). We pursue two main goals. First, we present extensive results for the temperature dependence of D. By exploiting time translation invariance within tDMRG, one can extract D for significantly lower temperatures than in previous tDMRG studies. Second, we discuss the numerical quality of the tDMRG data and elaborate on details of the finite-size scaling of the ED results, comparing calculations carried out in the canonical and grand-canonical ensembles. Abstract

We study the sudden expansion of spin-imbalanced ultracold lattice fermions with attractive interactions in one dimension after turning off the longitudinal confining potential. We show that the momentum distribution functions of majority and minority fermions approach stationary values quickly due to a quantum distillation mechanism that results in a spatial separation of pairs and majority fermions. As a consequence, Fulde-Ferrell-Larkin-Ovchinnikov (FFLO) correlations are lost during the expansion. Furthermore, we argue that the shape of the stationary momentum distribution functions can be understood by relating them to the integrals of motion in this integrable quantum system. We show that in the sudden expansion of a spin-balanced two-component Fermi gas into an empty optical lattice induced by releasing particles from a trap, over a wide parameter regime, the radius $R_n$ of the particle cloud grows linearly in time. This allow us to define the expansion velocity $V_{ex}$ from $R_n=V_{ex}t$. The goal of this work is to clarify the dependence of the expansion velocity on the initial conditions which we establish from time-dependent density matrix renormalization group simulations, both for a box trap and a harmonic trap. As a prominent result, the presence of a Mott-insulating region leaves clear fingerprints in the expansion velocity. We address the coherence of the dynamics of spin-currents with components transverse to an external magnetic field for the spin-1/2 Heisenberg chain. We study current autocorrelations at finite temperatures and the real-time dynamics of currents at zero temperature. Besides a coherent Larmor oscillation, we find an additional collective oscillation at higher frequencies, emerging as a coherent many-magnon effect at low temperatures. Abstract

We study the spin and heat conductivity of dimerized spin-1/2 chains in homogeneous magnetic fields at finite temperatures. At zero temperature, the model undergoes two field-induced quantum phase transitions from a dimerized, into a Luttinger, and finally into a fully polarized phase. We search for signatures of these transitions in the spin and heat conductivities. Using exact diagonalization, we calculate the Drude weights, the frequency dependence of the conductivities, and the corresponding integrated spectral weights. As a main result, we demonstrate that both the spin and heat conductivity are enhanced in the gapless phase and most notably at low frequencies. In the case of the thermal conductivity, however, the field-induced increase seen in the bare transport coefficients is suppressed by magnetothermal effects, caused by the coupling of the heat and spin current in finite magnetic fields. Abstract

Using the adaptive time-dependent density matrix renormalization group method, we numerically study the spin dynamics and transport in one-dimensional spin-1/2 systems at zero temperature. Instead of computing transport coefficients from linear response theory, we study the real-time evolution of the magnetization starting from spatially inhomogeneous initial states. In particular, we are able to analyze systems far away from equilibrium with this set-up. By computing the time-dependence of the variance of the magnetization, we can distinguish diffusive from ballistic regimes, depending on model parameters. For the example of the anisotropic spin-1/2 chain and at half filling, we find the expected ballistic behavior in the easy-plane phase, while in the massive regime the dynamics of the magnetization is diffusive. Our approach allows us to tune the deviation of the initial state from the ground state and the qualitative behavior of the dynamics turns out to be valid even for highly perturbed initial states in the case of easy-plane exchange anisotropies. We further cover two examples of nonintegrable models, the frustrated chain and the two-leg spin ladder, and we encounter diffusive transport in all massive phases. In the former system, our results indicate ballistic behavior in the critical phase. We discuss our findings in view of experiments on quasi-one dimensional quantum magnets and we propose that the study of the time-dependence of the spatial variance of particle densities could be instrumental in the characterization of the expansion of ultracold atoms in optical lattices as well., Comment: 12 pages Revtex4, 17 eps figures, version as published, minor revisions

Abstract

The choice of antibiotics and length of administration in the treatment of deep sternal wound infections (DSWI) is unclear. The reason for this is the lack of studies and local differences in resistance. An increase in resistance can be observed in gram-positive cocci, which are the most frequently detected in deep sternal infections. The duration of administration is often 2- 6 weeks or longer, although the benefit of prolonged antibiotic administration has not been confirmed by studies. We evaluated the antibiotic treatment during surgical treatment, consisting of surgical wound debridement and plastic chest reconstruction. METHODS: Retrospective analysis of patients (n = 260) who underwent reconstructive surgery in the Department of Plastic Surgery at Leipzig University Hospital from 01.05.2012 - 31.12.2020. The duration of intake, results of microbiological swabs and resistance were investigated. RESULTS: At the time of discharge, closed wound conditions were noted in 177 of 260 cases (68.1%). The largest proportion of patients (n = 238) was treated with a latissimus dorsi flap (91.5%). Antibiotic treatment was conducted in 206 of 260 cases (79.2%). The mean duration of antibiotic administration was 21.4 days (±17.6). Prolonged treatment over 14 days did not alter outcome (p = 0.226), in contrast, the number of multidrug resistances (p < 0.001). There was no prove of resistance against linezolid which is effective against the most common found infectious agents Staphylococcus epidermidis (n = 93; 24.0 %) & Staphylococcus aureus (n = 47; 12.1 %). CONCLUSION: There is no evidence of benefit from antibiotic therapy over 14 days, whereas multidrug resistance increases with prolonged antibiotic use. In the absence of infectious agents or clinical signs of inflammation, surgical treatment without additional antibiotic treatment is effective. Linezolid is a suitable antibiotic in the treatment of gram-positive infections which are the most frequent in DSWI. [ABSTRACT FROM AUTHOR]

Especially in the presence of artificial material or inadequate preliminary muscle flaps, the pedicled omentum flap is due to its immunological properties, the predetermined flap in salvage procedures. Methods: We treated 14 patients suffering a mediastinitis and open thorax using a pedicled omentoplasty as a salvage procedure because of persisting DSWIs. Omentoplasty was performed in combination with a split skin graft and the wound was closed by a vacuum-assisted therapy for 7 days. The patients’ sex and comorbid risk factors supporting DSWIs as well as the postoperative complications were recorded.Results: Retrospective analysis of 14 patients (10 males and four females) after a follow-up time of 24 months was performed. The average age was 75 years (range: 67–83). Heart surgery took place electively in eight cases, in three cases urgently and three for emergency reasons. The preoperative Euro Score was 16 (range 3.51–42.58). We had no flap loss in any patients. The skin graft showed a full take in all patients. Two patients needed revision of an abdominal wound dehiscence after laparotomy and one patient developed hernia in the late outcome.Discussion: The greater omentum flap has, over many years, become an ideal partner in the coverage and treatment of DSWIs. Especially due to its immunologic capacity and amorphous structure, it has the ability to fill up cavities and cover infected artificial material so residual infections can be controlled. 44. The Impact of Parental Bereavement on Young People: A Thematic Analysis of Using Online Web Forums as a Method of Coping

Abstract

A qualitative approach was used to explore how online web forums might facilitate recovery and the process of coping. Ten online web forums written by young people who have personally experienced the death of a parent formed the data of this study. Previous research suggests forum users do not receive the supportive reactions from face-to-face interactions that they desire. Thematic analysis found that forums created an environment where young people can process the bereavement of a parent. Forums allowed young people to use their experience of bereavement to positively support others with similar experiences. The findings imply that the process of using forums can positively impact individuals who have experienced the loss of a parent. This supports recommendations by professionals, to consider online forums as a coping strategy. This study presents one of the few analyses of web forums written by young people who have experienced parental bereavement.
