Distributed quantum computation in a quantum network [1]- [3] is based on the idea that qubits can be preserved and efficiently exchanged between long-lived, stationary network nodes via photonic links [4]. Although long qubit lifetimes have been observed [5]- [10], and non-qubit excitations have been memorized [11]- [14], the long-lived storage and efficient retrieval of a photonic qubit by means of a light-matter interface [15]- [20] remains an outstanding challenge. Here, we report on a qubit memory based on a single atom coupled to a high-finesse optical resonator. By mapping the qubit between an interface basis with strong light-matter coupling and a memory basis with low decoherence, we achieve a coherence time exceeding 100 ms with a time-independent storage-and-retrieval efficiency of 22%. The former constitutes an improvement by two orders of magnitude [21], [22] and thus implements an efficient photonic qubit memory with a coherence time that exceeds the lower bound needed for direct qubit teleportation in a global quantum internet. A quantum memory based on a rubidium atom shows a record-long storage time of 100 ms with a readout efficiency of 22%. Persons with severe mental illness and addiction are at higher risk for early morbidity and mortality than the general population, and are less likely to receive primary care and preventive health services. Primary and behavioral integrated care programs aim to reduce these health disparities by providing comprehensive health and wellness services. Gender in particular may play a significant role in individuals’ engagement and outcomes in such programs. Hence, this study examines the salient characteristics of behavioral health consumers accessing an integrated care program at a large community mental health center. Baseline gender differences in consumer demographics, substance use, psychological distress and functioning, physical health indicators, and risk factors for serious medical conditions are examined. Our results demonstrate that key gender differences exist and may warrant distinct treatment needs for men and women receiving integrated care. [ABSTRACT FROM AUTHOR]

Aim: The aim of this work was to determine racial disparities in access to minimally invasive proctectomy using a national database., Method: A retrospective review of the American College of Surgeons National Surgical Quality Improvement Program evaluated for surgical approach (robotic, laparoscopic or open), demographics and comorbidity, and then compared by race., Results: A total of 3511 patients (325 Asian, 2925 White, 261 African American/Black) with cancer who underwent a proctectomy between 2016 and 2020 were included. Both Asians and Whites had significantly higher rates of laparoscopic proctectomy relative to African Americans (38.5%, 33.8% and 28.7%, respectively; p = 0.0001). Asians had the highest rate of robotic proctectomy (38.2%, p = 0.0001). Conversely, Black patients had significantly higher rates of open proctectomy followed by Whites and then Asians (42.1%, 35.4% and 23.4%, respectively; p = 0.0001). In multivariable logistic regression with backward elimination, African Americans were 0.7 times as likely to undergo laparoscopic proctectomy and 1.4 times more likely to undergo open proctectomy than Whites (p = 0.043). Compared with Whites, Asians were 1.8, 1.7 and 1.9 times more likely to undergo minimally invasive, laparoscopic proctectomy and robotic proctectomy, respectively (p = 0.0001, p = 0.001, p = 0.0001)., Conclusion: Asians had the highest rate of laparoscopic and robotic proctectomy, while Blacks had the highest rate of open proctectomy. African Americans were least likely to undergo laparoscopic proctectomy compared with all races. Background: Hartmann's reversal (HR) is associated with significant technical difficulty and morbidity. Using the ACS-NSQIP database, we assessed the outcomes of HR as compared to elective left colectomy (LC)., Methods: The 2016-2019 ACS-NSQIP datasets were queried to identify patients undergoing HR and elective LC. Patients' demographics, comorbidities, and short-term surgical outcomes were evaluated using both univariable and multivariable methods., Results: The study included 7,632 HR cases and 29,162 LC cases. The HR group had more patients with ASA grade III (50% vs. 42.4%). HR had more open-operative cases (69.4 vs. 18.5%) and longer mean operative times (213 vs. 191 min) than LC. Postoperatively, the HR group had a longer mean hospital stay (5.5 vs. 4.1 days) and higher complication rate (18.3% vs. 10.3%). HR was associated with increased odds of having a concurrent ileostomy (OR 2.11), deep space/organ infection (OR 1.55), and at least one complication (OR 1.56)., Conclusion: HR is a more challenging operation with patients who fared worse than their LC counterparts. Since the early 2000s, many families impacted by Krabbe disease have tried to implement newborn screening for this rare fatal neurological disorder in their home state. However, despite grassroots efforts, states have been unable to agree to newborn screening for Krabbe disease due to poor testing mechanisms, lack of understanding of the developmental outcomes of transplantation, low incidence rate, and more. Over the past five years, many organizations and experts have made significant strides to help Krabbe disease meet the eligibility requirements for state panels and the Recommended Uniform Screening Panel (RUSP). Nevertheless, ethicists and newborn screening advisory committees continue to disregard the progress our community has made in the treatment and screening of Krabbe disease., (© 2022 Wiley Periodicals LLC.)

Objective: To examine glycemic variability within 1 month and 1 year following surgery among adult patients, with and without Type 2 Diabetes (T2D), treated for stage II-III colon cancer., Method: A retrospective analysis of electronic health record data was conducted. Glycemic variability (i.e., standard deviation [SD] and coefficient of variation [CV] of > 2 blood glucose measures) was assessed within 1 month and within 1 year following colon surgery. Chi-square (χ 2 ), Fisher's exact, and Mann-Whitney U tests were used for the analyses., Results: Among the sample of 165 patients with stage II-III colon cancer, those with T2D had higher glycemic variability compared to patients without T2D ( p < .001), with values within 1 month following surgery (SD = 44.69 mg/dL, CV = 27.4%) vs (SD = 20.55 mg/dL, CV = 17.53%); and within 1 year following surgery (SD = 45.04 mg/dL, CV = 29.04%) vs (SD = 21.36 mg/dL, CV = 18.6%). Associations were found between lower body mass index and higher glycemic variability (i.e., SD [r = -.413, p < .05] and CV [r = -.481, p < .01]) within 1 month following surgery in patients with T2D. Higher preoperative glucose was associated with higher glycemic variability (i.e., SD r = .448, p < .01) within 1 year in patients with T2D. Demographic and clinical characteristics were weakly associated with glycemic variability in patients without T2D., Conclusions: Patients with stage II-III colon cancer with T2D experienced higher glycemic variability within 1 month and within 1 year following surgery compared to those without T2D. Associations between glycemic variability and demographic and clinical characteristics differed by T2D status. Further research in prospective studies is warranted.

Nondestructive quantum measurements are central for quantum physics applications ranging from quantum sensing to quantum computing and quantum communication. Employing the toolbox of cavity quantum electrodynamics, we here concatenate two identical nondestructive photon detectors to repeatedly detect and track a single photon propagating through a 60 m long optical fiber. By demonstrating that the combined signal-to-noise ratio of the two detectors surpasses each single one by about 2 orders of magnitude, we experimentally verify a key practical benefit of cascaded nondemolition detectors compared to conventional absorbing devices.

Quantum teleportation enables the deterministic exchange of qubits via lossy channels. While it is commonly believed that unconditional teleportation requires a preshared entangled qubit pair, here we demonstrate a protocol that is in principle unconditional and requires only a single photon as an ex-ante prepared resource. The photon successively interacts, first, with the receiver and then with the sender qubit memory. Its detection, followed by classical communication, heralds a successful teleportation. We teleport six mutually unbiased qubit states with average fidelity F[over ¯]=(88.3±1.3)% at a rate of 6 Hz over 60 m.

One of the biggest challenges in experimental quantum information is to sustain the fragile superposition state of a qubit 1 . Long lifetimes can be achieved for material qubit carriers as memories 2 , at least in principle, but not for propagating photons that are rapidly lost by absorption, diffraction or scattering 3 . The loss problem can be mitigated with a nondestructive photonic qubit detector that heralds the photon without destroying the encoded qubit. Such a detector is envisioned to facilitate protocols in which distributed tasks depend on the successful dissemination of photonic qubits 4,5 , improve loss-sensitive qubit measurements 6,7 and enable certain quantum key distribution attacks 8 . Here we demonstrate such a detector based on a single atom in two crossed fibre-based optical resonators, one for qubit-insensitive atom-photon coupling and the other for atomic-state detection 9 . We achieve a nondestructive detection efficiency upon qubit survival of 79 ± 3 per cent and a photon survival probability of 31 ± 1 per cent, and we preserve the qubit information with a fidelity of 96.2 ± 0.3 per cent. To illustrate the potential of our detector, we show that it can, with the current parameters, improve the rate and fidelity of long-distance entanglement and quantum state distribution compared to previous methods, provide resource optimization via qubit amplification and enable detection-loophole-free Bell tests.

The big challenge in quantum computing is to realize scalable multi-qubit systems with cross-talk-free addressability and efficient coupling of arbitrarily selected qubits. Quantum networks promise a solution by integrating smaller qubit modules to a larger computing cluster. Such a distributed architecture, however, requires the capability to execute quantum-logic gates between distant qubits. Here we experimentally realize such a gate over a distance of 60 meters. We employ an ancillary photon that we successively reflect from two remote qubit modules, followed by a heralding photon detection, which triggers a final qubit rotation. We use the gate for remote entanglement creation of all four Bell states. Residents have the rights and responsibilities of both students and employees. Dismissal of a resident from a training program is traumatic and has lasting repercussions for the program director, the faculty, the dismissed resident, and the residency. A review of English language literature was performed using PUBMED and OVID databases, using the search terms, resident dismissal, resident termination, student dismissal, student and resident evaluation, legal aspects of education, and remediation. The references of each publication were also reviewed to identify additional appropriate citations. If the Just Cause threshold has been met, educators have the absolute discretion to evaluate academic and clinical performance. Legal opinion has stated that it is not necessary to wait until a patient is harmed to dismiss a resident. Evaluations should be standard and robust. Negative evaluations are not defamatory as the resident gave consent to be evaluated. Provided departmental and institutional polices have been followed, a resident can be dismissed without a formal hearing. Residencies are entitled to modify academic requirements and dismissal is not considered a breach of contract. Although there is anxiety regarding resident dismissal, the courts have uniformly supported faculty having this role. When indicated, failure to dismiss a resident also places the program director and the faculty at risk for educational malpractice.

Background: Treating opioid-addicted women with methadone in pregnancy increased the number of newborns suffering from neonatal abstinence syndrome (NAS). High-pitch crying, insomnia, tremor, myoclonic jerks, vomiting, diarrhoea and poor weight gain were reported symptoms, which were evaluated using the Finnegan (F)-score. Earlier phenobarbital or paregoric had been used to suppress symptoms. We surveyed the administration of pure mu-agonist morphine (MO) in comparison to the alcoholic opioid mixture in tincture of opium (TO). Thirty-three newborns were included in the survey, after informed consent by their parents., Results: NAS started 3-5 days after delivery and lasted for 27 or 30 days (mean) in the TO and MO groups, respectively. In either of the tested parameters, we found no significant differences between the two groups (2P < 0.05). The maximum F-score was similar in both groups, but the dose to suppress NAS was higher in the MO group (0.6-0.5 mg/day; total dose 61.6-42.7 mg of morphine). The duration of the therapy was longer in the MO than in the TO group (37.5-32.4 days). On the other hand the weight gain was better in the MO group than in the TO group (25-19 g/day), but was reduced in both groups compared with healthy newborns., Conclusions: Morphine is suitable to treat NAS in a similar manner as tincture of opium, but avoids unwanted effects of the alcoholic extracts with various alkaloids in the tincture of opium and allows better weight gain of the newborns.
