Abstract Introduction Eight-week glecaprevir/pibrentasvir (GLE/PIB) is indicated for treatment-naïve (TN) patients with chronic hepatitis C (CHC), with or without compensated cirrhosis. Given that the Taiwanese government is committed to eliminating hepatitis C virus (HCV) by 2025, this study aimed to measure real-world evidence for TN patients using 8-week GLE/PIB in the Taiwan HCV Registry (TACR). Methods The data of patients with CHC treated with 8-week GLE/PIB were retrieved from TACR, a nationwide registry program organized by the Taiwan Association for the Study of the Liver (TASL). Treatment efficacy, defined as a sustained virologic response at posttreatment week 12 (SVR12), was assessed in the modified intention-to-treat (mITT) population, which excluded patients who were lost to follow-up or lacked SVR12 data. The safety profile of the ITT population was assessed. Results A total of 7246 (6897 without cirrhosis; 349 with cirrhosis) patients received at least one dose of GLE/PIB (ITT), 7204 of whom had SVR12 data available (mITT). The overall SVR12 rate was 98.9% (7122/7204) among all patients, 98.9% (6780/6856) and 98.3% (342/348) among patients without and with cirrhosis, respectively. For the selected subgroups, which included patients with genotype 3 infection, diabetes, chronic kidney disease, people who injected drugs, and those with human immunodeficiency virus coinfection, the SVR12 rates were 95.1% (272/286), 98.9% (1084/1096), 99.0% (1171/1183), 97.4% (566/581), and 96.1% (248/258), respectively. Overall, 14.1% (1021/7246) of the patients experienced adverse events (AEs). Twenty-two patients (0.3%) experienced serious AEs, and 15 events (0.2%) resulted in permanent drug discontinuation. Only one event was considered treatment drug related. Conclusion Eight-week GLE/PIB therapy was effective and well tolerated in all TN patients, regardless of cirrhosis status.

ABSTRACTLow thermal conductivity and excellent high-temperature stability make lanthanum zirconate (La2Zr2O7, LZO) a very promising material for high-temperature insulation applications. In this study, LZO porous ceramics with ultra-high porosity and an open-pore structure were prepared by the replica method using a polyurethane template. The flowability and coating quality of the ceramic slurry were optimized. When the dispersant content of the slurry was 0.6 wt% and the alcohol to water ratio was 1:1, the slurry had low surface tension and low viscosity, leading to good coating quality of the ceramic slurry on the sponge template. The pore size between hollow skeletons was 100–500 μm with the hollow structure of the skeletons forming secondary pores. LZO porous ceramics showed high porosity of 87.6%-96.0%, low thermal conductivity of 0.048–0.126 W/(m·K), and compressive strength of 0.13–0.74 MPa by adjusting the ceramic content. A high temperature insulation performance test showed that the temperature difference between the cold side and the hot side of the sample was over 1000°C, indicating the excellent thermal insulation properties of LZO porous ceramics. LZO porous ceramics had good high-temperature stability without sintering shrinkage at heat treatment temperatures below 1500°C.

The role of the gut microbiota in host physiology has been previously elucidated for some marine organisms, but little information is available on their metabolic activity involved in transformation of environmental pollutants. This study assessed the metabolic profiles of the gut microbial cultures from grouper (Epinephelus coioides), green mussel (Perna viridis) and giant tiger prawn (Penaeus monodon) and investigated their transformation mechanisms to typical plastic additives. Community-level physiological profiling analysis confirmed the utilization profiles of the microbial cultures including carbon sources of carbohydrates, amines, carboxylic acids, phenolic compounds, polymers and amino acids, and the plastic additives of organophosphate flame retardants, tetrabromobisphenol A derivates and bisphenols. Using in vitro incubation, triphenyl phosphate (TPHP) was found to be rapidly metabolized into diphenyl phosphate by the gut microbiota as a representative ester-containing plastic additive, whereas the transformation of BPA (a representative phenol) was relatively slower. Interestingly, all three kinds of microbial cultures efficiently transformed the hepatic metabolite of BPA (BPA-G) back to BPA, thereby increasing its bioavailability in the body. The specific enzyme analysis confirmed the ability of the gut microbiota to perform the metabolic reactions. The results of 16S rRNA sequencing and network analysis revealed that the genera Escherichia-Shigella, Citrobacter, and Anaerospora were functional microbes, and their collaboration with fermentative microbes played pivotal roles in the transformation of the plastic additives. The structure-specific transformations by the gut microbiota and their distinct bioavailability deserve more attention in the future.

Basalt fiber reinforced recycled aggregate concrete (BFRAC) is a high-performance, environmentally friendly material that combines lightweight, high-strength fibers with low-carbon recycled aggregates (RAs), positioned for extensive use in building structures. However, research on its constitutive relationships is currently scarce, which partly restricts component design and analysis. In this context, the current study thoroughly explores the stress–strain relationship and variability of BFRAC under compression, using 240 cylinders for testing to investigate the influence of factors like coarse/fine RA sources, RA replacement rates, and fiber dosage. The study found that the addition of RAs and fibers reduced the workability of the mixture, particularly with the inclusion of fine RAs and short-cut fibers. Using coarse and fine RAs generally reduces the material’s elastic modulus, compressive strength, and post-peak ductility. Adding fibers can slightly improve compressive strength and peak strain, significantly reduce material brittleness, and have a minimal impact on elastic modulus. Importantly, the study noted that the pre-peak segment of the stress–strain curve of BFRAC is most sensitive to the addition of fine RAs, while the post-peak segment is most sensitive to fiber content. Despite this, using high-quality RAs up to 50% replacement and adding 0.4% by volume of fiber can make BFRAC with mechanical properties comparable to natural aggregate concrete. Based on the observed tests, this paper proposes constitutive relationships that incorporate skeleton curves and variability at different points for the compressive stress–strain behavior of BFRAC.

Existing semi-empirical formulas for predicting punching shear capacity in FRP bar reinforced concrete flat slabs without shear reinforcement often prove inaccurate and unstable. This is primarily due to limited modeling data, inadequate consideration of key variables and neglect of complex nonlinear relationships. To address these challenges, this study delves into the utilization of advanced machine learning (ML) algorithms to offer precise and dependable estimates of punching shear capacity in such structural components. The study initially compiled a comprehensive database comprising 165 sets of test data, integrating eight crucial variables for model development. Subsequently, four data-driven models including back propagation artificial neural network (BPANN), support vector regression (SVR), random forest (RF) and gradient boosting regression tree (GBRT) were formulated to estimate the shear capacity. The efficacy of these models was assessed in comparison to existing prediction formulas. To interpret the models, this study also introduced shapley additive explanation (SHAP) and partial dependence plot (PDP) to quantitatively evaluate the influence of variables on predicted results. Research findings suggest that: (a) Among 25 existing formulas, Ju et al.’s approach performs notably well, with R2, Pre/Exp, MAPE and RMSE values at 0.76, 1.02, 22.2 % and 142.8 kN, respectively. (b) ML models surpass traditional formulas in predictive accuracy, with R2, Pre/Exp, MAPE and RMSE values ranging from 0.89 to 0.93, 1.03–1.09, 4.8–9.5 % and 55.4–69.0 kN, respectively. The GBRT model demonstrates the highest precision. (c) SHAP analysis of the GBRT model reveals that effective slab height and column section aspect ratio are pivotal variables influencing punching shear capacity. (d) PDP analysis quantitatively illustrates how punching shear capacity varies with each key variable.

Background/Aims Despite the high efficacy of direct-acting antivirals (DAAs), approximately 1–3% of hepatitis C virus (HCV) patients fail to achieve a sustained virological response. We conducted a nationwide study to investigate risk factors associated with DAA treatment failure. Machine-learning algorithms have been applied to discriminate subjects who may fail to respond to DAA therapy. Methods We analyzed the Taiwan HCV Registry Program database to explore predictors of DAA failure in HCV patients. Fifty-five host and virological features were assessed using multivariate logistic regression, decision tree, random forest, eXtreme Gradient Boosting (XGBoost), and artificial neural network. The primary outcome was undetectable HCV RNA at 12 weeks after the end of treatment. Results The training (n=23,955) and validation (n=10,346) datasets had similar baseline demographics, with an overall DAA failure rate of 1.6% (n=538). Multivariate logistic regression analysis revealed that liver cirrhosis, hepatocellular carcinoma, poor DAA adherence, and higher hemoglobin A1c were significantly associated with virological failure. XGBoost outperformed the other algorithms and logistic regression models, with an area under the receiver operating characteristic curve of 1.000 in the training dataset and 0.803 in the validation dataset. The top five predictors of treatment failure were HCV RNA, body mass index, α-fetoprotein, platelets, and FIB-4 index. The accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of the XGBoost model (cutoff value=0.5) were 99.5%, 69.7%, 99.9%, 97.4%, and 99.5%, respectively, for the entire dataset. Conclusions Machine learning algorithms effectively provide risk stratification for DAA failure and additional information on the factors associated with DAA failure.

Abstract Background Asthma is the most common chronic disease among children and poses a significant threat to their health. This study aims to assess the relationship between various plasma proteins and childhood asthma, thereby identifying potential therapeutic targets. Methods Based on publicly available genome‐wide association study summary statistics, we employed a two‐sample Mendelian randomization (MR) approach to elucidate the causal relationship between plasma proteins and asthma. Mediation analysis was then conducted to evaluate the indirect influence of plasma proteins on childhood asthma mediated through risk factors. Comprehensive analysis was also conducted to explore the association between plasma proteins and various phenotypes using the UK Biobank dataset. Results MR analysis uncovered a causal relationship between 10 plasma proteins and childhood asthma. Elevated levels of seven proteins (TLR4, UBP25, CBR1, Rac GTPase‐activating protein 1 [RGAP1], IL‐21, MICB, and PDE4D) and decreased levels of three proteins (GSTO1, LIRB4 and PIGF) were associated with an increased risk of childhood asthma. Our findings further validated the connections between reported risk factors (body mass index, mood swings, hay fever or allergic rhinitis, and eczema or dermatitis) and childhood asthma. Mediation analysis revealed the influence of proteins on childhood asthma outcomes through risk factors. Furthermore, the MR analysis identified 73 plasma proteins that exhibited causal associations with at least one risk factor for childhood asthma. Among them, RGAP1 mediates a significant proportion (25.10%) of the risk of childhood asthma through eczema or dermatitis. Finally, a phenotype‐wide association study based on these 10 proteins and 1403 diseases provided novel associations between these biomarkers and multiple phenotypes. Conclusion Our study comprehensively investigated the causal relationship between plasma proteins and childhood asthma, providing novel insights into potential therapeutic targets.

Abstract This multicenter retrospective study was conducted to explore the effects of different courses and durations of invasive mechanical ventilation (MV) on the respiratory outcomes of very low birth weight infants (VLBWI) in China. The population for this study consisted of infants with birth weight less than 1500 g needing at least 1 course of invasive MV and admitted to the neonatal intensive care units affiliated with the Chinese Neonatal Network within 6 h of life from January 1st, 2019 to December 31st, 2020. Univariate and multivariate logistic regression analyses were performed to evaluate associations between invasive MV and respiratory outcomes. Adjusted odds ratios (ORs) were computed with the effects of potential confounders. (1) Among the 3183 VLBWs with a history of at least one course of invasive MV, 3155 (99.1%) met inclusion criteria and were assessed for the primary outcome. Most infants received one course (76.8%) and a shorter duration of invasive MV (62.16% with ventilation for 7 days or less). (2) In terms of the incidence of all bronchopulmonary dysplasia (BPD) (mild, moderate, and severe BPD), there were no significant differences between different invasive MV courses [For 2 courses, adjusted OR = 1.11 (0.88, 1.39); For 3 courses or more, adjusted OR = 1.07 (0.72, 1.60)]. But, with the duration of invasive MV prolonging, the OR of BPD increased [8–21 days, adjusted OR = 1.98 (1.59, 2.45); 22–35 days, adjusted OR = 4.37 (3.17, 6.03); ≥ 36 days, adjusted OR = 18.44 (10.98, 30.99)]. Concerning severe BPD, the OR increased not only with the course of invasive MV but also with the duration of invasive MV [For 2 courses, adjusted OR = 2.17 (1.07, 4.40); For 3 courses or more, adjusted OR = 2.59 (1.02, 6.61). 8–21 days, adjusted OR = 8.42 (3.22, 22.01); 22–35 days, adjusted OR = 27.82 (9.08, 85.22); ≥ 36 days, adjusted OR = 616.45 (195.79, > 999.999)]. (3) When the interaction effect between invasive MV duration and invasive MV course was considered, it was found that there were no interactive effects in BPD and severe BPD. Greater than or equal to three courses would increase the chance of severe BPD, death, and the requirement of home oxygen therapy. Compared with distinct courses of invasive MV, a longer duration of invasive MV (> 7 days) has a greater effect on the risk of BPD, severe BPD, death, and the requirement of home oxygen therapy.

The optimal strategy for the microelimination of HCV within community settings remains ambiguous. We evaluated the percentage of participants who achieved linkage to care (LTC) following the conclusion of a screening campaign and examined the diverse factors influencing LTC among these individuals. The effectiveness of recall intervention for the non-LTC population and its barriers were analyzed. We initiated an HCV patient recall program to identify HCV participants who might not be treated after the HCV screening campaign. The program staff recalled HCV participants who were lost to follow-up via telephone from March 2019 to June 2019. They were informed of HCV treatment’s importance, efficacy, availability, and safety. Among 185 participants infected with HCV, 109 (58.9%) obtained LTC. Compared with those who had LTC, those without LTC were older, had lower education levels, were less aware of their HCV infection, less frequently lived in urban areas, and had less health insurance. At the end of the recall program, 125 (67.6%) persons had linkage to care. The proportion of LTC increased by 8.7%. In total, 119 persons had an HCV RNA test, and 82 (68.9%) had viremia. Of the 82 patients with viremia, 78 (95.1%) received antiviral therapy, and 76 (97.4%) achieved a sustained virological response. After a community screening campaign, 59% of participants with anti-HCV-positive tests had LTC. The recall program increased this by 9%. However, 32% of HCV participants still could not be linked to care. Outreach care for non-LTC patients is a method worth trying in order to achieve the microelimination of HCV in rural communities.

California has experienced a surge in wildfires, prompting research into contributing factors, including weather and climate conditions. This study investigates the complex, multiscale interactions between large-scale climate patterns, such as the Boreal Summer Intraseasonal Oscillation (BSISO), El Niño Southern Oscillation (ENSO), and the Pacific Decadal Oscillation (PDO) and their influence on moisture and temperature fluctuations, and wildfire dynamics in California. The combined impacts of PDO and BSISO on intraseasonal fire weather changes; the interplay between fire weather index (FWI), relative humidity, vapor pressure deficit (VPD), and temperature in assessing wildfire risks; and geographical variations in the relationship between the FWI and climatic factors within California are examined. The study employs a multi-pronged approach, analyzing wildfire frequency and burned areas alongside climate patterns and atmospheric conditions. The findings reveal significant variability in wildfire activity across different climate conditions, with heightened risks during specific BSISO phases, La-Niña, and cool PDO. The influence of BSISO varies depending on its interaction with PDO. Temperature, relative humidity, and VPD show strong predictive significance for wildfire risks, with significant relationships between FWI and temperature in elevated regions (correlation, r > 0.7, p ≤ 0.05) and FWI and relative humidity along the Sierra Nevada Mountains (r ≤ −0.7, p ≤ 0.05).

Abstract Background Betel nut chewing involves the chewing of areca nuts or betel quid (areca nuts wrapped in betel leaves), which is associated with an increased risk of esophageal squamous cell carcinoma (ESCC). Statins have anticancer properties. We investigated the association between statin use and ESCC risk in betel nut chewers. Methods The study included 105 387 betel nut chewers matched statin users and nonusers. Statin use was defined as the use of ≥28 cumulative defined daily doses (cDDDs) of statin. The primary outcome was incidence of ESCC. Results The incidence rate of ESCC was significantly lower in statin users than in nonusers (2.03 vs. 3.02 per 100 000 person‐years). Statin users had a lower incidence rate ratio of 0.66 for ESCC (95% confidence interval [CI]: 0.43–0.85) relative to nonusers. After potential confounders were adjusted for, statin use was determined to be associated with a reduced risk of ESCC (adjusted hazard ratio [aHR], 0.68; 95% CI: 0.51–0.91). A dose–response relationship was observed between statin use and ESCC risk; the aHRs for statin use at 28–182 cDDDs, 183–488 cDDDs, 489–1043 cDDDs, and > 1043 cDDDs were 0.92, 0.89, 0.66, and 0.64, respectively. Conclusion Statin use was revealed to be associated with a reduced risk of ESCC in betel nut chewers.

α-SiAlON is a potential wave-transmittance material for radome materials, yet the comprehensive properties need to be improved. In this study, a series of SrxY0.5-xSi12-(m + n) Alm +nOnN16-n (x = 0.02, 0.04, 0.06, and 0.08) porous ceramics with porosities ranging from 44.15% to 58.62% were synthesized via tert-butanol gel-casting. The real part of the permittivity of porous ceramics was in the range of 3.13–4.18 (8.2–12.4 GHz) with over 70% wave transmittance in the thickness range of 5.5–9.5 mm. The thermal conductivity and flexural strength of porous ceramics were in the ranges of 1.063–1.703 W/(m·K) and 52.67–106.33 MPa, respectively. Compared with Y-α-SiAlON porous ceramics, the thermal insulation and mechanical properties of Sr/Y-α-SiAlON porous ceramics are improved without deteriorating wave transmittance properties. The results show that Sr/Y-α-SiAlON porous ceramics have the potential to be wave-transmittance materials by adjusting the wave transmittance, thermal insulation, and mechanical properties of α-SiAlON.

Vertical thin-film transistors (V-TFTs) with an InSnO-stabilized ZnO channel were fabricated. The vertical architecture enables devices with submicron channel lengths (≤500 nm) to afford delivering drain current greatly exceeding that of conventional planar TFTs. Due to the submicron length of the V-TFT channel, an on/off state current higher than 107 can be achieved even with a drain voltage of 0.01 V, and the subthreshold swing was kept in the tens of mV/dec range owing to the efficacious device preparation. In order to understand the influence of structures on the device performance, the source–drain (S/D) contact and the channel length of V-TFTs were designed and studied. The results show that the increase in the contact area between the active layer and the S/D region can reduce the S/D contact resistance, thus affecting the drain current across the threshold region. When the channel length is shortened to a deep submicron size, the electrostatic coupling between the source and drain electrodes will lead to a decrease in the S/D barrier. This leads to the leakage-induced barrier reduction effect of V-TFTs.

Rheumatoid arthritis (RA) is an inflammatory disease that leads to disability; however, existing therapies are still unsatisfactory. Activated fibroblast-like synoviocytes (FLSs) play an essential role in synovitis formation and joint destruction in RA. The Hedgehog signaling pathway is aberrantly activated and contributes to the aggressive phenotype of RA-FLSs. However, it remains uncertain whether inhibiting Smoothened (SMO), a critical component of the Hedgehog signaling pathway, is an effective treatment for RA. Here, we design a series of small interfering RNAs (siRNAs) that specifically target the SMO gene. With precise chemical modifications, siRNAs’ efficacy and stability are significantly improved, and the off-target effects are minimized. The optimized chemically modified siRNA (si-S1A3-Chol) decreases RA-FLS proliferation and invasiveness without the transfection reagent. Furthermore, si-S1A3-Chol injected intra-articularly effectively alleviates joint destruction and improves motor function in collagen-induced arthritis mouse models. Consequently, our results demonstrate that chemically modified siRNA targeting the Hedgehog signaling pathway may be a potential therapy for RA.

Abstract Background Factors of metabolic syndrome such as obesity are well‐known risk factors for gallstone disease (GSD). There are different indicators of obesity, including weight, body mass index, waist circumference, and waist‐to‐height ratio. The predictive ability of different obesity indicators for GSD remains unclear. Objective To explore the most efficient predictor of GSD among the different anthropometric indicators of obesity. Methods This population‐based cross‐sectional study included 2263 participants who completed a questionnaire detailing their demographics, medical history, and lifestyle between 2014 and 2017 in Taiwan. Blood samples were collected and physical examinations, including anthropometric measurements, were performed. Gallstone disease was ascertained using ultrasonography. Multivariate analyses were performed to identify independent risk factors for GSD. Results The overall prevalence of GSD was 8.8%. According to the multivariate analysis, individuals with a waist‐to‐height ratio ≥0.5 (odds ratio|odds ratios (OR) = 1.65, 95% confidence interval (CI) = 1.10−2.48, p = 0.017) had an increased risk of GSD. Diabetes was the main risk factor for GSD in men (OR = 2.06, 95% CI = 1.17−3.65, p = 0.013). Among women, waist‐to‐height ratio >0.5 (OR = 1.76, 95% CI = 1.03−3.02, p = 0.040) and current hormone drug use (OR = 2.73, 95% CI = 1.09−6.84, p = 0.033) were significant risk factors for gallstones. Conclusion GSD was independently associated with central obesity and exogenous hormone intake in women. Among the anthropometric indicators used to assess central obesity, waist‐to‐height ratio was the most accurate predictor of GSD.

Recent studies have shown that chlorogenic acid (CGA), which is present in coffee, has protective effects on the nervous system. However, its role in neonatal hypoxic-ischemic brain injury remains unclear. In this study, we established a newborn mouse model of hypoxic-ischemic brain injury using a modified Rice-Vannucci method and performed intraperitoneal injection of CGA. We found that CGA intervention effectively reduced the volume of cerebral infarct, alleviated cerebral edema, restored brain tissue structure after injury, and promoted axon growth in injured brain tissue. Moreover, CGA pretreatment alleviated oxygen-glucose deprivation damage of primary neurons and promoted neuron survival. In addition, changes in ferroptosis-related proteins caused by hypoxic-ischemic brain injury were partially reversed by CGA. Furthermore, CGA intervention upregulated the expression of the key ferroptosis factor glutathione peroxidase 4 and its upstream glutamate/cystine antiporter related factors SLC7A11 and SLC3A2. In summary, our findings reveal that CGA alleviates hypoxic-ischemic brain injury in neonatal mice by reducing ferroptosis, providing new ideas for the treatment of neonatal hypoxic-ischemic brain injury.

We present sub-50 fs soliton pulse generation from a diode-pumped Kerr-lens mode-locked laser based on an Yb3+-doped BaF2 crystal. Utilizing a spatially single-mode, fiber-coupled InGaAs laser diode at 976 nm as a pump source, the Yb:BaF2 laser generates pulses as short as 46 fs at 1060.1 nm with an average output power of 45 mW at a pulse repetition rate of ~65.6 MHz via soft-aperture Kerr-lens mode locking. To the best of our knowledge, this represents the first demonstration of Kerr-lens mode-locked operation of the Yb:BaF2 crystal, as well as the shortest pulse duration ever achieved from any diode-pumped mode-locked laser based on an Yb3+-doped alkaline-earth fluoride crystal.

Abstract Lanthanum zirconate (LZO) has high‐temperature advantages, including a high melting point, low thermal conductivity, and high structure stability, which has the potential to be used in high‐temperature thermal insulation field. However, low mechanical properties of LZO porous ceramics have limited further applications. In this work, LZO nanofibrous membranes were prepared by electrospinning, and the preparation parameters, including PVP content, spinning voltage, feeding rate, received distance, and the heating process, were investigated to optimize the microstructure of the nanofibrous membranes. The LZO nanofibrous membranes with a grain size of 34.5 nm displayed good morphology after calcining at 1000°C. The LZO fibrous membranes had a very low thermal conductivity of 0.0716 W/m·K at room temperature, showing excellent insulating performance. This work indicates that LZO nanofibrous membranes are promising candidate materials for thermal insulation applications.

In recent years crushing waste brick to produce recycled brick aggregates (RBAs) has become a viable solution for reducing environmental pollution and addressing the natural resource shortage in civil engineering. To promote the widespread use of the recycled brick aggregate concrete (RBAC) in construction, this study analyzes existing test results on the attributes of RBAs and the compressive mechanical behaviors of RBAC. The review results indicate significant differences and variabilities in the characteristics of RBAs compared to natural coarse aggregates and recycled concrete coarse aggregates. RBAs have the highest absorption capacity and crushing index among the three aggregates, leading to changes in the compressive failure mechanism and a decline in the mechanical properties of RBAC. Additionally, it is also observed that existing formulas do not adequately account for the deterioration of the compressive mechanical properties of RBAC. To tackle this problem, artificial intelligence (AI) approaches including artificial neural network and multigene genetic programming are utilized to develop precise models for predicting the compressive strength and elastic modulus of RBAC. It is found that RBAC’s these two mechanical indexes are mainly influenced by the standard strength of cement paste, water-to-cement ratio, sand-to-aggregate mass ratio, RBA replacement ratio and mass-weighted water absorption ratio of coarse aggregates. The AI models developed in this study accurately capture the trends of these factors and offer desirable predictive results.

Abstract Diabetic kidney disease is the leading cause of end-stage kidney disease worldwide; however, the integration of high-dimensional trans-omics data to predict this diabetic complication is rare. We develop artificial intelligence (AI)-assisted models using machine learning algorithms to identify a biomarker signature that predisposes high risk patients with diabetes mellitus (DM) to diabetic kidney disease based on clinical information, untargeted metabolomics, targeted lipidomics and genome-wide single nucleotide polymorphism (SNP) datasets. This involves 618 individuals who are split into training and testing cohorts of 557 and 61 subjects, respectively. Three models are developed. In model 1, the top 20 features selected by AI give an accuracy rate of 0.83 and an area under curve (AUC) of 0.89 when differentiating DM and non-DM individuals. In model 2, among DM patients, a biomarker signature of 10 AI-selected features gives an accuracy rate of 0.70 and an AUC of 0.76 when identifying subjects at high risk of renal impairment. In model 3, among non-DM patients, a biomarker signature of 25 AI-selected features gives an accuracy rate of 0.82 and an AUC of 0.76 when pinpointing subjects at high risk of chronic kidney disease. In addition, the performance of the three models is rigorously verified using an independent validation cohort. Intriguingly, analysis of the protein–protein interaction network of the genes containing the identified SNPs (RPTOR, CLPTM1L, ALDH1L1, LY6D, PCDH9, B3GNTL1, CDS1, ADCYAP and FAM53A) reveals that, at the molecular level, there seems to be interconnected factors that have an effect on the progression of renal impairment among DM patients. In conclusion, our findings reveal the potential of employing machine learning algorithms to augment traditional methods and our findings suggest what molecular mechanisms may underlie the complex interaction between DM and chronic kidney disease. Moreover, the development of our AI-assisted models will improve precision when diagnosing renal impairment in predisposed patients, both DM and non-DM. Finally, a large prospective cohort study is needed to validate the clinical utility and mechanistic implications of these biomarker signatures.

Background/purpose: The Taiwan Association for the Study of the Liver (TASL) HCV Registry (TACR) is a nationwide registry of chronic hepatitis C patients in Taiwan. This study evaluated antiviral effectiveness of ledipasvir (LDV)/sofosbuvir (SOF) in patients in the TACR. Methods: Patients enrolled in TACR from 2017–2020 treated with LDV/SOF were eligible. The primary outcome was the proportion of patients with sustained virologic response 12 weeks after end of treatment (SVR12). Results: 5644 LDV/SOF ± ribavirin-treated patients were included (mean age: 61.4 years; 54.4% female). Dominant viral genotypes were GT1 (50.8%) and GT2 (39.3%). 1529 (27.1%) patients had liver cirrhosis, including 201 (3.6%) with liver decompensation; 686 (12.2%) had chronic kidney disease. SVR12 was achieved in 98.6% of the overall population and in 98.2% and 98.7% of patients with and without cirrhosis, respectively. SVR12 rates in patients with compensated cirrhosis treated with LDV/SOF without RBV were >98%, regardless of prior treatment experience. SVR12 was 98.6%, 98.4%, 100%, 100%, and 98.7% among those with GT1, GT2, GT4, GT5, and GT6 infections, respectively.
