2. Thinking, making and living architecture in an ageing society : the typological problem in contemporary culture as an instrument of social inclusion for people living well with dementia

The twenty-first-century norm is represented by people living longer. This new demographic structure creates a number of societal challenges. One such challenge is that living longer and becoming old increases the likelihood of acquiring long-term conditions such as dementia. Another challenge is with the difficulties in defining a clear line between normal ageing and pathological ageing; a blurred distinction leads to stigmatising older adults as a social and economic burden. Therefore, there is a need for a care-model shift that is able to cope with a potential increase in demand for high-dependency and high-cost services and also address stigmatisation. Importantly, older adults’ mental and physical well-being should have a central role in this “shift.” A robust and productive relationship between people and space and well-being can have a positive impact. Of course, this shift has inevitable architectural repercussions. Taking this into account, the aim of this thesis is to investigate critically the comparable impact of the Humanitas© setting in Deventer (the Netherlands), the Rudolf© setting in Helsinki (Finland) and the Gojikara Mura© setting in Nagakute (Japan). Specifically, the Humanitas is a nursing home with a population of 50 older adults with dementia, 80 people with severe physical suffering, 20 people with social difficulties, ten people in short stay for recovery and six university students. The Rudolf is a senior home with a population of 18 young adults with mental impairments; 50 older adults with later stages of dementia; 52 older adults with early stages or no mental or physical impairments, and four young adults/university students. The Gojikara Mura© is a multigenerational community that accommodates a child day-care facility, a kindergarten (200 children between 3 – 5 years old), an adult day-care centre, an assisted living complex (50 older adult residents with different level of physical and mental impairments), a nursery school (300 enrolled students between 6 to 12, and 18 to 22 years old), and a nursing home (48 older adult residents with different level of physical and mental impairments). The analysis of these three case studies is based on a one-time post-occupancy evaluation framework and is organised into three parts defined as “thinking, making and living.” The method reveals the principles for conceiving a new architectural type: the “open type,” which is grounded in the notions of an “open architecture” and an “open city.” This “open type” promotes multidisciplinary, collaborative and socially inclusive design principles, and thereby order as a result. Consequently, a renewed adoption of Aristotelian ethics is revealed. They concern with the notion of dwelling that emphasises the value of socially inclusive forms of collective life. In this thesis, they are translated into a new pattern of care, which is built around participatory and coherent collaborative teamwork among different groups of healthcare providers, volunteers, residents and their families. The “open type” setting, therefore, addresses both a new architectural design and new care types with a focus on normalising ageing and tearing down personal and socioeconomic stigma. It may give form to a new type of housing stock not existing in Western countries and thereby institutions and governments must politically and economically support these initiatives.

Background and purpose: Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method: Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results: From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p=0.005) and more in smoker patients (OR 1.39; p=0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p=0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p=0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p=0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p=0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p=0.024), joint or muscle pain (G2, p=0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion: Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

- Abstract
Background and purpose Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p = 0.005) and more in smoker patients (OR 1.39; p = 0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p = 0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p = 0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p = 0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p = 0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p = 0.024), joint or muscle pain (G2, p = 0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

- Abstract
Background: The MuSC-19 project is an Italian cohort study open to international partners that collects data on multiple sclerosis (MS) patients with COVID-19. During the second wave of the pandemic, serological tests became routinely available. Objective: To evaluate the seroprevalence of anti-SARS-CoV-2 antibodies according to the use of disease-modifying therapy (DMT) in a subset of patients included in the MuSC-19 data set who had undergone a serological test. Methods: We evaluated the association between positive serological test results and time elapsed since infection onset, age, sex, Expanded Disability Status Scale score, comorbidities and DMT exposure using a multivariable logistic model. Results: Data were collected from 423 patients (345 from Italy, 61 from Turkey and 17 from Brazil) with a serological test performed during follow-up. Overall, 325 out of 423 tested patients (76.8%) had a positive serological test. At multivariate analysis, therapy with anti-CD20 was significantly associated with a reduced probability of developing antibodies after COVID-19 (odds ratio (OR) = 0.20, p = 0.002). Conclusion: Patients with MS maintain the capacity to develop humoral immune response against SARS-COV-2, although to a lesser extent when treated with anti-CD20 drugs. Overall, our results are reassuring with respect to the possibility to achieve sufficient immunization with vaccination.

Background: In patients with Multiple Sclerosis (pwMS) disease-modifying therapies (DMTs) affects immune response to antigens. Therefore, post-vaccination serological assessments are needed to evaluate the effect of the vaccine on SARS-CoV-2 antibody response. Methods: We designed a prospective multicenter cohort study enrolling pwMS who were scheduled for SARS-Cov-2 vaccination with mRNA vaccines (BNT162b2, Pfizer/BioNTech,Inc or mRNA-1273, Moderna Tx,Inc). A blood collection before the first vaccine dose and 4 weeks after the second dose was planned, with a centralized serological assessment (electrochemiluminescence immunoassay, ECLIA, Roche-Diagnostics). The log-transform of the antibody levels was analyzed by multivariable linear regression. Findings: 780 pwMS (76% BNT162b2 and 24% mRNA-1273) had pre- and 4-week post-vaccination blood assessments. 87 (11·2%) were untreated, 154 (19·7%) on ocrelizumab, 25 (3·2%) on rituximab, 85 (10·9%) on fingolimod, 25 (3·2%) on cladribine and 404 (51·7%) on other DMTs. 677 patients (86·8%) had detectable post-vaccination SARS-CoV-2 antibodies. At multivariable analysis, the antibody levels of patients on ocrelizumab (201-fold decrease (95%CI=128–317), p < 0·001), fingolimod (26-fold decrease (95%CI=16–42), p < 0·001) and rituximab (20-fold decrease (95%CI=10–43), p < 0·001) were significantly reduced as compared to untreated patients. Vaccination with mRNA-1273 resulted in a systematically 3·25-fold higher antibody level (95%CI=2·46–4·27) than with the BNT162b2 vaccine (p < 0·001). The antibody levels on anti-CD20 therapies correlated to the time since last infusion, and rituximab had longer intervals (mean=386 days) than ocrelizumab patients (mean=129 days). Interpretation: In pwMS, anti-CD20 treatment and fingolimod led to a reduced humoral response to mRNA-based SARS-CoV-2 vaccines. As mRNA-1273 elicits 3·25-higher antibody levels than BNT162b2, this vaccine may be preferentially considered for patients under anti-CD20 treatment or fingolimod. Combining our data with those on the cellular immune response to vaccines, and including clinical follow-up, will contribute to better define the most appropriate SARS-CoV-2 vaccine strategies in the context of DMTs and MS. - Abstract
Objective: This study was undertaken to assess the impact of immunosuppressive and immunomodulatory therapies on the severity of coronavirus disease 2019 (COVID-19) in people with multiple sclerosis (PwMS). Methods: We retrospectively collected data of PwMS with suspected or confirmed COVID-19. All the patients had complete follow-up to death or recovery. Severe COVID-19 was defined by a 3-level variable: mild disease not requiring hospitalization versus pneumonia or hospitalization versus intensive care unit (ICU) admission or death. We evaluated baseline characteristics and MS therapies associated with severe COVID-19 by multivariate and propensity score (PS)-weighted ordinal logistic models. Sensitivity analyses were run to confirm the results. Results: Of 844 PwMS with suspected (n = 565) or confirmed (n = 279) COVID-19, 13 (1.54%) died; 11 of them were in a progressive MS phase, and 8 were without any therapy. Thirty-eight (4.5%) were admitted to an ICU; 99 (11.7%) had radiologically documented pneumonia; 96 (11.4%) were hospitalized. After adjusting for region, age, sex, progressive MS course, Expanded Disability Status Scale, disease duration, body mass index, comorbidities, and recent methylprednisolone use, therapy with an anti-CD20 agent (ocrelizumab or rituximab) was significantly associated (odds ratio [OR] = 2.37, 95% confidence interval [CI] = 1.18–4.74, p = 0.015) with increased risk of severe COVID-19. Recent use (<1 month) of methylprednisolone was also associated with a worse outcome (OR = 5.24, 95% CI = 2.20–12.53, p = 0.001). Results were confirmed by the PS-weighted analysis and by all the sensitivity analyses. Interpretation: This study showed an acceptable level of safety of therapies with a broad array of mechanisms of action. However, some specific elements of risk emerged. - Abstract
Background: To minimize the risk of Progressive Multifocal Leukoencephalopathy and rebound in JCV-positive multiple sclerosis (MS) patients after 24 natalizumab doses, it has been proposed to extend the administrations interval. The objective is to evaluate the EID efficacy on MRI activity compared with the standard interval dosing (SID). Methods: Observational, multicentre, retrospective cohort study, starting from the 24th natalizumab infusion to the loss of follow-up or 2 years after baseline. Three hundred and sixteen patients were enrolled. The median dose interval (MDI) following the 24th infusion was 5 weeks, with a bimodal distribution (modes at 4 and 6 weeks). Patients were grouped into 2 categories according to the mean number of weeks between doses: < 5 weeks, SID; ≥ 5 weeks, EID. Results: One hundred and eighty-seven patients were in the SID group (MDI = 4.5 weeks) and 129 in the EID group (MDI 6.1 weeks). The risk to develop active lesions on MRI is similar in SID and EID groups during the 6 and 12 months after the 24th natalizumab infusion, respectively 4.27% (95% CI:0.84–7.70) vs 4.71% (95% CI:0.16–9.25%) [p = 0.89] and 8.50% (95% CI:4.05–12.95) vs 6.55% (95% CI:2.11–11.00%) [p = 0.56]. The EID regimen does not appear to increase the occurrence of MRI activity during follow-up. Conclusion: There is no evidence of the reduced efficacy of natalizumab in an EID setting regarding the MRI activity. This observation supports the need for a bigger randomized study to assess the need to change the standard of the natalizumab dosing schedule, to better manage JCV-positive patients.

- Abstract
Objective: To identify baseline factors associated with disease activity in patients with relapsing–remitting multiple sclerosis (RRMS) under teriflunomide treatment. Methods: This was an independent, multi-centre, retrospective post-marketing study. We analysed data of 1,507 patients who started teriflunomide since October 2014 and were regularly followed in 28 Centres in Italy. We reported the proportions of patients who discontinued treatment (after excluding 32 lost to follow-up) and who experienced clinical disease activity, i.e., relapse(s) and/or confirmed disability worsening, as assessed by the Expanded Disability Status Scale (EDSS). Decision tree-based analysis was performed to identify baseline factors associated with clinical disease activity during teriflunomide treatment. Results: At database lock (September 2020), approximately 29% of patients (430 out of 1,475) discontinued teriflunomide because of disease activity (~ 46%), adverse events (~ 37%), poor tolerability (~ 15%), pregnancy planning (~ 2%). Approximately 28% of patients experienced disease activity over a median follow-up of 2.75 years: ~ 9% had relapses but not disability worsening; ~ 13% had isolated disability worsening; ~ 6% had both relapses and disability worsening. The most important baseline factor associated with disease activity (especially disability worsening) was an EDSS > 4.0 (p < 0.001). In patients with moderate disability level (EDSS 2.0–4.0), disease activity occurred more frequently in case of ≥ 1 pre-treatment relapses (p = 0.025). In patients with milder disability level (EDSS < 2.0), disease activity occurred more frequently after previous exposure to ≥ 2 disease-modifying treatments (p = 0.007). Conclusions: Our study suggests a place-in-therapy for teriflunomide in naïve patients with mild disability level or in those who switched their initial treatment for poor tolerability. The twenty-first-century norm is represented by people living longer. This new demographic structure creates a number of societal challenges. One such challenge is that living longer and becoming old increases the likelihood of acquiring long-term conditions such as dementia. Another challenge is with the difficulties in defining a clear line between normal ageing and pathological ageing; a blurred distinction leads to stigmatising older adults as a social and economic burden. Therefore, there is a need for a care-model shift that is able to cope with a potential increase in demand for high-dependency and high-cost services and also address stigmatisation. Importantly, older adults’ mental and physical well-being should have a central role in this “shift.” A robust and productive relationship between people and space and well-being can have a positive impact. Of course, this shift has inevitable architectural repercussions. Taking this into account, the aim of this thesis is to investigate critically the comparable impact of the Humanitas© setting in Deventer (the Netherlands), the Rudolf© setting in Helsinki (Finland) and the Gojikara Mura© setting in Nagakute (Japan). Specifically, the Humanitas is a nursing home with a population of 50 older adults with dementia, 80 people with severe physical suffering, 20 people with social difficulties, ten people in short stay for recovery and six university students. The Rudolf is a senior home with a population of 18 young adults with mental impairments; 50 older adults with later stages of dementia; 52 older adults with early stages or no mental or physical impairments, and four young adults/university students. The Gojikara Mura© is a multigenerational community that accommodates a child day-care facility, a kindergarten (200 children between 3 – 5 years old), an adult day-care centre, an assisted living complex (50 older adult residents with different level of physical and mental impairments), a nursery school (300 enrolled students between 6 to 12, and 18 to 22 years old), and a nursing home (48 older adult residents with different level of physical and mental impairments). The analysis of these three case studies is based on a one-time post-occupancy evaluation framework and is organised into three parts defined as “thinking, making and living.” The method reveals the principles for conceiving a new architectural type: the “open type,” which is grounded in the notions of an “open architecture” and an “open city.” This “open type” promotes multidisciplinary, collaborative and socially inclusive design principles, and thereby order as a result. Consequently, a renewed adoption of Aristotelian ethics is revealed. They concern with the notion of dwelling that emphasises the value of socially inclusive forms of collective life. In this thesis, they are translated into a new pattern of care, which is built around participatory and coherent collaborative teamwork among different groups of healthcare providers, volunteers, residents and their families. The “open type” setting, therefore, addresses both a new architectural design and new care types with a focus on normalising ageing and tearing down personal and socioeconomic stigma. It may give form to a new type of housing stock not existing in Western countries and thereby institutions and governments must politically and economically support these initiatives.

- Abstract
Multiple sclerosis (MS) is an autoimmune inflammatory demyelinating central nervous system disorder that is more common in women, with onset often during reproductive years. The female:male sex ratio of MS rose in several regions over the last century, suggesting a possible sex by environmental interaction increasing MS risk in women. Since many with MS are in their childbearing years, family planning, including contraceptive and disease-modifying therapy (DMT) counselling, are important aspects of MS care in women. While some DMTs are likely harmful to the developing fetus, others can be used shortly before or until pregnancy is confirmed. Overall, pregnancy decreases risk of MS relapses, whereas relapse risk may increase postpartum, although pregnancy does not appear to be harmful for long-term prognosis of MS. However, ovarian aging may contribute to disability progression in women with MS. Here, we review sex effects across the lifespan in women with MS, including the effect of sex on MS susceptibility, effects of pregnancy on MS disease activity, and management strategies around pregnancy, including risks associated with DMT use before and during pregnancy, and while breastfeeding. We also review reproductive aging and sexual dysfunction in women with MS.

- Abstract
Extending the natalizumab interval after the 24th administration could reduce the risk of progressive multifocal leukoencephalopathy (PML). The objective is to evaluate the noninferiority of the efficacy of an extended interval dosing (EID) compared with the standard interval dosing (SID) of natalizumab. It is an observational, multicenter (14 Italian centers), retrospective cohort study, starting from the 24th natalizumab infusion to the loss of follow-up or 2 years after baseline. Patients were grouped in 2 categories according to the mean number of weeks between doses: < 5 weeks, SID; ≥ 5 weeks, EID. Three hundred and sixty patients were enrolled. Median dose interval (MDI) following 24th infusion was 4.7 weeks, with a bimodal distribution (modes at 4 and 6 weeks). Two hundred and sixteen patients were in the SID group (MDI = 4.3 weeks) and 144 in the EID group (MDI 6.2 weeks). Annualized relapse rate was 0.060 (95% CI = 0.033–0.087) in the SID group and 0.039 (95% CI = 0.017–0.063) in the EID group. The non-inferiority of EID versus SID was satisfied. In conclusion, there is no evidence of a reduced efficacy of natalizumab in an EID setting. This observation confirms previous results and together with the emerging evidence of a reduced risk of PML associated to an EID, supports the need of a randomized study to assess the need to change the standard of the natalizumab dosing schedule.

- Abstract
The market of the hybrid electric vehicles has been increasing for several years. Different commercial EV and PHEV solutions are available for passenger cars and light vehicles for freight deliveries. However, the market of heavy trucks still regards traditional ICE vehicles powered by diesel oil fuel. The recent interest for electric solutions have been pushing the development of the hybrid solutions, but only micro-hybrid systems are considered feasible for heavy truck applications. The proposed research aims to define a methodological approach with a virtual model in order to simulate the behavior of a hybrid heavy truck. The scope of this research is the feasibility analysis of a retrofit hybrid heavy truck. A real driving cycle has been used in order to obtain reliable results in terms of cost, energy consumption and gas emission. The layout of the hybrid system has been proposed as well as the sizing of battery and electric motor. A commercial tool has been used for the vehicle modelling and simulation. As a test case, an 18-ton truck has been analyzed with a 10-liter diesel engine. Firstly, the simulation of the diesel truck has been reproduced considering the real driving cycle data. Secondly, the simulation activity has been focused on the evaluation of the hybrid system behavior by investigating different battery sizes with the same boundary conditions related to the real driving cycle.

- Abstract
Nowadays in many industrial applications, i.e. electrical household appliances, it is necessary to have a robust and safe control for some variables involved in the analysis of the performances of different products. In addition, the recent ecodesign directives require products increasingly eco-friendly and eco-efficient, preserving high-performance but a low power consumption. For these reasons, the physical prototypes of products require many expensive and complex tests in term of time, resources and qualified personnel involved. To overcome these limitations, the proposed approach is focused on the use of virtual prototyping tools, which support and reduce the expensive physical experiments. The main objective of this paper is the development, implementation and testing of an innovative methodology, which could be an improvement for the sustainable design of induction hobs. Induction heating applied to the domestic cooking has significantly evolved since the first cooking hobs appeared. Different issues such as maximum power available for heating a pot, dimensional compactness of the hobs, or inverter electronics efficiency have achieved a great development. The proposed methodology provides the development of a multi-physic model which is able to estimate the efficiency of the induction hobs starting from the design data of the project. In particular, the multi-physic model is composed by an electromagnetic simulation and a thermal simulation. The electromagnetic simulation, starting from electrical values such as voltage, current and frequency, is able to simulate the eddy current induced in the bottom of the pot, and resistance leads to the Joulean heating of the material. The thermal simulation is able to measure the energy consumption during the operational phase and the temperature reached by the materials. Therefore, the thermal power obtained by the Joulean heating is, at the same time, the output of the electromagnetic simulation and the input of the thermal one. The proposed model can be applied to design product and simulate the performance considering different operating conditions such as different types of cookers, different coils and different materials. Through the use of virtual prototyping tools is possible to control the heat flux on the whole system (stove, pot, water), and to evaluate the energy efficiency during the operational phase. The proposed tool makes the productengineer more aware about decision-making strategies in order to achieve an energy saving, calculated over the whole life cycle.

- Abstract
Patients with multiple sclerosis on long-term injectable therapies may suffer from the so-called “needle fatigue”, i.e., a waning commitment to continue with the prescribed injectable treatment. Therefore, alternative treatment strategies to enhance patients’ adherence are warranted. In this independent, multicentre post-marketing study, we sought to directly compare switching to either teriflunomide (TFN), dimethyl fumarate (DMF), or pegylated interferon (PEG) on treatment persistence and time to first relapse over a 12-month follow-up. We analyzed a total of 621 patients who were free of relapses and gadolinium-enhancing lesions in the year prior to switching to DMF (n = 265), TFN (n = 160), or PEG (n = 196). Time to discontinuation and time to first relapse were explored in the whole population by Cox regression models adjusted for baseline variables and after a 1:1:1 ratio propensity score (PS)-based matching procedure. Treatment discontinuation was more frequent after switching to PEG (28.6%) than DMF (14.7%; hazard ratio [HR] = 0.25, p < 0.001) and TFN (16.9%; HR = 0.27, p < 0.001). We found similar results even in the re-sampled cohort of 222 patients (74 per group) derived by the PS-based matching procedure. The highest discontinuation rate observed in PEG recipient was mainly due to poor tolerability (p = 0.005) and pregnancy planning (p = 0.04). The low number of patients who relapsed over the 12-month follow-up (25 out of 621, approximately 4%) prevented any analysis on the short-term risk of relapse. This real-world study suggests that oral drugs are a better switching option than low-frequency interferon for promoting the short-term treatment persistence in stable patients who do not tolerate injectable drugs.

- Abstract
Objective: To investigate clinical and radiological outcomes of women with relapsing-remitting multiple sclerosis (RRMS) undergoing abortion. Methods: An independent, multicentre retrospective study was conducted collecting data from eight Italian MS centres. We compared the preconception and postabortion annualised relapse rate (ARR) and number of Gadolinium enhancing (Gd+) lesions, by analyses of covariance. Variables associated with postabortion clinical and MRI activity were investigated using Poisson regression models; each abortion was considered as a statistical unit. Results: From 1995 to 2017, we observed 188 abortions (17 elective) in 153 women with RRMS. Abortions occurred after a mean time of 9.5 (4.4) weeks from estimated conception date. In 86 events out of 188, conception happened during treatment with disease modifying drugs. The mean postabortion ARR (0.63±0.74) was significantly increased (p=0.037) compared with the preconception year (0.50±0.71) as well as the postabortion mean number of new Gd+ lesions (0.77±1.40 vs 0.39±1.04; p=0.004). Higher likelihood of relapses was predicted by higher preconception ARR, discontinuation of preconception treatment and elective abortion; the occurrence of new Gd+ lesions was associated with higher preconception number of active lesions, discontinuation of preconception treatment, shorter length of pregnancy maintenance and elective abortion. Conclusions: Abortion was associated with clinical and radiological inflammatory rebound remarkably in the first 12 months postevent. Deregulated proinflammatory processes arising at the early stages of pregnancy might play a role both in MS reactivation and abortion. Women with MS should be counselled about these risks of abortion and followed up accordingly.

- Abstract
Objective: To examine retrospectively the effects of plasmapheresis (PLEX) on the survival and clinical outcomes of patients with multiple sclerosis (MS) and natalizumab (NTZ)-associated progressive multifocal leukoencephalopathy (PML).Methods: The medical literature was searched for the terms natalizumab and progressive multifocal leukoencephalopathy. A total of 193 international and 34 Italian NTZ-PML cases were included. Clinical outcome was determined by comparing the patients' clinical status at PML diagnosis with status after PML resolution. The effects on survival and clinical outcome of PLEX, sex, age, country, pre-PML Expanded Disability Status Scale score, NTZ infusion number, prior immunosuppressant exposure, PML symptoms, PML lesion location at diagnosis, CSF JC virus status and copies, additional PML treatments and steroids, and PML immune reconstitution inflammatory syndrome ( IRIS) development were investigated with both univariate and multivariate analyses.Results: A total of 219 NTZ-PML cases were analyzed, and 184 (84%) underwent PLEX, which did not reduce the mortality risk or the likelihood of poor vs favorable outcomes. Country was predictive of mortality and poor outcome, while PML-IRIS development was predictive of poor outcome.Conclusions: PLEX did not improve the survival or clinical outcomes of Italian or international patients with MS and NTZ-PML, suggesting that this treatment should be performed cautiously in the future. Classification of evidence: This study provides Class III evidence that for patients with NTZ-PML, PLEX does not improve survival. The study lacks the statistical precision to exclude an important benefit or harm of PLEX.

Corticobasal degeneration (CBD) is an uncommon, sporadic, neurodegenerative disorder of mid- to late-adult life. We describe a further example of the pathologic heterogeneity of this condition. A 71-year-old woman initially presented dysarthria, clumsiness, progressive asymmetric bradykinesia, and rigidity in left arm. Rigidity gradually involved ipsilateral leg; postural instability with falls, blepharospasm, and dysphagia subsequently developed. She has been previously diagnosed as unresponsive Parkinson's Disease. At our clinical examination, she presented left upper-arm-fixed-dystonia, spasticity in left lower limb and pyramidal signs (Babinski and Hoffmann). Brain MRI showed asymmetric cortical atrophy in the right frontotemporal cortex. Neuropsychological examination showed an impairment in visuospatial functioning, frontal-executive dysfunction, and hemineglect. This case demonstrates that association of asymmetrical focal cortical and subcortical features remains the clinical hallmark of this condition. There are no absolute markers for the clinical diagnosis that is complicated by the variability of presentation involving also cognitive symptoms that are reviewed in the paper. Despite the difficulty of diagnosing CBD, somatosensory evoked potentials, motor evoked potentials, long latency reflexes, and correlations between results on electroencephalography (EEG) and electromyography (EMG) provide further support for a CBD diagnosis. These techniques are also used to identify neurophysiological correlates of the neurological signs of the disease.

Fingolimod is an effective disease modifying therapy for multiple sclerosis (MS). Beyond its main action on peripheral lymphocytes, several noteworthy side effects have been demonstrated in vitro, among which modulation of neural excitability. Our aim was to explore cortical excitability in vivo in patients treated with fingolimod 0.5mg/day.

Time-dependent B-0-(B) over bar(0) mixing is studied using about two million hadronic Z decays registered by L3 in 1994 and 1995. For this study three techniques are used. Tagging of the b-quark charge at decay time is performed by identifying leptons from semileptonic B decays. The flavour of the b quark at production time is determined from the charge of the lepton in the opposite hemisphere or by using a jet-charge technique. The proper time of the B-particle decay is obtained by reconstructing the production and decay vertices or by a measurement of the lepton impact parameter. The combined result for the frequency of B-d(0) meson oscillations is Delta m(d) = 0.444 +/- 0.040 ps(-1).

Highly common in multiple sclerosis (MS), fatigue severely impacts patients' daily lives. Previous findings of altered connectivity patterns led to the hypothesis that the distortion of functional connections within the brain-muscle circuit plays a crucial pathogenic role.
