1. Attitude towards cholera vaccination and its related factors in Jordan amid the 2022 Middle East outbreak

Abstract: Abstract Background An outbreak of cholera was reported in the Middle East by the second half of 2022. Raising public awareness and vaccination against cholera represent critical factors in the preventive efforts. The current study aimed to assess the knowledge of cholera and attitude towards its vaccination among a sample of the general public residing in Jordan. Methods An online self-administered questionnaire was distributed to the residents in Jordan using a snowball convenience-based sampling approach. The questionnaire based on previously published studies included items to evaluate sociodemographic variables, knowledge about cholera symptoms, transmission, and prevention and the willingness to accept cholera vaccination. Additionally, four items based on the validated 5 C scale in Arabic were included to assess the psychological factors influencing attitude to cholera vaccination. Results The final study sample comprised 1339 respondents, of whom 1216 (90.8%) heard of cholera before the study. Among those who heard of cholera, and on a scale from 0 to 20, the overall mean cholera Knowledge score (K-score) was 12.9 ± 3.8. In multivariate analysis, being over 30 years old and occupation as healthcare workers or students in healthcare-related colleges were significantly associated with a higher K-score compared to younger individuals and students in non-healthcare-related colleges. Overall, the acceptance of cholera vaccination if cases are recorded in Jordan, and if the vaccine is safe, effective, and provided freely was reported among 842 participants (69.2%), while 253 participants were hesitant (20.8%) and 121 participants were resistant (10.0%). In linear regression, the significant predictors of cholera vaccine acceptance were solely the three psychological factors namely high confidence, low constraints, and high collective responsibility. Conclusions In this study, the identified gaps in cholera knowledge emphasize the need to enhance educational initiatives. Although cholera vaccine acceptance was relatively high, a significant minority of the respondents exhibited vaccination hesitancy or resistance. The evident correlation between the psychological determinants and attitudes toward cholera vaccination emphasizes the need to consider these factors upon designing public health campaigns aimed at cholera prevention. The insights of the current study highlight the importance of addressing both knowledge gaps and psychological barriers to optimize cholera control strategies.

Abstract: Abstract Accurate and scalable quantification of amyloid-β (Aβ) pathology is crucial for deeper disease phenotyping and furthering research in Alzheimer Disease (AD). This multidisciplinary study addresses the current limitations on neuropathology by leveraging a machine learning (ML) pipeline to perform a granular quantification of Aβ deposits and assess their distribution in the temporal lobe. Utilizing 131 whole-slide-images from consecutive autopsied cases at the University of California Davis Alzheimer Disease Research Center, our objectives were threefold: (1) Validate an automatic workflow for Aβ deposit quantification in white matter (WM) and gray matter (GM); (2) define the distributions of different Aβ deposit types in GM and WM, and (3) investigate correlates of Aβ deposits with dementia status and the presence of mixed pathology. Our methodology highlights the robustness and efficacy of the ML pipeline, demonstrating proficiency akin to experts’ evaluations. We provide comprehensive insights into the quantification and distribution of Aβ deposits in the temporal GM and WM revealing a progressive increase in tandem with the severity of established diagnostic criteria (NIA-AA). We also present correlations of Aβ load with clinical diagnosis as well as presence/absence of mixed pathology. This study introduces a reproducible workflow, showcasing the practical use of ML approaches in the field of neuropathology, and use of the output data for correlative analyses. Acknowledging limitations, such as potential biases in the ML model and current ML classifications, we propose avenues for future research to refine and expand the methodology. We hope to contribute to the broader landscape of neuropathology advancements, ML applications, and precision medicine, paving the way for deep phenotyping of AD brain cases and establishing a foundation for further advancements in neuropathological research.

Abstract: Abstract Background Faculty development programs are crucial for promoting continuous learning, enhancing teaching effectiveness, and encouraging professional growth among medical educators. Problem-based learning was introduced as a teaching strategy in our Faculty of Medicine in 2007. Thereafter, several rounds of a faculty development program were conducted to help teachers recognize their role as facilitators and assess areas for improvement. Methods We conducted a mixed-methods study with a sample of 284 third-year medical students answering a questionnaire and 21 faculty members participating in focus groups. A validated 13-item questionnaire was used to investigate the students’ evaluation of their tutors’ performance in problem-based learning. Three sessions were then conducted with faculty members involved in problem-based learning to gain in-depth insights into their experiences and perspectives. Results The mean performance ranking for tutors awarded by the students was above halfway. Abstract: Abstract Background Despite the strong evidence supporting guideline-directed medical therapy (GDMT) in patients with heart failure with reduced ejection fraction (HFrEF), prescription rates in clinical practice are still lacking. Methods A survey containing 20 clinical vignettes of patients with HFrEF was answered by a national sample of 127 cardiologists and 68 internal/family medicine physicians. Each vignette had 4–5 options for adjusting GDMT and the option to make no medication changes. Survey respondents could only select one option. For analysis, responses were dichotomized to the answer of interest. Results Cardiologists were more likely to make GDMT changes than general medicine physicians (91.8% vs. 82.0%; OR 1.84 [1.07–3.19]; p = 0.020). Abstract: This study aimed to assess the prevalence and factors associated with lipid profile abnormalities of children aged 6 to 42 months in a Central-West Brazilian capital city. This cross-sectional study used data from the baseline of a cluster-randomized clinical trial conducted in parallel. It evaluated the lipid profile, usual nutrients intake (direct food-weighing method and 24-hour dietary recall), anthropometric parameters, and socioeconomic aspects of 169 children from early childhood education centers. Poisson regression with robust variance analysis was conducted. Of the total sample, 85% had dyslipidemia, 72% had high-density lipoproteins (HDL-c) levels below the desired range, 49% had increased triglycerides (TG), 17% exhibited elevated low-density lipoproteins (LDL-c), and 15% showed high total cholesterol (TC). An increase in the body mass index (BMI) for age z-score was associated with a higher prevalence of increased TG (PR = 1.22; 95%CI: 1.05-1.41; p = 0.009). Higher age in children was associated with an increased prevalence of high LDL-c (PR = 1.037; 95%CI: 1.01-1.07; p = 0.022) and TC (PR = 1.036; 95%CI: 1.00-1.07; p = 0.037), however it was a protective factor against low HDL-c (PR = 0.991; 95%CI: 0.98-1.00; p = 0.042). High energy intake was associated with low HDL-c (PR = 1.001; 95%CI: 1.00-1.00; p = 0.023). A higher prevalence of increased LDL-c (PR = 1.005; 95%CI: 1.00-1.01; p = 0.006) and decreased HDL-c (PR = 1.002; 95%CI: 1.00-1.00; p < 0.001) were associated with dietary cholesterol intake. Most of the children presented at least one alteration in serum lipids. Lipid profile abnormalities were associated with higher BMI, older age, and increased caloric and cholesterol intake.

Abstract: Information regarding the dietary requirements and consumption of young athletes is limited. Hence, the aim of this narrative review is to provide a comprehensive combination of research and review papers on the nutritional status of young athletes aged 5–18 years old, as well as quantitative, qualitative, wholesome foods, food choices, and eating disordered data concerning the dietary requirements for growing young athletes. This study involved systematic searches of electronic databases, including Google Scholar, PubMed, Science Direct, Scopus, and Web of Science. The specific criteria for identifying research papers published in English from July 1980 until May 2024 were included. Only 48 studies out of 1,262 were included in this narrative review. The findings of this study suggest that, compared with adults, junior athletes need a unique approach to meet their dietary needs. Growth, development, and general athletic performance depend on macronutrients, as they are vital nutrients for young active athletes. However, research on enhancing junior athletes’ performance is still in progress, and studies on hydration status, and eating disorders are limited.

Abstract: In the present paper, a numerical method is developed for solving non-homogeneous two-dimensional fractional integro-differential equations with a modified Atangana–Baleanu fractional derivative. The approach is based on the operational matrix method, involving the derivation of operational matrices for the terms of the equation to generate an algebraic system that is solved using mathematical software. First, basic definitions of fractional calculus and block pulse functions are presented. Then, the operational matrices and the method of solution are derived. The existence and uniqueness of the solution of the proposed equation are proved. Examples to test the numerical method are provided, demonstrating the efficiency of the proposed method.

Abstract: Vitamin D's role extends beyond classical calcium and phosphate homeostasis to encompass a pivotal influence on immune modulation and metabolic health. The mechanisms by which vitamin D exerts these effects involve its conversion to hormonally active calcitriol, which binds intracellular vitamin D receptors, initiating various downstream cascades. In this review, we tease out the evidence showing the relationship between vitamin D deficiency and prediabetes within the context of subclinical inflammation, with a special focus on the novel monocyte-to-HDL ratio (MHR), a novel inflammatory marker reflecting subclinical inflammation. This was based on a thorough literature review using reputable databases covering the period from 1980 to 2024. In light of this, we discuss calcitriol's anti-inflammatory effects and consequently link vitamin D deficiency to both overt and subclinical inflammation. Additionally, the utility of several biomarkers, notably MHR, in investigating this association is also discussed. We further reviewed the role of vitamin D deficiency in precipitating prediabetes and type 2 diabetes mellitus (T2DM) via insulin resistance, decreased insulin synthesis and secretion, and subclinical inflammation. Taken together, this mini review highlights that vitamin D deficiency is significantly associated with subclinical inflammation, playing a critical role in the development of prediabetes and the progression to T2DM. Addressing vitamin D deficiency through appropriate interventions may serve as a preventative measure against the development of prediabetes and T2DM.

Abstract: IntroductionPhenomics, an interdisciplinary field that investigates the relationships between genomics and environmental factors, has significantly advanced plant breeding by offering comprehensive insights into plant traits from molecular to physiological levels. This study examines the global evolution, geographic distribution, collaborative efforts, and primary research hubs in plant phenomics from 2000 to 2021, using data derived from patents and scientific publications. MethodsThe study utilized data from the EspaceNet and Lens databases for patents, and Web of Science (WoS) and Scopus for scientific publications. The final datasets included 651 relevant patents and 7173 peer-reviewed articles. Data were geocoded to assign country-level geographical coordinates and underwent multiple processing and cleaning steps using Python, Excel, R, and ArcGIS. Social network analysis (SNA) was conducted to assess collaboration patterns using Pajek and UCINET.ResultsResearch activities in plant phenomics have increased significantly, with China emerging as a major player, filing nearly 70% of patents from 2010 to 2021. The U.S. and EU remain significant contributors, accounting for over half of the research output. The study identified around 50 global research hubs, mainly in the U.S. (36%), Western Europe (34%), and China (16%). Collaboration networks have become more complex and interdisciplinary, reflecting a strategic approach to solving research challenges.DiscussionThe findings underscore the importance of global collaboration and technological advancement in plant phenomics. China's rise in patent filings highlights its growing influence, while the ongoing contributions from the U.S. and EU demonstrate their continued leadership. The development of complex collaborative networks emphasizes the scientific community's adaptive strategies to address multifaceted research issues. These insights are crucial for researchers, policymakers, and industry stakeholders aiming to innovate in agricultural practices and improve crop varieties.

11. Harnessing the prognostic potential of CT imaging in pediatric lymphoma: an in-depth analysis of disease evaluation and outcomes

Abstract: Introduction: Pediatric lymphomas are a significant childhood malignancy primarily treated with chemotherapy. While CT imaging is crucial for disease evaluation, its prognostic value remains under-explored. This study investigates the potential of CT characteristics to predict treatment response and clinical outcomes in pediatric lymphoma patients. Investigate the prognostic value of CT characteristics in pediatric lymphoma treated with chemotherapy. Materials and Methods: Retrospective analysis of 69 patients' medical records and CT scans. CT features (regression, size, nodal appearance, site involvement) were correlated with treatment response (regression, stable disease, progression, relapse, resolution) via univariate analysis. Results: Most patients (76.8%) achieved good outcomes with tumor regression. However, a subset displayed stable disease (11.6%), progression (7.2%), relapse (1.4%), or resolution (2.9%). CT characteristics associated with poor outcomes (p < 0.05) included: multiple site involvement (neck, chest, abdomen), larger tumor size (>3 cm), discrete nodal appearance Conclusion: CT features hold promise for prognostication in pediatric lymphoma. Integrating these findings into clinical practice may improve risk stratification and guide personalized treatment strategies.

Abstract: Abstract In a 3-arm presurgical trial, four-six weeks exemestane 25 mg three times/week (TIW) was non-inferior to 25 mg/day (QD) in suppressing circulating estradiol in postmenopausal women with ER-positive breast cancer. Since obesity may decrease exemestane efficacy, we analyzed changes in sex steroids, adipokines, Ki-67, and drug levels in relation to obesity. Postmenopausal women with early-stage ER-positive breast cancer were randomized to either exemestane 25 mg QD (n = 57), 25 mg TIW (n = 57), or 25 mg/week (QW, n = 62) for 4–6 weeks before breast surgery. Serum and tissue pre- and post-treatment biomarkers were stratified by body mass index (BMI)< or ≥30 kg/m2. Post-treatment median exemestane and 17-OH exemestane levels were 5–6 times higher in the QD arm compared to the TIW arm. For obese women, TIW maintained comparable reductions to QD in systemic estradiol levels, although the reduction in estrone was less with the TIW regimen. There was less suppression of SHBG with the TIW versus the QD dose schedule in obese women which should result in less systemic bioavailable estrogens. Metabolically, the effect of the TIW regimen was similar to the QD regimen for obese women in terms of leptin suppression and increase in the adiponectin-leptin ratio. Reduction in tissue Ki-67 was less for obese women on the TIW regimen than QD, although changes were similar for non-obese women. Our findings suggest that TIW exemestane should be explored further for primary cancer prevention in both normal weight and obese cohorts.

Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract: The increasing issue of plastic waste has become detrimental to human society, particularly with the increase in disposable plastic bottles in many countries. This study investigates the impact of incorporating plastic bottle waste fibers on the slump, density, compressive strength, split tensile strength, and flexural strength of concrete. This material was selected for its cost-effectiveness and wide availability, addressing the prevalent global concern of environmental pollution resulting from inadequate waste management practices. This study describes a systematic plan to fabricate and test cubes, cylinders, and beams using Fiber-Reinforced Concrete (FRC). A comparative analysis was performed between concrete reinforced with plastic bottle waste fibers, in varying ratios of 1, 2, and 3%, and plain concrete. The results showed a positive impact on concrete strength with fiber addition, although at the expense of reduced workability and decreased concrete density. In particular, a significant improvement in the ductility of the concrete was observed. The analysis shows that a fiber ratio of 2% emerges as the most optimal dosage to achieve improved concrete properties. This study provides valuable insights into the imperative pursuit of sustainable concrete production and the environmental challenges posed by plastic waste.

Abstract: Research on the therapeutic use of parasites has been ongoing since the development of the “hygiene hypothesis”. Parasites can stimulate the Th2-dependent response and suppress the Th1-dependent response, which is intensified in many diseases, especially allergic and autoinflammatory ones. In this review, we present the types of parasites used in helminth therapy and the range of diseases in which they may be useful. We also present the results of clinical trials conducted so far, which confirm the safety of such therapy and provide promising outcomes.

16. Tellurium Enrichment in Copper Tailings: A Mineralogical and Processing Study

Abstract: As the global demand for tellurium (Te) increases, it is crucial to develop efficient recovery methods that consider existing supply streams. This research combines gravity separation and froth flotation processes to enhance the recovery of Te minerals from tailings produced during the beneficiation of copper porphyry ores. Prior to processing, a systematic and comprehensive characterization study of copper tailing (CT) samples was conducted to examine the deportment of Te minerals in different mineral phases and to understand their locking and liberation behavior. Characterization techniques included inductively coupled plasma mass spectrometry (ICP-MS) and TESCAN’s integrated mineral analysis (TIMA). Copper tailing characterization showed that minerals with gold (Au), silver (Ag), bismuth (Bi), and Te were present in various forms, including native Au, electrum, tellurides, and sulfosalts. TIMA revealed that >90% of these minerals were primarily hosted in pyrite as less than 10 µm inclusions in the CT. TIMA also revealed that Te minerals exhibited fine-grained liberation of less than 20 μm. Moreover, TIMA results showed that >80% of mica and other silicate minerals were concentrated in size fractions < 38 μm, suggesting that desliming processes would positively impact Te enrichment. The results from the processing tests showed a Te recovery rate of ~77% and a Te enrichment ratio of 13 when using the combination of gravity separation and froth flotation at 90 g/t xanthate collector and 50 g/t glycol frother. The findings from this study show a significant potential for Te recovery from unconventional sources if appropriate physical beneficiation approaches are adopted.

Abstract: Background: Circadian Syndrome (CircS) encompasses cardiometabolic risk factors and comorbidities, indicating an elevated susceptibility to cardiovascular disease and type 2 diabetes. Methods: This cross-sectional study aimed to investigate the association between vitamin D levels and each of the following: CircS, metabolic syndrome (MetS), and the individual components of CircS. Data from 14,907 adults who participated in the National Health and Nutrition Examination Survey (NHANES) between 2007 and 2018 were utilized. CircS was defined based on MetS components, alongside depression, short sleep, and non-alcoholic fatty liver disease (NAFLD). Results: Our results indicated that low vitamin D levels exhibited meaningful associations with CircS, with vitamin D deficiency and inadequacy demonstrating 2.21-fold (95% CI 1.78–2.74, p < 0.001) and 1.33-fold (95% CI 1.14–1.54, p < 0.001) increases in CircS odds, respectively. The association between vitamin D deficiency and CircS was stronger than that with MetS. Additionally, a dose-response gradient in odds of CircS components, particularly with short sleep duration, was noted as serum vitamin D levels decreased. Conclusions: our findings highlight a significant association between low serum vitamin D levels and CircS and its components, particularly with short sleep. This suggests a potentially pivotal role of vitamin D in the pathogenesis of Circadian syndrome.

Abstract: Visual comfort includes the ability to provide sufficient daylight without exaggeration. When daylight levels are insufficient inside a place or when daylight is intense, inhabitants experience visual discomfort. This study examines visual comfort in prayer halls by investigating the effectiveness of daylighting performance, including daylight amounts, such as illuminance level and daylight factor, and glare, including daylight glare probability, in different mosque morphologies. The evaluation had done by applying a quantitative method via the most recently updated daylighting simulation software using Autodesk-Insight-360 for Revit-2023 and SolemmaClimateStudio for Rhinoceros-6. Results show that mosques with courtyard lighting have reduced glare, and windows placed uniformly along the vertical axis get lots of natural light. The study concluded that levels of visual comfort in the prayer halls, in turn, vary according to mosques morphology. The current research contributes to detecting visual discomfort in mosque architecture based on differences in typological and morphological design characteristics.

Abstract: Tire waste constitutes an undesirable surplus within urban industrial contexts, presenting a persistent annual increase on a global scale. Consequently, the reduction of tire waste through alternative approaches has attracted the interest of researchers around the world. This study evaluated the efficacy of using tire shreds as soil stabilizers to enhance the strength properties of the mixture in three proportions. Tire shred content of 10, 20, and 30% resulted in reduced CBR values of 3.3, 2.98, and 2.3%, respectively, compared to 4.4% without tire shred content. In addition, the direct shear test revealed that the increase in tire shred content significantly increased shear stress, as 10, 20, and 30% tire shred content resulted in 82.25, 84.14, and 85.87 kPa, respectively. Consequently, tire pieces can be used along with soil as an alternative mixture material in retaining structures.

Abstract: Abstract Objective We aim to assess the effect of low-pressure pneumoperitoneum on post operative pain and ten of the known inflammatory markers. Background The standard of care pneumoperitoneum set pressure in laparoscopic cholecystectomy is set to 12–14 mmHg, but many societies advocate to operate at the lowest pressure allowing adequate exposure of the operative field. Many trials have described the benefits of operating at a low-pressure pneumoperitoneum in terms of lower post operative pain, and better hemodynamic stability. But only few describe the effects on inflammatory markers and cytokines. Methods A prospective, double-blinded, randomised, controlled clinical trial, including patients who underwent elective laparoscopic cholecystectomy. Patients randomised into low-pressure (8–10 mmHg) vs. standard-pressure (12–14 mmHg) with an allocation ratio of 1:1. Perioperative variables were collected and analysed. Results one hundred patients were allocated, 50 patients in each study arm. Low-pressure patients reported lower median pain score 6-hour post operatively (5 vs. 6, p-value = 0.021) in comparison with standard-pressure group. Eight out of 10 inflammatory markers demonstrated better results in low-pressure group in comparison with standard-pressure, but the effect was not statistically significant. Total operative time and surgery difficulty was not significantly different between the two groups even in the hands of inexperienced surgeons. Conclusion low-pressure laparoscopic cholecystectomy is associated with less post operative pain and lower rise of inflammatory markers. It is feasible with comparable complications to the standard of care. Registered on ClinicalTrials.gov (NCT05530564/ September 7th, 2022).

Abstract: Abstract The recent discovery and characterization of pre-Descemet’s layer (PDL; also termed the Dua’s layer or the Dua-Fine layer) has advanced the understanding of various posterior corneal pathologies and surgeries in human. This study aimed to characterize the ultrastructure of the posterior stroma and interfacial zone of Descemet’s membrane (DM) in canine eyes. Eighteen canine corneo-scleral discs were included. Intrastromal air injection resulted in the formation of type 1 big bubble (BB) in 73% (n = 11/15) of corneas, with a mean diameter of 11.0 ± 1.3 mm. No type 2 BB was created. Anterior segment optical coherence tomography, histology and transmission electron microscopy confirmed that the wall of BB was composed of DM, in contact with remaining stroma (canine PDL; cPDL). The cPDL was populated with keratocytes, of varying thickness of 16.2 ± 4.2 µm in close apposition to the DM, and composed of collagen bundles arranged in transverse, longitudinal and oblique directions. The interfacial zone, between DM and cPDL, showed fibril extension in all three directions, predominantly longitudinal. Irregular extensions of DM material into cPDL stroma were observed. No long-spaced collagen was detected. In conclusion, there exists a well-defined cleavage plane between the posterior stroma and cPDL, with similar but not identical characteristics as in humans, that is revealed by pneumodissection. This adds to our understanding of the anatomy of the posterior most canine cornea, which will have significant clinical impact on posterior corneal surgery and understanding of corneal pathology in dogs.

22. Impact of USMLE Step 1 Transition to Pass/Fail Scoring System on Medical Students' Anxiety, Sleep Quality, and Burnout

Abstract: BACKGROUND: Originally designed to evaluate the application of foundational scientific knowledge in clinical contexts, the United States Medical License Exam Step 1 evolved into a comparative tool for assessing candidates' educational foundations. This transition heightened the pressure on medical students to excel in the exam. In response, collaborative efforts involving the National Board of Medical Examiners prompted a significant change from reporting scores to a pass/fail system. However, the true impact of this shift remains insufficiently explored. This study aims to assess the emotional toll - encompassing burnout, anxiety, depression, and sleep quality - experienced prior to taking the Step 1 exam. Additionally, it aims to uncover potential gender-based disparities in perceived stress, anxiety, and depression. METHODS: The study encompasses the entirety of third-year medical students at Drexel University College of Medicine, who were invited to participate in a comprehensive survey. Drawing from retrospection, the survey relies on self-reported data regarding anxiety, depression, sleep quality index, and burnout levels. Data compilation was anonymized and executed through the Qualtrics platform. RESULTS: A total of 102 medical students completed the survey, with a 97% pass rate for the USMLE Step 1. Despite their excellent performance, 75% of students reported inadequate sleep quality, and 68% exhibited mild to moderate anxiety levels. Among them, a higher percentage of females (83%) experienced mild to moderate anxiety compared to their male counterparts (50%). Furthermore, 66% of students felt that their commitment to education exceeded what was reasonable for their well-being. CONCLUSION: The transition from traditional scoring to a pass/fail system was ostensibly intended to enhance the mental well-being of medical students. Nevertheless, our findings underscore that students continue to grapple with heightened levels of stress, anxiety, and burnout during the pivotal month leading up to the Step 1 examination.

It explored the perspective of FOM stakeholders on the challenges and factors in the teaching and learning domains that are affecting achieving learning outcomes. METHODS Faculty teaching in the second and third year, students in third and fourth year, and administrative personnel were included. An electronic survey was created. Chi-Squared test for independence was used to test the association between the correspondent group and different statements. Kruskal-Wallis ANOVA was used to examine the differences in medians among the questions utilizing ranks to evaluate different questions, followed by post hoc pairwise comparison with Bonferroni-corrected alpha. RESULTS A strong association was seen between the correspondent level and perspective on time allocated for modules and vertical integration of the curriculum. Among the most highly ranked challenges for using various teaching strategies were the loaded curricula content and the limitation of time, with an overall median rank of 8. Student readiness and motivation were also significant factors, however, students considered motivation less of a challenge. CONCLUSION Continuous assessment of factors affecting teaching and learning will bridge gaps in curriculum.

Abstract: Water quality sensors have recently received a lot of attention due to their impact on human health. Due to their distinct features, environmental sensors are based on carbon quantum dots (CQDs). In this study, CQDs were prepared using the electro-chemical method, where the structural and optical properties were studied. These quantum dots were used in the environmental sensor application after mixing them with three different materials: CQDs, Alq3 polymer and CQDs and Alq3 solutions using two different methods: drop casting and spin coating, and depositing them on silicon. The sensitivity of the water pollutants was studied for each case of the prepared samples after measuring the change in resistance of the samples at a temperature of 30 oC. Through the results, it was found that the highest sensitivity of sample 3 to the carbon continuous dot was in the case of the contaminant fructose and was 99.55%, while the highest sensitivity of sample 4 was for the one sensitive to the contaminant (mercury chloride) and was 81. As for sample 1, the highest sensitivity was in the case of detecting the contaminant lead chloride and was 80. The results showed that the best sensor was obtained using a spin-coating technique when the solution sample of CQDs+Alq3 was placed on a silicon slide in fructose and the sensitivity was 200%. This demonstrates the importance of quantum dots in measuring the sensitivity of water pollutants. The thin film thickness was measured to be 500 nm.

Abstract: Rapid industrialization and urbanization have led to widespread metal contamination in aquatic ecosystems. This study explores the metal tolerance and biosorption characteristics of four bacterial strains (Serratia sp. L2, Raoultella sp. L30, Klebsiella sp. R3, and Klebsiella sp. R19) isolated from Saint Clair River sediments. These strains effectively removed various metal cations (As3+, Pb2+, Cu2+, Mn2+, Zn2+, Cd2+, Cr6+, and Ni2+) in single and multi-metal solutions. Minimum inhibitory concentration (MIC) assays revealed strain-specific variations in metal tolerance, with L2 and L30 exhibiting higher tolerance. Surprisingly, R3 and R19, despite lower tolerance, demonstrated superior metal removal efficiency, challenging the notion that tolerance dictates removal efficacy. In single-metal solutions, R3 and R19 excelled at extracting various metal ions, while competitive binding in multi-metal solutions hindered removal. However, R3 and R19 retained higher removal efficiencies, possibly due to enhanced flocculation activities facilitating metal-ion contact. Comprehensive Fourier-transform infrared (FTIR) analysis highlighted the strains’ metal-binding capabilities, with novel peaks emerging after metal exposure, indicative of extracellular polymeric substance (EPS) production. Scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDX) confirmed metal accumulation on bacterial surfaces and within cytoplasmic regions and revealed morphological changes and metal adsorption patterns, emphasizing the strains’ ability to adapt to metal stress. Scanning transmission microscopy (STEM) and EDX analysis uncovered metal accumulation within bacterial cells, underscoring the complexity of microbial-metal interactions. This study also confirms that the simultaneous presence of an aqueous solution may cause a mutual inhibition in the adsorption of each metal to the EPS resulting in reduced metal uptake, which emphasizes the need to select specific bacterial strains for a given metal-containing effluent. The differences in metal distribution patterns between Klebsiella sp. R19 and Raoultella sp. L30 suggest species-specific metal accumulation strategies driven by environmental conditions and metal availability. The heavy metal-removing capabilities and the ability to grow over a wide range of metal concentrations of the strains used in this study may offer an advantage to employ these organisms for metal remediation in bioreactors or in situ.

A conceptual model was constructed based on previous literature. The model includes government policy, the capacity of external suppliers, and the capacity of the project team, top management support, as independent variables where their influence on the acceptance of telemedicine in Jordanian public hospitals as a dependent variable is examined. Respondents were chosen using a Purposive sampling technique. Questionnaires were delivered to 320 respondents using Google Forms. SEM was used for statistical analysis. The findings revealed that all the proposed factors including government policy, external supplier capacity, project team capacity, and top management support have a significant influence toward accepting telemedicine. The results of this study may aid Jordanian public hospitals in making the best use of the proposed factors to increase the acceptance of telemedicine in Jordanian public hospitals.

Имеются несколько хронологий развития архитектурных стилей. В статье рассмотрены этапы появления сооружений криволинейной формы в виде поверхностей разных классов. Показано, что первые тонкие оболочки и оболочечные структуры имели форму сферической, цилиндрической поверхностей и некоторых простейших поверхностей вращения. К концу ХХ века из более чем шестисот известных анали- тических поверхностей нашли применение 24. Показано, что пять периодов в развитии архитектуры криволинейных форм совпадают по времени с пятью периодами проектирования и строительства тонких оболочек и оболочечных структур. В анной статье авторы только констатирует реальное положение в архитектуре криволинейных форм ХХ века и отмечают возросший интерес архитекторов и строителей к таким объектам к концу ХХ века.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Abstract: The authors report a case of a patient with a history of IgA nephropathy that, during the admission for pneumonia, was found to have an incidental finding of a huge mitral valve (MV) mass on transthoracic echocardiography. The differential diagnosis was challenging because the clinical scenario raised the suspicion of possible infective endocarditis and the imaging features were suggestive of a myxoma or vegetation. The patient underwent urgent excision of the mass with MV replacement due to the high risk of embolism. Intraoperative findings were consistent with clot or vegetation. Pathology result of thrombus was beyond our imagination and at the best of our knowledge one case only has been reported. Awareness about native MV thrombosis and its etiologic factors, workup, and management is key for better medical and surgical management planning because this condition is extremely rare and challenging in the clinical and imaging arena.

Abstract: Background: An elective caesarean section (CS) has been associated with high anxiety scores, and there are associations between higher anxiety scores and younger age, primigravidae, higher educational level, and previous experience with anaesthesia. In this study, the aim is to measure anxiety scores associated with an elective CS using two measuring scales and identify women's characteristics and obstetrics variables that are associated with higher scores. Methods: A cross-sectional study was conducted between Nov 15, 2019 and Nov 15, 2020. Women were included if they were 18 years of age or more, had viable pregnancies, and were admitted for an elective CS. Anxiety scores were measured on admission using the visual analogue scale for anxiety (VASA) and then the State-Trait Anxiety Inventory (STAI-Y). Associated factors were studied using logistic regression analyses. Results: Three hundred women were recruited. Means (SD) for the participant's age and gestation age were 30.5 (5.7) years and 37.6 (1.4) weeks, respectively. Additionally, 29.3% of the participants having a CS were primigravidae and 62.3% were for maternal indications. Furthermore, 55%, 59%, and 61% of the women had scores above the means of VASA and STAI-S components 1 and 2, respectively.Variables that showed statistically significant associations with higher anxiety scores were that the woman's age was 25–34, the CS was for foetal indications, the choice of anaesthesia was general, and the source of information for the choice of anaesthesia was a layperson. Conclusion: Higher anxiety scores are prevalent among women admitted for an elective CS. STAI-Y and VASA correlated well, and the short VASA may replace the lengthy STAI-Y in clinical practice. Identification of women with risk factors may help in implementing strategies to reduce anxiety.

39. Is It a Choice? Examining Neoliberal Influences in Three Ontario Education Reforms

Abstract: In this article, we draw on various critical perspectives to theorize neoliberal choice and examine how it has been deployed to market new educational reforms in Ontario. We begin by offering a contemporary framing of neoliberalism that looks at its core elements as well as its chameleon-like tendencies to draw on neoconservative elements as needed. We also furnish critiques of neoliberalism by engaging Adams et al.'s (2019) description of neoliberal "choice" as one component of a larger psychological exercise in support of capitalism. We then examine how the language of choice has been used to position three recent Ontario education reforms: (a) mandatory e-learning; (b) growth of international students; and (c) the revision of curricula according to economic ends. Finally, we argue that the implementation of these reforms ironically has produced less choice for stakeholders through austerity and standardization.

Abstract: Abstract Background Tamoxifen is one of the fundamental pillars of adjuvant endocrine therapy for hormone receptor-positive breast cancer; however, 30–50% of patients receiving tamoxifen experience tumor relapse. CYP2D6, encoded by an extremely polymorphic CYP2D6 gene, is the rate-limiting enzyme of tamoxifen bioactivation. This study aimed at determining the frequencies of the most clinically relevant CYP2D6 alleles and evaluating their impact on the responsiveness to tamoxifen in a cohort of Syrian breast cancer patients. Methods This case–control study encompassed positive estrogen and/or progesterone receptor, stage 1–3 breast cancer female patients receiving tamoxifen at Al-Bairouni University Hospital, the major National Oncology Center in Syria. Successfully genotyped eligible patients (n = 97) were classified according to their response into; no recurrence group (n = 39) who had completed a five-year recurrence-free adjuvant tamoxifen therapy, and recurrence group (n = 58) who had experienced recurrence. Several star alleles including CYP2D6*4, CYP2D6*10, CYP2D6*41, and CYP2D6*69 were identified via targeted sequencing of specific polymerase chain reaction (PCR) products and phenotypes were assigned according to activity score (AS). The correlation between genotypes and disease-free survival (DFS) was assessed using Kaplan–Meier method and log-rank test. Hazard ratios were estimated using Cox proportional hazards regression models. Results The allelic frequencies of CYP2D6*41, CYP2D6*10, CYP2D6*4, and CYP2D6*69 were found to be 9.28%, 7.22%, 7.22%, and 2.58%, respectively. No statistically significant differences were observed in the frequencies of CYP2D6 phenotypes between the two arms (P = 0.24), nor the incidence of tamoxifen-induced hot flashes (P = 0.109). Poor metabolizers (PMs) tended to display shorter DFS than intermediate metabolizers (IMs) and normal metabolizers (NMs) combined (adjusted HR = 2.34, 95% CI = 0.84–6.55, P = 0.104). Notably, patients homozygous for the null CYP2D6*4 allele (1847A/A) had an elevated risk of disease recurrence compared to patients with 1847G/G genotype (adjusted HR = 5.23, 95% CI = 1.22–22.49, P = 0.026). Conclusions Our findings show no association between CYP2D6 phenotype and treatment outcomes of tamoxifen in Syrian breast cancer patients. Nevertheless, a worse DFS was revealed in patients with 1847A/A genotype (*4/*4).

41. Basis, Process and Outcomes of a Student Involvement Project for Curriculum Review at the Imperial College School of Medicine

Abstract: Introduction: To help create a Bachelor of Medicine, Bachelor of Surgery (MBBS) curriculum centred around the student voice, the Imperial College School of Medicine (ICSM) recruited two medical students for a two-week student-staff collaboration in Summer 2019 for its wider curriculum review. This writeup discusses the background, processes, and outcomes of the collaboration and includes some student reflections.Methods: The team comprised a member of the faculty and two medical students (the authors). We met daily for two weeks and focussed on the Bioregulatory Systems (BRS) module of Year 1. There were three key areas of work: learning objectives, large-group sessions, and small-group sessions. Each aspect involved planning, implementation, and reflection. For example, learning objectives were recategorized and reorganised, students fed back on a new slide template for large-group sessions, and new small-group sessions were designed. Feedback from the staff was collected verbally, and the medical students submitted feedback in the form of a mid-project interview, a post-project report, andinformally.Results: We achieved such outcomes as reorganising and refining learning objectives, improving large-group teaching sessions, and refining and creating small-group teaching sessions. Following the collaboration, we had a debrief session.Conclusion: This collaboration was highly valuable for both students and faculty; the feedback revealed that the ideas,discussions, and outputs had a substantial impact. Overall, student-staff collaboration will become increasingly valuable as we emerge from COVID-19; we hope this write-up informs and inspires more ‘students as partners’ projects worldwide.

These patients may experience dyspnea and hypoxia, leading to the development of acute respiratory distress syndrome. Studies have reported an increased risk of severe sarcopenia in COVID-19 patients during and after recovery. This narrative review aimed to summarize and synthesize available studies on the association between sarcopenia and mortality in critically ill COVID-19 patients. A total of 22 studies conducted on hospitalized COVID-19 patients were included in this review. Of those, 17 studies reported a direct association, while 5 studies showed no association between sarcopenia and mortality in severe COVID-19 patients. It is important to maintain muscle quality and quantity in defense against COVID-19. The measurement of lean muscle mass should be included in the risk assessment of severely ill COVID-19 patients as part of the therapy plan.

Abstract: Three Schiff base ligands, NQ, CQ and HQ, were prepared from the reaction of quinoline-3-carbohydrazide with 2-nitrobenzaldehyde, 2-chlorobenzaldehyde and 2,4-dihydroxybenzaldehyde, respectively, and were investigated for their coordination to Cu (II), Ni(II), Co(II), Cd(II), Cr(III) and Fe(III) chlorides. The NQ preparation and the X-ray structure of NQ and CQ, as well as the transition metal complexes of NQ, CQ and HQ, were reported for the first time. FTIR, 1H-NMR, magnetic susceptibility and elemental analysis were used to study the coordination of ligands to the metal ions. Based on the magnetic susceptibility and elemental analysis results, octahedral structures of the complexes such as [CuL2Cl(OH)], [FeL2Cl2(OH)] and [CoL2Cl(OH)] were proposed for L = NQ, CQ and HQ. The relatively large Cd(II) exhibited [CdL3(OH)2]. The FTIR study revealed that NQ and CQ are coordinated to the metal ions via azomethine nitrogen and carbonyl oxygen while HQ through azomethine nitrogen and phenolic oxygen. Despite the high solvation power of DMSO solvent in 1H-NMR experiments, the azomethine HC=N peak at 9.3 ppm is the most affected by complexation with metal ions. On the other hand, quinoline nitrogen seems to be a weaker coordinating site than the azomethine nitrogen. The HQ ligand, containing phenolic groups, and its complexes with Cu and Ni were found to have inhibitory effects on human breast adenocarcinoma MCF-7 and human chronic myelogenous leukemia K562. Nevertheless, metal ions did not exhibit a significant synergistic effect on the antiproliferative activity of the ligands investigated.

Kotov proposed to separate the surfaces of congruent sections into a separate class and to include the surfaces of plane-and-parallel translation, surfaces of revolution, carved surfaces of Monge, cyclic surfaces with a generatrix circle of constant radius, rotative, spiroidal, and helical surfaces in it. The aim of the research is to obtain generalized parametric equations of surfaces of congruent sections of the pendulum type on right cylinders with plane-and-parallel translation of movable rigid superellipses. Analytical geometry methods are used. Computer systems MathCad and AutoCAD are applied to visualize surfaces. The results consist in the derivation of parametric equations of the studied surfaces in a general form convenient for the use of computer modeling methods. The technique is demonstrated on five examples with congruent mobile superellipses. The possibility of using obtained surface shapes in parametric architecture, free-form architecture, and in shaping of the surfaces of some technical products is noted.

However, it is unclear what constitutes person-centered care in populations affected by conflict, as they may have specific concerns in the dimensions of physical, emotional, social, and spiritual wellbeing. This study aimed to identify what matters to patients with advanced cancer and family caregivers in Jordan including refugees, to inform appropriate person-centered assessment and palliative care in conflict-affected populations.MethodsCross-sectional face-to-face, semi-structured interviews were conducted at two sites in Amman. Adult patients with advanced cancer and family caregivers were purposively sampled to maximize diversity and representation. Interviews were digitally audio recorded, anonymized, and transcribed verbatim for thematic analysis.FindingsFour themes were generated from 50 patients (22 refugees; 28 Jordanians) and 20 caregivers (7 refugees; 13 Jordanians) (1). Information, communication, and decision-making. Truth-telling and full disclosure from clinicians was valued, and participants expressed concerns that information was not shared in case patients would disengage with treatment. (2) Priorities and concerns for care and support. Participants’ top priority remained cure and recovery (which was viewed as possible). Other priorities included returning to their “normal” life and their “own” country, and to continue contributing to their family. (3) Role of spirituality and Islam. Most participants had strong faith in God and felt that having faith could comfort them. For refugees whose social network was fractured due to being away from home country, prayer and Quran reading became particularly important. (4) Unmet support needs of family caregivers. Family caregivers were affected physically and emotionally by worrying about and caring for the patients. They needed support and training, but often could not access this.DiscussionTruth-telling is highly valued and essential to achieving person-centered care and informed decision-making. This study also reveals specific concerns in conflict-affected populations, reflecting the experience of prior losses and fracturing of existing social networks and support. The role of religion is crucial in supporting refugee communities, and consideration should be paid to the needs of patients and caregivers when caring for a patient at home without access to their communities of origin and the support they accessed.

Abstract: Perovskite has developed as a photovoltaic material. Its power conversion energy has reached 22,1%. One of the perovskite materials was perovskite with methylammonium cation. But, the material has low thermal stabilization, so it needs to evolve by switching cations like cesium. Hence, further research is necessary to compare the electronic structure of both perovskite material to understand how efficiency increase by changing its cation using computational material methods. The computation was done using the ab-initio method with Density Functional Theory (DFT) formulation, Generalized-Gradient Approximation (GGA) as XC functional, and electron-ion core interaction modeled by pseudopotential (PP) relativistic data. The computational results show that MAPbI3 and CsPbI3 have direct band gaps of 1,64 eV and 1,46 eV. The cation switching from MA to Cs has decreased band gap value and generated electron-hole couples, represented by the curvature of the density of states (DOS).

This study aims to assess the influence of social isolation measures due to the COVID-19 pandemic on the mental health of women in their third trimester and postpartum.Material and methodsThis is a cross-sectional survey-based study conducted in Khalid University Hospital, Riyadh, Saudi Arabia, between the months of April to May 2021, to explore depression and anxiety levels in females who gave birth during the COVID-19 pandemic. In addition to background demographic data, the survey included Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder Scale-7 (GAD-7) were utilized to detect symptoms of depression and anxiety, respectively.ResultsA total of 283 women were included in this study, almost half of them were ante-natal (n-141) and the rest were post-natal (n = 124). 62.3% were in the age groups of 25–35 years. Based on the PHQ-9 scoring, 65% of the study sample had depression (ranging from mild to severe). Moreover, based on GAD-7 scoring, 49.1% had anxiety (ranging from mild to severe). No association was found between PHQ-9 and GAD-7 scores and different sociodemographic and obstetric factors. Additionally, the mean scores of women infected with COVID-19 vs. women who has never been diagnosed with COVID-19 were closely comparable.ConclusionsWe reported a high prevalence of depression and anxiety among pregnant women during COVID-19 pandemic. Policymakers and health care providers are advised to implement targeted preventive measures for pregnant women to improve mental health in times of epidemics.

Abstract: Abstract CYP2D6 is a key drug-metabolizing enzyme implicated in the biotransformation of approximately 25% of currently prescribed drugs. Interindividual and interethnic differences in CYP2D6 enzymatic activity, and hence variability in substrate drug efficacy and safety, are attributed to a highly polymorphic corresponding gene. This study aims at reviewing the frequencies of the most clinically relevant CYP2D6 alleles in the Arabs countries. Articles published before May 2021 that reported CYP2D6 genotype and allelic frequencies in the Arab populations of the Middle East and North Africa (MENA) region were retrieved from PubMed and Google Scholar databases. This review included 15 original articles encompassing 2737 individuals from 11 countries of the 22 members of the League of Arab States. Active CYP2D6 gene duplications reached the highest frequencies of 28.3% and 10.4% in Algeria and Saudi Arabia, respectively, and lowest in Egypt (2.41%) and Palestine (4.9%). Frequencies of the loss-of-function allele CYP2D6*4 ranged from 3.5% in Saudi Arabia to 18.8% in Egypt. The disparity in frequencies of the reduced-function CYP2D6*10 allele was perceptible, with the highest frequency reported in Jordan (14.8%) and the lowest in neighboring Palestine (2%), and in Algeria (0%). The reduced-function allele CYP2D6*41 was more prevalent in the Arabian Peninsula countries; Saudi Arabia (18.4%) and the United Arab Emirates (15.2%), in comparison with the Northern Arab-Levantine Syria (9.7%) and Algeria (8.3%). Our study demonstrates heterogeneity of CYP2D6 alleles among Arab populations. The incongruities of the frequencies of alleles in neighboring countries with similar demographic composition emphasize the necessity for harmonizing criteria of genotype assignment and conducting comprehensive studies on larger MENA Arab populations to determine their CYP2D6 allelic makeup and improve therapeutic outcomes of CYP2D6- metabolized drugs.

50. Prevalence of text neck syndrome in children and adolescents using smartphones in Erbil city

Abstract: Background: Neck and shoulder pain is a common health issue among children and teenagers in developing countries. Objective: This study aimed to identify the prevalence of text neck syndrome and its associated factors in children and adolescents. Materials and Methods: In this cross-sectional study, children and adolescents who attended primary health centers (PHCs) along with their parents were included. Four main PHCs in Erbil city in Iraqi Kurdistan in 2022 were selected for this study. Results: The prevalence of text neck pain was 69.0% among children. The level of neck disability score was 17.15 out of 21 among children. Adolescents (96.05%) those who used smartphones >3 h (83.56%), those studying secondary level (94.24%), and those with psychological, social, and physical functions and children who slept less, played fewer sports, used more computers, watched more TV, and played the game more than 3 h were more likely to have text neck syndrome. The study showed that children who spent more time on smartphones, watching TV, playing games, and those with physical and social functions had a high level of neck disability. Conclusion: This study found a high prevalence of text neck syndrome among schoolchildren in this region. The high prevalence of text neck syndrome was associated with more using smartphones and watching TV and a high prevalence of physical, psychological, and social issues.
