35. Biopolymeric 3D printed implantable scaffolds as a potential adjuvant treatment for acute post-operative pain management

Abstract

ABSTRACTBackgroundPain is characterized as a major symptom induced by tissue damage occurring from surgical procedures, whose potency is being experienced subjectively, while current pain relief strategies are not always efficient in providing individualized treatment. 3D printed implantable devices hold the potential to offer a precise and customized medicinal approach, targeting both tissue engineering and drug delivery.Research design and methodsPolycaprolactone (PCL) and PCL – chitosan (CS) composite scaffolds loaded with procaine (PRC) were fabricated by bioprinting. Geometrical features including dimensions, pattern, and infill of the scaffolds were mathematically optimized and digitally determined, aiming at developing structurally uniform 3D printed models. Printability studies based on thermal imaging of the bioprinting system were performed, and physicochemical, surface, and mechanical attributes of the extruded scaffolds were evaluated. The release rate of PRC was examined at different time intervals up to 1 week.ResultsPhysicochemical stability and mechanical integrity of the scaffolds were studied, while in vitro drug release studies revealed that CS contributes to the sustained release dynamic of PRC.ConclusionsThe printing extrusion process was capable of developing implantable devices for a local and sustained delivery of PRC as a 7-day adjuvant regimen in post-operative pain management.

Abstract

ABSTRACTIntroduction3D printing (3DP) applications in medicine are intensively investigated, creating an opportunity to provide patient-tailored therapy by delivering a drug with an accurate dose and release profile. Moving away from the ‘one size fits all’ paradigm, it could be beneficial for treating mental and neurological disorders, improving the efficiency and safety of the therapy. The aim of this critical review is to assess recent advances and identify gaps regarding 3DP in this important and challenging field, by focusing on recent research examples.Areas coveredApplications of the 3DP techniques for solid dosage forms in mental and neurological disorders have been covered and discussed, together with recent advantages, limitations, and future directions.Expert opinionThe personalize treatment, which is considered as the most significant advantage of the 3DP technique, can be beneficial in mental and neurological disorders therapy, where the dose should be adjusted to the patient. Printing of medicines enables creating the structure modifications and thus controlling the drug release or combining multiple drugs into one tablet, simplifying the dose regimen. Medications printed on-demand, in health-care facilities, could address the special needs of pediatric patients and help avoid interruptions in the supply chain. Despite promising advances, the described methods have limitations and need further investigation before being scaled-up to an industrial manufacturing environment. There is also a need to establish protocols for the preparation and registration of 3DP dosage forms.

Abstract

Skin wounds not only cause physical pain to patients but also pose an economic burden to society. Consequently, effective approaches to promote skin repair remain a challenge. Specifically, chitosan-based hydrogels are ideal candidates to promote wound healing at different stages and while diminishing the factors that impede this process (such as excessive inflammatory and chronic wound infection). Furthermore, the unique biological properties of a chitosan-based hydrogel enable it to serve as both a wound dressing and a drug delivery system (DDS). In the present work, chitosan (CS) graft copolymer with [2-(methacryloyloxy)ethyl] trimethyl ammonium chloride (CS-MTAC), a cationic monomer with promising antibacterial properties, was synthesized. The successful synthesis of the copolymer was confirmed, while it was studied for its swelling ability and water absorption capacity, as well as for its biocompatibility and antibacterial properties. Expecting to improve its printability, the copolymer was blended with elastin (EL), collagen (COL), and increasing concentrations of gelatin (GEL). The hydrogel with 6% w/v CS, 4% w/w EL, 4% w/w COL and 1% w/v GEL was selected for its potential to be 3D-printed and was neutralized with ammonia vapors or ethanol/sodium hydroxide solution and loaded with levofloxacin. The feasibility of CS-MTAC/EL/COL/GEL bioink, loaded with Levo, as a suitable candidate for wound healing and drug delivery applications, has been demonstrated. [ABSTRACT FROM AUTHOR]

49. Towards the determination of surface energy at the nanoscale

Abstract

Surface energy, the work required to generate the surface of a solid or liquid, is an important property influencing a wide range of interfacial processes. This property determines physical interactions between phases, e.g., the degree of hydrophilicity of a solid surface and the surface adhesion of biological entities including bacteria and cells. The determination of surface energy by means of liquid/solid contact angle goniometry is well established, although only averaged values on the macro scale can be obtained and there may be practical limitations of the method, e.g., materials where liquid penetration occurs. The main aim of the research work described in this thesis has been to develop a technique to determine surface energy using atomic force microscopy, where the measurement may be obtained at a resolution of the order of the probing tip radius (50- 90 nm). To produce suitable substrate materials of known surface chemistry, self-assembled alkanethiol structures on gold have been prepared. Contact angle goniometry, atomic force microscopy, and infrared spectroscopy have been used to investigate the surface-preparation and solution-deposition conditions for the reproducible formation of the self-assembled molecular structures on gold-coated tips and substrates for atomic force microscopy. Procedures have been established for removing adsorbed material, including self-assembled monolayers, and for producing clean gold substrates. Preliminary data show that surface-saturated self-assembled monolayers form reproducibly on prolonged (> 16 h) exposure of gold-coated glass substrates to ethanolic solutions of lD-functionalised alkanethiols in the concentration range 80 - 160 mmol dm-3. The data also show that exposure for 16 h to alkanethiol concentrations in the range 160-240 mmol dm-3 promote bilayer formation whereas concentrations of 240-320 mmol dm-3 result in the deposition of multilayers, the average orientation of which is parallel to that of the first molecular layer; the use of parent I-undecanethiol solutions at concentrations of 1-80 mmol dm-3 results in incomplete monolayer coverage. The self-assembled alkanethiol structures deposited on gold from ethanolic solutions have been found to be susceptible to both chemical and structural changes: ethanol provides a medium for the formation of S-alkyl hydrogen thiocarbonates and related compounds via reaction with dissolved atmospheric CO2. Deposition from ethanolic solutions results in multilayered structures, incorporating these thiocarbonates and thiocarbonate-derived compounds, which at room temperature are susceptible to time-dependent structural rearrangement and molecular migration. To assess the applicability of the atomic force microscopy method for determining surface energy, forces of adhesion have been measured for interactions involving self-assembled molecular structures or polymer-film structures that had each been deposited onto gold-coated glass substrates and onto the probing, gold-coated cantilever. The data have been fitted into mathematical models that allow the calculation of surface energy by considering the work done for the separation of the identically coated contacting surfaces. The obtained surface energy values are in close agreement with those from corresponding contact-angle determinations, highlighting the potential usefulness of the technique for the study of surfaces at a resolution level approaching 1000 atoms. Comparative studies have shown that the employment of the atomic force microscopy technique may be preferable for the study of materials that are susceptible to penetration by liquids or for the investigation of small samples « contact angle drop diameter).
