Abstract

Having a growth mindset has been hailed as one of the most critical advancements in understanding students’ motivation in recent years. The attention on the growth mindset indicates an increased surge in the public’s interest in using evidence-based intervention models to facilitate learning. Because the positive benefits of growth mindsets are apparent, changing ones’ mindset becomes the core focus of growth mindset literature. But aside from “changing” students’ mindsets, finding the right kind of psychological interventions that leverage student’s growth or fixed mindset with sustainable improvement for students can be as, if not more important. The current study seeks to fill this research gap. Our first study indicated that learners with a relatively high growth mindset benefit from a self-reflection learning strategy. However, this very same self-reflection strategy can harm the learning benefits for those learners with a fixed mindset. The second study used experimental manipulation to show that learners with a growth mindset can enhance their learning process and outcomes from self-reflection strategies. In contrast, learners with a fixed mindset can improve their learning process and result from self-compassion strategies. The current study identified two distinct psychological mechanisms – self-reflection and self-compassion and linked them with mindsets with the learning process and learning outcomes. It also demonstrates how learners with growth and fixed mindsets can directly benefit from these two psychological mechanisms in an experimental design.

2. Intramuscular midazolam, olanzapine, or haloperidol for the management of acute agitation: A multi-centre, double-blind, randomised clinical trial

Abstract

Background: The safety and effectiveness of intramuscular olanzapine or haloperidol compared to midazolam as the initial pharmacological treatment for acute agitation in emergency departments (EDs) has not been evaluated. Methods: A pragmatic, randomised, double-blind, active-controlled trial was conducted from December 2014 to September 2019, in six Hong Kong EDs. Patients (aged 18–75 years) with undifferentiated acute agitation requiring parenteral sedation were randomised to 5 mg intramuscular midazolam (n = 56), olanzapine (n = 54), or haloperidol (n = 57). Primary outcomes were time to adequate sedation and proportion of patients who achieved adequate sedation at each follow-up interval. Sedation levels were measured on a 6-level validated scale (ClinicalTrials.gov Identifier: NCT02380118). Findings: Of 206 patients randomised, 167 (mean age, 42 years; 98 [58·7%] male) were analysed. Median time to sedation for IM midazolam, olanzapine, and haloperidol was 8·5 (IQR 8·0), 11·5 (IQR 30·0), and 23·0 (IQR 21·0) min, respectively. At 60 min, similar proportions of patients were adequately sedated (98%, 87%, and 97%). There were statistically significant differences for time to sedation with midazolam compared to olanzapine (p = 0·03) and haloperidol (p = 0·002). Adverse event rates were similar across the three arms. Dystonia (n = 1) and cardiac arrest (n = 1) were reported in the haloperidol group. Interpretation: Midazolam resulted in faster sedation in patients with undifferentiated agitation in the emergency setting compared to olanzapine and haloperidol. Midazolam and olanzapine are preferred over haloperidol's slower time to sedation and potential for cardiovascular and extrapyramidal side effects. Funding: Research Grants Council, Hong Kong.

Abstract

Plants and algae are faced with a conundrum: harvesting sufficient light to drive their metabolic needs while dissipating light in excess to prevent photodamage, a process known as nonphotochemical quenching. A slowly relaxing form of energy dissipation, termed qH, is critical for plants' survival under abiotic stress; however, qH location in the photosynthetic membrane is unresolved. Here, we tested whether we could isolate subcomplexes from plants in which qH was induced that would remain in an energy-dissipative state. Interestingly, we found that chlorophyll (Chl) fluorescence lifetimes were decreased by qH in isolated major trimeric antenna complexes, indicating that they serve as a site for qH-energy dissipation and providing a natively quenched complex with physiological relevance to natural conditions. Next, we monitored the changes in thylakoid pigment, protein, and lipid contents of antenna with active or inactive qH but did not detect any evident differences. Finally, we investigated whether specific subunits of the major antenna complexes were required for qH but found that qH was insensitive to trimer composition. Because we previously observed that qH can occur in the absence of specific xanthophylls, and no evident changes in pigments, proteins, or lipids were detected, we tentatively propose that the energy-dissipative state reported here may stem from Chl-Chl excitonic interaction.

Abstract

Abstract Background Tian Xian Liquid (TXL) is a Chinese medicine decoction and has been used as an anticancer dietary supplement. The present study aims to investigate the effects of TXL on the apoptosis of HT-29 cells and tumor growth in vivo. Method HT-29 colon cancer cells were treated with gradient dilution of TXL. The mitochondrial membrane potential was measured by JC-1 assay. The release of cytochrome c from mitochondrial and apoptosis-related proteins Bax, Bcl-2, cleaved caspase-3, 9 were examined by Western blot analysis. HT-29 cells were implanted in nude mice to examine the effects of TXL on tumor growth. Result TXL inhibited HT-29 xenografted model and showed a strong and dose-dependent inhibitory effect on the proliferation of HT-29 cells. Mitochondrial membrane potential was reduced by TXL at the concentration of 0.5% above. For Western blot analysis, an increase in Bax expression and a decrease in Bcl-2 expression were observed in TXL-treated cells. TXL treatment increased the protein level of cleaved casepase-3 and caspase-9, and the release of cytochrome c in cytoplasm was up-regulated as well. Conclusion TXL significantly inhibits cell proliferation in the HT-29 cells and HT-29 xenografted model via the mitochondrial cell death pathway.

Abstract

Introduction: Even though randomized controlled trials could not show a significant survival benefit for the use of neoadjuvant systemic therapy (NST), it is increasingly recommended for patients with clinically node-positive breast cancer due to its implications on prognosis, locoregional downstaging and response-driven adjuvant systemic therapy. The aim of this study was to assess the need for international standardization of treatment recommendations by evaluating clinical practice heterogeneity in use of NST for patients with clinically node-positive breast cancer in Europe. Methods: The study was preplanned in the international multicenter phase-III OPBC-03/TAXIS trial (ClinicalTrials.gov Identifier: NCT03513614) after randomization of the first 500 patients with clinically node-positive breast cancer who underwent axillary lymph node dissection (ALND) or axillary radiation (ART) without ALND after tailored axillary surgery (TAS) in the context of extended regional nodal irradiation. Clinically node-positive breast cancer was defined by confirmed nodal disease at the time of initial diagnosis; in case of neoadjuvant therapy, residual nodal disease was mandatory. Investigators were encouraged to enroll all eligible patients consecutively. However, TAXIS is unique inasmuch as its pragmatic design allows both the neoadjuvant and adjuvant setting according to the preferences of the treating physicians and institutions and thus provides an excellent opportunity to study patterns and trends in use of NST in patients with clinically positive nodes in Europe. Results: A total of 500 patients with a median age of 57 years (IQR: 48-69 years) were included at 44 breast centers in 6 European countries from August 2018 to June 2022. Subtype was hormone receptor (HR) positive (+) and human epidermal growth factor receptor 2 (HER2) negative (-) in 393 (80.0%), HR+/HER2+ in 52 (10.6%), HR-/HER2+ in 5 (1.0%) and HR-/HER2- in 34 (6.9%) patients. The rate of patients undergoing NST was 31.4% with a significant upward trend over time during the study period (from 20.0% in 2018 to 38.1% in 2022; p=0.044). The use of NST varied significantly by country (p=< 0.001) and by site (p=0.015). For patients with clinical AJCC tumor stage II and III, the rates of patients undergoing NST in Switzerland were 26.5% (18 of 68) and 35.9% (92 of 256), in Germany 22.2% (2 of 9) and 30.4% (7 of 23), in Austria 50% (7 of 14) and 60% (9 of 15) and in Hungary 0% (0 of 15) and 20.7% (18 of 87), respectively (p=0.019 and 0.004). Large differences by country were found for ER+/HER2- breast cancer, ranging from 13.1% (11 of 84) in Hungary to 47.8% (11 of 23) in Austria (p=0.007). Within Switzerland, which was the country with most included patients (328 of 500) and participating sites (n=25), the rate of patients undergoing NST for ER+/HER2- breast cancer varied considerably by site, ranging from 10% (2 of 20) to 50% (11 of 22). Discussion: This study revealed substantial heterogeneity in clinical practice in Europe, indicating the need for development of and adherence to consistent guidelines to standardize the international use of NST. Abstract

In this study, an investigation on the influences of different manufacturing techniques on the heat transfer and pressure drop in the developing and fully developed regions of mini-tube under different flow regimes is introduced. The purpose of this research is to experimentally investigate the heat transfer and pressure drop characteristics using 3D-printed tubes and traditional stainless steel tubes in the vertical direction under isothermal and non-isothermal boundary conditions. Experiments are conducted using distilled water (Prandtl numbers varying from 4 and 7) at Reynolds numbers of 800-10000 with heat fluxes between 30 and 500 kW·m-2. Test tubes with inside diameters of 2 mm are used, and the average surface roughness is 1.6 μm and 15.3 μm, respectively. The results are compared with previous studies. It is verified that the heat transfer characteristics are almost the same for the traditional tube and the 3D-printed tube in the laminar region. The average deviation between these two tubes is 7.7%. However, for the turbulent region, the Nusselt numbers of 3D-printed tube in the turbulent region increases by an average of 45% as compared with a traditional tube. The friction factors under heating conditions also increased by an average of 209%. In addition, the 3D-printed tube enters the transition region earlier. The results show that the average critical Reynolds number of a traditional tube and 3D-printed tube is around 2300 and 2000, respectively. Correlations in the turbulent region are developed to predict the friction factors and heat transfer coefficients with good accuracy.

Abstract

Neuroleptic malignant syndrome (NMS) is a rare and acute adverse drug reaction associated with antipsychotic therapy. However, few data on the risk and epidemiology of NMS are available. The aim of this study was to ascertain the incidence risk and all-cause mortality of NMS associated with antipsychotic use, and to assess the association of recent antipsychotic exposure and NMS. We did a population-based study using data from the Hong Kong Hospital Authority’s Clinical Data Analysis and Reporting System database. Cases had a first diagnosis of NMS between 1 January 2004 and 30 November 2017. A case-crossover analysis was used to compare antipsychotic exposure 30 days before the diagnosis of NMS (index date) and a reference period 91–120 days before the index date. To adjust for potential time trends in antipsychotic exposure, we sampled from cases to match current cases and future cases, and further adjusted for select medications and acute medical conditions. 297,647 patients were prescribed antipsychotics, and the incidence risk of NMS was 0.11%. Of the 336 cases included in the case-crossover analysis, 20 (6%) died within 30 days after the index date; only one case had NMS recorded as the primary cause of death. When compared with the reference period, cases were more frequently prescribed multiple antipsychotics (15.8% vs 26.8%; standardized mean difference [SMD] 0.27) and short-acting injectable antipsychotics (3.6% vs 13.7%; SMD 0.37) during the 30 days prior to the diagnosis of NMS. Odds ratios for antipsychotic exposure in the case-crossover, case-crossover adjusted for time trend, and case-crossover adjusted for time trend and potential confounders analysis were 8.00 (95% confidence interval 3.42–18.69), 5.88 (2.46–14.04), and 4.77 (1.95–11.66). Our results suggest that recent use of antipsychotics is associated with NMS. Although a case-only design inherently controls for confounding by time-invariant factors, residual confounding by acute medical conditions with similar presentations to NMS cannot be fully excluded.

Abstract

Introduction: Chemotherapy is recommended for patients with luminal breast cancer and more than three positive nodes. In addition, recent landmark trials raised the question if the exact number of positive nodes is required to indicate genomic testing. In the neoadjuvant setting, response-driven therapy is increasingly used and may be influenced by surgical staging of the axilla. The present study addressed the role of axillary lymph node dissection (ALND) as decision aid for systemic therapy in a contemporary cohort of patients with clinically node-positive breast cancer in the adjuvant and neoadjuvant setting. Methods: The study was preplanned in the international multicenter phase-III OPBC-03/TAXIS trial (ClinicalTrials.gov Identifier: NCT03513614). The first 500 patients with clinically node-positive breast cancer who were randomized after tailored axillary surgery (TAS) to undergo ALND or axillary radiotherapy (ART) without ALND in the context of extended regional irradiation were included from August 2018 to June 2022. Clinically node-positive breast cancer was defined by confirmed nodal disease at the time of initial diagnosis; in case of neoadjuvant therapy, the finding of residual nodal disease was mandatory for randomization. TAS consisted of removal of palpably suspicious findings and the sentinel nodes with the option of image guidance. In the ART arm, the total number of positive nodes was not known. We analyzed the impact of ALND on rate and type of systemic therapy. Results: A total of 500 patients with a median age of 57 years (IQR: 48-69 years) were included at 44 breast centers from six European countries. Subtype was hormone receptor (HR) positive (+) and human epidermal growth factor receptor 2 (HER2) negative (-) in 393 (80.0%), HR+/HER2+ in 52 (10.6%), HR-/HER2+ in 5 (1.0%) and HR-/HER2- in 34 (6.9%) patients. Of 343 patients (68.6%) who were treated in the adjuvant setting, 297 had HR+/HER2- disease. Of these 297 patients, 145 (48.8%) underwent ART without ALND and 152 (51.2%) underwent ALND after TAS. In the ART arm, the median number of lymph nodes removed was five (IQR 4-8), three (IQR 1-4) of which were positive and in the ALND arm, the number was 19 (IQR 14-26), four (IQR 2-9) of which were positive (p < 0.001). The use of ALND had no significant impact on the rate of patients with HR+/HER2- disease undergoing adjuvant chemotherapy (51.0% in the ART and 57.9% in the ALND arm, p=0.2), and there were no significant differences in type of systemic therapy with the exception of tamoxifen, which was 18.4% with ALND versus 9.0% without (p=0.018). A total of 143 patients (28.6%) underwent neoadjuvant chemotherapy, 13 had neoadjuvant antihormonal treatment and one had neoadjuvant double HER2-blockade without chemotherapy. Of the 143 patients who received neoadjuvant chemotherapy, 71 (49.7%) underwent ART without ALND and 72 (50.3%) underwent ALND. In the ART arm, the median number of lymph nodes removed was four (IQR 3-6), one (IQR 1-3) of which was positive and in the ALND arm, the number was 16 (IQR 12-19), two (IQR 1-5) of which were positive (p < 0.001). The use of ALND in patients after neoadjuvant treatment had no significant impact on the rate of adjuvant systemic therapy (71.8% in the ART and 65.3% in the ALND arm, p=0.4), with no significant differences in type of chemotherapy (e.g., capecitabine: 11.3% vs 12.5%, p=0.8; T-DM1: 11.3% vs. 11.1%, p>0.9) or antihormonal therapy (e.g., aromatase inhibitors: 49.3% vs. 41.7%, p=0.4; tamoxifen: 11.3% vs. 5.6%, p=0.2). Abstract

Sleep disruption is a common complaint among patients with post-traumatic stress disorder (PTSD). Modern technology of activity monitoring (actigraphy) enables extended, objective, unobtrusive recording and measuring of daytime and nighttime activity. We conducted a meta-analysis to investigate the actigraphic sleep patterns in PTSD compared with healthy controls.We searched through seven electronic databases from inception to July 2022. Only case-control studies comparing rest-activity variables measured by actigraphy devices between clinically diagnosed PTSD patients and healthy individuals were included.We identified 12 eligible studies comparing 323 PTSD patients and 416 healthy controls. Using a random-effects model, we showed that PTSD patients have significantly lower sleep efficiency (SMD: -0.26, 95 % CI = -0.51 to -0.004, p .05, IThis study included a limited number of studies. Publication bias was not examined on all variables, which could lead to an overestimation of effect size. Four studies involved veterans, which likely differ from civilians regarding traumatic exposure.This meta-analytic review highlighted a pattern of sleep disturbances in PTSD patients compared with non-PTSD individuals. High-quality, large-scale studies are necessary to draw a definitive conclusion regarding the distinctive sleep profile in PTSD. Future research can pay attention to sleep-specific mechanisms underlying PTSD and explore the momentary interactions between sleep-wake variables.

Abbot, Rich, et al. (Virgo and MAGIC Collaboration), Advanced LIGO and Advanced Virgo are monitoring the sky and collecting gravitational-wave strain data with sufficient sensitivity to detect signals routinely. In this paper we describe the data recorded by these instruments during their first and second observing runs. The main data products are gravitational-wave strain time series sampled at 16384 Hz. Abstract

Many structural analysis problems in civil engineering and mechanical engineering can be treated as plane stress and plane strain problems introduced in the theory of elasticity. One of the popular analytical methods to tackle plane analysis is to determine Airy stress function. In general, the Airy stress function depends on the analyzed domain and the applied loads; however, the number of problems that can be solved by employing this method is limited because of the formidable challenges of guessing trial function. In many cases, the trial Airy stress functions are selected based on the results of a simple beam model or experimental results. This paper introduces a solution of the plane stress subjected to horizontal shear forces by using a polynomial Airy stress function, in which the trail function is predicted from the results of the elementary beam theory of an equivalent model. The numerical investigation on stress distributions was presented, and it showed that although the internal shear force acting on cross-sections have not appeared, shear stress still appeared, and the shear stress diagram had both negative and positive areas.

Abstract

Background The safety and effectiveness of intramuscular olanzapine or haloperidol compared to midazolam as the initial pharmacological treatment for acute agitation in emergency departments (EDs) has not been evaluated. Methods A pragmatic, randomised, double-blind, active-controlled trial was conducted from December 2014 to September 2019, in six Hong Kong EDs. Patients (aged 18–75 years) with undifferentiated acute agitation requiring parenteral sedation were randomised to 5 mg intramuscular midazolam (n = 56), olanzapine (n = 54), or haloperidol (n = 57). Primary outcomes were time to adequate sedation and proportion of patients who achieved adequate sedation at each follow-up interval. Sedation levels were measured on a 6-level validated scale (ClinicalTrials.gov Identifier: NCT02380118). Findings Of 206 patients randomised, 167 (mean age, 42 years; 98 [58·7%] male) were analysed. Median time to sedation for IM midazolam, olanzapine, and haloperidol was 8·5 (IQR 8·0), 11·5 (IQR 30·0), and 23·0 (IQR 21·0) min, respectively. At 60 min, similar proportions of patients were adequately sedated (98%, 87%, and 97%). There were statistically significant differences for time to sedation with midazolam compared to olanzapine (p = 0·03) and haloperidol (p = 0·002). Adverse event rates were similar across the three arms. Dystonia (n = 1) and cardiac arrest (n = 1) were reported in the haloperidol group. Interpretation Midazolam resulted in faster sedation in patients with undifferentiated agitation in the emergency setting compared to olanzapine and haloperidol. Midazolam and olanzapine are preferred over haloperidol's slower time to sedation and potential for cardiovascular and extrapyramidal side effects. Funding Research Grants Council, Hong Kong.

Abstract

Social entrepreneurship is a complex form of organization, which includes technological organizations, where market-based methods are used to solve social problems. Currently, the issue continues to increase in popularity, but remains poorly understood and its motivations remain undertheorized. The researchers affirm that compassion can complement the individual’s own traditional motivations to influence social entrepreneurship. A model of three mechanisms is presented (integrative thinking, prosocial cost-benefit analysis and commitment to alleviate the suffering of others) that transform compassion into social entrepreneurship and identify the institutional conditions under which it is most likely to be carry on. In this article, we present an introduction on the definition of compassion, a model of how compassion encourages entrepreneurship and a model of the emerging organizational capacity for compassion, and we also discuss the contribution of both models.

Amfetamine and methamfetamine abuse remains a prevalent health problem, increasing the burden on healthcare. Naltrexone, a µ-opioid receptor antagonist, has been suggested as a promising treatment for amfetamine and methamfetamine use disorder.To review the current evidence for the efficacy and safety of naltrexone as a pharmacological treatment for amfetamine and methamfetamine use disorder. The primary outcome was defined as abstinence or reduction of use. Secondary outcomes were, attenuated "positive" subjective effects (e.g., "feel good," "craving," etc.) of amfetamine or methamfetamine after naltrexone treatment, adverse events and physiological changes (e.g., blood pressure, heart rate).This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. A systematic literature search was conducted on 2 April 2017, and updated on 31 March 2018. Records were retrieved from databases including PubMed, EMBASE Classic plus EMBASE 1980 via Ovid, and the databases were searched using keywords and/or headings: (naltrexone AND amfetamine AND dependence) OR (naltrexone AND amfetamine AND craving) OR (vivitrol AND amfetamine) OR (revia AND amfetamine) OR (naltrexone AND amfetamine) OR (naltrexone AND methamfetamine dependence) OR (naltrexone AND methamfetamine AND craving) OR (vivitrol AND methamfetamine) OR (revia AND methamfetamine) OR (naltrexone AND ice) OR (naltrexone AND crystal meth) OR (naltrexone AND methamfetamine). Studies investigating the effects of naltrexone on amfetamine or methamfetamine use were eligible for inclusion. All studies were rated as low risk of bias using the Cochrane tool for risk of bias.Among 591 identified studies, there were four randomized controlled trials. Two studies investigated the effects of naltrexone on amfetamine use disorder and two on methamfetamine use. Compared to placebo, the abstinence rate was increased significantly (p 0.05) by naltrexone in one of two amfetamine studies, whereas there was no statistical difference in the only study reporting methamfetamine use. In one out of two amfetamine studies, naltrexone significantly attenuated either craving levels or subjective effects (e.g., "want more," "like effect") relative to placebo (p 0.05). Additionally, only in one of two methamfetamine studies did naltrexone produce a significant reduction (p 0.05) in craving levels or attenuated subjective effects. Both amfetamine and methamfetamine studies showed good tolerability of naltrexone, with few adverse events seen.There is presently insufficient evidence to support the use of naltrexone in amfetamine and metamfetamine use disorders. There is a compelling need for high-quality studies to further evaluate the potential use of naltrexone.

Abstract

Continuum robot is with better flexibility and manipulability compared to traditional rigid multiple joints robot in operating in confined flexuose space. Among continuum robots, tendon-driven robot with compressible and extensible length has bigger working space than that of the non-extensible robot, though both of them can travel along the nonlinear spatial curved path. However, for the existing extensible robots, they realize extensible action based a pre-compression action, i.e., not from the natural no-load status for elastic components, like springs. This type of designs imposes restrictions on the length of the whole robot. In this paper, we present a novel tendon-driven continuum robot with extensible length from the natural no-load status for elastic components to overcome this limitation. The main contribution of our work is that the new design achieves extension based on extension of nitinol alloy super elastic bar from natural no-load status. Therefore, continuum robot based on this new design shows deeper reachability in natural orifices. Kinematic model of the proposed robot is derived and the workspace of the robot is demonstrated for one and two bending modules. A prototypical robot is built and its contractible and extensible performance is illustrated through experiments. This type of continuum robot may be useful in diagnosis and therapies for clinical scenarios, like laparoscopic surgeries and natural orifice transluminal endoscopic surgery (NOTES).

Abstract

This study analyses the importance of teaching social entrepreneurship in state universities. A survey was applied to university students to find out if personality traits are related to the characteristics of social entrepreneurship. This was achieved through Alpha Cronbach and Pearson/Spearman correlation analysis. The results show that students have the personality traits to become social entrepreneurs. However, social entrepreneurship courses are not taught in universities of Tijuana city. Therefore, it is strongly suggested to teach social entrepreneurship in universities of the city and the state because students possess social entrepreneurship traits and are interested in the subject.

Abstract

Background: Neuroleptic malignant syndrome (NMS), a rare but serious adverse reaction of antipsychotics, however, the little evidence risk and epidemiology of NMS is available. We aimed to determine the risk of NMS associated with exposure to any antipsychotic, and four antipsychotics commonly implicated in cases of NMS: haloperidol, olanzapine, quetiapine, and risperidone. To characterize the incidence and case fatality risk of NMS in patients taking antipsychotics. Methods: We conducted a population-based cohort analyzing health records of 297 647 antipsychotic users identified from January 1st, 2004 to December 31st, 2016 and 328 in patients with incident diagnosis of NMS during the same period, to measure the incidence and case fatality risk of NMS. We further extended the NMS identification period to November 30th, 2017 and conducted a case-crossover study analyzing data for a total of 336 eligible patients to estimate the risk of NMS associated with any antipsychotic, haloperidol, olanzapine, quetiapine, and risperidone, by comparing the use of antipsychotics during day 1 to 30 (case period) and day 91 to 120 (reference period) preceding the diagnosis of NMS. Findings: In the case-crossover study, 336 patients were diagnosed with NMS (210 males [62.5%]; mean age, 49.7 years; case fatality risk 6.0%). After adjustment for time trends in exposure, concurrent medications, and medical conditions, diagnosis of NMS was associated with exposure to any antipsychotic (OR, 4.77; 95% CI, 1.95-11.66), haloperidol (OR, 4.40; 95% CI 1.97-9.81), and quetiapine (OR, 4.32; 95% CI, 1.70-10.97), while there was no observed association for risperidone (OR, 2.00; 95% CI, 0.93-4.32) or olanzapine (OR, 1.23; 95% CI, 0.47-3.18). In the cohort of patients exposed to antipsychotics, NMS occurred with an incidence of 1.10 per 1000 persons. Interpretation: Antipsychotics are associated with an acutely increased risk of NMS. Patients had statistically significant increased odds of exposure to, haloperidol and quetiapine but not risperidone or olanzapine, during the 30 days prior to the diagnosis of NMS, as compared with the earlier reference period. Clinicians who prescribe antipsychotics should weigh the acutely increased risk of NMS with the potential benefits of antipsychotic therapy, especially for haloperidol and quetiapine. In recent years, the waste management and recycling industry has been facing multiple challenges such as protecting the public from harmful hazardous waste, promoting recycling and material source separation into common material streams. For instance, separating waste of hazardous wastes such as paint or batteries can be costly during the source separation process after waste is collected. What is therefore needed is a smart waste management system that is capable of identifying waste materials prior to the separation process. To overcome these challenges and more, we introduce recycle.io, a serverless Internet of Things (IoT) architecture for smart waste management systems. Using recycle.io, it is then possible to determine in real-time the types of source material violations prior to the waste collection. In this manner, waste management systems can identify sources of violations and rectify this by bringing awareness to the public or issuing fines to prevent violations from occurring. We demonstrate usefulness of our approach throughout the paper.

Abstract

Electrical conductivity of starch suspension during ohmic heating was applied to analyze the starch gelatinization process. Native starches from wheat, corn, rice, potato, sweet potato, and pea were prepared on a starch to water ratio of 1:3 (w/w) with adding 0.05 M potassium chloride. A laboratory scale ohmic heating setup was designed to heat the starch suspension from room temperature to 90°C at a controlled heating rate of 10°C/min operating at 50 Hz and 110 V. The results show that starch gelatinization temperatures could be precisely calculated from electrical conductivity of starch suspension during ohmic heating process. The starch gelatinization temperatures based on ohmic heating were comparable to those measured by differential scanning calorimetry (DSC). Highly significant correlation of onset temperature (R=0.9972) and peak temperature (R=0.9950) were observed. The ohmic heating method could provide an alternative way to DSC with a promising potential for on-line and rapid evaluation of starch gelatinization temperatures.

Abstract

Research is fun! At least, that is my experience. As Editor-in-Chief of the journal, I extend this fun to all association members with no hesitation. In recent years, the desire of local clinicians involved in clinical studies has been strengthened with the promotion of continuous education. However, it is not an easy job to perform clinical studies, not to mention submitting your work to various journals for publication. Having a control group in a research study may give rise to ethical issues, and obtaining a large enough sample of patients is yet another common obstacle. Locally, we face problems like how can we blind the reviewer of a study given that both the reviewer and the investigator therapists work in the same clinic, and how the investigators work with the subjects are easily noticeable by the reviewer therapist. No matter how difficult it is, we're still performing studies regularly as this remains the most evidential way to prove the efficacy of our treatment and to advance our profession. Last year, only a few articles were accepted for publication in order to maintain the high standards of the Journal. However in the past year, the Journal received triple the usual number of submissions despite various hurdles around us. Some of the submitted manuscripts were of a high standard and quality as noted by the manuscript reviewers. I must also take this opportunity to thank all the reviewers who were part of the manuscript reviewing process. Your comments were most helpful to the authors for improving their manuscripts, and the Journal could not have been published without your commitment and willingness to share your time and expertise. “Research” virtually means “to investigate thoroughly”. I hope that in the coming year, researchers will bear this fundamental meaning in mind and work in an enjoyable state of mind.

The structure of the N-terminal-truncated Type IVb structural pilin (t-PilS) from Salmonella typhi was determined by NMR. Although topologically similar to the recently determined x-ray structure of pilin from Vibrio cholerae toxin-coregulated pilus, the only Type IVb pilin with known structure, t-PilS contains many distinct structural features. The protein contains an extra pair of beta-strands in the N-terminal alphabeta loop that align with the major beta-strands to form a continuous 7-stranded antiparallel beta-sheet. The C-terminal disulfide-bonded region of t-PilS is only half the length of that of toxin-coregulated pilus pilin. A model of S. typhi pilus has been proposed and mutagenesis studies suggested that residues on both the alphabeta loop and the C-terminal disulfide-bonded region of PilS might be involved in binding specificity of the pilus. This model structure reveals an exposed surface between adjacent subunits of PilS that could be a potential binding site for the cystic fibrosis transmembrane conductance regulator.
