Background Vulvar lichen sclerosus (LS), a poorly recognized chronic inflammatory skin disease, may represent a therapeutic challenge. Pimecrolimus cream 1% is a nonsteroidal, selective inflammatory cytokine inhibitor that has recently been indicated for some inflammatory cutaneous diseases. Objective To investigate the efficacy, tolerability, and safety of 1% pimecrolimus cream therapy in postmenopausal women with LS. Methods A total of 16 patients applied pimecrolimus cream 1% twice a day over the first 3 months and then as required. All the patients completed this study and were then followed up over the next 12 months. The symptoms and clinical appearance of the lesions in every subject were recorded before and after treatment using a severity scale. Results Using pimecrolimus, most of the patients exhibited a significant improvement with respect to symptoms and the clinical appearance of the disease. After 3 months of treatment, complete remission was seen in 11 patients, partial remission in four, whereas one patient experienced no response at all. Over the subsequent 12 months of follow-up, 10 patients exhibited complete remission while five had partial remission. Four cases with complete remission experienced a few relapses during the follow-up period. Older patients and those with an advanced stage of the disease responded poorly. No significant side effects were observed. Conclusions Pimecrolimus cream 1% appears to be an effective and well-tolerated therapeutic alternative option in the treatment of early stage of vulvar LS. Pimecrolimus may reduce the incidence of flare ups, improve long-term disease control, and enhance the patients’ quality of life, especially in postmenopausal women.

Zygomatic hemangioma is an extremely rare, benign, slow-growing tumor occurring mostly in adult women. The radiologic findings are diagnostic. Total excision of the tumor with primary reconstruction of the defect is the preferred treatment modality. We present a case of zygomatic hemangioma and a brief review of the literature.

Hydatic cyst is an illness that appears in consequence of the cystic form of small strap-shaped worm Echinococcus granulosis. Frequently, cysts exist in the lungs and liver. Peritoneal involvement is rare, and generally occurs as a result of second inoculation from rupture of a liver-located hydatic cyst. Primary ovarian hydatic cyst is very rare. A 56-year-old female patient was admitted to Emergency Service with the complaint of stomachache and swollen abdomen. From ultrasonographic examination, a right ovarian 52 × 45-mm heterogeneous semi-solid cystic mass and right hydronephrosis were detected. As a result of the tomographic examination, the right ovarian growth was judged to be a 60 × 45-mm lobule contoured, septal, heterogeneously cystic mass (ovarian carcinoma). Depending on these indicators and with the diagnosis of ovarian carcinoma, laparotomy was planned. During the observation, a mass that compressed on the right ureter and dilatation in the right ureter were determined. The mass was approximately 6 cm long and smoothly contoured, including widespread adhesions, and also obliteration of the pouch of Douglas. The mass was excised and total abdominal hysterectomy and bilateral salpingo-oopherectomy performed. After a pathological examination, hydatid cyst was diagnosed. Although pointing at the issue of the distinctive diagnosis of pelvic and peritoneal mass, it should be realized that the existence of primary peritoneal and pelvic involvement of the hydatic cyst is generally a result of the second inoculation, and is also more common in regions in which Echinococcus granulosa is endemic and livestock production is prevalent.

Extramedullary haematopoiesis is a compensatory phenomenon in diseases where erythrocyte production is diminished or destruction is accelerated. The authors describe a rare case of meningeal extramedullary haematopoiesis mimicking subdural hematoma. CT scan revealed a hyperdense subdural lesion at the right frontal region following head injury. The patient was operated on and the pathological examination of the subdural lesion showed extramedullary haematopoiesis.

Angiolipomas of the neck are extremely rare; to our knowledge, only 2 cases have been previously reported. We report a new case, which occurred in a 28-year-old woman. The mass was totally resected, and the patient showed no recurrence at the 18-month follow-up.

Laryngeal chondromas are rare benign tumors originating from the cartilagenous framework of the larynx. Since its first description in 1882, about 250 cases have been reported in the literature. Depending on the size of the lesion, excision via an endoscopic or open surgical approach is the treatment of choice. The majority of these patients present with an endolaryngeal mass. To encounter an external neck mass as the reason for the initial presentation is a rare occurrence. In this article we report a case of laryngeal chondroma of cricoid cartilage origin, which presented as a neck mass. The lesion was totally excised using a transcervical approach. No tracheotomy or laryngeal stent was needed postoperatively. Despite its rarity, this diagnostic possibility should always be kept in mind for elderly patients with complaints of progressive voice changes and exertional dyspnea.

Nail involvement in lichen striatus is nearly always accompanied by skin lesions. A case of childhood lichen striatus involving the thumb nail is presented.

To investigate the relationship between human papilloma virus (HPV) infection persistence and serum folate, vitamin B12, homocysteine, neopterin levels in cervical dysplasia. Through the years 2007–2008, 122 women who have admitted to Gynecology Clinic were evaluated for cervical cytology, demographical characteristics, HPV infection, serum folate, vitamin B12, homocysteine, albumin, and neopterin levels. Considering all the cases, the highest percentage of the HPV-infected patients was in high-grade squamous intraepithelial lesion (HSIL) group (83%, n = 25). The serum folate levels in all patient groups [HSIL 10.0 ± 0.4 ng/ml, low-grade squamous intraepithelial lesion (LSIL) 10.6 ± 0.5 ng/ml, atypical squamous cells-undetermined significance (ASCUS) 11.1 ± 0.8 ng/ml] were lower than control group (11.9 ± 0.5 ng/ml; p

Uterine tumors resembling ovarian sex cord-stromal tumors (UTROSCTs) are very rare. In this article, we present 3 cases that manifest classical histomorphological features alongside diverse immunohistochemical findings. As a distinctive finding, one of the patients had UTROSCT in the uterus and an ovarian sex cord tumor, called granulosa cell tumor, in the left ovary, simultaneously. Problems in diagnosing such pathologic condition generally arise because of the variable histologic picture of UTROSCT and may cause problems for general and other nongynecologic surgical pathologists. Immunohistochemically, these tumors express different markers that indicate their polyphenotypic origins.

Toxoplasmosis is caused by an obligate intracellular protozoan, Toxoplasma gondii, which can infect birds and mammals including humans. The disease is common throughout the world, and the most common clinical form is generalized or localized lymphadenopathy. However, in the literature, toxoplasmosis manifesting as a parotid mass is extremely rare. A 37-year-old female was admitted to our department with a painless mass in the parotid region, which had been present for one month. Upon physical examination, a hard, mobile, superficially located, and painless parotid mass with a size of 2x2 cm was detected. Following the diagnosis of a parotid neoplasm, a superficial parotidectomy was performed. Histopathological examination of the surgical specimen supported a diagnosis of toxoplasmosis lymphadenitis. Specific serum immunoglobulin levels were studied and results also confirmed the diagnosis of toxoplasmosis. The patient was given rovamycine for three weeks, and on 12 months follow-up, the patient was still free of recurrence.

Aspergillosis of the maxillary sinus is rarely seen in nonimmunocompromised patients. Among paranasal sinus infections associated with aspergillosis, the most common subtype is Aspergillus fumigatus. A 41-year-old woman presented with facial pain. She had a five-year history of root canal filling in the right upper molar teeth. Paranasal sinus tomography showed fungal sinusitis of radiopaque appearance, giving the impression of a foreign body in the maxillary sinus. The patient underwent endoscopic sinus surgery to purge infection of the maxillary sinus. Pathological examination revealed aspergillosis of the maxillary sinus.

A 63-year-old woman presented with dyspnea, non-productive cough and chest pain. On computed tomography of the thorax, near total atelectasis of the left lung was reported. On bronchoscopic examination, a mass lesion was observed in the mucosa of the left main bronchus leading to near total obstruction. Her medical history revealed that she had been treated with total abdominal hysterectomy and adjuvant radiation therapy for a leiomyosarcoma of the uterus one year ago. She was diagnosed as having a metastatic leiomyosarcoma and was treated with palliative radiation therapy. Six months later, she presented with disseminated disease involving the thorax and the pelvis. She refused further treatment and died one month later. Patients with leiomyosarcomas of the uterus might follow an aggressive clinical course, with relatively early metastases at unusual sites.

We report a case of mucinous cystadenoma in a horseshoe kidney which radiologically resembled a simple renal cyst. In the published literature, three cases of mucinous cystadenoma of renal origin have been reported. Although these tumors are believed to originate from the renal pelvis, the cyst in the present case originated from renal parenchyma. The significance of this particular case is the radiological features, which mimick a simple renal parenchymal cyst and contribute to the histopathological definition of an extremely rare disease.

Vaginal leiomyoma is uncommon and borderline leiomyoma of the vagina is an extreme rarity. Leiomyoma of the vagina may undergo malignant change to become borderline malignancy or leiomyosarcoma. A 38-year-old woman, complaining of foul vaginal discharge and dyspareunia with 2 months duration, underwent total excision of tumor. The removed tumor was diagnosed as borderline case of leiomyoma histologically. Unfortunately, she had tumor recurrence within 4 months. Thus, it was more likely that the primary diagnosis was incorrect and that the diagnosis leiomyosarcoma has been missed. She underwent total abdominal hysterectomy, bilateral salphingo-oophorectomy and total vaginectomy. The final diagnosis was leiomyosarcoma. Three months later tumor recurred again and re-excision followed by chemotherapy was performed. However, she died of her disease on her 5th month following three courses of chemotherapy.

Almost all nasal polyps originate from the mucosa of the lateral walls of the nasal cavity or from the paranasal sinuses. A choanal polyp is the intranasal portion of a cyst that has arisen from the wall of the maxillary sinus near the ostium. Medially based polyps, such as those that arise from the nasal septum, are rare. The literature cites a wide range of incidence rates for polyps originating from this structure, but choanal extension of this type of polyp is extremely unusual. This report describes a polyp that arose from the superior aspect of the posterior nasal septum and extended through the choana into the nasopharynx. The histology of this choanal lesion was typical of nasal polyps, but the site of origin is rare. The ethiopathogenesis of nasal polyps with its common location remains controversial so it is difficult to speculate what mechanism triggered the development of this lesion on the nasal septum. Some form of local inflammation may have induced choanal polyp formation at this atypical site.

Autoimmune progesterone dermatitis (APD) is an uncommon cutaneous disorder characterized by exacerbations during the luteal phase of the menstrual cycle. We describe a 27-year-old woman with a recurrent skin eruption for 3 years. She had no history of exposure to synthetic progesterones. At each menses, the patient developed scaly, erythematous maculopapular lesions over the face. Intradermal skin test reaction to progesterone was positive. Progesterone sensitivity was also demonstrated by challenge test with intramuscular progesterone acetate. These features were consistent with the diagnosis of APD. Our patient was treated successfully with conjugated estrogen for 6 months. At one year follow-up, the patient had had no recurrence of facial eruption.

We report an 8-year-old boy with a primary subcutaneous sacrococcygeal ependymoma, a rare tumor that is thought to arise in embryologic rests. The lesion was completely removed in our patient, who has been followed without recurrence for 20 months. Our experience, together with that of the other 15 cases in the world literature, supports surgical excision as the mainstay of treatment.

A histopathologic study in eighty six patients with ectopic pregnancy and in eighty six control patients was undertaken to evaluate the association between ectopic pregnancy and tubal pathology in our population. Fifty six (65%) and nine (10.4%) cases with chronic salpingitis (CS) were diagnosed in the study group and in the control group, respectively. This difference was statistically significant (p < 0.001). Twelve of 56 cases with chronic salpingitis were salpingitis isthmica nodosa (SIN) and no patient with SIN was observed in the control group. SIN was always concomittant with chronic salpingitis in our study. Based on these findings, we concluded that chronic salpingitis and SIN have an important role in the etiology of ectopic pregnancy in our population and SIN is significantly associated with chronic salpingitis.

Although syringoma of the eyelids and the cheeks are well known and defined, vulvar syringoma is a rare clinical variant of this benign eccrine tumor. A case of syringoma of the vulva exacerbated during pregnancies with regression in the periods in between is presented. Histopathological examination showed typical features of syringomas. Only 20 patients with vulvar syringoma have been previously reported in the literature.
