1. Development of simplified air drag models including crosswinds for commercial heavy vehicle combinations.

Abstract: Accurate range prediction requires good knowledge of the prevailing wind conditions and how they affect the energy consumption of the ego vehicle. A few different simplified vehicle air drag models that explicitly include the effect from crosswinds are presented and compared through some objective criteria. The models are developed from the normal air drag equation where the effect from wind is implicit and therefore often forgotten or neglected. The purpose is to find a low-complexity model complementing CFD models and wind tunnel tests, that can be used for range estimation and predictive energy management algorithms. To simplify online estimation, a requirement is that the air drag models only contain a few tuning parameters. The models are validated against CFD calculations for a few vehicle combinations and the best models show good accuracy for air attack angles up to at least 60 degrees. It is shown that the parameters of the simplified models can loosely be connected to some basic geometrical attributes of a vehicle combination so it should be possible to give at least a rough estimate of the parameters of a simplified model based on these geometrical attributes. This is useful for making a first estimate of the aerodynamic properties of a vehicle combination after major changes in the exterior, e.g. when adding a trailer. It also highlights that the size and the shape of the vehicle side may be mainly responsible for the high longitudinal air drag sensitivity to crosswinds for large vehicle combinations. Abstract: Background: Ozanimod, a selective sphingosine-1-phosphate receptor modulator, is under investigation for the treatment of inflammatory bowel disease. Methods: We conducted a phase 3, multicenter, randomized, double-blind, placebo-controlled trial of ozanimod as induction and maintenance therapy in patients with moderately to severely active ulcerative colitis. In the 10-week induction period, patients in cohort 1 were assigned to receive oral ozanimod hydrochloride at a dose of 1 mg (equivalent to 0.92 mg of ozanimod) or placebo once daily in a double-blind manner, and patients in cohort 2 received open-label ozanimod at the same daily dose. At 10 weeks, patients with a clinical response to ozanimod in either cohort underwent randomization again to receive double-blind ozanimod or placebo for the maintenance period (through week 52). The primary end point for both periods was the percentage of patients with clinical remission, as assessed with the three-component Mayo score. Key secondary clinical, endoscopic, and histologic end points were evaluated with the use of ranked, hierarchical testing. Safety was also assessed. Results: In the induction period, 645 patients were included in cohort 1 and 367 in cohort 2; a total of 457 patients were included in the maintenance period. 4. Laulakaa kovempaa! Miten motivoida ja tukea yläkoulun oppilaan laulamista?

Abstract: Tiivistelmä. Tässä pro gradu -tutkielmassa selvitin sitä, mitkä tekijät vaikuttavat yläkoululaisten laulumotivaatioon, ja kuinka musiikinopettaja voi vaikuttaa siihen. Laulutaidon kokemiseen vaikuttavat fyysisten ja perinnöllisten tekijöiden lisäksi identiteetti, uskomukset, harrastuneisuus sekä ympäröivä kulttuuri. Tutkimuksen teoreettinen viitekehys käsittelee nuoren kehitystä sekä laulun motivointia kouluympäristössä. Nuoren kehitystä tarkastellaan kognitiivisen ja identiteetin kehityksen sekä äänenmurroksen ja laulun kehityksen avulla. Keskeisiä ilmiöitä ovat myös nuoren vokaalinen minäkuva sekä laulajan identiteetti. Lauluidentiteettiä käsitellään myös kuorolaisen ja sukupuolivähemmistön näkökulmista. Motivaatiota käsitellessä käyn läpi erilaisia motivaatioteorioita, jotka voivat olla hyödyllisiä tarkasteltaessa laulun motivoimista kouluympäristössä. Käsitttlen myös uskomuksia, koska ne vaikuttavat ihmisen tapaan käsitellä omaa laulukykyä. Käyn läpi myös laulamista kouluympäristössä. Tutkimukseni on laadullinen. Analysoin aineiston laadullisen sisällönanalyysin avulla. Tutkimusaineistoni on toteutettu puolistrukturoitujen teemahaastattelujen avulla. Haastatteluihini osallistui kahdeksan (N=8) musiikinopettajaa ympäri Suomea. Musiikinopettajien työkokemus vaihteli parista vuodesta kolmeenkymmeneen vuoteen. Kuusi haastatteluun osallistuneista oli musiikkikasvatuksen maistereita, ja kaksi valmistuneet laulupedagogeiksi. Oppilaan motivaatioon vaikuttivat hänen kulttuuri- ja perhetaustansa sekä henkinen hyvinvointi, vertaisten luoma paine, luokan sisäinen ryhmädynamiikka sekä oppilaan ja opettajan välinen vuorovaikutus. Laulun motivoimiseen vaikuttivat vahvasti kappalevalinta, ryhmän sisäinen dynamiikka sekä oppilaiden tukeminen ja kannustaminen.

Abstract: Normothermic machine perfusion (NMP) allows objective assessment of donor liver transplantability. Several viability evaluation protocols have been established, consisting of parameters such as perfusate lactate clearance, pH, transaminase levels, and the production and composition of bile. The aims of this study were to assess 3 such protocols, namely, those introduced by the teams from Birmingham (BP), Cambridge (CP), and Groningen (GP), using a cohort of high-risk marginal livers that had initially been deemed unsuitable for transplantation and to introduce the concept of the viability assessment sensitivity and specificity. To demonstrate and quantify the diagnostic accuracy of these protocols, we used a composite outcome of organ use and 24-month graft survival as a surrogate endpoint. The effects of assessment modifications, including the removal of the most stringent components of the protocols, were also assessed. Of the 31 organs, 22 were transplanted after a period of NMP, of which 18 achieved the outcome of 24-month graft survival. The BP yielded 94% sensitivity and 50% specificity when predicting this outcome. The GP and CP both seemed overly conservative, with 1 and 0 organs, respectively, meeting these protocols. Modification of the GP and CP to exclude their most stringent components increased this to 11 and 8 organs, respectively, and resulted in moderate sensitivity (56% and 44%) but high specificity (92% and 100%, respectively) with respect to the composite outcome. This study shows that the normothermic assessment protocols can be useful in identifying potentially viable organs but that the balance of risk of underuse and overuse varies by protocol.

Abstract: International audience; Objectives: Our aim was to assess if switching from a protease inhibitors (PI)-based regimen to a PI-free one is associated with an increased risk of Kaposi Sarcoma (KS) relapse among patients living with HIV (PLHIV) with history of KS and controlled HIV replication.Methods: In a retrospective analysis of the prospectively collected Dat'AIDS database we selected patients who both had a past KS history and a HIV-1 viral load below 200 copies/mL while being PI-treated. We searched for KS relapses while persistent virological success was maintained for at least 6 months, whether patients kept taking the PI, or switched to PI-free regimen.Results: Among the 216 patients with past KS event and a history of HIV-1 infection efficiently treated by a PI-based regimen, 148 patients (68.5%) later switched to a PI-sparing regimen. Their baseline characteristics were not different from non-switching patients. We described 7 cases of relapse (3.2% of the 216 patients). Five cases of relapse occurred in switching patients (3.4%). The remaining two relapses occurred in PI-treated patients (2.9%). At KS relapse, CD4 cell count was 459 cells/μL (range 225-560) for switching patients, compared with 362 and 136 cells/μL for the other two patients.Conclusions: In this large cohort of PLHIV with a history of KS and ART-controlled HIV replication, KS relapses were described in 3.2% of the patients, and were not more frequent when a PI-containing ART regimen has been switched to a PI-free regimen. Our results do not support a specific effect of PI on KS.

Abstract: Tiivistelmä. Tässä kirjallisuuskatsauksena suoritetussa kandidaatin tutkielmassa etsitään keinoja edistää sukupuolitietoista musiikkikasvatusta. Instrumenttivalinnoissa ja tunneilla käyttäytymisessä on pitkään huomattu esiintyvän sukupuolittuneisuutta. Suurin osa klassisen instrumentinsoittajista ovat tyttöjä, kun taas pojat edustavat enemmistöä bändisoittimissa. Tunneilla pojat ovat taas äänekkäämpiä ja tytöt hiljaisempia. Aloin kiinnostumaan aiheesta, kun huomasin niin monen asian olevan sukupuolittunutta ja halusinkin selvittää, mistä se johtuu ja mitä sukupuolten tasa-arvon eteen voisi tehdä musiikinopettajana. Tutkimuskysymyksenä minulla on: Miten edistää sukupuolitietoista musiikkikasvatusta? Avaan tutkielmassani käsitteet sukupuoli, sukupuoliroolit sekä sukupuolitietoisuus. Sukupuoli on monien tekijöiden summa, joka muuttuu jatkuvasti. Ihminen voi kokea itsensä naisena, miehenä tai jotain sen väliltä tai ulkopuolelta. Jokainen määrittää itse oman sukupuolensa. Sukupuolirooleilla taas tarkoitetaan sitä, kuinka ihmiseltä odotetaan sosiaalisen sukupuolen mukaista käyttäytymistä tyttönä, naisena, poikana tai miehenä. Sukupuoliroolit kohdistuvat aina olemiseen tyttönä tai poikana, minkä vuoksi käytän itse tutkielmassa sukupuolesta puhuttaessa tyttö tai poika -käsitettä. Sukupuolitietoisuus opetuksessa käsitetään ajattelutapana, jossa sukupuoli huomioidaan biologian lisäksi kulttuurisesti ja sosiaalisesti muotoutuneina. Sukupuolitietoisella opetuksella halutaan purkaa sukupuolittavia yhteiskunnallisia ja kulttuurillisia rakenteita. Sillä pyritään antamaan jokaiselle mahdollisuus yksilöllisyydelle ilman paineita toteuttaa tietynlaisia odotuksia tai ennakkoluuloja sukupuolen suhteen Käsitteiden lisäksi käsittelen tutkielmassani sukupuolitietoisuuden näkymistä opintosuunnitelmassa, tasa-arvolakia sekä aiempia tutkimuksia aiheeseen liittyen. Tulos- luvussa kokoan tapoja edistää sukupuolitietoista musiikkikasvatusta, joita teoreettisessa viitekehyksessä ilmenee. Lopussa pohdin muun muassa sitä, kuinka musiikinopettaja koulutuksessa sukupuolitietoisuutta pitäisi jo tuoda esille enemmän kuin mitä sitä tällä hetkellä tuodaan.

Abstract: Background The arrival of highly effective, well-tolerated, direct-acting antiviral agents (DAA) led to a dramatic decrease in hepatitis C virus (HCV) prevalence. Human immunodeficiency virus (HIV)-HCV–coinfected patients are deemed a priority population for HCV elimination, while a rise in recently acquired HCV infections in men who have sex with men (MSM) has been described. We describe the variations in HIV-HCV epidemiology in the French Dat’AIDS cohort. Methods This was a retrospective analysis of a prospective cohort of persons living with HIV (PLWH) from 2012 to 2018. We determined HCV prevalence, HCV incidence, proportion of viremic patients, treatment uptake, and mortality rate in the full cohort and by HIV risk factors. Results From 2012 to 2018, 50 861 PLWH with a known HCV status were followed up. During the period, HCV prevalence decreased from 15.4% to 13.5%. HCV prevalence among new HIV cases increased from 1.9% to 3.5% in MSM but remained stable in other groups. Recently acquired HCV incidence increased from 0.36/100 person-years to 1.25/100 person-years in MSM. The proportion of viremic patients decreased from 67.0% to 8.9%. MSM became the first group of viremic patients in 2018 (37.9%). Recently acquired hepatitis represented 59.2% of viremic MSM in 2018. DAA treatment uptake increased from 11.4% to 61.5%. More treatments were initiated in MSM in 2018 (41.2%) than in intravenous drug users (35.6%). In MSM, treatment at the acute phase represented 30.0% of treatments in 2018. Conclusions A major shift in HCV epidemiology was observed in PLWH in France from 2012 to 2018, leading to a unique situation in which the major group of HCV transmission in 2018 was MSM. Abstract: International audience; Background - Although antiretroviral therapy (ART) has reduced the risk of Kaposi sarcoma (KS), KS cases still occur in HIV-infected people. Objective - To describe all KS cases observed between 2010 and 2015 in a country with high ART coverage. Methods - Retrospective study using longitudinal data from 44 642 patients in the French Dat'AIDS multicenter cohort. Patients' characteristics were described at KS diagnosis according to ART exposure and to HIV-plasma viral load (HIV-pVL) (≤50 or >50) copies/mL. Results - Among the 209 KS cases diagnosed during the study period, 33.2% occurred in ART naïve patients, 17.3% in ART-experienced patients and 49.5% in patients on ART, of whom 23% for more than 6 months. Among these patients, 24 (11.5%) had HIV-pVL ≤50 cp/mL, and 16 (66%) were treated with a boosted-PI-based regimen. The distribution of KS localization did not differ by ART status nor by year of diagnosis. Limitations - Data on human herpesvirus 8, treatment modalities for KS and response rate were not collected. Conclusion - Half of KS cases observed in the study period occurred in patients not on ART, reflecting the persistence of late HIV diagnosis. Factors associated with KS in patients on ART with HIV-pVL ≤50 cp/mL remain to be explored.

Abstract: Background We assessed prevalence of multimorbidity (MM) according to year of human immunodeficiency virus (HIV) diagnosis in elderly people living with HIV (PLWH). Methods This was a cross-sectional study of MM in PLWH aged ≥70 years from the Dat’AIDS French multicenter cohort. MM was defined as at least 3 coexistent morbidities of high blood pressure, diabetes mellitus, osteoporosis, non-AIDS cancer, chronic renal failure, cardiovascular and cerebrovascular disease, obesity, undernutrition, or hypercholesterolemia. Logistic regression models evaluated the association between MM and calendar periods of HIV diagnosis (1983–1996, 1997–2006, and 2007–2018). The secondary analysis evaluated MM as a continuous outcome, and a sensitivity analysis excluded PLWH with nadir CD4 count Results Between January 2017 and September 2018, 2476 PLWH were included. Median age was 73 years, 75% were men, median CD4 count was 578 cells/μL, and 94% had controlled viremia. MM prevalence was 71%. HBP and hypercholesterolemia were the most prevalent comorbidities. After adjustment for age, gender, smoking status, hepatitis C and hepatitis B virus coinfection, group of exposure, nadir CD4 count, CD4:CD8 ratio, and last CD4 level, calendar period of diagnosis was not associated with MM (P = .169). MM was associated with older age, CD4/CD8 ratio Conclusions MM prevalence was high and increased with age, low CD4/CD8 ratio, and nadir CD4 count
