Gastric cancer (GC) is one of the leading types of fatal cancer worldwide. Epigenetic manipulation of cancer cells is a useful tool to better understand gene expression regulatory mechanisms and contributes to the discovery of novel biomarkers. Our research group recently reported a list of 83 genes that are potentially modulated by DNA methylation in GC cell lines. Herein, we further explored the regulation of one of these genes, LRRC37A2, in clinical samples. LRRC37A2 expression was evaluated by RT-qPCR, and DNA methylation was studied using next-generation bisulphite sequencing in 36 GC and paired adjacent nonneoplastic tissue samples. We showed that both reduced LRRC37A2 mRNA levels and increased LRRC37A2 exon methylation were associated with undifferentiated and poorly differentiated tumours. Moreover, LRRC37A2 gene expression and methylation levels were inversely correlated at the +45 exon CpG site. We suggest that DNA hypermethylation may contribute to reducing LRRC37A2 expression in undifferentiated and poorly differentiated GC. RACIONAL: No Brasil, o câncer gástrico é o quarto tumor maligno mais frequente entre os homens e sexto entre as mulheres. A causa é multivariada e os componentes de risco conhecidos. Ele tem seu prognóstico e tratamento definidos pela localização e estadiamento do tumor e número de linfonodos ressecados e acometidos. OBJETIVO: O Consenso Brasileiro sobre Câncer Gástrico foi idealizado pela ABCG com o intuito de emitir diretrizes que possam orientar os profissionais médicos no atendimento aos pacientes portadores desta afecção. MÉTODOS: Foram respondidas e resumidas 43 questões refletindo consenso ou não sobre diagnóstico e tratamento que poderão ser empregadas como orientação para sua abordagem multiprofissional. O método envolveu três etapas. Inicialmente 56 cirurgiões do aparelho digestivo e médicos de especialidades correlatas reuniram-se para formular as perguntas que foram enviadas aos participantes para embasarem as respostas em evidências científicas e na experiência pessoal. Resumos aos temas foram apresentados, discutidos em plenário e votados em duas outras reuniões. Os temas abrangeram 53 perguntas envolvendo: diagnóstico e estadiamento (seis questões); tratamento cirúrgico (35 questões); quimioterapia e radioterapia (sete questões); e anatomopatologia, imunoistoquímica e perspectivas (cinco questões). Considerou-se consenso a concordância de mais de 70% dos votos em cada tema. RESULTADOS: Todas as respostas foram apresentadas e votadas, e em 42 delas houve consenso. CONCLUSÃO: Pôde ser elaborado consenso na maior parte das questões que acompanham o atendimento dos pacientes com câncer gástrico, permitindo a elas serem encaminhadas para a confecção de Diretrizes sobre o tema.BACKGROUND: In Brazil, gastric cancer is the fourth most common malignancy among men and sixth among women. The cause is multivariate and the risks are well known. It has prognosis and treatment defined by the location and staging of the tumor and number of lymph nodes resected and involved. AIM: The Brazilian Consensus on Gastric Cancer promoted by ABCG was designed with the intention to issue guidelines that can guide medical professionals to care for patients with this disease. METHODS: Were summarized and answered 43 questions reflecting consensus or not on diagnosis and treatment that may be used as guidance for its multidisciplinary approach. The method involved three steps. Initially, 56 digestive surgeons and related medical specialties met to formulate the questions that were sent to participants for answers on scientific evidence and personal experience. Summaries were presented, discussed and voted in plenary in two other meetings. They covered 53 questions involving: diagnosis and staging (six questions); surgical treatment (35 questions); chemotherapy and radiotherapy (seven questions) and anatomopathology, immunohistochemistry and perspective (five questions). It was considered consensus agreement on more than 70% of the votes in each item. RESULTS: All the answers were presented and voted upon, and in 42 there was consensus. CONCLUSION: It could be developed consensus on most issues that come with the care of patients with gastric cancer and they can be transformed in guidelines.

RACIONAL: O câncer do esôfago ainda constitui neoplasia de difícil tratamento e de prognóstico muito ruim. OBJETIVO: Comparar a resposta do tratamento exclusivo com teleterapia isolada versus teleterapia associada à braquiterapia endoluminal de doentes portadores de carcinoma espinocelular avançado do esôfago. MÉTODO: Foram estudados 49 doentes portadores de carcinoma espinocelular avançado do esôfago médio em estádio clínico III (TNM). Os doentes foram divididos em dois grupos de doentes. O primeiro grupo foi submetido à teleterapia exclusiva com acelerador linear de partículas, dose média de 6000 cGy e o segundo à teleterapia na dose de 5040 cGy associada à braquiterapia endoluminal com Iridium 192 na dose de 1500 cGy. A braquiterapia foi iniciada uma a duas semanas após o término da teleterapia e dividida em três aplicações semanais de 500 cGy. Foram avaliadas as variáveis idade, sexo, raça, hábitos (tabagismo e etilismo), índice de massa corpórea (IMC), complicações com o tratamento, benefícios (melhora da dor e satisfação alimentar) e sobrevivência. RESULTADOS: Os resultados quanto a qualidade de vida (satisfação alimentar, paliação da disfagia e dor) foram melhores no grupo submetido à teleterapia associada a braquiterapia. A sobrevivência foi maior no grupo de teleterapia associada a braquiterapia. CONCLUSÃO: Apesar do índice de cura do câncer espinocelular do esôfago ser quase nulo quando tratado com irradiação isolada, esta terapêutica constitui-se em uma forma de tratamento paliativo para grande parte dos doentes em que existe contra-indicação cirúrgica.BACKGROUND: Squamous cell carcinoma of the esophagus is still a difficult tumor to treat with very poor prognosis. AIM: To compare the response to teletherapy treatment (external beam radiotherapy) alone versus teletherapy combined with brachytherapy for patients with advanced squamous cell carcinoma of the esophagus. METHODS: Were studied 49 patients with advanced squamous cell carcinoma of the esophagus on clinical stage III (TNM-1999). They were separated into two groups. The first, underwent radiation therapy alone with linear accelerator of particles, average dose of 6000 cGy and the second to external beam radiation therapy at a dose of 5040 cGy combined with brachytherapy with Iridium 192 at a dose of 1500 cGy. Brachytherapy started one to two weeks after the end of teletherapy, and it was divided into three weekly applications of 500 cGy. Age, gender, race, habits (smoking and drinking), body mass index (BMI), complications with treatment benefits (pain relief and food satisfaction) and survival were analyzed. RESULTS: The quality of life (food satisfaction, and pain palliation of dysphagia) were better in the group treated with external beam radiation therapy combined with brachytherapy. Survival was higher in the brachytherapy combined with external beam radiation therapy alone. CONCLUSION: Although the cure rate of squamous cell cancer of the esophagus is almost nil when treated with irradiation alone, this therapy is a form of palliative treatment for most patients in whom surgical contraindication exists.

PURPOSE: To evaluate chemoprevention by celecoxib in cases of reflux-induced gastric adenocarcinoma, in Wistar rats that underwent gastrojejunostomy. METHODS: Sixty male Wistar rats of average age three months underwent surgery and were distributed into three groups: group 1, exploratory laparotomy; group 2, gastrojejunostomy; and group 3, gastrojejunostomy and daily celecoxib administration. After 53 weeks, the animals were sacrificed. Changes in the mucosa of the gastric body of group 1 and in the gastrojejunal anastomosis of groups 2 and 3, observed in histopathological and immunohistochemical examinations, were compared. All statistical analyses were performed using Epi-Info®, version 3.4.3. RESULTS: Comparison between groups 2 and 3 relative to the presence of adenocarcinoma showed a statistically significant difference (p=0.0023). Analysis of the association between groups 2 and 3 relative to COX-2 expression also showed a statistically significant difference (p=0.0018). CONCLUSION: Celecoxib had an inhibiting effect on gastric carcinogenesis induced by enterogastric reflux in an animal model.OBJETIVO: Avaliar a quimioprevenção pelo celecoxibe no adenocarcinoma gástrico induzido por refluxo, em ratos Wistar, submetidos a gastrojejunostomia. MÉTODOS: Sessenta ratos machos Wistar, com média de idade de três meses foram operados e distribuídos em 03 grupos: Grupo 1 - Os animais foram submetidos a laparotomia exploradora. Grupo 2 - Os animais foram submetidos a gastrojejunostomia. Grupo 3 - Os animais foram submetidos a gastrojejunostomia e tomaram celecoxib, diariamente. Após um período de 53 semanas, os animais foram sacrificados. As alterações da mucosa do corpo gástrico dos animais do grupo 1 e da anastomose gastrojejunal dos animais dos grupos 2 e 3 foram analisadas no exame histopatológico e imuno-histoquímica e foram comparadas. Todas as análises estatísticas foram realizadas pelo programa Epi Info®, versão 3.4.3. RESULTADOS: No cotejo entre os animais dos grupos 2 e 3 com relação à presença de adenocarcinoma observou-se uma diferença estatística significante (p=0,0023). A análise de associação entre os grupos 2 e 3 com relação à expressão da COX-2, também evidenciou uma diferença estatística significante (p=0,0018). CONCLUSÃO: O celecoxib teve efeito inibidor da carcinogênese gástrica, induzida pelo refluxo em ratos.

RACIONAL/OBJETIVO: Em vista do aumento na incidência do carcinoma da cárdia nos últimos anos, este trabalho visa estudar os aspectos clínico-patológicos, da proliferação celular e da apoptose tumorais, suas correlações e eventuais influências no seu prognóstico. MATERIAL E MÉTODO: Estudaram-se 40 doentes submetidos a gastrectomia total por carcinoma da cárdia, no período de 1988 a 2001, com seguimento clínico de no mínimo 3 anos. Excluíram-se aqueles com tratamento químio ou radioterápico prévio, neoplasia precoce, óbito no intra-operatório ou por outras causas não relacionadas ao câncer. Analisou-se sexo, idade, tipo histológico de Laurén, padrão de crescimento tumoral de Ming, estádio e presença ou ausência de metaplasia intestinal e/ou Helicobacter pylori na mucosa adjacente. O índice apoptótico foi avaliado por cortes histológicos corados pela hematoxilina-eosina. O índice de proliferação celular foi avaliado por meio da imunoexpressão ao PCNA, sendo a técnica imunoistoquímica utilizada a da estreptoavidina-biotina-peroxidase. Para análise da sobrevida, excluíram-se os casos com metástase à distância ao diagnóstico. Foram utilizados testes t de Student, de Mann-Whitney, curvas de Kaplan-Meier e modelo de regressão de Cox. O nível de significância adotado foi menor de 0,05. RESULTADOS: A idade média foi de 61 anos (mediana: 63); houve predomínio do sexo masculino (72,5%), tipo histológico difuso (55%) e infiltrativo (72,5%) e de estádios mais avançados (III e IV: 67,5%). Não houve associação com metaplasia intestinal e/ou H. pylori. O índice apoptótico médio foi de 7,05 em 10 campos consecutivos de grande aumento e de 11,40 em 500 células (2,28%). A positividade média ao PCNA foi de 275,05 células em 10 campos consecutivos de grande aumento e de 409,33 em 500 células (81,9%). Houve correlação positiva do tipo histológico intestinal com PCNA e com a apoptose em 10 campos consecutivos de grande aumento. Houve correlação positiva entre a apoptose e PCNA tumorais. A sobrevida média foi de 28,41 meses. A idade (acima de 63) e o índice apoptótico (acima de 7,05), em análise multivariada, mostraram correlação negativa com a sobrevida. CONCLUSÕES: O adenocarcinoma da cárdia ocorreu mais nos indivíduos do sexo masculino, com idade média de 61 anos, com predomínio do tipo difuso e estádios mais avançados. A sobrevivência ainda é baixa. Houve correlação positiva entre a apoptose e proliferação celular tumoral. Tanto a idade como a apoptose foram fatores prognósticos independentes no câncer da cárdia.BACKGROUND/AIMS: In view of the increased incidence of carcinoma of the cardia over recent years, this work had the aim of studying the clinicopathological aspects, cell proliferative and tumor apoptotic indices of this neoplasm, their interrelations and possible influences on the prognosis. MATERIAL AND METHODS: Forty cases of adenocarcinoma of the cardia were studied between 1988 and 2001, with a minimum clinical follow-up of 3 years. Patients were excluded if they had previous chemotherapy or radiotherapy treatment, presented early neoplasia, or died during the operations or for other reasons unrelated to cancer. Gender; age, Laurén and Ming histological type, staging, and the presence or absence of intestinal metaplasia, epithelial dysplasia and Helicobacter pylori in the adjacent mucosa were analyzed. The apoptotic index was evaluated via hematoxylin-eosin in the primary tumor. To analyze the cell proliferation tumor, PCNA was utilized. The immunohistochemical technique utilized was streptavidin-biotin-peroxidase. For the survival analysis, cases with distant metastasis upon diagnosis were excluded. For the statistical analysis, the Student t and Mann-Whitney tests, Kaplan-Meier curves and Cox regression model were utilized. RESULTS: The mean age was 61 years (median: 63). There was predominance of the male gender (72.5%), diffuse histological type (55%) and infiltrative histological type (72.5%), and the more advanced stages (III and IV: 67.5%). There was no association with intestinal metaplasia and/or H. pylori. No epithelial dysplasia was detected in adjacent mucosa in any of the cases. The mean apoptotic index was 7.05 in 10 high power fields and 11.40 in 500 cells (2.28%). The mean positivity to PCNA was 275.05 cells in ten high power fields and 409.33 in 500 cells (81.9%). There was a positive correlation between the cell proliferative and apoptotic indices. There was a positive correlation for intestinal histological type with PCNA and apoptotic indices, in 10 high power fields. The mean survival was 28.41 months. Age over 63 and apoptotic index over 7.05 showed a negative correlation with survival, in multivariate analysis. CONCLUSIONS: Adenocarcinoma of the cardia predominated in male adults of mean age 61 years, and the predominant type was diffuse in more advanced stages. There was a correlation between apoptosis and tumoral cell proliferation. Survival in cases of adenocarcinoma of the cardia is still low. Both age and apoptosis were independent prognostic factors in cancer of the cardia.

INTRODUÇÃO: O tumor estromal gastrointestinal (GIST) é o sarcoma mais comum do aparelho digestivo. Essa neoplasia ocorre devido à mutação do gene KIT com consequente ativação constitutiva da proteína KIT. O tratamento primário é cirúrgico e consiste na sua ressecção completa. Entretanto, alguns grupos de pacientes apresentam risco elevado de recorrência mesmo após operação com ressecção completa (R0), indicando diferenças no comportamento biológico. Estudos clínicos comprovaram a atividade clínica do mesilato de imatinibe, fazendo dele a primeira linha de tratamento padrão nos GISTs metastáticos ou irressecáveis, mudando muito o desfecho clínico dessa doença em relação aos benefícios anteriormente obtidos com a quimioterapia antineoplásica. MÉTODO: Foi realizada revisão da literatura com consulta nos periódicos das bases Medline/Pubmed, Scielo e Lilacs cruzando os descritores: tumor estromal gastrointestinal, Gist, tratamento, adjuvância. Além desta revisão foi adicionada a experiência pessoal dos autores. CONCLUSÃO: Melhor refinamento dos critérios de prognóstico tem permitido selecionar de forma mais adequada pacientes para o tratamento adjuvante com imatinibe. Os resultados de maior evidência até o momento respaldam o tratamento adjuvante por um ano, o que produz benefício significativo na sobrevida livre de recidiva, mas não na sobrevida global desses pacientes.INTRODUCTION: Gastrointestinal stromal tumor (GIST) is the most common sarcoma of the digestive tract. This cancer occurs due to mutation of the KIT gene resulting in constitutive activation of KIT protein. The primary treatment is surgical and consists of complete resection. However, some groups of patients at high risk of recurrence even after surgery with complete resection (R0), indicate differences in biological behavior. Clinical studies have demonstrated the clinical activity of imatinib mesylate, making it the standard first-line treatment in metastatic or unresectable GISTs, changing the outcome of this disease in relation to the benefits obtained previously with cancer chemotherapy. METHODS: Was performed a literature review with consultation in Medline/Pubmed, Lilacs and Scielo crossing the key words: gastrointestinal stromal tumor, GIST, treatment, adjuvant treatment. In addition to this review was added to the authors' personal experience. CONCLUSION: Better refinement of prognostic criteria is allowed to select the most appropriate patients for adjuvant treatment with imatinib. The results are yet evident on basis of one year, which produces significant benefit in relapse-free survival but not overall survival in these patients.

OBJECTIVE: To evaluate the protective effect of celecoxib in the esophageal mucosa in rats undergoing esofagojejunostomy. METHODS: Sixty male Wistar rats from the vivarium of the University of Health Sciences of Alagoas were used for the experiment. The animals were divided into four groups: Group I, 15 rats undergoing esofagojejunostomy with the use of celecoxib postoperatively; Group II, 15 rats undergoing esofagojejunostomy without the use of celecoxib; Group III, 15 rats undergoing celiotomy with bowel manipulation; and Group IV, 15 rats without surgery and using celecoxib. The observation period was 90 days. After the death of the animals, the distal segment of the esophagus was resected and sent for microscopic analysis. RESULTS: esofagojejunostomy caused macroscopic and microscopic esophagitis. Esophagitis was equal in both groups I and II. In groups III and IV esophageal lesions were not developed. CONCLUSIONS: celecoxib had neither protective nor inducing effect on esophagitis, but had a protective effect on dysplasia of the animals of group I.

OBJETIVO: Avaliar a expressão da proteína p53 no adenocarcinoma gástrico e correlacioná-la com variáveis clínicas e anatomopatológicas, tais como: idade, sexo, infiltração da parede gástrica (T), tipo histológico (Laurén), grau de diferenciação histológica, comprometimento linfonodal, estadiamento (TNM) e sobrevida. MÉTODO: Foram analisados os registros médicos e reestudadas as lâminas de peças cirúrgicas de 45 doentes com adenocarcinomas gástricos submetidos à gastrectomia parcial e total no Serviço de Cirurgia Oncológica da Santa Casa de Misericórdia de Maceió-AL e no Hospital Universitário da Universidade Federal de Alagoas, no período de 1991 a 2002. A expressão da proteína p53 foi avaliada pelo método imunohistoquímico com o anticorpo monoclonal DO-7 e comparada com idade, sexo, infiltração na parede gástrica, tipo histológico, grau de diferenciação, comprometimento linfonodal, estadiamento e sobrevida. RESULTADOS: Dos 45 doentes, 27 eram do sexo masculino (60%). A média das idades foi 53,9 anos (26 - 75 anos), e mediana de 57 anos. Em 40 doentes (88,9%) o tumor foi classificado como bem diferenciado. Quanto à infiltração na parede gástrica, em 28 doentes (62,2%) foram classificados como profundos. Em 25 doentes (55,6%) não havia comprometimento linfonodal. O estudo histológico revelou que 29 doentes (64,4%) apresentavam tumores classificados como tipo intestinal de Laurén. O estadiamento TNM demonstrou que 33 (73,3%) doentes apresentavam tumores avançados. Quanto à expressão da p53, 18 doentes (40%) foram considerados positivos. O tempo médio de seguimento foi de 1020,4 dias (63 - 3920 dias) e mediana de 798 dias. Trinta e um (68,9%) doentes evoluíram para óbito. ABSTRACT Background : The II Brazilian Consensus on Gastric Cancer of the Brazilian Gastric Cancer Association BGCA (Part 1) was recently published. On this occasion, countless specialists working in the treatment of this disease expressed their opinion in the face of the statements presented. Aim : To present the BGCA Guidelines (Part 2) regarding indications for surgical treatment, operative techniques, extension of resection and multimodal treatment. Methods: To formulate these guidelines, the authors carried out an extensive and current review regarding each declaration present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases initially with the following descriptors: gastric cancer, gastrectomy, lymphadenectomy, multimodal treatment. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results : Of the 43 statements present in this study, 11 (25,6%) were classified with level of evidence A, 20 (46,5%) B and 12 (27,9%) C. Regarding the degree of recommendation, 18 (41,9%) statements obtained grade of recommendation 1, 14 (32,6%) 2a, 10 (23,3%) 2b e one (2,3%) 3. Conclusion : The guidelines complement of the guidelines presented here allows surgeons and oncologists who work to combat gastric cancer to offer the best possible treatment, according to the local conditions available.

ABSTRACT Background: The II Brazilian Consensus on Gastric Cancer by the Brazilian Gastric Cancer Association (ABCG) was recently published. On this occasion, several experts in gastric cancer expressed their opinion before the statements presented. Aim: To present the ABCG Guidelines (part 1) regarding the diagnosis, staging, endoscopic treatment and follow-up of gastric cancer patients. Methods: To forge these Guidelines, the authors carried out an extensive and current review regarding each statement present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases with the following descriptors: gastric cancer, staging, endoscopic treatment and follow-up. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results: Of the 24 statements, two (8.3%) were classified with level of evidence A, 11 (45.8%) with B and 11 (45.8%) with C. As for the degree of recommendation, six (25%) statements obtained grade of recommendation 1, nine (37.5%) recommendation 2a, six (25%) 2b and three (12.5%) grade 3. Conclusion: The guidelines presented here are intended to assist professionals working in the fight against gastric cancer with relevant and current information, granting them to be applied in the daily medical practice.

ABSTRACT Background: Since the publication of the first Brazilian Consensus on Gastric Cancer (GC) in 2012 carried out by the Brazilian Gastric Cancer Association, new concepts on diagnosis, staging, treatment and follow-up have been incorporated. Aim: This new consensus is to promote an update to professionals working in the fight against GC and to provide guidelines for the management of patients with this condition. Methods: Fifty-nine experts answered 67 statements regarding the diagnosis, staging, treatment and prognosis of GC with five possible alternatives: 1) fully agree; 2) partially agree; 3) undecided; 4) disagree and 5) strongly disagree A consensus was adopted when at least 80% of the sum of the answers “fully agree” and “partially agree” was reached. This article presents only the responses of the participating experts. Comments on each statement, as well as a literature review, will be presented in future publications. Results: Of the 67 statements, there was consensus in 50 (74%). In 10 declarations, there was 100% agreement. Conclusion: The gastric cancer treatment has evolved considerably in recent years. This consensus gathers consolidated principles in the last decades, new knowledge acquired recently, as well as promising perspectives on the management of this disease.

PURPOSE: To evaluate the immunohistochemical expression of p16, Ki-67, p53 and Bcl-2 proteins in gastrointestinal stromal tumors (GIST); to assess the possible association between these variables and clinical and histopathological factors of cancer; and to check for prognostic value of these variables (survival and recurrence). METHODS: A sample of 55 patients treated surgically for GIST in three hospitals was studied. The surgically excised tumors were confirmed as GIST by KIT, vimentin, desmin S100 protein, CD117, 1A4 and CD34 assessment in paraffin blocks. RESULTS: Only 9 (16%) cases of GIST were positive for p53, p16 was positive among 43.6%; 80% of GISTs showed staining for Bcl-2. The proliferative index (expressed as the proportion of positive cells) assessed by immunohistochemical expression of Ki-67 was high in 49% of cases. Elevated Ki-67 scores were associated to high histological grade (p=0.0026) and mitosis index, MI (p=0.0001). High Ki-67 index was associated to death. Expression of p53, p16 and Bcl-2 did not correlate to morphological or clinical variables. CONCLUSIONS: Ki-67 immunohistochemical evaluation should be included in preoperative evaluation of GIST biopsies or surgical specimens as a prognostic tool for clinical staging; and all other proteins studied (Bcl-2, p53 and p16) did not play a role in GIST metabolic or carcinogenic process, remaining without prognostic value.OBJETIVO: Avaliar a expressão imunoistoquímica de p16, Ki-67, p53 e Bcl-2 proteínas em tumores gastrointestinais estromais (GIST); determinar a possível associação entre essas variáveis e fatores clínicos e histopatológicos de câncer, e para verificar o valor prognóstico destas variáveis (sobrevivência e recorrência). MÉTODOS: Uma amostra de 55 pacientes tratados cirurgicamente para GIST em três hospitais foi estudada. Os tumores extirpados cirurgicamente foram confirmados como GIST por KIT, vimentina, proteína desmina S100, CD117, 1A4 e avaliação de CD34 em blocos de parafina. RESULTADOS: Apenas nove (16%) casos de GIST foram positivos para p53, p16 foi positiva em 43,6%, 80% dos GIST apresentaram coloração para Bcl-2. O índice proliferativo (expresso como a proporção de células positivas), avaliado pela expressão imunoistoquímica de Ki-67, foi elevado em 49% dos casos. Escores de Ki-67 elevados foram associados com alto grau histológico (p=0,0026) e índice de mitose, MI (p=0,0001). Alto índice de Ki-67 foi associado à morte. Expressão da p53, p16 e Bcl-2 não se correlacionou com as variáveis morfológicas ou clínicas. CONCLUSÕES: A avaliação imunoistoquímica de Ki-67 deve ser incluída na avaliação pré-operatória de biópsias ou peças cirúrgicas de GIST como uma ferramenta prognóstica para o estadiamento clínico, e todas as outras proteínas estudadas (Bcl-2, p53 e p16) não desempenharam um papel no processo metabólico ou carcinogênico em GIST, mantendo-se sem valor prognóstico.

CONTEXT: Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor. Cellular proliferation and apoptosis is gaining importance for predicting prognosis in several cancers. OBJECTIVE: To investigate the Ki67 and p53 immunostaining in GISTs. METHODS: Specimens from 40 patients with GIST were assessed for immunohistochemical expression of Ki67 and p53. The tumors were divided according the risk of recurrence in two groups: I with high or intermediate risk and; II with low or very low risk. RESULTS: Among the 40 patients, 21 were men, the mean age was 56 years, 16 occurred in the small intestine and 13 in the stomach, 5 in the retroperitonium, 4 in the colon or rectum and 2 in the mesenterium. Thirty two tumors were from group I and 8 from group II. Half of the patients developed recurrence, being 90% of the group I (P = 0.114). The tumor Ki67 labelling index ranged from 0.02 to 0.35 (mean level 0.12). This index was marginally higher in the group I patients with recurrence (P = 0.09) compared to the patients of the same group without recurrence. p53 staining was expressed in 65% of the GISTs. A higher frequency of p53 and Ki67 had been found in the group I tumors when compared to the other group (P = 0.022; OR = 8.00 - IC 95%: 1.32-48.65). CONCLUSION: The most common site was the small intestine and 80% had a malignant potential justifying the high recurrence observed. No significant correlation was found between p53 and overall outcome of the patients. In group I patients, the evaluation Ki67LI may be a marker of prognosis. The positivity of both markers is higher among the patients with worst prognosis than in the others.CONTEXTO: Os tumores estromais gastrointestinais (GIST) são os tumores mesenquimais mais frequentes. A proliferação intestinal e a apoptose são cada vez mais importantes na avaliação do prognóstico de diversos cânceres. OBJETIVO: Avaliar a imunoexpressão de Ki67 e p53 em GIST. MÉTODOS: Foram estudados a expressão de Ki67 e p53 por imunoistoquimica em tumores de 40 pacientes com GIST. Os tumores foram divididos segundo o risco de recurrência em 2 grupos: I com risco alto ou intermediário e II com risco baixo ou muito baixo. RESULTADOS: Entre os 40 pacientes, 21 eram do sexo masculino, a idade média foi de 56 anos, 16 ocorreram no intestino delgado, 13 no estômago, 5 no retroperitônio, 4 no cólon e reto, e 2 no mesentério. Trinta e dois tumores foram classificados no grupo I e 8 no grupo II. Metade dos pacientes desenvolveu recurrência, sendo 90% de cólon (P = 0,114). O índice de proliferação tumoral Ki67 variou entre 0,02 e 0,35 (média = 0,12). Este índice foi marginalmente superior nos tumores do grupo I com recurrência (P = 0,09), quando comparado aos do mesmo grupo sem recurrência. A expressão do p53 foi observada em 65% dos GISTs. Nos tumores do grupo I foi observada com maior frequência, expressão de p53 e Ki67 (P = 0,022; OR = 8.00 - IC 95%: 1,32-48,65). CONCLUSÃO: A localização mais comum foi no intestino delgado, 80% tinham potencial maligno, justificando a alta recurrência encontrada. Não se observou correlação significante entre p53 e evolução dos pacientes. Nos pacientes do grupo I, a avaliação do KI67LI pode ser um marcador de prognóstico. A positividade dos dois marcadores é maior entre os pacientes de pior prognóstico.

BACKGROUND: Higher proliferation is commonly observed in cancer cells. Apoptosis can be a useful measure of a tumor cell kinetic. Alteration of the balance between proliferation and apoptosis is associated with cancer. AIM: To study proliferation and apoptosis on gastric cancer and in intestinal metaplasia. METHODOLOGY: Twenty-two samples from gastric adenocarcinomas and 22 biopsies from intestinal metaplasia were studied. The apoptotic bodies in hematoxylin-eosin slides and the expression of p53, bcl-2 and Ki67 were determined by immunohistochemistry. RESULTS: The number of the apoptotic cells was higher in cancer. Ki 67LI increased from intestinal metaplasia to gastric cancer. p53 was positive in 68% of the patients with cancer, more frequently in advanced stage and negative in samples of intestinal metaplasia. Although there was no significant difference between the groups, bcl-2 was positive in 45% of gastric cancer tissue and in 68% of metaplasia. In gastric cancer patients bcl-2 was expressed in early gastric cancer more frequently than in advanced stage. CONCLUSION: The positivity of bcl-2 was higher in metaplasia and probably is involved in the progression of carcinogenesis. p53 was negative in metaplasia and positive in more than half of the gastric cancer, mostly in stage IV, suggesting a late event in gastric cancer.RACIONAL: O aumento da proliferação celular é comumente observado no câncer e a apoptose pode ser medida útil na avaliação da cinética celular do tumor. Alterações do equilíbrio entre proliferação celular e apoptose estão associadas ao câncer. OBJETIVO: Avaliar a proliferação celular e a apoptose no câncer gástrico e na metaplasia intestinal. CASUÍSTICA E MÉTODOS: Vinte e dois tecidos de adenocarcinoma gástrico e 22 biopsias de metaplasia intestinal foram estudados. Os corpos apoptóticos foram estudados em lâminas coradas pela hematoxilina-eosina e a expressão de p53, bcl-2 e Ki67 por imunoistoquímica. RESULTADOS: O número de corpos apoptóticos foi maior no câncer. O Ki67LI aumentou no câncer gástrico em relação ao tecido com metaplasia intestinal. O p53 foi positivo em 68% dos pacientes com câncer, principalmente nos tumores avançados e negativo nos indivíduos com metaplasia intestinal. O bcl-2 foi expresso em 45% dos tecidos com câncer gástrico e em 68% dos tecidos com metaplasia, no entanto, esta diferença não foi significante. Nos doentes com câncer gástrico precoce, o bcl-2 foi expresso com maior freqüência quando comparados aos com câncer em estádio avançado. CONCLUSÃO: A expressão do bcl-2 foi maior no tecido com metaplasia, sugerindo envolvimento desta proteína na evolução da carcinogênese. O p53 foi negativo nos tecidos com metaplasia intestinal e positivo em 50% dos tecidos tumorais, principalmente nos doentes com câncer estádio IV, sugerindo ser um evento tardio no câncer gástrico.

Racional - O tratamento e o prognóstico dos pacientes com câncer gástrico dependem, principalmente, do estádio clínico. Os marcadores tumorais séricos e do lavado peritonial podem auxiliar a avaliar o risco de recurrência da doença. Casuística e Métodos - Quarenta pacientes com câncer gástrico (11 estádio I ou II e 29 estádio III ou IV) e 24 com doença benigna foram estudados prospectivamente. Todos os doentes foram submetidos a laparotomia. O sangue e o lavado peritonial foram colhidos durante o ato cirúrgico, antes da retirada do tumor, para determinação dos marcadores CEA e CA72-4. Resultados - Vinte e cinco por cento e 47,5% dos pacientes com câncer gástrico apresentam elevação dos níveis séricos de CEA e CA72-4. Através das curvas ROC definiram-se os valores de corte dos marcadores no lavado peritonial. Através destas curvas, observaram-se que 60% e 57,5% apresentavam CEA e CA72-4 elevado, respectivamente no grupo com câncer gástrico. Os valores de CEA e CA72-4 foram maiores nos pacientes estádios III e IV. No lavado peritonial, os níveis de CEA foram maiores nos doentes com tumores T3-4. Os valores de CA72-4 no lavado peritonial diferenciaram o grupo controle do grupo com câncer gástrico. Conclusão - O CA72-4 foi o marcador sérico mais sensível no diagnóstico de câncer gástrico. Entretanto, no lavado peritonial, o marcador mais sensível foi o CEA. Os valores de CEA foram superiores nos tumores que ultrapassam a serosa e inferiores nos tumores que se restringem a mucosa e muscular.Background - The treatment and the prognosis of gastric cancer patients depends mainly on clinical stage. Serum and peritoneal tumoral markers levels can be helpful to evaluate individual risk for recurrence. Aims - To evaluate the sensibility of the tumoral markers in the serum and in the peritoneal washing on diagnosis of gastric cancer. Patients and Methods - Forty patients with adenocarcinoma of the stomach (11 stage I or II and 29 III or IV) and 24 patients with benign diseases were studied prospectively. All of them were submitted to laparotomy. Blood and peritoneal washed was collected during surgery before tumoral resection, for determination of CEA and CA72-4. Results - CEA and CA 72-4 serum levels were elevated in 25% and 47,5% respectively. Through the curves ROC, we defined the cut-off values for the markers in washed peritoneal fluid. Through these values CEA and CA72-4 rose in 60% and 57.5% respectively. The values of CEA and of CA 72-4 in the serum and in washed peritoneal fluid were higher in cancer patients stage III and IV. CEA levels in the peritoneal washed fluid were higher in the patients with tumor T3-4. Washed peritoneal CA72-4 differed the control group from the cancer group. Conclusion - CA72-4 was the most sensitive marker in the serum of the patients with gastric cancer. Otherwise in the washing peritoneal fluid the most sensitive marker was CEA. These levels were higher in patients with surpass the serosa and lower in patients with mucosa or muscular tumors.

CONTEXT: Synchronous associations of esophageal and gastric cancers are not a common finding, especially with differing histological types and both tumors in advanced forms. A case with such an association is presented, in which an unusual therapy was proposed: palliative gastrectomy and esophageal intubation. CASE REPORT: A 75-year-old white man was referred to our service complaining of malaise and weight loss for one year and dysphagia and vomiting for 2 months. The patient had sought out medical consultation as a result of the latter two complaints.

OBJETIVO: Avaliar os resultados da esofagectomia trans-hiatal no tratamento do megaesôfago chagásico avançado. MÉTODO: Foram estudados retrospectivamente 28 pacientes portadores de megaesôfago chagásico avançado (MCA), graus III e IV, segundo a classificação radiológica de Rezende (adotada pela Organização Mundial de Saúde), e que foram submetidos à esofagectomia subtotal trans-hiatal no Serviço de Clínica Cirúrgica do Hospital Universitário Prof. Alberto Antunes (HUPAA) da Universidade Federal de Alagoas, entre 1982 e 2000. Foram analisadas, as seguintes variáveis: A) Queixas clínicas pré-operatórias versus as pós-operatórias (disfagia, regurgitação, pirose, diarréia, dumping, plenitude pós-prandial, pneumonia e o estado ponderal). B) avaliação radiológica pós-operatória da boca anastomótica esofagogástrica cervical e do estômago transposto. C) avaliação endoscópica pós-operatória do coto esofágico e da boca anastomótica. RESULTADOS: O seguimento variou de 4 a 192 meses, média de 58,18 meses. Dezesseis pacientes eram do sexo feminino e 12 masculinos. Idade mínima de 16 e máxima de 67 anos, média de 36,5 anos. Não houve mortalidade nesta série. Houve resolução plena da disfagia na maioria dos pacientes (20/28 - 71,4%), um (3,6%) referiu disfagia leve que não necessitou tratamento e 7/28 (25%) necessitaram de uma ou mais sessões de dilatação. Nenhum necessitou de dilatação permanente. A pirose foi o sintoma mais importante no seguimento tardio (35,7%), seguida da regurgitação (25%), diarréia (14,3%), plenitude pós-prandial (10,7%) e dumping (3,6%). Houve ganho ponderal em 87,5% dos pacientes avaliados. A esofagite no coto esofágico foi o achado endoscópico mais significativo (46,4%). O esôfago de Barrett no coto remanescente foi encontrada em 10,7% dos casos. A maioria dos achados radiológicos foi normal, embora três doentes (10,7%) tenham apresentado estase gástrica. CONCLUSÃO: A esofagectomia trans-hiatal mostrou-se eficaz para o tratamento da disfagia no megaesôfago chagásico avançado, embora com morbidade elevada, porém com mortalidade nula.

OBJETIVO: Identificar os fatores prognósticos clínicos e anatomopatológicos nos portadores de tumor estromal gastrointestinal (GIST) gástrico submetidos à ressecção cirúrgica. MÉTODO: Estudo retrospectivo realizado no Instituto Nacional do Câncer (INCA), incluindo 29 casos de GIST gástrico c-Kit positivo submetidos à ressecção cirúrgica entre 1983 e 2004. Variáveis clínicas e anatomopatológicas foram investigadas quanto ao significado prognóstico, correlacionando-as com sobrevida global e sobrevida livre de doença. RESULTADOS: O acompanhamento mediano foi de 35 meses. A sobrevida global, estimada em cinco anos, foi de 53%. As variáveis tamanho tumoral maior que 13,5 cm e presença de recidiva tiveram implicação prognóstica na sobrevida global conforme análise univariada (p=0,01 e p=0,03, respectivamente). A análise multivariada evidenciou que tamanho tumoral maior que 13,5 cm e presença de recidiva representaram fatores prognósticos independentes relacionados à sobrevida global (p=0,01 e p=0,03, respectivamente). As características tamanho tumoral maior que 13,5 cm e índice mitótico influenciaram significativamente (p=0,04 e p=0,04) a sobrevida livre de doença (análise univariada), porém apenas tamanho tumoral maior que 13,5 cm apresentou-se como fator prognóstico independente (p=0,04) relacionado à sobrevida livre de doença, quando utilizada a análise multivariada. CONCLUSÕES: O tamanho tumoral e a presença de recidiva representaram fatores prognósticos independentes relacionados à sobrevida global. Apenas o tamanho tumoral apresentou impacto adverso independente na sobrevida livre de doença.
