PURPOSE:To evaluate the surgical technique for subretinal implantation of two sizes of PRIMA photovoltaic wireless microchip in two animal models, and refine these surgical procedures for human trials. METHODS:Cats and Macaca fascicularis primates with healthy retina underwent vitrectomy surgery and were implanted with subretinal wireless photovoltaic microchip at the macula/central retina. The 1.5mm PRIMA chip was initially studied in feline eyes. PRIMA implant (2mm,1.5mm sizes) arrays were studied in primates. Feasibility of subretinal chip implantation was evaluated with a newly-developed surgical technique, with surgical complications and adverse events recorded. RESULTS:The 1.5mm implant was placed in the central retina of 11 feline eyes, with implantation duration 43-106 days. The 1.5mm implant was correctly positioned into central macula of 11 primate eyes, with follow-up periods of minimum 6 weeks (n = 11), 2 years (n = 2), and one eye for 3 years. One primate eye underwent multi-chip 1.5mm implantation using two 1.5mm chips. The 2mm implant was delivered to 4 primate eyes. Optical coherence tomography confirmed correct surgical placement of photovoltaic arrays in the subretinal space in all 26 eyes. Intraoperative complications in primate eyes included retinal tear, macular hole, retinal detachment, and vitreous hemorrhage that resolved spontaneously. Postoperatively, there was no case of significant ocular inflammation in the 1.5mm implant group. CONCLUSIONS:We report subretinal implantation of 1.5mm and 2mm photovoltaic arrays in the central retina of feline and central macula of primate eyes with a low rate of device-related complications. The in vivo PRIMA implantation technique has been developed and refined for use for a 2mm PRIMA implant in ongoing human trials.

Abstract Background The epiretinal membrane (ERM) is a degenerative condition associated with age, which can cause loss of vision and/or metamorphopsia. The treatment of symptomatic ERM involves surgical removal including a vitrectomy followed by peeling of the ERM using a microforceps. As the internal limiting membrane (ILM) is adherent to the ERM, it is sometimes removed with it (spontaneous peeling). If ILM remains in place, it can be removed to reduce ERM recurrence. However, it is important to clarify the safety of ILM peeling, while it increases surgical risks and cause histological disorganization of the retina that can lead to microscotomas, may be responsible for definitive visual discomfort. Methods PEELING is a prospective, randomized, controlled, single-blind, and multicentered trial with two parallel arms. This study investigates the benefit/risk ratio of active ILM peeling among individuals undergoing ERM surgery without spontaneous ILM peeling. Randomization is done in the operating room after ERM removal if ILM remains in place. After randomization, the two groups—“active peeling of the ILM” and “no peeling of the ILM”—are compared during a total of three follow-up visits scheduled at month 1, month 6, and month 12. Primary endpoint is the difference in microscotomas before surgery and 6 months after surgery. Patients with spontaneous peeling are not randomized and are included in the ancillary study with the same follow-up visits and the same examinations as the principal study. Relevant inclusion criteria involve individuals aged > 18 years living with idiopathic symptomatic ERM, including pseudophakic patients with transparent posterior capsule or open capsule or lensed patients with age-related cataracts. The calculated sample size corresponds to 53 randomized eyes (one eye/patient) per arm that means 106 randomized eyes (106 randomized patients) in total and a maximum of 222 included patients (116 spontaneous peeling). Discussion ILM peeling is often practiced in ERM surgery to reduce ERM recurrence. It does not impair postoperative visual acuity, but it increases the surgical risks and causes anatomical damages. If active ILM peeling is significantly associated with more microscotomas, it may contraindicate the ILM peeling during primitive idiopathic ERM surgery. To assess anatomical and functional outcomes after switching from dexamethasone implant (DEXi) to fluocinolone acetonide implant (FAci) in 113 diabetic macular edema eyes, a multicentric retrospective observational study was conducted. Seventy-five eyes (73.5%) were switched 1–8 weeks after their last DEXi. The mean best-corrected visual acuity improved to 59.8 letters at month 4 and remained stable during the follow-up. The mean central macular thickness (CMT) significantly decreased during the follow-up, with a minimum of 320.9 μm at month 3. The baseline CMT was higher in eyes that received the last DEXi >8 weeks versus p < 0.021). After FAci injection, additional treatments were needed in 37 (32.7%) eyes. A longer diabetes duration (p = 0.009), a longer time between the last DEXi and the first FAci (p = 0.035), and a high baseline CMT (p = 0.003) were risk factors for additional treatments. The mean intraocular pressure was 8 weeks before the switch. Switching from DEXi to FAci in DME is effective and safe. A short time between the last DEXi and the first FAci reduced CMT fluctuations and the need for early additional treatments.

We present the case of a 72-year-old male with a right carotid aneurysm incidentally discovered after an ischemic stroke. After management of the stroke, stent-assisted coil embolization was performed to treat the aneurysm. Two simultaneous and early complications occurred: new ischemic strokes and branch retinal artery occlusions, respectively in the right occipital lobe and in the right eye. Treatment of unruptured carotid aneurysm using stent-assisted coil embolization can lead to severe complications involving both the brain and the retina. Multidisciplinary and close surveillance of patients should be performed to diagnose adverse effects of this endovascular treatment.

25. PRIMA subretinal wireless photovoltaic microchip implantation in non-human primate and feline models.

PURPOSE:To evaluate the surgical technique for subretinal implantation of two sizes of PRIMA photovoltaic wireless microchip in two animal models, and refine these surgical procedures for human trials. METHODS:Cats and Macaca fascicularis primates with healthy retina underwent vitrectomy surgery and were implanted with subretinal wireless photovoltaic microchip at the macula/central retina. The 1.5mm PRIMA chip was initially studied in feline eyes. PRIMA implant (2mm,1.5mm sizes) arrays were studied in primates. Feasibility of subretinal chip implantation was evaluated with a newly-developed surgical technique, with surgical complications and adverse events recorded. RESULTS:The 1.5mm implant was placed in the central retina of 11 feline eyes, with implantation duration 43-106 days. The 1.5mm implant was correctly positioned into central macula of 11 primate eyes, with follow-up periods of minimum 6 weeks (n = 11), 2 years (n = 2), and one eye for 3 years. One primate eye underwent multi-chip 1.5mm implantation using two 1.5mm chips. The 2mm implant was delivered to 4 primate eyes. Optical coherence tomography confirmed correct surgical placement of photovoltaic arrays in the subretinal space in all 26 eyes. Intraoperative complications in primate eyes included retinal tear, macular hole, retinal detachment, and vitreous hemorrhage that resolved spontaneously. Postoperatively, there was no case of significant ocular inflammation in the 1.5mm implant group. CONCLUSIONS:We report subretinal implantation of 1.5mm and 2mm photovoltaic arrays in the central retina of feline and central macula of primate eyes with a low rate of device-related complications. The in vivo PRIMA implantation technique has been developed and refined for use for a 2mm PRIMA implant in ongoing human trials.

26. Assessment of Spontaneous Retinal Arterial Pulsations in Acute Central Retinal Vein Occlusions

Central retinal vein occlusion (CRVO) is a common retinal disease. Recent works mentioned spontaneous retinal arterial pulsations (SRAPs) as a feature of some CRVOs. This is a retrospective study on patients presenting with CRVO who were followed up for at least 6 months. The objective was to identify SRAP in the acute phase of the disease and determine their relationship with patients’ characteristics and visual prognosis. A 10-second infrared film centered on the optic disc was recorded within a month of the onset of symptoms, and SRAPs were detected in two-thirds of the cases. Patients with SRAP were significantly younger than those without SRAP. Mean central macular thickness was significantly higher in the absence of SRAP, which was translated into a more severe macular edema; however, this difference faded with time. BCVA tended to be higher in the presence of SRAP at the 6-month follow-up when adjusted to baseline. This study demonstrates that SRAPs are a frequent finding, easily detected by infrared fundus video recording, and associated with a younger age and lesser macular edema.

The Athena X-ray Integral Unit (X-IFU) is the high resolution X-ray spectrometer, studied since 2015 for flying in the mid-30s on the Athena space X-ray Observatory, a versatile observatory designed to address the Hot and Energetic Universe science theme, selected in November 2013 by the Survey Science Committee. Based on a large format array of Transition Edge Sensors (TES), it aims to provide spatially resolved X-ray spectroscopy, with a spectral resolution of 2.5 eV (up to 7 keV) over an hexagonal field of view of 5 arc minutes (equivalent diameter). The X-IFU entered its System Requirement Review (SRR) in June 2022, at about the same time when ESA called for an overall X-IFU redesign (including the X-IFU cryostat and the cooling chain), due to an unanticipated cost overrun of Athena. In this paper, after illustrating the breakthrough capabilities of the X-IFU, we describe the instrument as presented at its SRR, browsing through all the subsystems and associated requirements. We then show the instrument budgets, with a particular emphasis on the anticipated budgets of some of its key performance parameters. Finally we briefly discuss on the ongoing key technology demonstration activities, the calibration and the activities foreseen in the X-IFU Instrument Science Center, and touch on communication and outreach activities, the consortium organisation, and finally on the life cycle assessment of X-IFU aiming at minimising the environmental footprint, associated with the development of the instrument. Thanks to the studies conducted so far on X-IFU, it is expected that along the design-to-cost exercise requested by ESA, the X-IFU will maintain flagship capabilities in spatially resolved high resolution X-ray spectroscopy, enabling most of the original X-IFU related scientific objectives of the Athena mission to be retained. Albinism is a group of genetic disorders characterized by general skin and retinal hypopigmentation. It is in most cases an autosomal recessive condition. Foveal hypoplasia (FH) is one of the main criteria for the diagnosis of albinism. The aim of this study was to analyze the macular profile of the parents of patients with albinism.This study included a case series of 27 patients with albinism seen in Rothschild Foundation between April 2017 and February 2020. Spectral-domain optical coherence tomography (SD-OCT) and OCT angiography (OCT-A) were performed in every patient when possible and in every available parents. FH was graded according to Thomas' classification based on OCT. Next generation sequencing-based gene panel testing was performed in parents and children when a FH was detected on OCT in a parent.Twenty-seven patients with albinism were examined. Nine parents had FH based on the OCT B-scan (33%). In parents without FH based on the SD-OCT B-scan (67%), OCT-A showed a reduced avascular zone in the deep vascular plexus in 4 parents. Six parents carried variants that could explain their phenotype, including TYR R402Q hypomorphic alleles.This study showed the presence of FH in parents of patients with albinism, and aimed to genetically explain this phenotype.

33. Incidence of macular displacement after rhegmatogenous retinal detachment using ultra-widefield fundus autofluorescence

Purpose The incidence of retinal displacement after rhegmatogenous retinal detachment (RRD) surgery is variable and its clinical consequences are unclear. The aim of this study was to assess the incidence and clinical features of retinal displacement after RRD surgery by using ultra-widefield (UWF) imaging. Methods Retrospective observational study including all consecutive patients who underwent RRD surgery at the Rothschild Foundation Hospital. Postoperative data included the visual acuity and symptoms of visual impairment. Macular retinal displacement occurrence and its features were assessed and measured by using the autofluorescence images. Results A total of 123 eyes were included. UWF fundus autofluorescence revealed the presence of macular retinal displacement in 14 (11%) eyes. All displacements were inferior, with a mean angle of 3.8°. Patients with and without macular displacement did not differ in postoperative visual acuity. The retinal detachment extent and preoperative macular involvement were not significantly associated with the occurrence of retinal displacement. Conclusion In this representative cohort of eyes that underwent RRD surgery with systematic screening for postoperative retinal displacement by UWF fundus autofluorescence, 11% of eyes experienced an inferior retinal shift. As in other cohorts, the presence of metamorphopsia was not associated with the occurrence of retinal shift.

34. Postsurgical Macular Edema after Rhegmatogenous Retinal Detachment: Risk Factors for Recalcitrance to First-Line Therapy and Long-Term Response to Dexamethasone Intravitreal Implant

- Abstract
Objectives: To assess the risk factors for recalcitrance to first-line therapy and the long-term response to dexamethasone intravitreal implant (Ozurdex®) of postsurgical macular edema (PSME) after rhegmatogenous retinal detachment (RRD). Methods: This was a retrospective consecutive cohort study of patients who underwent RRD repair between January 2014 and December 2020 at the Rothschild Foundation Hospital and who experienced PSME with at least 18 months of follow-up. Results: Of the 1 152 patients screened, 36 eyes had a PSME (incidence: 3.1%). The mean follow-up was 45.2 months (18.0-80.5 months). Twenty-five eyes (69.4%) were recalcitrant to first-line therapy and received at least one Ozurdex® (mean number: 2.7 [1-12]). On multivariate analysis, perfluorocarbon liquid (PFCL)-assisted drainage was found to significantly increase the risk of recalcitrance (adjusted odds ratio 8.65; 95% confidence interval 1.97-15.33; p = 0.01). A significant difference in best-corrected visual acuity (BCVA) and central macular thickness (CMT) was found before Ozurdex® and at the last follow-up visit: 0.57 ± 0.47 LogMAR vs. 0.34 ± 0.32 LogMAR (p = 0.02) and 483.0 ± 124.0 µm to 354.6 ± 96.5 µm (p=0.001). The absence of serous retinal detachment and the presence of hyperreflective foci at baseline were negative prognostic factors both for recalcitrance and for response to Ozurdex®. Two patients (8%) experienced hypertony, which was well controlled with hypotonic drops. Conclusion: PSME after RRD is highly recalcitrant. Ozurdex® could be reasonably proposed as a first-line treatment, at least if it occurs after PFLC-assisted drainage, given the favorable benefit/risk ratio in the long term.

Resume But Evaluer la baisse de la PIO et la symptomatologie oculaire chez les patients nouvellement diagnostiques de GPAO et traites en monotherapie par le Latanoprost. Patients et methodes Il s’est agi d’une etude multicentrique, transversale et descriptive. Nous avons inclus les adultes nouvellement diagnostiques de GPAO. Tous les patients ont recu une goutte de Latanoprost 0,005 % conserve dans chaque œil tous les soirs pendant 12 semaines. La variation de la PIO et l’apparition des signes et symptomes oculaires etaient evalues pendant et a la fin du traitement. Resultats Au total, nous avons inclus 524 patients avec un taux de participation a 93,3 % a 12 semaines. La moyenne d’âge etait de 52,79 ± 17,33 ans et le sexe-ratio H/F etait de 1,39. La PIO moyenne a l’inclusion etait de 21,68 ± 9,72 mmHg. Apres 2 semaines de traitement, la PIO moyenne etait de 15,49 ± 5,81 mmHg, soit une reduction de 28,55 %. Apres 12 semaines de traitement, la PIO moyenne etait de 13,16 ± 3,54 mmHg, soit une reduction de 39,30 %. Les symptomes enregistres etaient domines par la sensation de grains de sable dont la frequence etait de 4,72 % a S2 et 2,45 % a S12. Les signes etaient majoritairement representes par l’hyperhemie conjonctivale (4,33 % a S2 et 1,84 % a S12). Conclusion Le Latanoprost donne en premiere intention et en monotherapie dans le GPAO chez le melanoderme baisse considerablement la PIO. L’incidence des effets indesirables reste faible ; elle est plus importante en debut de traitement.

Background Fovea plana is defined as an immature macula diagnosed by OCT, showing the unusual shunt of the inner retinal layers into the fovea. The incidence of fovea plana in the adult population remains to be determined. The aim of this study was to determine the incidence of fovea plana in the French population with age-related cataract. Methods Consecutive patients who underwent cataract surgery in Rothschild Foundation Hospital, France, between January and March 2021, with preoperative analyzable OCT scans available, were retrospectively screened in order to determine the incidence of fovea plana in these population. Ophthalmological characteristics of patients were reported, and detailed. Result Fovea plana was encountered in 20 out of 204 patients during the 3 months corresponding to an incidence of 9.8%. One of those patients had stage 2 fovea plana. Conclusion Although fovea plana is defined as an immature macula, it is not rare in preoperative population. This macular aspect was not associated with poor visual acuity in our cohort.

The role of earthworms on biogeochemical carbon cycling is a major knowledge gap resulting from the difficulty of isolating and exploring the effects provided by the diversity of organisms. In this study, we investigated the effect of six earthworm species belonging to three ecological categories on soil organic carbon (SOC) mineralisation. To this end, we produced casts with the six species using subsoil material with low SOC content and miscanthus litter. Cast were subjected to laboratory ageing for 140 days. During this process, we monitored physicochemical parameters, CO2 emissions and determined the micro-scale organisation of the casts’ particulate organic matter and pores using X-ray tomography.Our results showed contrasting properties of fresh casts from the 3 main ecological categories, in accordance with the earthworm species’ morphological or behavioral strategies, indicating that those were maintained in artificial environments. However, species-specific changes in cast properties throughout ageing increased intragroup variability among ecological categories. As a result we observed earthworm species-specific evolution of CO2 mineralisation rates during casts ageing. We found that at least half of the variability in CO2 emissions was explained by cast microstructural changes, related to the spatial arrangement between particulate organic matter, porosity, and mineral particles. We conclude that earthworm species-specific traits may play a role in organic carbon protection through their impact on microstructural cast properties.
