Traditionally characterized by a familialist approach to long-term care (LTC), with the legal obligation for families to care for their relatives, France has developed several policy measures and evolved toward a mixed model, combining public measures and family care. The chapter investigates LTC policy in France by exploring the balance between formal and informal care and its evolution during the last decades. Building on the scholarly debate on familialization versus defamilialization policies, the chapter argues that different policy measures have contributed to a turn toward “optional familialism,” according to which families are encouraged to provide family care and are (directly or indirectly) given alternatives through the provision of public and market care. Based on qualitative empirical research conducted in several research projects since the 2000s and an updated literature review, it explores the impact of the development of a cash-for-care scheme which can support both familialism or defamilialism depending on the context, and which, in France, facilitated the externalization of care. It also investigates the introduction, since the 2000s, of direct policy interventions toward informal caregivers, which question the defamilialization process. It identifies three types of policy intervention – compensation measures (means to reward caregivers’ time financially or via social security rights), conciliation measures (interventions to help caregivers who have a job to combine work and care), and supportive measures (to assist caregivers in their role and enable them to carry out their caring activities) – and concludes with the importance of supportive measures in France and the trend taken toward optional familialism.

Comment rendre compte de la pluralité des expériences du vieillir, de leur transformation et des manières dont nos sociétés en régulent les tensions, sinon en les replaçant dans leurs contextes et leurs dynamiques sociales ? Dans le prolongement des analyses de Simone Pennec à l’université de Bretagne Occidentale (Brest) et à partir d’objets et de terrains d’études variés en France principalement, et aussi en Belgique, au Brésil, au Canada, en Suisse, au Mexique, cet ouvrage analyse les manières dont les individus articulent leurs différents engagements au cours de la vie. Il montre comment certaines manières d’avancer en âge peuvent être soutenues ou au contraire déqualifiées. Les quatre parties de l’ouvrage rendent compte des effets des catégorisations des politiques publiques, de la participation des personnes vieillissantes aux différents espaces sociaux, du statut et des expériences des personnes et de leur entourage face aux transformations liées au vieillissement et du travail de soin, de la diversité de ses contextes, de ses registres et de ses acteurs. Cet ouvrage est destiné à des enseignants, des étudiants et des chercheurs en sciences sociales, à des praticiens de l’intervention sociale et des professionnels de santé, comme à l’ensemble des acteurs professionnels, politiques, militants ou citoyens concernés. Il a été coordonné par cinq enseignant·e·s-chercheur·e·s en sociologie : Françoise Le Borgne-Uguen (UBO-Brest), Florence Douguet (UBS-Lorient), Guillaume Fernandez (UBO-Brest), Nicole Roux (UBO-Brest) et Geneviève Cresson (Université Lille 1).

International audience; On estime aujourd’hui à 8,3 millions le nombre de personnes de 16 ans ou plus qui aident régulièrement et à titre non professionnel un proche en perte d’autonomie à domicile [66]. Les activités accomplies sont multiples, allant d’une aide concrète aux tâches domestiques ou aux soins de la personne elle-même, à une activité plus diffuse et difficile à appréhender de coordination des différentes aides mobilisées et de suivi de la situation [16]. Véritables chefs d’orchestre de l’arrangement d’aide autour de la personne, les aidants restent les pivots de l’accompagnement de leur proche. Mais comment s’organisent-ils dans leur quotidien ? L’analyse du profil des aidants [66] montre que 47 % d’entre eux occupent un emploi ou sont apprentis, 7 % sont au chômage, 33 % sont retraités, 13 % sont d’autres inactifs. Se pose dès lors la question de l’articulation entre la vie personnelle et la vie professionnelle de ces aidants, conjoints ou enfants d’un parent en perte d’autonomie. (Intro.)

International audience; Context and main issue: In France, the necessity to better coordinate health and social care interventions in elderly care has been a longstanding preoccupation of the state. Even if these policies date back to the 1980s, there has recently been a strong emphasis on them as response to persistent fragmentations. Since the beginning of the 2010s, various policies with similar objectives have been initiated. They introduced a number of new components – concepts, tools, and also new dedicated professionals – meant to be implemented by local institutional actors and impacting directly local managers and frontline professionals’ activities. One of these new professionals dedicated to coordination are case managers for elderly people with complex health and social care needs [gestionnaires de cas] introduced by the MAIA scheme – Method of action for the integration of health and social services in the field of autonomy [Méthode d'action pour l'intégration des services d'aide et de soins dans le champ de l'autonomie]. This paper proposes a comprehensive analysis of the activities developed by complex case managers in two local territories. Analysis is based on inputs from both public policy analysis and sociology of professions. Special attention is paid on the reception of those case managers by pre-existing care professionals developing similar coordination tasks, on the one hand, and acceptance strategies implemented by the new complex case manager, on the other. Methodology: The paper is based on the analysis of the local implementation of two MAIA schemes investigated in a postdoc research. It includes a review of available evidence including national and local official reports and research articles and original qualitative semi-directive interviews led between 2015 and 2016. Interviewees include the new professionals introduced by the scheme (n=8) and a selection of institutional, organisational and professional actors they are meant to interact with to develop their activities (n=22).Results: Two different levels of analysis are proposed. The first concerns the local reception of the MAIA schemes. Analysis shows strong resistance to their introduction at at least three interdependent levels: at the institutional level by the main institutional local actors, at the organisational level by the managers of other health and social care organisations, and at the care managers’ level by frontline professionals with similar coordination activities. The second level focuses on the care managers’ level. We show that they face difficulties to make emerge a common professional identity due to unclear professional status and lack of ‘pragmatic’ legitimacy. Despite this, they also need to develop strategies to facilitate their acceptance by the other frontline professionals and develop their activity. Whereas mobilising different skills to which the new care managers are more or less prepared, these strategies have in common to be deployed in situations of concrete and complex interactions in which time is needed to make themselves known and recognised in order to be able to delimitate locally their own field of action

As "community care" has no exact equivalent in the French context, the authors focus on one of the major sector of social and community care which concerns elderly people confronted to a reduction of their autonomy, which correspond to the international denomination: long-term care (LTC). It presents the emergence, the current organization and main challenges of this policy in France.

On 28 March 2019, six months after the launch of a major national consultation process to tackle the challenges of ageing, Dominique Libault, Councillor of State and President of the High Council of Financing for Social Protection, handed over his Report on autonomy and old age to Agnès Buzyn, Minister of Solidarity and Health. The Report outlines the foundations of “a strong, new policy on old age in France”. It builds on a major national consultation carried out between September 2018 and February 2019, and involving all actors concerned with old age along with experts in the field.

La littérature scientifique analyse l’aide au long cours apportée à un proche âgé selon deux grandes perspectives : une perspective épidémiologique du « fardeau »qui appréhende l’aide par ses effets sur la santé des aidants – présentés comme les « victimes cachées » des pathologies et des fragilités de leur parent et une perspective économique qui cherche à quantifier cette aide par un volume horaire, et à en évaluer les coûts économiques directs, et indirects, par les effets sur l’activité professionnelle et l’emploi des aidants. Le rapport propose d’approfondir l’analyse de l’intensité de l’aide, en la saisissant non seulement par ses effets (économiques ou sanitaires), mais par le sens qu’elle revêt au sein de configurations d’aide où sont engagés de manière variable selon les situations des membres de la parenté de la personne âgée, des proches (voisins ou amis) et des professionnels de l’aide à domicile. A partir de l’analyse qualitative de 19 configurations d’aide, saisies à travers 33 entretiens avec des personnes aidées et certains de leurs aidants, le rapport montre comment les variations de l’intensité de l’aide sont socialement construites, tant dans les parcours de vie des individus, dans des contextes socialisateurs antérieurs ou alternatifs à l’aide, que dans les configurations où cette aide se déploie. La première partie revient sur les ressorts sociaux et individuels de l’entrée et du maintien dans l’aide, dans la dialectique des mouvements d’engagement et de désengagement du care. Elle insiste sur l’articulation des positions et des trajectoires sociales dans la définition et la réalisation du soutien au proche âgé. Elle analyse les manières dont les bricolages initiaux de l’aide, peu coûteux, sont déterminants dans la stabilisation de l’accompagnement et dans le maintien dans l’aide. L’intensité de l’aide varie alors non seulement selon les besoins d’aide manifestés par la personne âgée, mais aussi selon les exigences de l’aide définies et portées par les aidants, telles qu’elles se sont constituées au fil de leur histoire personnelle et familiale. La deuxième partie analyse les formes de répartition et de régulation de l’aide entre les membres apparentés, et leurs effets sur l’intensité de l’aide, en lien avec les professionnels lorsque certaines tâches sont externalisées. Si certaines configurations, très asymétriques, concentrent responsabilité de care et réalisation de l’aide sur une seule aidante, majorant l’intensité ressentie par celle-ci, d’autres manifestent des conceptions plus partagées et mieux articulées du soutien au parent âgé, qui permettent un équilibre dynamique de la configuration, malgré des contributions différenciées (et parfois objectivement inégales).

Traditionally characterised by a familialist approach to elderly care, France has evolvedtowards a mixed model, combining public and family care. Long-term care (LTC) is afragmented policy field associating different levels of governance and severalinstitutional, organisational and professional actors. It is based on a cash-for-carescheme, the personal autonomy allowance (allocation personnalisée d’autonomie – APA).Managed at the local level by the département (French local authority in charge of socialpolicies), the APA is paid, at home or in institutions, to any person aged 60 or over whoneeds assistance to accomplish daily living activities. At home, it finances a specific ‘careplan’ elaborated with a multidisciplinary team of professionals, and covers the needswhich are not taken into account by the health insurance system.Different in-kind services have been introduced since the 1970s (home-based careservices, residential care, day care centres) to organise the care of elderly people. Yet,considering demographic trends – by 2060 one third of French citizens will be over 60,including almost 5 million people over 85 compared with 1.4 million in 2015 – thesemeasures are insufficient. In a spending cuts context the sustainability of the publicfinances is a major challenge. The overall cost of public expenditure on policies aimed atolder people represents EUR 21 billion – i.e. 1.05% of GDP. Different solutions have beenadopted in recent decades to face the current difficulties and anticipate the comingdemographic situation.• Since the 2010s greater emphasis has been placed on the role of informal carers.Home-based care, which is considered as the least-cost option, cannot developwithout an informal carer acting as the coordinator of the care arrangement set-up. Policy measures to support informal carers have been introduced with theobjective of facilitating investment in informal carers: care leave, respite solutions,training and education.• The priority given to home-based care also implies a restructuring of the care worksector, based on a complex price system and offering precarious work conditions.• Better coordination of the different schemes and actors, belonging to different –health or social care – policy sectors, involving different territorial levels and amultiplicity of professionals, is another solution which has been put forward atboth national and local levels. A variety of schemes have been created such asnetworks, pathways, coordination or integration schemes, with the objective offacilitating continuity of care for elderly people and rationalising the elderly carefield.• Prevention is also a pillar of recent policy proposals.All these orientations inform the main objectives of the 2015 Act on adapting society toan ageing population, which proposes a move away from the medico-social perspectiveon old age (focused on elderly people and dating back to the 1990s) towards a morecomprehensive approach, integrating all the different dimensions of ageing. The objectiveis not only to meet the needs of frail elderly people but also to anticipate their difficultiesand incapacities by developing prevention and by adapting society to ageing. The 2015Act also emphasises the role of local actors and the development of measures at theterritorial level.However, the measures above are considered insufficient to meet the needs of elderlypeople and of their families. The EUR 700 million annual budget to finance the differentmeasures is without doubt highly insufficient. In the medium term, given the predicteddemographic changes, the question of the financial operation of the LTC sector thereforeremains open. The possibility of creating a fifth social security branch, which first arose inthe 1990s, has come up again. The government, aware of the difficulties of creating anew user contribution, is putting a greater accent on prevention, thus anticipating andreducing the costs of dependency.

National audience; This article describes the changing modalities of action designed to address social inequalities in health recommended in France, in the light of WHO and other international guidelines. Content analysis of policy recommendations identified eleven categories of actions and five types of changes as a function of target populations and the resources deployed. Recommendations have evolved from interventions designed to improve access to primary care to interventions on the social determinants of health, from reinforcement of individual capacities to global empowerment, from interventions on target populations to social inclusion of everyone, from promotion of health investments to increased financial regulation, from a favourable public health policies approach to a health approach in all policies.Multiple modalities of action have been proposed in France, but have remained poorly implemented. They have evolved over time, usually in response to international reference texts with a certain time gap. Internationally recommended interventions differ considerably from interventions focussing on individual capacities, while the new public health law in France reintroduced targeted behavioural interventions. These findings raise a number of other questions, especially concerning the way in which research into social inequalities in health influence public decisions and the institutional treatment of health in all policies.

International audience; This chapter (chapter 7) investigates the 'elderly care work at home' sector in France and analyses the objectives and impact of the various policy measures related to its development. It argues that though formalization of care work appears to be a major trend in elderly care policy, this process does not usher in the development of a homogeneous professionalized care sector. The chapter highlights the complexity of the formalization process of the care work sector in France and demonstrates that formalization, which facilitates the recognition of care as work, does not necessarily mean professionalization. It outlines the diversity of the resources available to support families in caring activities: heterogeneous professional care and semi-formal forms of care work with the possibility to employ a relative and informal family carer. The chapter also outlines the main axes of the policy measures in favour of the formalization of care work, and the key role of both elderly care policy and employment policy.

International audience; Over the past decades the English adult social care system has seen a set of policy reforms combined with funding cuts and austerity measures. This chapter utilizes secondary and primary data sources to investigate the current state of the English social care workforce with a particular focus on the increasing role of migrants; persistent low wages and job related stress. The findings highlight the continued role of migrants in the sector with a clear shift in their profile that is consistent with the direction of the UK immigration policies. From 2013migrants joining the care sector from Bulgaria and Romania alone have exceeded all other migrants entering the sector. The analysis indicates the persistent low wages in the sector, which are facilitated by a number of factors including austerity measures and the increasing role of the private sector and outsourcing of services. The nature of care work combined with difficult working conditions created various situations where individual workers felt aconsiderable level of stress. The latter is likely to impact not only on workers’ ability to complete their work to the highest quality but also has a potential negative impact on their own well-being and increases the likelihood of job quitting.
