Introduction VTE‐BLEED is a validated score for identification of patients at increased risk of major bleeding during extended anticoagulation for venous thromboembolism (VTE). It is unknown whether VTE‐BLEED high‐risk patients also have an increased risk for recurrent VTE, which would limit the potential usefulness of the score. Methods This was a post hoc analysis of the randomized, double‐blind, placebo‐controlled PADIS‐PE trial that randomized patients with a first unprovoked pulmonary embolism (PE) initially treated during 6 months to receive an additional 18‐month of warfarin vs. placebo. The primary outcome of this analysis was recurrent VTE during 2‐year follow‐up after anticoagulant discontinuation, that is, after the initial 6‐month treatment in the placebo arm and after 24 months of anticoagulation in the active treatment arm. This rate, adjusted for study treatment allocation, was compared between patients in the high‐ vs. low‐risk VTE‐BLEED group. Results In complete case analysis (n = 308; 82.4% of total population), 89 (28.9%) patients were classified as high risk; 44 VTE events occurred after anticoagulant discontinuation during 668 patient‐years. The cumulative incidence of recurrent VTE was 16.4% (95% confidence interval [CI], 10.0%‐26.1%; 14 events) and 14.6% (95% CI, 10.4%‐20.3%; 30 events) in the high‐risk and low‐risk VTE‐BLEED groups, respectively, for an adjusted hazard ratio of 1.16 (95% CI, 0.62‐2.19). Conclusion In this study, patients with unprovoked PE classified at high risk of major bleeding by VTE‐BLEED did not have a higher incidence of recurrent VTE after cessation of anticoagulant therapy, supporting the potential yield of the score for making management decisions on the optimal duration of anticoagulant therapy.

Introduction Les formes rapidement progressives de PID (RP-PID) sont frequentes au cours des dermatomyosites (DM), en particulier associees a un anticorps (AC) anti-MDA5 (PID-MDA5). Les facteurs predictifs de survenue d’une RP-PID sont mal connus. L’objectif principal etait de determiner les elements facteurs predictifs de survenue des formes RP au cours de PID-MDA5. Methodes Il s’agit d’une etude retrospective multicentrique nationale de patients adultes ayant une PID-MDA5. La RP-PID, a ete definie par une deterioration respiratoire ou Resultats 92 patients ont ete inclus (29 (31 %) hommes, âge median 50,1 ans (38,7–58,6), 57,6 % afro-caribeens, 79,3 % non fumeurs) ( Fig. 1 ). CVF mediane: 68 % (52,2–86,8), DLCO mediane: 47 % (40,5–60,5). Des lesions ulcerations etaient notees chez 42.4 % des patients et 69.6 % etaient amyopathiques. L’extension de la PID etait > 20 % chez 34,8 % des patients. Le pattern initial etait le suivant: PINS (33,7 %), PINS-PO (29,3 %), PID indeterminee (19,6 %) et PIA (7,6 %). Le delai de suivi median etait de 36 mois (7.7-63). Douze patients ont presente un pneumomediastin au cours du suivi. Quarante et un patients (44,5 %) avaient une PID-RP. Les donnees demographiques, le tabac, les signes cutanes et musculaires etaient comparables mais les PID-RP avaient une fievre plus frequente (65.9 vs 29.4 % p = 0,001), et plus de condensations (75.6 vs 49 % p = 0,008). Vingt-neuf patients sont decedes dont 25 dans le groupe PID-RP et 3 ont ete greffes, tous dans le groupe PID-RP. L’extension de la PID, la fievre et la ferritinemie au diagnostic etaient associees a la survenue d’une PID-RP en analyse de regression logistique. La survenue d’une PID-RP etait associee a un pronostic defavorable avec 28 % de survie estimee a 5 ans (p Fig. 1 ). Les patients ayant une forme « limitee » avaient une meilleure survie que ceux ayant une forme « etendue » (p = 0,012). Enfin, en analyse multivariee, l’âge, l’extension de la PID, la ferritinemie etaient des facteurs pronostiques associes a la mortalite. Conclusion Dans cette serie francaise de PID-MDA5, la fievre, la ferritinemie et l’extension de la PID au diagnostic sont associes a la survenue d’une PID-RP.

One in three patients were classified as high-risk, 10% of whom had cancer diagnosed. The RIETE score identifies a subgroup at high risk for cancer. SummaryBackground Most recent trials evaluating extensive screening strategies for occult cancer in patients with unprovoked venous thromboembolism have failed, because, among other reasons, of an overall low rate of occult cancer. The RIETE investigators recently proposed a score aimed at identifying a subgroup at higher risk. Methods We retrospectively computed the RIETE score for all patients included in the MVTEP study, which evaluated the accuracy of [¹⁸F]fluorodeoxyglucose-positron emission tomography in the screening of occult cancer in patients with unprovoked venous thromboembolism. Performance of the RIETE score was assessed according to the proportion of patients classified in each risk group, and the corresponding rates of cancer diagnosis. Results Among the 386 patients included in the analysis, 136 patients (35.3%) were classified as high risk by the RIETE score. Cancer was diagnosed in 16 (11.8%) of them, whereas it was diagnosed in nine (3.6%) of the 250 patients with a low RIETE cancer score: odds ratio of 3.6 (95% confidence interval [CI] 1.53–8.32). The area under the receiver operating characteristic curve was 0.63 (95% CI 0.51–0.74). Conclusion The RIETE score seems to be able to identify a subgroup at high risk for cancer (10%) in our specific dataset of patients with unprovoked venous thromboembolism.

Introduction Les symptomes neurologiques sont des motifs frequents de consultation. Un diagnostic doit etre pose pour permettre une prise en charge adaptee. Devant un syndrome cerebelleux chez un patient presentant une intoxication ethylique, un bilan exhaustif doit etre conduit pour ne pas meconnaitre une etiologie autre ou surajoutee. Observation Un homme de 55 ans est adresse en consultation de medecine interne en 2014 pour decouverte d’une thyroidite de Hashimoto – anticorps anti-TPO positifs, supplementation par levothyroxine 100 μg permettant d’obtenir une TSH normale. Il presente un syndrome cerebelleux evoluant depuis plusieurs annees. Ses antecedents comportent notamment une thrombopenie iatrogene a l’escitalopram, un choc septique sur une infection urinaire en 2012 avec insuffisance renale chronique sequellaire, une hypercholesterolemie, deux chirurgies pour hernie discale, une intoxication ethylo-tabagique sevree depuis 2012 ; le bilan biologique retrouve une gammapathie de signification indeterminee. Les troubles de la marche evoluent depuis 2012 avec une ataxie et une demarche talonnante ; on retrouve aussi des paresthesies en chaussettes. En 2013, l’electromyogramme ne retrouvait pas d’argument pour une polyneuropathie distale mais des signes de denervation aigue dans le territoire radiculaire S1 gauche et chronique en L5 droit. Malgre des infiltrations epidurales pour ces atteintes radiculaires surajoutees, la marche reste difficile. En 2014 lors de la consultation de medecine interne, il est decide de realiser un bilan du syndrome cerebelleux. Le reste du bilan auto-immun est negatif. Il n’y a pas de carence vitaminique. La ponction lombaire ne retrouve pas d’hypercellularite ; le profil neuro-immunitaire et l’electrophorese sont normaux. L’IRM cerebrale ne retrouve pas d’anomalie cerebelleuse mais un anevrisme asymptomatique de la bifurcation sylvienne droite qui sera par la suite embolise. Le TEP-scanner ne retrouve pas d’hypermetabolisme pathologique pouvant faire evoquer une lesion neoplasique. Ainsi, dans l’hypothese d’une atteinte cerebelleuse secondaire a la thyroidite de Hashimoto, une corticotherapie de 60 mg/jour est debutee permettant une nette regression des troubles de la marche en 15 jours. Apres decroissance progressive, la corticotherapie a pu etre arretee en 2016 sans rechute jusqu’a present. Discussion L’encephalopathie de Hashimoto est definie par trois criteres diagnostiques : anticorps anti-TPO et/ou antithyroglobuline positifs, symptomes neurologiques et reponse favorable aux immunomodulateurs [1] , [6] . Il faut savoir l’evoquer devant des symptomes neurologiques chez un patient presentant une dysthyroidie car le traitement par corticoides est efficace et y penser meme si le patient est en periode d’euthyroidie [2] . A noter que les anticorps diriges contre la partie NH2 terminale de l’α-enolase sont consideres comme des biomarqueurs specifiques de l’encephalopathie de Hashimoto [5] . Plusieurs mecanismes physiopathologiques sont proposes : possible role pathogene direct des anticorps anti-TPO ou simples marqueurs [2] , perturbation de la myelinogenese in vitro par les anticorps contre le systeme nerveux central [2] , possible deficit de liberation du glutamate [6] … Entre 16 % [4] et 28 % [6] des patients ont une ataxie cerebelleuse, 6 % de maniere isolee ou predominante [6] . Dans la litterature, l’atteinte cerebelleuse touche des patients de 38 a 84 ans [6] , plutot des femmes [6] . Cliniquement, il s’agit d’une ataxie axiale ; le nystagmus est atypique. Il n’y a pas ou peu d’atrophie cerebelleuse sur l’imagerie. Les patients avec les anticorps diriges contre la partie NH2 terminale de l’α-enolase n’ont pas de nystagmus et semblent mieux repondre aux traitements immunomodulateurs. L’atteinte mime souvent une degenerescence spinocerebelleuse [3] , [4] , [5] . Conclusion Nous rapportons ici un cas rare d’atteinte cerebelleuse en lien avec une thyroidite de Hashimoto. Nous avons regulierement des patients qui consultent pour « symptome neurologique » ou pour hypothyroidie. Il faut donc savoir evoquer en cas d’association [1] , [6] d’un signe neurologique, meme non systematise, et d’une thyroidite de Hashimoto une encephalopathie de Hashimoto au cours du bilan etiologique. En effet, cette pathologie repond tres bien a la corticotherapie [1] , [6] .

Introduction Environ 10 % des patients atteints de sarcoidose presentent une hypercalcemie. L’hypercalcemie secondaire aux metastases osseuses malignes et l’hyperparathyroidie primaire sont les deux principales etiologies retrouvees en cas d’hypercalcemie symptomatique. Le recours au TEP-scanner lors du bilan etiologique d’hypercalcemie est une pratique courante pour conforter une hypothese de malignite. Observation Une patiente de 50 ans etait hospitalisee pour la prise en charge et le bilan etiologique d’une hypercalcemie. Ses antecedents comportaient une neoplasie mammaire gauche traitee par chirurgie, radio-chimiotherapie puis hormonotherapie de septembre 2012 a aout 2016 ; une osteopenie secondaire a l’hormonotherapie et supplementee par vitamine D ; une sarcoidose pulmonaire de stade 1 diagnostiquee en 2012 sous surveillance simple et une hypertension arterielle. La symptomatologie de la patiente associait une asthenie progressive et une anorexie, accompagnees de nausees, d’une perte de 5 kg, d’une polyarthralgie d’horaire mixte et d’une dyspnee de stade II. Un bilan biologique mettait en evidence une hypercalcemie maligne symptomatique a 4 mmol/L et une insuffisance renale aigue avec une creatinine a 172 μmol/L. La patiente est alors traitee par une hyperhydratation, des biphosphonates. La supplementation par vitamine D est arretee. Dans l’hypothese d’une hypercalcemie secondaire a des metastases osseuses d’un cancer du sein, les explorations biologiques complementaires montraient une hypophosphoremie a 0,68 mmol/L, une PTH effondree a 5 ng/L, une vitamine D a 70 nmol/L, une 1,25OH-D3 fortement augmentee a 646 pmol/L, un ECA > 150 U/L, un CA 15-3 augmente a 33,4 kU/L, une PTH-rp inferieure a 0,8 pmol/L, une cholestase hepatique avec des phosphatases alcalines a 174 U/L et des GammaGT a 102 U/L. Le TEP scanner montrait une hyperfixation intense et diffuse de l’ensemble du squelette ; une hyperfixation de multiples formations ganglionnaires cervicales, axillaires, mediastinales et hilaires, lombo-aortiques, iliaques et inguinales ; une hyperfixation intense de multiples nodules pulmonaires bilateraux avec un aspect en lâcher de ballons ; une hyperfixation intense heterogene et diffuse en regard du parenchyme hepatique, du parenchyme splenique. Une biopsie osseuse realisee a la demande des oncologues montrait, apres etude anatomo-pathologique, une inflammation granulomateuse epithelioide et giganto-cellulaire sans necrose en faveur d’une localisation osseuse de sarcoidose. Il n’y avait pas d’argument un faveur d’une recidive neoplasique. Nous avons retenu le diagnostic d’hypercalcemie secondaire a un traitement par vitamine D chez une patiente presentant une sarcoidose. La patiente etait traitee par corticoides a la dose de 1 mg/kg, soit 60 mg par jour en association avec de l’hydroxychloroquine. Une normalisation de la calcemie a ete obtenue au bout d’un mois et de la fonction renal en deux mois. La biopsie renale montrait quelques rares foyers de calcifications intra- et peri-tubulaires secondaires d’un episode de tubulopathie sans lesions de nephrocalcinose ou d’infiltration interstitielle granulomateuse. Discussion Dans la sarcoidose, l’hypercalcemie est secondaire a l’augmentation de la synthese de vitamine 1,25OH-D3 secretee par les macrophages presents dans les granulomes des alveoles pulmonaires [1] . Cette augmentation de la concentration du taux de 1,25OH-D3 entraine une hypercalcemie et une hypercalciurie souvent responsables de tubulopathie renale et de lithiase. Une supplementation inappropriee en vitamine D en cas de granulomatose active exercerait un retrocontrole positif sur l’activite inflammatoire des granulomes et leur formation [2] . L’atteinte osseuse dans la sarcoidose est extremement rare et concerne 1 a 13 % des patients selon les etudes. Elle est sous-estimee car souvent asymptomatique. En TEP-scanner, l’atteinte osseuse diffuse de la sarcoidose se manifeste par une hyperfixation diffuse secondaire a l’accumulation de fluorodesoxyglucose dans les granulomes et pouvant etre consideree a tort comme des metastases osseuses malignes. C’est la biopsie osseuse et l’examen anatomo-pathologique qui permettent d’etayer le diagnostic par la presence caracteristique d’une infiltration granulomateuse epithelioide et giganto-cellulaire sans necrose dans les fragments osseux analyses [3] . Conclusion Les granulomatoses, l’hypercalcemie et les lithiases renales calciques sont des contre-indications a la supplementation vitamino-calcique. L’utilisation du TEP scanner semble etre un examen utile au diagnostic de sarcoidose osseuse mais une etiologie maligne doit etre eliminee.

Resume La drepanocytose est une pathologie repandue mais parfois mal apprehendee par les pneumologues qui y sont peu confrontes. Elle merite d’etre connue, compte tenu de sa principale complication respiratoire : le syndrome thoracique aigu (STA). Etat des connaissances La drepanocytose est une maladie autosomique recessive provoquant dans certaines conditions, une falciformation des hematies. Elle touche principalement les individus originaires des pays d’Afrique centrale, de l’ouest et des Antilles. Le STA associe une douleur thoracique ou de la fievre a des infiltrats radiologiques d’apparition recente, chez un individu porteur d’un syndrome drepanocytaire majeur. Ses principales etiologies sont l’infection, l’embolie graisseuse, et l’hypoventilation alveolaire. Il est cependant souvent difficile d’en determiner l’etiologie car il realise un cercle vicieux auto-entretenu. Les bases de la prise en charge, largement sous-evaluees, reposent sur le traitement etiologique, et les mesures visant a eviter l’aggravation liee principalement aux complications, en particulier microcirculatoires, de la maladie. Conclusions Le STA est une complication respiratoire grave de la drepanocytose a laquelle le pneumologue peut etre amene a etre confronte. Une prise en charge adaptee et rapide est necessaire. Les mesures therapeutiques sont simples mais sous-evaluees.

Resume Propos Malgre un traitement approprie, 10 a 100 % des patients atteints de thrombose veineuse profonde (TVP) des membres inferieurs presenteront une maladie veineuse post-thrombotique (MPT). L’objectif de ce travail etait d’evaluer l’incidence de la MPT au sein des patients de la cohorte EDITH et d’estimer l’association entre les caracteristiques initiales de la maladie thrombotique et le risque ulterieur de survenue d’une MPT. Methodes Cent quatre-vingts patients de la cohorte EDITH âges de moins de 70 ans inclus a l’occasion d’un premier episode de TVP des membres inferieurs sans signes cliniques d’insuffisance veineuse chronique au moment de l’inclusion ont ete convoques quatre ans apres leur episode initial thromboembolique. La presence de MPT a ete evaluee a l’aide du score de Villalta. Resultats Quatre-vingt-quinze patients (45 hommes, âge moyen 50,7 ± 16,9 ans) ont repondu a la convocation. Parmi eux, 28,4 % (IC 95 % 19,3–37,5) ont developpe une MPT, aucun n’a developpe de MPT severe. Les signes cutanes les plus frequemment retrouves chez les patients porteurs d’une MPT etaient les varices (59 %), une corona phlebectatica (48 %), un œdeme de jambe (30 %) et une dermite ocre (26 %). Aucun facteur de risque teste pour la MPT (âge, sexe, IMC, localisation, recidive et aspect symptomatique de thrombose initiale, thrombophilie, etiologie, antecedents familiaux de TVP) n’a pu etre mis en evidence. Conclusion La MPT est une pathologie frequente mais le manque d’uniformite des criteres diagnostiques entre les etudes ne permet pas d’etablir clairement ses facteurs de risque.

Resume Les traitements antiretroviraux permettent actuellement une survie importante des patients infectes par le virus VIH, en faisant reculer les complications infectieuses. Trois classes therapeutiques antiretrovirales contre le VIH sont disponibles. Parallelement, chez cette population jeune, sont apparues des complications cardiovasculaires inhabituelles, en rapport avec des anomalies biologiques iatrogenes (dyslipidemies et insulino-resistance), meme si cette population est exposee aux facteurs de risque classiques d’atherosclerose. Les traitements antiretroviraux semblent a l’origine de ces complications en induisant un diabete de type 2 dans 4 a 20 % des cas, une insulino-resistance chez 15 a 60 % des patients, une hypertriglyceridemie dans 15 a 74 % des cas selon les etudes et une hypercholesterolemie chez 20 a 60 % des sujets, surtout en cas de lipodystrophie associee. La mise en route d’un traitement antiretroviral motive un bilan prealable lipidique, comprenant un dosage du cholesterol total, HDL et triglycerides, et glycemique apres 12 heures de jeune. La surveillance evolutive de ces parametres biologiques parait souhaitable tous les 3 a 6 mois en cas d’anomalie, meme si d’autres causes surajoutees de dyslipidemies secondaires doivent etre evoquees. En cas d’elevation isolee des taux de LDL-cholesterol persistante, l’introduction d’un traitement par statine parait licite. En prevention secondaire, quel que soit le contexte, les recommandations rejoignent actuellement le consensus de la population generale. Ces patients necessitent donc une surveillance et une prise en charge specifiques en dehors du probleme de leur immunodeficience. (J Mal Vasc 2004 ;29 : 192-199).

Introduction Le bilan de chute doit comporter la recherche de facteurs precipitants tels que l’hypotension orthostatique et les troubles metaboliques (dysnatremie, hypoglycemie…). Nous rapportons le cas d’une patiente âgee admise pour chute dans un contexte d’hypoglycemie severe. L’etiologie retrouvee est une hypoglycemie auto-immune, associee a des anticorps anti-recepteurs de l’insuline, resolutive sous corticotherapie orale. Observation Mme R., 83 ans, sans antecedent notable ni traitement medicamenteux, est adressee aux urgences pour chute. L’examen somatique met en evidence des troubles de conscience sans signe de localisation neurologique et une hypothermie a 33,1 °C. Devant une glycemie capillaire indosable, trois ampoules de G30 % sont administrees par voie intraveineuse, permettant une regression des symptomes. Les episodes d’hypoglycemie, confirmes sur sang veineux, se repetent plusieurs fois par jour a jeun pendant l’hospitalisation. Le bilan etiologique de ces hypoglycemies ne met pas en evidence d’insuffisance hepatique, cardiaque ou renale, ni de cause toxique (alcool ou medicament) ou endocrinienne. Le TEP scanner ecarte une cause tumorale. Pour une glycemie a 3,3 mmol/L, l’insulinemie est indosable, le peptide C abaisse (0,340 μg/L) et l’HbA 1c a 5,6 %. En l’absence de cause evidente et devant la persistance des hypoglycemies, nous evoquons une origine immunologique. Les anticorps anti-insuline sont negatifs mais les anti-recepteurs de l’insuline reviennent tres positifs, signant le diagnostic d’insulinoresistance de type B. La suite du bilan elimine un pic monoclonal et retrouve des AAN positifs a 1/160 e sans specificite. Il n’y a pas d’argument pour une pathologie auto-immune associee. Une corticotherapie orale a 1 mg/kg/jour associee a une alimentation fractionnee et adaptee permet la disparition progressive des hypoglycemies. A 6 semaines, nous debutons une decroissance progressive des corticoides. A 3 mois, la patiente reste asymptomatique. Discussion Devant une hypoglycemie authentifiee par une glycemie veineuse ≤ 2,8 mmol/L ou 0,5 g/L, il faut realiser un bilan etiologique systematique a la recherche : En premiere intention d’une cause iatrogene (insuline, sulfamide, glinides, quinine, salicyles…), fonctionnelle (diagnostic d’elimination, HGPO sur 5 h), d’une intoxication ethylique (OH, CDT, plaquettes, γGT, VGM), d’une insuffisance hepatique (TP, albumine, signe clinique de cirrhose) ou d’une insuffisance d’organe terminale (creatinine, fraction d’ejection du ventricule gauche en echocardiographie). En deuxieme intention d’une cause tumorale : tumeur volumineuse (TEP scanner), insulinome (insulinemie elevee, echoendoscopie…), tumeur secretant de l’IGF2 (insuline basse, big-IGF2) ; puis d’une cause hormonale : insuffisance surrenalienne (cortisolemie a 8 h), hypothyroidie (TSH), deficit en hormone de croissance (hypoglycemie insulinique…). En troisieme intention, d’une cause auto-immune. L’hypoglycemie auto-immune est une etiologie rare d’hypoglycemie. Il en existe deux types : –l’« insulin auto-immune syndrome » avec des anticorps anti-insuline. Ces anticorps captent l’insuline (d’ou les hyperglycemies) puis la liberent sans synchronisation avec la glycemie (d’ou les hypoglycemies). Troisieme cause d’hypoglycemie au Japon, parfois associee a la prise d’un medicament ; –l’insulinoresistance de type B avec des anticorps anti-recepteur de l’insuline. L’hypothese physiopathologique principale est l’effet agoniste des anticorps a un titre faible lors de leur fixation sur le recepteur de l’insuline (d’ou les hypoglycemies) et antagoniste a un titre eleve (d’ou les hyperglycemies). La majorite des patients presentent des hyperglycemies et un Acanthosis nigricans signant l’insulinoresistance. Notre patiente presente une forme a priori hypoglycemiante pure, rare. Ces autoanticorps peuvent s’inscrire dans le cadre d’une maladie auto-immune systemique (type lupus erythemateux dissemine) ou en association a une gammapathie monoclonale. Le traitement doit etre adapte selon ce contexte. Parfois, la remission est spontanee. Conclusion Ce cas illustre comment un bilan de chute peut mener au diagnostic d’hypoglycemie organique ; ici, d’une hypoglycemie auto-immune a anticorps anti-recepteur de l’insuline, rarissime. La physiopathologie de cette affection reste encore mal connue. Le pronostic semble defavorable dans la litterature avec des difficultes therapeutiques, toutefois, notre patiente est en remission sous corticotherapie.

We report the case of a 71-year-old man with acute colonic pseudo-obstruction that complicates a pneumococcal meningo-encephalitis. After 48 h of conservative management with nothing by mouth, nasogastric suction, fluid and electrolyte correction, withdrawal of any anticholinergic agents, a pharmacological approach with 2 mg of neostigmine was successful in intensive care unit. This treatment was effective in over 80% of patients of recent reports. Neostigmine might be considered as first-line therapy in patients who do not have major contraindications to its use, because of less frequent iatrogenic risk than colonoscopic decompression or surgery.

Introduction Capnocytophaga canimorsus est un bacille gram negatif commensal de la gueule du chien et du chat, transmis par contact, a culture lente, fragile et difficile a identifier. Il se place dorenavant juste derriere Pasteurella multocida pour les infections apres morsure de chien et Streptococcus pneumoniae pour les infections chez les splenectomises. Un cas avec atteinte rhumatologique et erytheme annulaire centrifuge, classiquement rares, nous ont incites a en revoir l’incidence, le contexte et l’expression clinique. Materiel et methodes Nous avons realise une extraction des patients chez lesquels une culture ou une PCR universelle a C. canimorsus se sont revelees positives a partir des bases de donnees du departement d’information medicale et du laboratoire de microbiologie du CHRU de Brest entre 2000 et 2016. Resultats Sept patients (5 hommes et 2 femmes) ont ete recenses. Ils avaient de 36 a 88 ans et trois etaient immunodeprimes (diabete, splenectomie et polyarthrite rhumatoide diabetique sous biotherapie) mais un seul avait eu une consommation d’alcool excessive (alors sevree). Trois lechages de plaie (chiens) et une griffure (chat) etaient retrouves, ainsi que la presence de chiens a domicile dans un cas. L’expression clinique predominante etait orthopedique dans deux cas (spondylodiscite, oligoarthrite), cutanee dans deux cas (plaie d’une cheville, escarre inflammatoire), meningee dans deux cas et valvulaire dans un cas. Sur le plan cutane, il existait un purpura associe a l’endocardite et un erytheme annulaire centrifuge associe a l’oligoarthrite comme possibles expressions immunologiques secondaires. Le diagnostic etait porte grâce a la culture (trois hemocultures, deux biopsies cutanees et un liquide cephalorachidien), ou par PCR universelle (spondylodiscite). L’antibiotherapie, reposant sur des C3G en grande majorite, l’association levofloxacine–clindamycine ou rifampicine–doxycycline de facon anecdotique, a ete associee dans un cas a une chirurgie (parage de la plaie de cheville). Discussion Sur le plan des manifestations cutanees, nous retrouvons dans notre serie une plaie infectee, deux portes d’entrees cutanees a type de plaies qui furent lechees, une escarre inflammatoire lechee, un erytheme annulaire centrifuge et un purpura. Dans la litterature, les atteintes cutanees sont aussi diverses : purpura, petechies, gangrene, exantheme maculopapuleux ou maculeux, plaies mais aussi un exantheme urticarien, un syndrome de Stevens-Johnson associe a un choc septique… Conclusion Il faut penser a une infection a C. canimorsus devant un purpura dans les suites d’une morsure de chien mais aussi devant des manifestations cutanees moins frequentes chez des patients presentant des facteurs de risques.

Resume Introduction La pemphigoide est une maladie bulleuse auto-immune, frequente chez l’adulte âge, de localisation classiquement symetrique et generalisee. Nous rapportons une observation ou les lesions bulleuses epargnaient le membre inferieur gauche siege d’un lymphœdeme acquis secondaire a un curage ganglionnaire. Observation Une femme de 74 ans etait hospitalisee pour une eruption bulleuse generalisee. Le membre inferieur gauche n’etait pas atteint et il etait egalement indemne de prurit. Ce membre etait le siege d’un lymphoedeme acquis secondaire a un curage ganglionnaire. L’examen histologique standard confirmait le diagnostic de pemphigoide en montrant une bulle sous-epidermique alors que l’immunofluorescence directe mettait en evidence une fixation lineaire d’IgG et de C3 le long de la membrane basale. Le western blot trouvait des anticorps anti-BPAg2. L’examen histologique d’une biopsie realisee du cote du lymphoedeme, indemne de bulle, montrait un derme depourvu de tout infiltrat inflammatoire, en revanche il existait une fixation lineaire d’IgG et de C3. Discussion D’autres cas de pemphigoide localisee ont ete rapportes, survenant sur des zones traitees par ultraviolets ou radiotherapie. Il existe egalement des observations d’atteinte elective sur un lymphœdeme : l’hypothese avancee etant celle d’un ralentissement du flux lymphatique favorisant la rencontre antigene-anticorps. Notre observation est originale et nous formulons 2 hypotheses devant le caractere protecteur de ce lymphoedeme. Il pourrait exister une alteration de l’immunite cellulaire locale, qui diminuerait l’activation lymphocytaire T. Il pourrait egalement exister une alteration des voies nerveuses comme en temoigne l’absence de prurit, qui rendrait impossible ou insuffisante la reaction inflammatoire.

An important step in the pathogenesis of Neisseria meningitidis is the crossing of two cellular barriers, one in the nasopharynx and one in the brain. To approach the mechanisms by which this bacterium can achieve these goals, we studied the interactions between N. meningitidis and a monolayer of polarized tight junction-forming T84 cells grown on filter units. A capsulated, piliated, Opa-, and Opc- N. meningitidis strain is shown to be capable of adhering to and crossing this monolayer several orders of magnitude more efficiently than an isogenic nonpiliated derivative. This bacterial interaction does not affect the barrier function of tight junctions, as assessed by (i) the absence of modification of the transepithelial resistance, (ii) the lack of increase of [3H]inulin penetration across the monolayer, and (iii) the absence of delocalization of ZO-1, a tight junction protein. Electron microscopy studies and confocal examinations demonstrated that N. meningitidis (i) induces cytoskeletal rearrangements with actin polymerization beneath adherent bacteria, (ii) is intimately attached to the apical membrane of the cells, and (iii) can be internalized inside cells. Immunofluorescent staining with antipilus antibodies showed evidence that meningococcal piliation was dramatically reduced at later time points of bacterial cell interaction compared to the early phase of this interaction. In addition, adhesive bacteria recovered from an infected monolayer are piliated, capsulated, Opa-, and Opc-, a phenotype similar to that of the parental strain. Taken together, these data demonstrate that following pilus-mediated adhesion, N. meningitidis is involved in an intimate attachment which requires a bacterial component different from Opa and Opc and that meningococci cross a monolayer of tight-junction-forming epithelial cells by using a transcellular pathway rather than a paracellular route.

From a research sample of 138 corpses, divided into four subgroups of ambient storage temperature (0-5 degrees C, 6-10 degrees C, 11-15 degrees C and 16-23 degrees C) four linear regression formulae of actual versus estimated post-mortem interval were obtained ('interval' formulae) using a single outer ear temperature measurement on both sides. This method showed the best correlation coefficient among five other methods previously proposed for time of death determination (rectal temperature, vitreous K+, CSF K+, blood log NA+/K+ and log Cl-), however its results were less accurate than those obtained with a multivariate equation combining several of the above mentioned methods. Eventually an equation expressing time of death (TOD) as a function of outer ear temperature (OE T degrees) and ambient temperature was also established from the whole research sample ('global' formulae). On a different sample of 141 corpses the regression formulae ('interval' and 'global') for the outer ear temperature were compared to three methods based on a single rectal temperature measurement ('rule of thumb' 1 and 2, Henssge nomogram) and therefore useful at the scene; the results of all methods were compared within the four subgroups of ambient temperature as well as in three subgroups of different post-mortem interval lengths (7 h,10 h,15 h). In all cases the outer ear temperature formulae provided better results than the rectal temperature methods (especially Henssge nomogram and rule of thumb 1). Moreover they did not show any post-mortem plateau which was present in almost 30% of cases when rectal temperature was measured in corpses kept at ambient temperature above 15 degrees C. Our results show that outer ear temperature measurement is the method which provides the best simplicity/quality ratio and should therefore be proposed for use at the scene when conditions are similar to those of our experiment (within buildings). A software equipped thermometer is required in order to use in each case the appropriate formula and confidence interval.

Introduction L’oligoarthrite est une atteinte inflammatoire de 2 ou 3 articulations. Elle a de nombreuses etiologies qu’il faut s’acharner, a l’aide de l’anamnese, de la clinique, des examens paracliniques, a rechercher, gage d’un traitement etiologique adapte. Nous rapportons ici une etiologie exceptionnelle d’oligoarthrite associee a une eruption cutanee. Observation Monsieur T., educateur canin de 50 ans, consulte pour fievre depuis 72 heures associee a une douleur du coude gauche depuis 24 heures. Ses comorbidites sont une silicose, un asthme, des lombalgies chroniques, une gonarthrose bilaterale traitees par formoterol, montelukast, ebastine, budesonide, sulfate de terbutaline. L’examen retrouve une fievre a 40 °C, une arthrite du coude gauche et du genou droit mais ni souffle cardiaque, ni lesion cutanee. Le lendemain, il presentera trois lesions d’erytheme annulaire centrifuge. Devant cette oligoarthrite aigue febrile associee a un syndrome inflammatoire biologique avec une hyperleucocytose et l’absence de signes radiographiques notables, nous realisons un bilan etiologique. L’analyse du liquide articulaire du coude retrouve un liquide inflammatoire avec une predominance de PNN, sans microcristaux ni germe a l’examen direct et a la culture avec une PCR ARN 16S negative. Les serologies : Campylobacter, Yersinia pseutobuberculosis et Y. enterocolitica, Chlamydia trachomatis et C. pneumoniae, Mycoplasma pneumoniae, Francisella tularensis, Treponema pallidum, Borrelia burgdorferi, VIH, parvovirus B19, hepatitis B et C virus reviennent negatives. De meme que les marqueurs de rhumatisme inflammatoire (AAN positifs au 1/320e mouchetes sans specificite, anti-CCP, FR et ANCA negatifs). L’origine iatrogene est ecartee. C’est apres 7 jours de culture qu’une des cinq hemocultures revient positive a Capnocytophaga canimorsus. Cette infection est traitee par ceftriaxone (2 g IV/jour, 7 jours) puis levofloxacine (500 mg × 2/jour per os 4 semaines) avec efficacite. Le patient ne presente pas de facteur majeur d’immunodepression. Seul un diabete a ete decouvert, mais il a une exposition professionnelle patente avec notion de lechage de plaies du visage par ses chiens. Discussion C. canimorsus est un Bacille gram negatif [1] a culture lente [1] . Difficile a identifier [1] , on peut dorenavant s’aider de la PCR universelle [1] ou de la spectrometrie de masse. Contrairement aux autres Capnocytophaga d’origine bucco-dentaire et frequemment opportunistes, C. canimorsus est un germe commensal de la gueule du chien mais aussi du chat et du lapin qui se transmet par morsure, griffure et autres contacts [1] . Son traitement repose sur une antibiotherapie notamment par penicilline. T. Butler a recense 484 cas dans sa revue de la litterature en 2015 [1] , il s’agit en general d’hommes [1] de plus de 50 ans [1] en contact avec des chiens ou des chats [1] , presentant comme autres facteurs de risque une splenectomie [1] ou une asplenie fonctionnelle [1] et/ou une consommation excessive d’alcool [1] . Cependant, la plupart des cas decrits sont survenus chez des patients apparemment en bonne sante [1] . L’infection par C. canimorsus associe des symptomes varies, domines toutefois par le sepsis et la meningite [1] , et se place dorenavant juste derriere Pasteurella multocida pour les infections apres morsure de chien et Streptococcus pneumoniae pour le sepsis chez les splenectomises [1] . Par contre, l’atteinte rhumatologique reste exceptionnelle : une spondylodiscite chez un homme de 54 ans [2] , une bi-arthrite des genoux chez un homme de 59 ans a distance d’une arthroplastie [5] , une mono-arthrite du genou avec exantheme chez un homme de 59 ans apres arthroplastie [4] et une mono-arthrite de la premiere metatarso-phalangienne chez un homme de 67 ans [3] . Chez tous [2] , [3] , [4] , [5] , un contact avec un chien a ete retrouve. Cette observation a ete l’occasion de recenser sur les 15 dernieres annees les infections a C. canimorsus diagnostiquees dans notre CHRU soit 5 cas : une endocardite avec purpura, deux plaies infectees, une spondylodiscite et l’oligoarthrite avec erytheme annulaire centrifuge. Chez quatre, un contact avec un chien a ete retrouve et chez un, un contact avec un chat. Trois patients presentaient un diabete et un etait splenectomise. Conclusion Nous rapportons ici une cause inhabituelle d’oligoarthrite avec eruption cutanee sous la forme d’erythemes annulaires centrifuges. Il s’agit de l’expression clinique rare d’une bacteriemie a C. canimorsus, chez un patient en bonne sante apparente mais ayant une exposition patente. Cette observation rappelle la necessite d’un bilan exhaustif des oligoarthrites incluant la PCR16S sur le liquide de ponction, la pratique d’hemocultures systematiques sur milieu performant cultivees au moins 7 jours.

International audience; OBJECTIVE: The authors had for aim to describe the effectiveness and the safety of a saquinavir/ritonavir (SQV/r) regimen, 1000/100mg twice daily, in HIV-infected pregnant patients. PATIENTS AND METHOD: We made a prospective and observational study of HIV positive female patients beginning or going on SQV/r antiretroviral treatment (ART) during pregnancy. RESULTS: Sixty-two patients were enrolled from July 2007 to June 2009 in 10 infectious diseases units in France. Thirty-six women (group 1) were ART naive on inclusion, 20 (group 2) had been previously treated and then switched to SQV/r, six (group 3) were treated with SQV/r before pregnancy. 58 patients delivered while on SQV/r regimen after a median pregnancy duration of 39 WA. Eighty percent had a viral load below 50 copies/mL and 93% below 400 copies/mL: respectively 77% and 93.5% in group 1, 83% and 89% in group 2, 83% and 100% in group 3. The median SQV minimum concentrations (C(min)) measured at the third trimester and at delivery were adequate, respectively 0.91 mg/L and 0.86 mg/L. Most women (52%) had a vaginal delivery; 12 (21%) had an elective caesarean section, for obstetrics factors in eight cases. None of the newborns were HIV-infected at 6 months of age (n = 59, one death at day 3). Only one severe adverse event occurred due to saquinavir (maternal grade 3 hepatotoxicity). CONCLUSION: SQV/r 1000/100mg twice daily seems to be effective and safe in HIV-infected pregnant women with adequate saquinavir C(min).

National audience; Purpose. - Despite appropriate therapy 10 to 100% of patients with deep vein thrombosis (DVT) of the lower limbs will develop post-thrombotic syndrome (PTS). The aim of this study was to evaluate the incidence of PTS in the EDITH cohort and to estimate the association between initial patients' characteristics and the risk of development of PTS. Methods. - One hundred and eighty patients included in the EDITH study for a first event of DVT of the lower limbs without clinical signs of venous insufficiency were recalled 4 years after their initial thrombotic event. PTS was diagnosed according to the Villalta score. Results. - Ninety-five patients (45 men, mean age 50.7±16.9 years) were evaluated for PTS. Among them, 28.4% (95% CI 19.3-37.5) developed PTS but none had severe PTS. The most frequent clinical signs of PTS were varicose veins (59%), corona phlebectatica (48%), swelling leg (30%) and pigmented dermatitis (26%). No single risk factor was associated with PTS development (age, sex, BMI thrombophilia, etiology, localization, recurrence, symptomatic DVT and familial history of DVT). Conclusion. - PTS is a frequent disease. However, lack of uniformity of diagnosis criteria in the different studies does not make possible the estimation of PTS risk factors.

International audience; BACKGROUND: Despite an initial impressive impact, a critical appraisal of the link between pregnancy loss and inherited thrombophilias is currently growing. Furthermore, little is known about the paternal thrombophilic phenotype and pregnancy loss. OBJECTIVE: We sought an association between unexplained pregnancy loss and parental factor V Leiden (FVL) and Prothrombin G20210A (PTG) mutations. METHODS: Design - Incident case-control study. Setting- University Hospital of Brest (France). Patients - Women and their partners from the West Brittany area, consecutively referred for unexplained pregnancy losses (two or more consecutive losses at or before 21 weeks of gestation, or at least one later loss). Controls - Women and their partners with no history of pregnancy loss and at least one normal pregnancy, from the same geographic area, recruited using electoral lists. Statistical analysis - Comparison of FVL and PTG allele frequency between cases and controls using the chi-square test. Separate analyses were performed according to the type of pregnancy loss (early recurrent or later loss). RESULTS: 311 women (mean age: 32.8) and 284 of their partners were enrolled as cases while 599 women (mean age: 34.3) and 297 of their partners were recruited as controls. The prevalence of female, male or couple thrombophilic mutations was not statistically different between cases and controls whatever the definition of pregnancy loss retained. CONCLUSIONS: Presently, there is no clinical indication to routinely test for FVL and likely PTG mutations in women with early recurrent pregnancy loss. Moreover, our results did not reveal that paternal thrombophilic polymorphism should be further explored.

We report the development to eight years of paralysis diaphragmatic bilateral attributed to a Parsonage-Turner syndrome: the lack of recovery is proved by respiratory functional follow-up. International audience; BACKGROUND: Previous studies have reported conflicting results regarding recurrent pregnancy loss and skewed X-chromosome inactivation. Hence, we sought an association by carrying out a specifically designed incident paired case-control study with required statistical power. METHODS: Design incident 1:3 matched case-control study, from 2003 to 2007. Setting: University Hospital of Brest. Patients: Women, from the Brittany area, consecutively referred for at least two unexplained consecutive spontaneous abortions. Controls: Women from the same geographic area, with no history of pregnancy loss and at least one normal pregnancy, recruited using electoral lists and then paired with cases, with respect to age, to within 1 year. Intervention: Assessment of skewed X-chromosome inactivation. Statistical analysis: Comparison of the ratio of >90% skewed X-chromosome inactivation by conditional logistic regression. RESULTS: Five hundred and forty-three controls (mean age: 34.3 years) were paired within 1 year to 200 cases. The cases (mean age: 33.6 years) had experienced between 2 and 14 consecutive losses (median 3). The rate of >90% skewed X-chromosome inactivation was not statistically different (P = 0.33, odds ratio: 0.58, 95% confidence interval: 0.19-1.77) between cases and paired controls, 2.27% versus 4.1%, respectively. CONCLUSIONS: We conclude that there is no association between skewed X-chromosome inactivation and recurrent pregnancy loss, defined as two or more unexplained consecutive spontaneous abortions.

Bullous pemphigoid is an autoimmune disease, common in the elderly and generally of symmetrical and systemic localization. We report a case with sparing of the lower limb and acquired lymphedema secondary to lymph node surgery.A 74-year-old woman was hospitalized for a bullous eruption. The left lower limb was completely spared and was unaffected by pruritus. Acquired lymphedema was seen in this limb secondary to lymph node surgery. Standard histopathology tests confirmed the diagnosis of bullous pemphigoid with subepidermal blistering, while a direct immunofluorescence antibody test showed linear binding of IgG and C3 throughout the basement membrane. Western blotting revealed anti-BPAg2 antibodies. Skin biopsy on the lymphedema spared by the disease revealed no inflammatory infiltrate in the dermis. However, linear binding of anti-IgG and anti-C3 autoantibodies was observed.Other cases of localized bullous pemphigoid appearing on body areas treated by UV or radiotherapy have been reported. Cases of bullous pemphigoid with predilection for areas of lymphedema have also been previously described: the hypothesis has been advanced of reduced lymphatic flow, with increased antigen-antibody contact enabling better binding. Our case is original and, given the protective nature of this lymphedema, suggests two hypotheses. There could be deterioration of local cellular immunity, with decreased activation of T lymphocytes. They could also be impairment of nervous conduction, as suggested by the absence of pruritus, with partial or total inhibition of neurogenic inflammation.

Current antiretroviral therapy protocols enable long-term survival of HIV-infected patients, decreasing the risk of infectious complications. Three classes of anti-HIV treatments are available. With longer survival, unusual cardiovascular complications related to iatrogenic biological anomalies (dyslipidemia and impaired glucose tolerance) have appeared among this young population which is exposed to usual risk factors of atherosclerosis. Antiretroviral therapies are suspected to cause these complications, inducing maturity-onset diabetes in 4 to 20% of patients, impaired glucose tolerance in 15 to 60%, hypertriglyceridemia in 15 to 74% depending on the survey, and hypercholesterolemia in 20 to 60%, especially in case of associated lipodystrophia. A lipid battery including total cholesterol, HDL, and triglycerides, and 12-h fasting blood glucose should be obtained before initiating antiretroviral therapy. Any anomalous finding should be followed carefully with regular surveillance every 3 to 6 months and search for other causes of secondary dyslipidemia. In the event of casual and persisting elevation of LDL-cholesterol levels, a statin treatment can be introduced. For secondary prevention, irrespective of the context, recommendations currently merge with the consensus applying to the general population. These patients require careful surveillance of cardiovascular risk factors and a specific care in addition to treatment of their immunodeficiency.

We report the case of a 71-year-old man with acute colonic pseudo-obstruction that complicates a pneumococcal meningo-encephalitis. After 48 h of conservative management with nothing by mouth, nasogastric suction, fluid and electrolyte correction, withdrawal of any anticholinergic agents, a pharmacological approach with 2 mg of neostigmine was successful in intensive care unit. This treatment was effective in over 80% of patients of recent reports. Neostigmine might be considered as first-line therapy in patients who do not have major contraindications to its use, because of less frequent iatrogenic risk than colonoscopic decompression or surgery.

The determination of age at death is an important part of physical and forensic anthropology. Because of resistance to decomposition and the simplicity/accuracy ratio, the direct observation of the os pubis by Suchey-Brooks phase analysis is the preferred reference system for determination of age at death. We propose an age-prediction system using a pubic symphysis numerical database obtained from CT x-ray through quantification of age-linked parameters. Our system increases the accuracy of age estimation and at the same time preserves the integrity of the archeological material.

Activated protein C (APC) resistance is related to a single point mutation in the factor V gene (FV:Q506) and appears to be the most common inherited risk factor for venous thromboembolism. A reliable screening test is therefore useful. We aimed to evaluate a new APC resistance test, on the basis of the procoagulant activity present in one snake venom of a crotalidae family: STA Staclot APC-R. We studied 36 consecutive patients with an acute deep venous thrombosis (DVT) confirmed by compression ultrasonography and carrying the FV:Q506 allele, assessed by DNA analysis, 103 of their family members and 35 consecutive patients with a proven DVT but who did not carry the FV:Q506 allele. Blood samples were collected within 24 h of admission for the DVT cases and on the day of medical registration for the family members. Tests were performed blind. The STA Staclot APC-R test, using a cut-off value of 0.80, had an overall sensitivity of 100% (95% CI, 95–100) and a specificity of 98.8% (95% CI, 92.0–99.6). An acute thrombosis process did not influence the performance of the test. Background Data on the outcome of patients treated for non-infectious mixed cryoglobulinemia vasculitis (CryoVas) in the era of hepatitis C virus screening are lacking. Although in most patients, induction therapy leads to complete or partial remission, relapses in patients with non-infectious mixed CryoVas remain a major problem. Objectives To identify potential predictors of early relapses occurring with the first 12 months in patients with non-infectious mixed CryoVas. Methods Eighty-one French centers of Internal Medicine, Nephrology, Rheumatology, Hematology, Dermatology and Neurology from University and general hospitals have included 242 patients with non-infectious mixed CryoVas diagnosed between January, 1995 and July, 2010. Patients exhibiting complete or partial remission after induction therapy and followed-up for at least 12 months were analyzed for predictors of early relapses, i.e. occurring within the first 12 months. All variables with a p value Results 106 women and 39 men (sex ratio F/M 2.7), mean age 61.2±11.8 years, were included. Forty out of the 145 patients (28%) experienced an early relapse within the first 12 months of follow-up. Relapses occurred after a median time of 9.5 months (3-12). These relapses involved the skin in 75%, the joints and peripheral nerve in 28%, the kidneys in 25% and the gastrointestinal tract in 5%. Baseline factors associated with an early relapse in univariate analysis were purpura (P=0.006), cutaneous necrosis (P=0.006) and articular involvement (P=0.025). No biological feature was associated with an early relapse. In multivariate analysis, these factors remained significantly associated with an early relapse: purpura [OR 3.35 (1.02-10.97), p=0.046], cutaneous necrosis [OR 4.46 (1.58-12.57), p=0.005] and articular involvement [OR 2.20 (1.00-4.78), p=0.048]. The factors associated with an early relapse during the follow-up, i.e., the treatments used and the response to therapy, in univariate analysis were the use of corticosteroids alone (P=0.026) and the absence of achievement of complete clinical or immunological response (P=0.034 and 0.0005, respectively). In contrast, the initial dosage of corticosteroids and the use of plasmapheresis were not associated with lower rate of early relapse. In multivariate analysis, the only factor associated with an early relapse was the absence of complete immunological response [OR 0.07 (0.01-0.51), P=0.009]. Conclusions In patients with non-infectious mixed cryoglobulinemia vasculitis, main predictors of early relapses after initial remission are purpura, cutaneous necrosis and articular involvement. The use of combination therapy associating corticosteroids and immunosuppressive agents and the achievement of a complete immunological response were associated with lower rates of early relapse. We report on a retrospective study evaluating infectious morbidity associated with totally implantable venous access devices (TIVAD) (Port-A-Cath) in HIV-infected patients. This study of 84 consecutive HIV-infected patients requiring 89 TIVAD between January 1990 and October 1993 was performed in the Department of Infectious Diseases Hopital de l'Institut Pasteur, Paris, France. The total number of catheter days was 11,595. Eighteen of 89 patients with TIVAD (20%) were infected, causing 25 infectious events (25/89: 28%) among 17 different patients (17/84: 20%). The infection rate was 0.22 per 100 catheter days. Mean onset of infection was 82 days. Twenty microorganisms were isolated: Staphylococcus aureus in eight cases (40%), coagulase-negative Staphylococcus in six cases (30%), Streptococcus D faecalis in one case; Gram-negative bacilli were found in five cases (25%). All patients received an intravenous antibiotherapy combined with a local lock treatment in eight cases. Nine TIVAD removals were performed. One death was related to the TIVAD infection. No additional predisposing factor for infection was identified other than the implied condition of the HIV infection. The population and material in this study were homogeneous. The TIVAD infection rate was comparable to other published reports. Prospective evaluation comparing tunneled catheter and TIVAD in HIV-infected patients is needed.

Resume La paralysie diaphragmatique est une pathologie rare, dont les etiologies et les formes evolutives sont nombreuses. Nous rapportons l’evolution a huit ans d’une paralysie diaphragmatique bilaterale imputee a un syndrome de Parsonage-Turner : l’absence de recuperation est authentifiee par le suivi fonctionnel respiratoire. Les possibilites therapeutiques, limitees, sont discutees.
