1. Pre-operative intramuscular ranitidine and cimetidine

Abstract

Summary A controlled trial was carried out on 120 healthy patients presenting for elective surgery. The patients were divided randomly into three groups, which received respectively, 50 mg ranitidine, 100 mg ranitidine, or 300 mg cimetidine intramuscularly at least 45 minutes before operation. Following induction of anaesthesia, a nasogastric tube was passed, the stomach contents aspirated and analysed for volume and pH. Thirteen per cent of cimetidine-treated patients had a gastric pH of 2.5 or less, compared to 8% of those given ranitidine 50 mg and 3% of those given ranitidine 100 mg; however, these differences were not statistically significant. No side effects attributable to the administration of either drug were observed. It is concluded that intramuscular administration of ranitidine or cimetidine is an effective method of reducing the number of patients at risk of acid aspiration during anaesthesia. However, neither drug eliminates the risk of acid aspiration in all patients. and thus careful anaesthetic technique to protect the airway remains essential.

Abstract

Summary Having used the Boyle's bottle vaporizer apparatus out of necessity in a developing country, the concentration of agent that had been administered was investigated retrospectively. Three anaesthetic agents, halothane, isoflurane and enflurane, were measured at different temperatures, using a Boyle's anaesthetic machine and a Boyle's bottle in circuit with a Magill breathing system connected to a Rascal II Agent Monitor. Bubbling a fresh gas flow of 5 l.min−1 through the anaesthetic liquids generated concentrations in excess of 12%. Elevating the initial temperature of the vaporizer increased the delivered concentration, although this effect was short-lived. Therapeutic concentrations of vapour were achieved for all three agents by avoiding bubbling and manipulating the ‘splitting ratio’ lever. The Boyle's bottle vaporizer may be used with modern anaesthetic agents such as halothane, isoflurane and enflurane. However, the limitations of and variations between vaporizers should be borne in mind. An agent monitor employed at the patient end of the circuit would be an important safety feature.

Abstract

In a previous study, logistic regression analysis was used to determine the association of independent fixed patient factors with the incidence of postoperative nausea and vomiting (PONV). Female sex, previous history of PONV, use of postoperative opioids, previous history of motion sickness and an interaction between male sex and previous history of PONV were combined in an equation from which risk of PONV could be estimated. The present study was designed to test this equation in a group of patients with wide selection criteria. Data on 400 patients were collected in relation to pre-, per- and postoperative factors which may influence the incidence of PONV. The equation was used to predict PONV, and actual outcome was compared with that predicted. The overall incidence of PONV was 36%. The equation predicted an overall probability of PONV of 27.4%. If the model was used to define individual patients as predicted to have or not to have PONV, it was correct only 71% of the time. However, there was good agreement between the actual incidences of PONV and those predicted among the 16 risk groups created by the model.

This article presents a "case report" written from the viewpoints of the doctor, the scientist, and the professor.

Abstract

Summary The Association of Anaesthetists of Great Britain and Ireland's most recent publication on controlled drugs notes that the Environmental Protection Act (1992) does not permit the disposal of controlled drugs into the water sewerage system. The Association suggests that, in order to conform to the Misuse of Drugs Regulations (1985). controlled drugs may best be disposed of using some sort of absorbent material, for example a small amount of cat litter or wallpaper paste, in the bottom of a sharps bin. The absorbent capacity of three types of cat litter, wallpaper paste and a commercial absorbent material were tested. Both the commercial absorbent and wallpaper paste displayed limited absorbent qualities unless the container was agitated. The cat litter, Snowflake, absorbed more controlled drug than the others (one-way ANOVA, F2.12= 250.85, p < 0.01). The use of absorbent materials in sharps bins for the disposal of controlled drugs in clinical areas has limitations and may not be suitable. It is concluded, therefore, that small volumes of controlled drugs should be disposed of directly into clinical waste and that large volumes should be dealt with using a commercial absorbent in its own container.

Abstract

The publication of the 1991–1992 report of the National Confidential Enquiry into Perioperative Deaths (NCEPOD) [10] drew attention to prophylaxis against venous thromboembolism in surgical patients as an area of clinical practice that requires attention. The report found that pulmonary embolism is an important cause of mortality in postoperative surgical patients and was responsible for 7 % of all deaths. The role of prophylaxis against venous thromboembolism was not investigated in great detail but the findings suggested that it is not being given to all patients in whom it could be considered appropriate. In high-risk hip replacement patients, for example, 33 % did not receive any prophylaxis against venous thromboembolism. The recommendations made in the NCEPOD report were that every hospital should have an agreed policy for prophylaxis against venous thromboembolism, all surgeons should be made aware of the problem and implementation of the policy should be audited regularly. The aim of this review of the prevention of venous thromboembolism is to identify patients who benefit, discuss the merits of different methods, investigate areas of controversy and explore the role of anaesthetists.

Abstract

The knowledge and attitudes of healthcare professionals in accident and emergency departments in Australia are poorly understood. The authors assessed knowledge of and attitudes toward HIV/AIDS and homophobia were assessed in a convenience sample of 111 healthcare professionals in three New South Wales teaching hospitals. Results indicated that respondents with low knowledge of HIV/AIDS used the media as their major source of education and had higher levels of fear of infection than their colleagues with higher knowledge. The study findings imply that quality health care for people with HIV/AIDS requires programs that provide correct information and address the fear of infection.

Abstract

Despite substantial progress, adolescents remain at high risk for human immunodeficiency virus (HIV) infection. Two independent random digit dial statewide Massachusetts surveys of 16- to 19-year-old persons conducted August through September 1986 and 1988 revealed that the proportion of teenagers who had discussed acquired immunodeficiency syndrome (AIDS) in schools increased from 52% to 82%. Knowledge about how AIDS is transmitted was also significantly higher in 1988. The proportion of teenagers using drugs other than alcohol and marijuana declined from 13% to 9%, and intravenous drug use declined from 1% to 0.1%. At the same time, among sexually active teenagers, the proportion who reported changes in sexual behavior to avoid AIDS increased from 16% to 34%. The proportion who adopted condom use to avoid AIDS increased from 2% to 19%. However, the overall proportion of teenagers who reported sexual intercourse in the past year increased significantly from 55% to 61%. In 1988, among sexually active teenagers, 37% never used condoms and 33% used them only some of the time. Of all respondents, 18% reported unprotected sex with more than one partner in the previous year, and 3% reported unprotected sex with intravenous drug users. Although mass media and school education may increase knowledge and stimulate some teenagers to change behavior, for others more personal forms of counseling may be needed. The effects of increasing physician counseling warrant special study. Teenagers who had spoken to physicians about AIDS were much more likely to have adopted condom use to avoid HIV exposure. However, although 80% of adolescents saw a physician in the past year, only 13% were counseled about AIDS.

22. Death in the dental chair: an avoidable catastrophe?

Abstract

1In their editorial the authors make the case for these views most elegantly, but I am a little concerned by their final, and isolated, conclusion that substitution of halothane by sevoflurane is the definitive requirement. All inhalation anaesthetics lower the threshold for arrhythmias. I would accept that the problem is somewhat greater with halothane than with sevoflurane, but I feel they are a little hard on a drug that has served us well for many years. Changing to sevoflurane will have significant cost implications — something that I would be happy to endorse if halothane really is the key cause of death in the dental chair. Arrhythmias with halothane are, in my view, nearly always associated with poorly conducted anaesthesia resulting in either carbon dioxide retention or hypoxaemia. In the dental chair, other factors (notably fear and intense surgical stimulation) are also relevant, but we must recognize that the proper conduct of the anaesthetic remains paramount. Many of the deaths referred to by Worthington, Flynn and Strunin have involved factors of much greater significance than the simple choice of inhalation agent. Sevoflurane will certainly reduce the incidence of arrhythmias, but will it reduce the incidence of avoidable catastrophe?

Abstract

The measurement of plasma fentanyl concentration by radioimmunoassay (RIA) and gas liquid chromatography (GLC) using either a flame ionization detector (FID) or nitrogen/phosphorus detector (NPD) has been compared. RIA is a satisfactory but expensive method of measuring plasma fentanyl concentration. GLC using an FID is not as satisfactory as RIA, but when an NPD is used the results are equal to those of RIA. In addition, other analgesics which are chemically similar to fentanyl, such as alfentanil, may also be measured by the GLC/NPD combination using the same set of operating conditions. By contrast, an RIA method is usually specific for only one compound, and measurement of additional drugs would almost certainly necessitate the development of new assay kits for each one.

Abstract

During hysteroscopy the uterus may be distended with carbon dioxide (CO 2 ), nitrous oxide (N 2 O), or Hyskon (a high molecular weight dextran). An initial study in 27 patients (group 1) using arterialized venous blood samples demonstrated rises in carbon dioxide tension (PCO 2 ) when N 2 O was insufflated by using a laparoscopy insufflating device—a constant-pressure, variable-volume gas source. Cardiovascular collapse occurred in one patient in this group, most probably as a result of macropulmonary emboli of N 2 O. The rise in PCO 2 is accounted for by an increase in physiologic dead space. In another 24 patients (group 2) the gaseous media were introduced by using a constant-volume, variable-pressure gas source; this resulted in minimal changes in arterial PCO 2 . The choice of whether a gaseous or liquid distending medium is used for hysteroscopy is governed by the state of the endometrium. If a gaseous medium is indicated, then CO 2 is preferable to N 2 O and should be introduced with a constant-volume, variable-pressure gas source.

Abstract

In 32 patients with chronic liver disease, undergoing major abdominal surgery, anaesthesia was induced with either Althesin (alphaxalone and alphadolone acetate) or thiopentone, and maintained with nitrous oxide, oxygen and increments of either pethidine or fentanyl. The patients were ventilated artificially to maintain PaCO, values in the normal range. Pancuronium bromide was used as the muscle relaxant. Liver function tests at 24 hr and 5 day after surgery showed only minor changes compared with findings before operation. There was no significant difference between the Althesin and thiopentone groups. In two patients with severe obstructive jaundice there was difficulty in reversing the effects of pancuronium, but "pancuronium resistance" was observed in all patients. It is concluded that the anaesthetic sequence described, with either Althesin or thiopentone induction, is satisfactory with respect to changes in liver function in patients with chronic liver disease undergoing major surgery.

Abstract

Pentazocine 30 mg. or 15 mg or a placebo, was administered randomly to forty-nine patients undergoing conservative dental treatment in combination with a local analgesic block and intravenous diazepam. Simple cardiorespiratory measurements were made throughout the treatment period. Patients in the 30 mg pentazocine group required some 6 mg diazepam less than the placebo (control) group (P less than 0.05). Patients receiving 15 mg pentazocine also required less diazepam compared to the control group, but this difference was not statistically significant. There were no significant differences between the three groups either in recovery times or the cardiorespiratory measurements.
