Periprostatic fat has a metabolic activity on the prostate via cytokines that act paracrine on several signaling pathways including tumorigenesis. We investigated whether there was an association between preprostatic fat abundance and prostate cancer (PCa) aggressiveness.We performed a retrospective study including patients who underwent radical prostatectomy in our center from the prospective RESCAP database. Preoperative MRIs were re-read and different measurements of preprostatic fat (PPF) were performed. The maximum thickness (PPFmax) and the minimum thickness (PPFmin) were measured on a median T2 sagittal section. The total volume of preprostatic fat (PPFV) and volume normalized by prostate volume (NPPFV) were calculated semi-automatically by segmentation on continuous axial sections of 3mm. The association of these parameters with the aggressiveness criteria of PCa (ISUP 3-5 on biopsies and on operative specimen, intermediate or high-risk disease according to D'AMICO, PSA10, upgrading risk at radical prostatectomy) was measured as well as the association between normal, overweight, and obese BMI classes and the aggressiveness criteria used.One hundred and twenty-one men were included in this study. In both the univariate analysis and the multivariate analysis, none of the preprostatic fat measurements (PPFmax, PPFmin, PPFV and NPPFV) were associated with PCa aggressiveness criteria. There was also no association between BMI class and tumor aggressiveness.In this study, there is no association between the abundance of preprostatic fat and PCa aggressiveness according to the periprostatic fat measurements achieved.III.

Objectifs Il n’existe a ce jour aucune donnee sur la prise en charge de l’echec d’une premiere injection intra-detrusorienne de toxine botulique chez les patients spina bifida. L’objectif de cette etude etait de rapporter les resultats des reinjections et de comparer l’efficacite des differentes strategies de reinjection chez les patients spina bifida en echec d’une premiere injection de toxine botulique. Methodes Tous les patients spina bifida ayant eu au moins une injection intra-detrusorienne de toxine botulique (IDBTX) entre 2002 et 2016 dans 14 centres francophones ont ete inclus dans une etude retrospective. Seul les patients avec un echec de la premiere injection ont ete inclus dans cette etude. L’echec etait defini comme la persistance soit des urgenturies, soit de l’incontinence urinaire, soit de l’hyperactivite detrusorienne. La strategie de prise en charge variait en fonction des centres : reinjection de la meme toxine a la meme dose, reinjection de la meme toxine a un dosage plus eleve, changement de toxine (« switch ») ou enterocystoplastie d’agrandissement. Resultats Parmi une cohorte de 125 patients, 40 patients en echec d’une premiere injection intra-detrusorienne ont ete inclus (32 %). Les injections sont restees inefficaces chez 63 % des patients malgre une a quatre reinjections, et deux autres patients ont eu une efficacite pendant quelques injections avant d’echapper : 70,4 % ont ainsi finalement eu une enterocystoplastie d’agrandissement. Ainsi 48,1 % des patients ont eu des injections efficaces a un moment de leur prise en charge, 40,7 % avaient des injections toujours efficaces a la fin de la periode d’etude, dont 3 chez qui l’efficacite n’etait que partielle (efficacite clinique mais pas urodynamique), soit seulement 25,9 % des patients avec une efficacite satisfaisante et durable. Aucun patient ayant ete en echec des 3 premieres injections n’a finalement repondu. Conclusion Les strategies de « manipulation toxinique » et de reinjections semblent peu efficaces chez les patients spina bifida en echec d’une premiere injection de toxine botulique ne permettant d’obtenir un resultat satisfaisant que chez 25,9 % d’entre eux ce qui pourrait plaider pour un recours plus rapide a l’enterocystoplastie d’agrandissement chez ces patients.

Renal transplantation is performed only in university hospital centres, in accredited transplanting centres. The aim of this study is to analyse the learning curve of this operation and its impact on the graft survival.Monocentric retrospective study in which 3 groups have been defined: Juniors 1, Juniors 2 and Seniors corresponding respectively to the first thirty transplantations and to the last thirty transplantations of 5 clinical leaders, and 30 transplantation graft of referent seniors. Data have been registered in a database. Operation times, lukewarm ischemic times and postoperative complications have been compared within the 3 groups.A clear difference of operation time has been noted within the 3 groups with an average time of 202 minutes for Juniors 1, 173 minutes for Juniors 2 and 140 minutes for Seniors (P0.0001). Likewise, concerning lukewarm ischemic time and vascular anastomosis time respectively with an average time of 72, 59 and 40 min (P0.0001). Vascular complications occurred in 20% of cases in Juniors 1, 44.3% of cases in Juniors 2 and 17% of cases in Seniors (P=0.65). There were no significant differences of survival without urinary complications: 20% of complications for Juniors 1, 10% for Juniors 2 and 17% for Seniors (P=0.63). Similarly results have been obtained with analysing complications following Clavien's order.This study reveals that renal transplantations operated by young surgeons require longer operation and lukeward ischemic time but without significant repercussions on the surgical complication rate and the global survival. This stresses on the importance of surgical training during medicine internship.

Objectifs Evaluation de l’efficacite de la photovaporisation prostatique (PVP) par laser Greenlight 180 W dans le traitement de l’hypertrophie benigne de prostate (HBP) pour des volumes prostatiques superieurs a 120 cc. Methodes Cette etude de cohorte prospective unicentrique a ete realisee entre 2007 et 2013 sur 335 patients traites par PVP pour prise en charge d’une HBP. Soixante-deux patients avaient un volume prostatique superieur a 120 cc. Les caracteristiques des patients etaient comparables dans les deux groupes. Le suivi des patients a ete realise grâce a l’International Prostate Symptom Score (IPSS), Quality of life (Qol), mesure du Qmax et du residu post-mictionnel (RPM). Resultats Nous n’avons pas trouve de difference significative pour les resultats fonctionnels a 1 an (IPSS a 5,6 vs 6,16, p = 0,6 ; Qol a 1,38 vs 1,08, p = 0,318 ; Qmax a 17,2 vs 17,32, p = 0,95 ; RPM a 21,8 vs 33,4, p = 0,29). L’analyse des resultats postoperatoires montre de maniere significative que les troubles urinaires irritatifs etaient moindres a 1 mois dans le groupe superieur a 120 cc (urgenturie 25,6 vs 12,9 %, p = 0,032 ; pollakiurie 24,6 vs 13 %, p = 0,029) et une absence de difference significative a plus long terme (6 et 12 mois) autant sur les troubles irritatifs que sur le risque de retention urinaire. Conclusion Notre etude prouve l’efficacite de la photovaporisation prostatique chez les patients avec un volume prostatique eleve sans majoration du nombre de complications postoperatoires et a long terme. Mais nous constatons egalement que les signes urinaires irritatifs sont moins presents a court terme en cas de volume prostatique superieur a 120 cc.
