Abstract

We report a characterization of the multiband flux variability and correlations of the nearby (z=0.031) blazar Markarian 421(Mrk 421) using data from Mets ̈ahovi, Swift, Fermi-LAT, MAGIC, FACT, and other collaborations and instruments from 2014November till 2016 June. Mrk 421 did not show any prominent flaring activity, but exhibited periods of historically low activity above 1 TeV (F>1TeV<1.7×10−12ph cm−2s−1) and in the 2–10 keV (X-ray) band (F2−10 keV<3.6×10−11erg cm−2s−1),during which the Swift-BAT data suggest an additional spectral component beyond the regular synchrotron emission. The highest flux variability occurs in X-rays and very high-energy (E>0.1 TeV)γ-rays, which, despite the low activity, show a significant positive correlation with no time lag. The HRkeV and HRTeV show the harder-when-brighter trend observed in many blazars, but the trend flattens at the highest fluxes, which suggests a change in the processes dominating the blazar variability. Enlarging our dataset with data from years 2007 to 2014, we measured a positive correlation between the optical and the GeV emission over a range of about 60 d centred at time lag zero, and a positive correlation between the optical/GeV and the radio emission over a range of about 60 d centred at a time lag of 43+9−6d. This observation is consistent with the radio-bright zone being located about 0.2 parsec downstream from the optical/GeV emission regions of the jet. The flux distributions are better described with a lognormal function in most of the energy bands probed, indicating that the variability in Mrk 421 is likely produced by a multiplicative process.

Abstract

Long-duration γ-ray bursts (GRBs) originate from ultra-relativistic jets launched from the collapsing cores of dying massive stars. They are characterized by an initial phase of bright and highly variable radiation in the kiloelectronvolt-to-megaelectronvolt band, which is probably produced within the jet and lasts from milliseconds to minutes, known as the prompt emission. Subsequently, the interaction of the jet with the surrounding medium generates shock waves that are responsible for the afterglow emission, which lasts from days to months and occurs over a broad energy range from the radio to the gigaelectronvolt bands. The afterglow emission is generally well explained as synchrotron radiation emitted by electrons accelerated by the external shock. Recently, intense long-lasting emission between 0.2 and 1 teraelectronvolts was observed from GRB 190114C. Here we report multifrequency observations of GRB 190114C, and study the evolution in time of the GRB emission across 17 orders of magnitude in energy, from 5 × 10^(−6) to 10^(12) electronvolts. We find that the broadband spectral energy distribution is double-peaked, with the teraelectronvolt emission constituting a distinct spectral component with power comparable to the synchrotron component. This component is associated with the afterglow and is satisfactorily explained by inverse Compton up-scattering of synchrotron photons by high-energy electrons. We find that the conditions required to account for the observed teraelectronvolt component are typical for GRBs, supporting the possibility that inverse Compton emission is commonly produced in GRBs.

Abstract

Very-high Energy (VHE) gamma-ray astroparticle physics is a relatively young field, and observations over the past decade have surprisingly revealed almost two hundred VHE emitters which appear to act as cosmic particle accelerators. These sources are an important component of the Universe, influencing the evolution of stars and galaxies. At the same time, they also act as a probe of physics in the most extreme environments known - such as in supernova explosions, and around or after the merging of black holes and neutron stars. However, the existing experiments have provided exciting glimpses, but often falling short of supplying the full answer. A deeper understanding of the TeV sky requires a significant improvement in sensitivity at TeV energies, a wider energy coverage from tens of GeV to hundreds of TeV and a much better angular and energy resolution with respect to the currently running facilities. The next generation gamma-ray observatory, the Cherenkov Telescope Array Observatory (CTAO), is the answer to this need. In this talk I will present this upcoming observatory from its design to the construction, and its potential science exploitation. CTAO will allow the entire astronomical community to explore a new discovery space that will likely lead to paradigm-changing breakthroughs. In particular, CTA has an unprecedented sensitivity to short (sub-minute) timescale phenomena, placing it as a key instrument in the future of multi-messenger and multi-wavelength time domain astronomy. Abstract

Objectives To verify the intrasession reliability and the agreement between strength measurement of hip and knee muscles using hand-held dynamometer stabilized by a belt or by an examiner. Design Test-retest design. Setting Knee and hip muscles strength were measured bilaterally using hand-held dynamometer stabilized by a belt and by an examiner. Participants 24 young and healthy participants. Main outcome measures The reliability was verified by the intraclass correlation coefficient2,1 and the standard error of measurement. Agreement between stabilization methods was verified by the Bland Altman's method. Results Reliability was excellent for all muscle groups when stabilized by a belt (intraclass correlation coefficient = 0.78 to 0.95) and by the examiner (intraclass correlation coefficient = 0.83 to 0.97); standard error of measurement ranged between 1kgf to 4kgf at both methods, but they are proportionally lower when stabilized by the examiner. No agreement between both methods was identified for all knee strength measurements and for bilateral hip flexion, right internal and external rotation and left adduction. Conclusions The hand-held dynamometer is reliable for hip and knee strength evaluation despite of the stabilization method. However, for the majority of the movements, greater strength and lower error are expected when the examiner stabilizes it.

Abstract

Extreme high-frequency-peaked BL Lac objects (EHBLs) are blazars that exhibit extremely energetic synchrotron emission. They also feature nonthermal gamma-ray emission whose peak lies in the very high-energy (VHE, E > 100 GeV) range, and in some sources exceeds 1 TeV: this is the case for hard-TeV EHBLs such as 1ES 0229+200. With the aim of increasing the EHBL population, 10 targets were observed with the MAGIC telescopes from 2010 to 2017, for a total of 265 hr of good-quality data. The data were complemented by coordinated Swift observations. The X-ray data analysis confirms that all but two sources are EHBLs. The sources show only a modest variability and a harder-when-brighter behavior, typical for this class of objects. At VHE gamma-rays, three new sources were detected and a hint of a signal was found for another new source. In each case, the intrinsic spectrum is compatible with the hypothesis of a hard-TeV nature of these EHBLs. The broadband spectral energy distributions (SEDs) of all sources are built and modeled in the framework of a single-zone, purely leptonic model. The VHE gamma-ray-detected sources were also interpreted with a spine-layer model and a proton synchrotron model. The three models provide a good description of the SEDs. However, the resulting parameters differ substantially in the three scenarios, in particular the magnetization parameter. Abstract

Astronomy and astrophysics 647, A163 (2021). doi:10.1051/0004-6361/202039687, Context. QSO B1420+326 is a blazar classified as a flat-spectrum radio quasar (FSRQ). At the beginning of the year 2020, it was found to be in an enhanced flux state and an extensive multiwavelength campaign allowed us to trace the evolution of the flare.Aims. We search for very high-energy (VHE) gamma-ray emission from QSO B1420+326 during this flaring state. We aim to characterize and model the broadband emission of the source over different phases of the flare.Methods. The source was observed with a number of instruments in radio, near-infrared, optical (including polarimetry and spectroscopy), ultraviolet, X-ray, and gamma-ray bands. We use dedicated optical spectroscopy results to estimate the accretion disk and the dust torus luminosity. We performed spectral energy distribution modeling in the framework of combined synchrotron-self-Compton and external Compton scenario in which the electron energy distribution is partially determined from acceleration and cooling processes.Results. During the enhanced state, the flux of both SED components of QSO B1420+326 drastically increased and the peaks were shifted to higher energies. Follow-up observations with the MAGIC telescopes led to the detection of VHE gamma-ray emission from this source, making it one of only a handful of FSRQs known in this energy range. Modeling allows us to constrain the evolution of the magnetic field and electron energy distribution in the emission region. The gamma-ray flare was accompanied by a rotation of the optical polarization vector during a low -polarization state. Also, a new superluminal radio knot contemporaneously appeared in the radio image of the jet. Abstract

MAGIC Collaboration: et al., [Context]: It has become evident that one-zone synchrotron self-Compton models are not always adequate for very high-energy (VHE) gamma-ray-emitting blazars. While two-component models perform better, they are difficult to constrain due to the large number of free parameters., [Aims]: In this work, we make a first attempt at taking into account the observational constraints from very long baseline interferometry (VLBI) data, long-term light curves (radio, optical, and X-rays), and optical polarisation to limit the parameter space for a two-component model and test whether or not it can still reproduce the observed spectral energy distribution (SED) of the blazars., [Methods]: We selected five TeV BL Lac objects based on the availability of VHE gamma-ray and optical polarisation data. We collected constraints for the jet parameters from VLBI observations. We evaluated the contributions of the two components to the optical flux by means of decomposition of long-term radio and optical light curves as well as modelling of the optical polarisation variability of the objects. We selected eight epochs for these five objects based on the variability observed at VHE gamma rays, for which we constructed the SEDs that we then modelled with a two-component model., [Results]: We found parameter sets which can reproduce the broadband SED of the sources in the framework of two-component models considering all available observational constraints from VLBI observations. Moreover, the constraints obtained from the long-term behaviour of the sources in the lower energy bands could be used to determine the region where the emission in each band originates. Finally, we attempt to use optical polarisation data to shed new light on the behaviour of the two components in the optical band. Our observationally constrained two-component model allows explanation of the entire SED from radio to VHE with two co-located emission regions., VFR and EL were supported by Academy of Finland projects 317636 and 320045. Abstract

Aims. We measure the Crab Nebula γ-ray spectral energy distribution in the ~100 TeV energy domain and test the validity of existing leptonic emission models at these high energies. Methods. We used the novel very large zenith angle observations with the MAGIC telescope system to increase the collection area above 10 TeV. We also developed an auxiliary procedure of monitoring atmospheric transmission in order to assure proper calibration of the accumulated data. This employs recording optical images of the stellar field next to the source position, which provides a better than 10% accuracy for the transmission measurements. Results. We demonstrate that MAGIC very large zenith angle observations yield a collection area larger than a square kilometer. In only ~ 56 h of observations, we detect the γ-ray emission from the Crab Nebula up to 100 TeV, thus providing the highest energy measurement of this source to date with Imaging Atmospheric Cherenkov Telescopes. Abstract

We report on a multiband variability and correlation study of the TeV blazar Mrk 421 during an exceptional flaring activity observed from 2013 April 11 to 19. The study uses, among others, data from GLAST-AGILE Support Program (GASP) of the Whole Earth Blazar Telescope (WEBT), Swift, Nuclear Spectroscopic Telescope Array (NuSTAR), Fermi Large Area Telescope, Very Energetic Radiation Imaging Telescope Array System (VERITAS), and Major Atmospheric Gamma Imaging Cherenkov (MAGIC). The large blazar activity and the 43 hr of simultaneous NuSTAR and MAGIC/VERITAS observations permitted variability studies on 15 minute time bins over three X-ray bands (3-7 keV, 7-30 keV, and 30-80 keV) and three very-high-energy (VHE; >0.1 TeV) gamma-ray bands (0.2-0.4 TeV, 0.4-0.8 TeV, and >0.8 TeV). We detected substantial flux variations on multi-hour and sub-hour timescales in all of the X-ray and VHE gamma-ray bands. The characteristics of the sub-hour flux variations are essentially energy independent, while the multi-hour flux variations can have a strong dependence on the energy of the X-rays and the VHE gamma-rays. The three VHE bands and the three X-ray bands are positively correlated with no time lag, but the strength and characteristics of the correlation change substantially over time and across energy bands. Our findings favor multi-zone scenarios for explaining the achromatic/chromatic variability of the fast/slow components of the light curves, as well as the changes in the flux-flux correlation on day-long timescales. We interpret these results within a magnetic reconnection scenario, where the multi-hour flux variations are dominated by the combined emission from various plasmoids of different sizes and velocities, while the sub-hour flux variations are dominated by the emission from a single small plasmoid moving across the magnetic reconnection layer. © 2020. Abstract

Introduction Recombinant human C1 esterase inhibitor (rhC1-INH) is indicated in the United States for treating hereditary angioedema (HAE) attacks in adolescents/adults. Data are limited on rhC1-INH treatment in women with HAE who are pregnant. Methods Pregnant women with HAE from the United States and Europe who received rhC1-INH were followed to full term and were assessed for adverse events (AEs) that occurred during pregnancy as well as for neonatal outcomes. Results Ten pregnant women (age, 20-33 years) with HAE treated with rhC1-INH were identified. One woman received a 4200-IU dose pre-delivery as short-term prophylaxis and 9 were treated with rhC1-INH (2100 IU-4200 IU) for 1 (n=1), 2 (n=2), 4 (n=1), 6 (n=1), 8 (n=1), 9 (n=2), or 40 (n=1) HAE attacks. Ten laryngeal attacks occurred in 2 patients; all 10 attacks responded to rhC1-INH within 2 to 4 hours postdose. In addition, 4 facial attacks occurred in 1 patient and 1 in another patient that were successfully treated with 1 rhC1-INH dose and no rescue medications. There were no AEs considered related to rhC1-INH during the pregnancy period. Of 6 women with birth delivery method details available, 4 had a vaginal delivery and 2 a cesarean, all without complications. All 10 women gave birth at full term to healthy babies. No fetal distress or congenital abnormalities were reported. Conclusions rhC1-INH treatment for HAE attacks in pregnant women was generally safe and well tolerated. All 10 women delivered healthy babies at full term without complications.

Abstract

Renal endothelin (ET) production was investigated in rats after renal mass ablation, a model of progressive renal disease characterized by glomerular hemodynamic alterations and capillary thrombosis, and in sham-operated animals. Thrombin stimulation of renal cortical tissue from rats with renal mass reduction, 45 but not 7 days after surgery, generated significantly (P less than 0.01) more ET than tissue from sham-operated animals. Exposure to thrombin of isolated glomeruli from remnant but not sham kidneys also significantly (P less than 0.01) increased ET production compared with unstimulated glomeruli. At day 45, in rats with renal mass ablation ET plasma levels were numerically lower, whereas urinary excretion rate of ET was significantly (P less than 0.01) increased compared with sham-operated animals. After a 50-min intravenous infusion of 125I-ET to normal rats and animals with renal mass ablation, less than 0.3% and 0.03%, respectively, of total infused radioactivity was recovered in urine. These results indicate that thrombin-stimulated ET production by renal cortical tissues is increased in rats with renal mass reduction. Despite normal or low-normal plasma ET levels, urinary excretion of the peptide is markedly increased in this model. The exogenously labeled ET added to circulation is not excreted by the kidney, suggesting that enhanced urinary excretion rate would reflect an increased renal production of the peptide in rats with remnant kidney.

Abstract

Starburst galaxies and star-forming active galactic nuclei are among the candidate sources thought to contribute appreciably to the extragalactic gamma-ray and neutrino backgrounds. NGC 1068 is the brightest of the star-forming galaxies found to emit gamma-rays from 0.1 to 50 GeV. Precise measurements of the high-energy spectrum are crucial to study the particle accelerators and probe the dominant emission mechanisms. We have carried out 125 hr of observations of NGC 1068 with the MAGIC telescopes in order to search for gamma-ray emission in the very-high-energy band. We did not detect significant gamma-ray emission, and set upper limits at the 95% confidence level to the gamma-ray flux above 200 GeV f < 5.1 × 10−13 cm−2 s−1. This limit improves previous constraints by about an order of magnitude and allows us to put tight constraints on the theoretical models for the gamma-ray emission. By combining the MAGIC observations with the Fermi-LAT spectrum we limit the parameter space (spectral slope, maximum energy) of the cosmic ray protons predicted by hadronuclear models for the gamma-ray emission, while we find that a model postulating leptonic emission from a semi-relativistic jet is fully consistent with the limits. We provide predictions for IceCube detection of the neutrino signal foreseen in the hadronic scenario. We predict a maximal IceCube neutrino event rate of 0.07 yr−1. [ABSTRACT FROM AUTHOR]

Abstract

Acciari, V. A., et al. (MAGIC Collaboration), PSR J0218+4232 is one of the most energetic millisecond pulsars known and has long been considered as one of the best candidates for very high-energy (VHE; >100 GeV) ?-ray emission. Using 11.5 yr of Fermi Large Area Telescope (LAT) data between 100 MeV and 870 GeV, and ~90 hr of Major Atmospheric Gamma Imaging Cherenkov (MAGIC) observations in the 20 GeV to 20 TeV range, we searched for the highest energy ?-ray emission from PSR J0218+4232. Based on the analysis of the LAT data, we find evidence for pulsed emission above 25 GeV, but see no evidence for emission above 100 GeV (VHE) with MAGIC. We present the results of searches for ?-ray emission, along with theoretical modeling, to interpret the lack of VHE emission. Abstract

Over the past 20 years, several eye-tracking technologies have been developed. This article aims to present a new type of eye tracker capable of producing detailed information on eye and head movements using an array of magnetoresistive detectors fixed on the patient's head and a small magnet inserted into a contact lens, adapted to the curvature of the cornea of the subject. The software used for data analysis can combine or compare eye and head movements and can represent them as 2D or 3D images. Preliminary data involve an initial patient who was asked to perform several tasks to establish the accuracy, reliability, and tolerance of the magnetic eye tracker and software. The tasks included assessment of saccadic eye movements and pursuit, "drawing" alphabetic shapes or letters, and reading. Finally, a Head Impulse Test (HIT) was performed to estimate the VOR gain, comparing the standard deviation established via vHIT with that established via this magnetic eye tracker (mHIT). This prototypical device is minimally invasive, lightweight, relatively cheap, and tolerable, with a high degree of reliability and precision. All these characteristics could lead to the future use of the magnetic eye tracker in neurological and otoneurological fields.

Abstract

We present a set of results obtained with an innovative eye-tracker based on magnetic dipole localization by means of an array of magnetoresistive sensors. The system tracks both head and eye movements with a high rate (100-200 Sa/s) and in real time. A simple setup is arranged to simulate head and eye motions and to test the tracker performance under realistic conditions. Multimedia material is provided to substantiate and exemplify the results. A comparison with other available technologies for eye-tracking is drawn, discussing advantages (e.g., precision) and disadvantages (e.g., invasivity) of the diverse approaches, with the presented method standing out for low cost, robustness, and relatively low invasivity.

26. Recombinant human C1 esterase inhibitor for hereditary angioedema attacks: A European registry.

Abstract

Background: Hereditary angioedema (HAE) due to C1 esterase inhibitor deficiency (C1-INH-HAE) is characterized by recurrent swelling attacks. A European treatment registry was established to review the adverse event profile and efficacy of recombinant human C1 esterase inhibitor (rhC1-INH) for HAE attacks., Methods: Individuals with C1-INH-HAE were enrolled following a decision to treat with rhC1-INH and provision of written informed consent. Medical history and baseline HAE information were collected at screening. Healthcare providers entered data on HAE attacks, response to treatment, and adverse events using a web-based questionnaire., Results: From July 1, 2011, through December 1, 2019, 71 patients with C1-INH-HAE (30 male/41 female; mean age, 47.3 years; age range, 19-78 years) in 9 countries reported 2356 attacks and were treated with rhC1-INH. Before registry entry, patients, including 20 (28.2%) who were on maintenance therapy/prophylaxis at registry enrollment, experienced a mean of 25 HAE attacks per year (median, 16 [range, 0-185]). Most treated HAE attacks were abdominal (46.1%), followed by peripheral (38.3%), oro-facial-pharyngeal (14.8%), urogenital (3.2%), and laryngeal (2.6%). The mean rhC1-INH dose was 3307 U (43.3 U/kg). Patients reported symptom improvement within 4 h for 97.8% of attacks (2305/2356) with rhC1-INH; most attacks (99.8%; 2351/2356) required only 1 dose. Five attacks were treated with a second dose (total rhC1-INH dose administered for attack, 4200 U). Abstract

On January 14, 2019, the Major Atmospheric Gamma Imaging Cherenkov telescopes detected GRB 190114C above 0.2 TeV, recording the most energetic photons ever observed from a gamma-ray burst. We use this unique observation to probe an energy dependence of the speed of light in vacuo for photons as predicted by several quantum gravity models. Based on a set of assumptions on the possible intrinsic spectral and temporal evolution, we obtain competitive lower limits on the quadratic leading order of speed of light modification.

28. Functional leg length discrepancy between theories and reliable instrumental assessment: a study about newly invented NPoS system.

Background: In spite the instinct social&financial impact of Leg Length Discrepancy (LLD), controversial and conflicting results still exist regarding a reliable assessment/correction method. For proper management it's essential to discriminate between anatomical&functional Leg Length Discrepancy (FLLD). With the newly invented NPoS (New Postural Solution), under the umbrella of the collaboration of PRM Department, Tor Vergata University with Baro Postural Instruments srl, positive results were observed in both measuring& compensating the hemi-pelvic antero-medial rotation in FLLD through personalized bilateral heel raise using two NPoS components: Foot Image System (FIS) and Postural Optimizer System (POS). This led our research interest to test the validity of NPoS as a preliminary step before evaluating its implementations in postural disorders., Methods: After clinical evaluation, 4 subjects with FLLD have been assessed by NPoS. Over a period of 2 months, every subject was evaluated 12 times by two different operators, 48 measurements in total, results have been verified in correlation to BTS GaitLab results., Results: Intra-Operator&inter-operator variability analysis showed statistically insignificant differences, while inter-method variability between NPoS and BTS parameters expressed a linear correlation., Conclusion: Results suggest a significant validity of NPoS in assessment&correction of FLLD, with high degree of reproducibility with minimal operator dependency. Abstract

Background: Hereditary angioedema (HAE) is a rare, debilitating, potentially life-threatening condition characterized by recurrent acute attacks of edema of the skin, face/upper airway, and gastrointestinal and urogenital tracts. During a laryngeal attack, people with HAE may be at risk of suffocation, while other attacks are often associated with intense pain, disfigurement, disability, and/or vomiting. The intensity of some symptoms is known only to the person experiencing them. Thus, interview studies are needed to explore such experience and patient-reported outcome measures (PROMs) are required for systematic assessment of symptoms in the clinical setting and in clinical trials of treatments for acute HAE attacks., Objective: The aim of this interview study was to assess the content validity and suitability of four visual analog scale (VAS) instruments for use in clinical studies. The VAS instruments were designed to assess symptoms at abdominal, oro-facial-pharyngeal-laryngeal, peripheral, and urogenital attack locations. This is the first known study to report qualitative data about the patient's experience of the rare disorder, HAE., Methods: Semi-structured exploratory and cognitive debriefing interviews were conducted with 27 adults with a confirmed clinical/laboratory diagnosis of HAE (baseline plasma level of functional plasma protein C1 esterase inhibitor [C1INH] <50% of normal without evidence for acquired angioedema). There were 17 participants from the US and 10 from Italy, with mean age 42.5 (SD 14.5) years, range 18-72 years, mean HAE duration 21.3 (SD 14.1) years, range 1-45 years, 67% female, and 44% VAS-naïve. Experience of acute angioedema attacks was first explored, noting spontaneous mentions by participants of HAE symptomatology. Cognitive debriefing of the VAS instruments was undertaken to assess the suitability, comprehensibility, and relevance of the VAS items. Asymptomatic participants completed the VAS instruments relevant to their angioedema experience, reporting as if they were experiencing an acute angioedema attack at the time. Interviews were conducted in the clinic setting in the US and Italy over an 8-month period., Results: Participants mentioned spontaneously almost all aspects of acute angioedema attacks covered by the four VAS instruments, thus providing strong support for inclusion of nearly all VAS items, with no important symptoms missing. Predominant symptoms found to be associated with acute angioedema attacks were edema and pain, and there was evidence of varying degrees of disruption to everyday activities supporting the inclusion of an overall severity item reflecting the disabling effects of HAE symptoms. VAS item wording was understood by participants., Conclusion: This interview study explored and reported the patient experience of HAE attacks. It demonstrated the content validity of the four anatomical location HAE VAS instruments and their suitability for use in clinical trials of recombinant human C1INH (rhC1INH) treatment for ascertaining trial participants' assessments of the severity of acute angioedema symptoms.

Abstract

The authors present their experience with the treatment of haemorrhoidal prolapse using the Longo technique. From March 2005 to February 2006, 56 patients underwent the above-mentioned procedure, 49 of them (87.5%) suffering from third-degree and 7 (12.5%) from fourth-degree prolapse. The most common symptom, present in all patients, was rectal bleeding. Fifty patients (89.3%) were treated as one-day surgery cases. All patients were monitored at 1 week and1 month after the operation, 29 patients (51.8%) were monitored for 6 months postoperatively and 4 patients (7.1%) for 1 year. The mean follow-up was 5.6 months (range: 1 week-1 year). In our experience, the most important clinical evidence consisted in very low postoperative pain intensity, early return to work, a low rate of minor complications and the absence of major complications. It is necessary to ensure that both the details of technique and the indications for the procedure are fully considered and complied with and that an effective learning curve is implemented in order to achieve good postoperative results. We suggest the adoption of an eclectic approach that does not preclude more traditional operative techniques for the treatment of fourth-degree prolapse and prolapse with large external haemorrhoids. We consider this to be a sine qua non for an appropriate surgical approach to the disease.

Abstract

Systemic and renal hemodyanmic changes in normal pregnancy have been attributed in part to altered vascular synthesis of vasodilatory prostaglandins. Besides vasodilatory substances, endothelium also generates vasoconstrictors, including endothelin. We evaluated the capacity of placental tissue from normal pregnant women to express endothelin gene and to generate endothelin. Placental tissue expressed a single 2.3 kb preproendothelin messenger ribonucleic acid and produced comparable amounts of endothelin 3, Big endothelin 1, and endothelin 1 and a minor quantity of endothelin 2. To investigate the possible influence of placental endothelin production on plasma levels of the peptide, plasma endothelin concentrations were measured in normal pregnant women at delivery and were found to be numerically higher than those measured in nonpregnant subjects. Urinary excretion of endothelin, taken as a marker of the renal synthesis of the peptide, tended to increase, although not significantly, in the first 14 weeks of pregnancy. Abstract

Renal endothelin (ET) production was investigated in rats after renal mass ablation, a model of progressive renal disease characterized by glomerular hemodynamic alterations and capillary thrombosis, and in sham-operated animals. Thrombin stimulation of renal cortical tissue from rats with renal mass reduction, 45 but not 7 days after surgery, generated significantly (P less than 0.01) more ET than tissue from sham-operated animals. Exposure to thrombin of isolated glomeruli from remnant but not sham kidneys also significantly (P less than 0.01) increased ET production compared with unstimulated glomeruli. At day 45, in rats with renal mass ablation ET plasma levels were numerically lower, whereas urinary excretion rate of ET was significantly (P less than 0.01) increased compared with sham-operated animals. After a 50-min intravenous infusion of 125I-ET to normal rats and animals with renal mass ablation, less than 0.3% and 0.03%, respectively, of total infused radioactivity was recovered in urine. These results indicate that thrombin-stimulated ET production by renal cortical tissues is increased in rats with renal mass reduction. Despite normal or low-normal plasma ET levels, urinary excretion of the peptide is markedly increased in this model. The exogenously labeled ET added to circulation is not excreted by the kidney, suggesting that enhanced urinary excretion rate would reflect an increased renal production of the peptide in rats with remnant kidney.
