Abstract

ABSTRACT The feeling that life has meaning or purpose is essential for human beings. Absence of meaning can lead to an existential vacuum. Purpose in life, akin to meaning in life, derives from the humanistic psychology theory of Frankl, and they are considered synonymous. Other scholars, however, regard the two constructs as distinct. The objective of the review was to map both concepts in the gerontological literature, examining how they manifest in aging. The review of the databases was carried out between January 2019 and March 2020. Thirty articles were included. In the context of aging, results were conflicting. The literature review yielded evidence to affirm that meaning and purpose in life are distinct concepts, despite sharing the same existential-philosophical roots.

2. High stability microwave discharge ion sources

Abstract

Abstract A new plasma heating mechanism for Microwave Discharge Ion Sources (MDIS) was discovered. Unprecedented beam stability was observed during the commissioning of the Proton Source for the European Spallation Source (PS-ESS) where several thousand source configurations were tested using a custom software tool. Data analysis and plasma simulation revealed that the new behaviour is generated by a completely new plasma heating schema activated by a precise magnetic configuration peculiarity. The stability showed in this configuration, denominated High Stability Microwave Discharge Ion Source (HSMDIS), is excellent and the emittance of the produced beam is lower than produced by standard MDIS configuration. High linearity between power and beam current was observed making easier the use of the source. This new mode of operation can be easily implemented in all existing sources.

Abstract

Background: Thrombotic thrombocytopenic purpura (TTP) is a potentially fatal disease that requires early diagnosis and treatment that can be made possible by applying the PLASMIC score. This study aims to evaluate this score applicability for patients with suspected TTP in a developing country. Methods: This was a retrospective study performed at a tertiary hospital in the northeastern region of Brazil. Patients were analyzed in two groups: ADAMTS13 activity 10%. Patients were stratified according to the PLASMIC score, and the level of agreement between the PLASMIC score and the ADAMTS13 activity was evaluated. Abstract

Examine the association between glycemic control and cognition. Included subjects ≥60 years who participated in the 2013 to 2014 National Health and Nutrition Examination Survey and completed one of the followings: Consortium to Establish a Registry for Alzheimer’s Disease Word List (CERAD-WL), Animal Fluency (AF), Digit Symbol Substitution Test (DSST), and CERAD-Delayed Recall (CERAD-DR). Stratified participants into: No type 2 diabetes (T2D; N = 557), Controlled T2D ( N = 41), Uncontrolled T2D ( N = 120), and Untreated T2D ( N = 86). Multiple regression was used to examine the association between variables. After adjusting for demographics and cardiovascular risk factors, Uncontrolled T2D was associated with lower DSST (β = −3.164, p = .04), and Untreated T2D was associated with a trend for having lower CERAD-DR (β = −.496, p = .06) scores. T2D, independent of glycemic control, is associated with cognitive impairment and this relationship is influenced by modifiable and non-modifiable risk factors.

8. Weight loss severity and functional decline among the oldest old in a middle-income country: the fibra study longitudinal findings

Abstract

Background and objective: Nutritional status is a key modifiable risk factor associated with disability, and further evidence suggests that weight change is also linked to this adverse outcome. Thus, the aim of this study is to evaluate weight loss severity and functional decline in instrumental activities of daily living (IADL) in a seven-year period among a sample of Brazilian oldest old adults. Methods: This is a longitudinal prospective study using data from FIBRA study (Frailty in Older Brazilians), a population-based investigation carried out in 2008/2009, with follow up data collected in 2016/2017 from participants who were 80 years and older in the follow-up in Campinas, Brazil. Of the 167 participants who were interviewed with complete data in 2016-2017, 16 had improved their functional status and were excluded, so the final sample was restricted to 151 participants who maintained or declined functional status. We considered functional decline when a subject had a greater number of IADL dependencies at follow‐up than at baseline. Logistic regression was performed to assess the effect of weight loss, according to severity (moderate weight loss: 5-10% of body weight; severe weight loss >10%) in increasing the number of disabilities, in relation to the group with stable weight, controlling for covariates (gender, age, education and morbidity). An alpha level Results: During the follow-up period, 60.3% of the participants kept stable weight, 21.8% had moderate weight loss and 17.9% had severe weight loss. During the follow-up, only severe weight loss was associated to higher risk of functional decline (OR=2.74;p=0.032). Conclusions: weight loss consisted in a risk factor to functional decline in this sample of oldest old, but severity of the loss must be considered as only severe loss was significant. Given that weight loss is expected for the oldest old, it is important to quantify this loss and measure its severity, and also to identify the magnitude of the loss that is associated with functional decline.

Objective Although benign epilepsy with centrotemporal spikes (BECTS) is an idiopathic, age-related epilepsy syndrome with favorable outcome, recent studies have shown impairment in specific neuropsychological tests. The objective of this study was to analyze the comorbidity between dyslexia and BECTS. Method Thirty-one patients with clinical and electroencephalographic diagnosis of BECTS (group A) and 31 paired children (group B) underwent a language and neuropsychological assessment performed with several standardized protocols. Our findings were categorized as: a) dyslexia; b) other difficulties; c) without difficulties. Our results were compared and statistically analyzed. At the Istituto Nazionale di Fisica Nucleare–Laboratori Nazionali del Sud (INFN-LNS), the commissioning of the high intensity proton source for the European Spallation Source (PS-ESS) has been recently completed. Optical emission spectroscopy measurements carried out with PS-ESS permitted, in a noninvasive way, the evaluation of the electron density, temperature, and relative abundances of the neutral components of a hydrogen plasma. This approach is helpful in finding the optimal source parameters exploiting protons or H_{2}^{+} beams production.

Abstract

Our aim was to clarify the correlation of attention deficit hyperactivity disorder (ADHD) with epilepsy and behavior problems. This was a cross-sectional study. Sixty children with idiopathic epilepsy were interviewed using the MTA-SNAP IV Teacher and Parent Rating Scale, Vineland Adaptive Behavior Scales and Conners’ Rating Scales. We used the chi-square test to analyze the correlation of epilepsy variables in patients with and without ADHD with a significance level of 0.05. Eight patients had ADHD symptoms (13%), seven had the inattentive ADHD subtype and only three had behavioral problems. When epileptic patients with and without ADHD symptoms were compared we found no significant difference in regard to epilepsy variables. All patients were controlled and 43% were either without AED or undergoing withdrawal. Our study revealed a low comorbidity of ADHD symptoms and epilepsy due to low interference of seizures and drug treatment on the comorbid condition.

Abstract

Purpose: Secondary stroke prevention is an essential but unmet need for stroke survivors. Current secondary stroke prevention guidelines from the American Heart Association recommend following a Mediterranean-type diet to reduce recurrent stroke risk. However, there is a lack of published data on dietary behavior interventions and outcomes among stroke survivors. Therefore, we conducted a scoping review to characterize published dietary behavior interventions and outcomes among community-dwelling stroke survivors to identify gaps in scientific knowledge. Methods: PubMed was used to find research articles published between January 2000 and April 2021. To be included, articles must have undergone peer-review and described both a dietary intervention and dietary outcome among community-dwelling stroke survivors. Study characteristics and dietary outcomes were extracted by 3 team members for studies that met inclusion criteria. Results: 649 articles were initially identified but 637 were excluded based on eligibility criteria. Study characteristics and outcomes of the 12 included studies are described in the table. All studies (9 randomized controlled trials, 3 cohort studies) delivered multimodal health behavior interventions that included a dietary component. Interventions were delivered weekly to every other month, lasted 1-24 months, and provided 1-132 contact hours. Dietary outcomes included food frequency questionnaires and self-reported fruit, vegetable, salt, or fat consumption. Eight studies reported improvement in dietary outcomes; 6 of which reported ≥16 contact hours. Conclusions: This review provides support for the efficacy of dietary behavior interventions in stroke survivors. Intervention format and outcomes varied greatly, which impedes the ability to determine efficacy of dietary interventions in this population and represents a gap in scientific knowledge. Comparative effectiveness studies that compare outcomes across existing interventions may narrow this knowledge gap.

15. Evidence Surrounding Dietary Behavior Interventions in Community-Dwelling Stroke Survivors: A Scoping Review

Abstract

Objective: To identify and summarize studies that have implemented dietary behavior interventions and reported dietary outcomes in community-dwelling adult stroke survivors. Design: Scoping Review; Setting: Not applicable. Participants: Studies eligible for review included peer-reviewed studies describing both a dietary intervention and a dietary outcome among community-dwelling stroke survivors published between January 2000 and June 2022. Results: We identified 14 articles that included both a dietary behavior intervention and a dietary outcome. All studies (5 pilot randomized controlled trials (RCTs), 6 non-pilot RCTs, 3 cohort studies) delivered multimodal health behavior interventions that included a dietary component. Interventions were delivered weekly to every other month and lasted for 1–24 months. Most studies did not use standardized assessments to assess dietary outcomes; however, 8 studies reported improvement in dietary outcomes. Conclusions: This review provides support for the efficacy of dietary behavior interventions in stroke survivors. Variability in intervention format and assessment measures across studies impedes the ability to conduct meta-analyses of outcomes to inform research knowledge and clinical practice. Additional research is needed to determine mechanisms-of-action for dietary behavior change in stroke survivors.

Abstract

OBJECTIVES: To evaluate the presence of neurological soft signs (NSS) and to correlate them with the Wechsler Intelligence Scale for Children (WISC III) in patients with rolandic epilepsy (RE). METHODS: Forty children and adolescents aged between 9 and 15 years were studied. They were divided into two groups: G1 - patients with RE (n=20) - and G2 - healthy controls without epilepsy (n=20). They were assessed with the Quick Neurological Screening Test (QNST II) - clinical trial to search for NSS -, and the WISC III - neuropsychological test. RESULTS: No statistical difference between groups was found in WISC III and QNST II. However, children with poorer motor skills had worse performance in the QNST II and also in the execution intelligence quotient - IQ (p=0.001) and in total IQ (p=0.004), thus showing a positive correlation between them. CONCLUSIONS: The QNST II is a good screening tool for the neurologist to detect abnormalities in fine motor skills.

Abstract

Electron cyclotron resonance ion sources based charge breeders (ECR-CB) are fundamental devices for Isotope Separation On Line (ISOL) facilities aiming at postaccelerating radioactive ion beams (RIBs). Presently, low intensity RIBs do not allow a conventional tuning of the ECR-CB: as a consequence, it has to be set with a stable 1+ pilot beam first, switching then to the radioactive one without changing any parameter; this procedure is usually called “blind tuning.” Besides having different masses, pilot and radioactive beams can also differ in terms of the rms transverse emittance ε_{rms} and/or longitudinal energy spread ΔE, so the choice of a given pilot beam can determine the overall performances of the final breeding stage. This paper shows a numerical study of how the capture efficiency of the PHOENIX charge breeder is affected by the aforementioned beam paramaters: the analysis reveals the two-step nature of the process, highlighting the role of the injection optics and the plasma capture capability in the overall performances of this device. Abstract

The present paper describes important features of the immune response induced by the Cry1Ac protein from Bacillus thuringiensis in mice. The kinetics of induction of serum and mucosal antibodies showed an immediate production of anti-Cry1Ac IgM and IgG antibodies in serum after the first immunization with the protoxin by either the intraperitoneal or intragastric route. The antibody fraction in serum and intestinal fluids consisted mainly of IgG1. In addition, plasma cells producing anti-Cry1Ac IgG antibodies in Peyer's patches were observed using the solid-phase enzyme-linked immunospot (ELISPOT). Cry1Ac toxin administration induced a strong immune response in serum but in the small intestinal fluids only anti-Cry1Ac IgA antibodies were detected. The data obtained in the present study confirm that the Cry1Ac protoxin is a potent immunogen able to induce a specific immune response in the mucosal tissue, which has not been observed in response to most other proteins.

Abstract

In 2008, we published the first set of guidelines for standardizing research in autophagy. Since then, this topic has received increasing attention, and many scientists have entered the field. Our knowledge base and relevant new technologies have also been expanding. Thus, it is important to formulate on a regular basis updated guidelines for monitoring autophagy in different organisms. Despite numerous reviews, there continues to be confusion regarding acceptable methods to evaluate autophagy, especially in multicellular eukaryotes. Here, we present a set of guidelines for investigators to select and interpret methods to examine autophagy and related processes, and for reviewers to provide realistic and reasonable critiques of reports that are focused on these processes. These guidelines are not meant to be a dogmatic set of rules, because the appropriateness of any assay largely depends on the question being asked and the system being used. Moreover, no individual assay is perfect for every situation, calling for the use of multiple techniques to properly monitor autophagy in each experimental setting. Finally, several core components of the autophagy machinery have been implicated in distinct autophagic processes (canonical and noncanonical autophagy), implying that genetic approaches to block autophagy should rely on targeting two or more autophagy-related genes that ideally participate in distinct steps of the pathway. Along similar lines, because multiple proteins involved in autophagy also regulate other cellular pathways including apoptosis, not all of them can be used as a specific marker for bona fide autophagic responses. Here, we critically discuss current methods of assessing autophagy and the information they can, or cannot, provide. Our ultimate goal is to encourage intellectual and technical innovation in the field.

Abstract

This work describes the utilization of an injected ^{23}Na^{1+} ion beam as a diagnostics of the helium plasma of a charge breeder electron cyclotron resonance ion source. The obtained data allows estimating the upper limit for the ion-ion collision mean-free path of the incident sodium ions, the lower limit of ion-ion collision frequencies for all charge states of the sodium ions and the lower limit of the helium plasma density. The ion-ion collision frequencies of high charge state ions are shown to be at least on the order of 1–10 MHz and the plasma density is estimated to be on the order of 10^{11} cm^{-3} or higher. The experimental results are compared to simulations of the ^{23}Na^{1+} capture into the helium plasma. The results indicate that the lower breeding efficiency of light ions in comparison to heavier elements is probably due to different capture efficiencies in which the in-flight ionization of the incident 1+ ions plays a vital role.

Abstract

The Advanced Ion Source for Hadrontherapy (AISHa) is an ECR ion source operating at 18 GHz, developed with the aim of producing high intensity and low emittance highly charged ion beams for hadrontherapy purposes. Due to its unique peculiarities, AISHa is a suitable choice for industrial and scientific applications. In the framework of the INSpIRIT and IRPT projects, in collaboration with Centro Nazionale di Adroterapia Oncologica (CNAO), new candidates for cancer treatment (including metal ion beams) are being developed. Moreover, within the IONS experiment, AISHa will be the test-bench for the development of an innovative active plasma chamber designed to increase plasma confinement by changing plasma fluxes. OES technique will be also used to refine techniques of non-invasive plasma diagnostics. Finally, a dedicated setup is under realization to provide impinging beams and detection systems for target production in nuclear physics experiments.

Abstract

Acetaminophen or paracetamol is a widely used analgesic and antipyretic drug and appears to be safe if used at normal therapeutic doses, but in large doses produce liver and / or kidney damage in humans and experimental animals. Prostaglandin endoperoxide synthase (PGES) and cytochrome P-450 are the key enzymes in humans as they are responsible for the analgesic and toxicity effects of paracetamol, respectively. At present, the development of new derivatives still has few impacts on clinical applications of safe compounds. Thus, in this work are discussed, a series of approaches on the design and development of acetaminophen derivatives. Some efforts were realized in our own research group.

Abstract

OBJECTIVE To describe the technique and findings of the 'veterinary focused assessment with sonography for trauma-airway, breathing, circulation, disability and exposure' protocol in dogs suffering from trauma. MATERIALS AND METHODS Prospective observational study on a new point-of-care ultrasound protocol on 64 dogs suffering from trauma and comparison of findings with radiology. RESULTS Comparison of the results of this new ultrasound protocol for trauma patients with radiography findings for pneumothorax, pleural effusion, alveolar-interstitial syndrome and abdominal effusion revealed positive agreement of 89, 83, 100 and 87% and negative agreement of 76, 83, 76 and 92%, respectively. Novel findings of the 'veterinary focused assessment with sonography for trauma-airway, breathing, circulation, disability and exposure' exam, which were not previously reported for dogs undergoing focused assessment with sonography for trauma, included alveolar-interstitial syndrome (suggestive of pulmonary contusions), diaphragmatic hernia, retroperitoneal effusion and tracheal injury. Our new technique may also help identify increased intracranial pressure via changes in optic nerve sheath diameter and haemodynamic instability through the evaluation of the caudal vena cava and cardiac function. CLINICAL SIGNIFICANCE The described ultrasound examination protocol can be rapidly performed on dogs suffering from trauma during resuscitation and it may detect injuries previously undetectable using other veterinary point-of-care ultrasound protocols.

Abstract

The objective of the present study was to evaluate the effect of dynapenia, central obesity and the presence of chronic diseases in 8-year mortality of community-dwelling older adults.Participants comprised 610 older adults, aged ≥65 years at baseline, who participated in the "Frailty in Brazilian Older People" study carried out in 2008. Baseline data, such as weight, height, waist circumference, muscle strength, sex, self-reported diseases and physical activity, were assessed. Vital status in 2016 was assessed by Mortality Information System database of Campinas. The χThere was a statistical difference between alive and deceased groups for the variables sex, age group, physical activity, waist circumference and dynapenia. Path analysis explored the relationship among the studied variables. Direct paths showed a positive association to mortality for those who presented the combination of more diseases (β = 0.105), male sex (β = 0.108), low physical activity (β = 0.121), low handgrip strength (β = 0.090) and no central obesity (β = -0.143). When indirect paths were concerned, variables "central obesity", the presence of "more diseases" and dynapenia had a mediator role.Central obesity was not positively associated with higher mortality, unless it was associated with the presence of chronic diseases. Dynapenia showed a direct effect on mortality, but not combined with central obesity. The findings of this study shed light on complex relationships between nutritional status and elderly mortality through the use of simple measurements. Geriatr Gerontol Int 2019; 19: 108-112.

To determine the direct and indirect relationships of sociodemographic, emotional and behavioral characteristics of family caregivers of Alzheimer's disease patients who have neuropsychiatric symptoms.A total of 134 family caregivers answered a questionnaire with sociodemographic data, and questions regarding the context of care, Neuropsychiatric Inventory, Zarit Burden Interview, Dementia Management Strategies Scale and the Beck Depression Inventory. A path analysis was used to model the direct and indirect relationships among the selected variables.Being a female caregiver, presenting high levels of burden, a moderate degree of depressive symptoms, distress (Neuropsychiatric Inventory total distress score), active management strategies and criticism management strategies were significantly associated with the total score of neuropsychiatric symptoms. The caregiver burden and use of criticism management strategies were directly related to neuropsychiatric symptoms in older adults with Alzheimer's disease. The use of active management strategies and depressive symptoms of caregivers had indirect relationships with neuropsychiatric symptoms through caregiver burden.Overburdened caregivers and the use of dysfunctional management strategies are associated with a greater presence of neuropsychiatric symptoms in Alzheimer's disease patients. These aspects should be considered when developing interventions for caregivers to manage neuropsychiatric symptoms and overall treatment of patients with dementia. Abstract

Chemotherapeutic drug treatment can result in the emergence of drug-resistant cells. By culturing an interleukin-3 (IL-3)-dependent cell line, FL5.12 cells in the presence of the chemotherapeutic drug doxorubicin, we isolated FL/Doxo cells which are multi-drug resistant. Increased levels of drug efflux were detected in FL/Doxo cells which could be inhibited by the MDR1 inhibitor verapamil but not by the MRP1 inhibitor MK571. The effects of TP53 and MEK1 were examined by infection of FL/ Doxo cells with retroviruses encoding either a dominant negative TP-53 gene (FL/ Doxo+ TP53 (DN) or a constitutively-activated MEK-1 gene (FL/Doxo + MEK1 (CA). Elevated MDR1 but not MRP1 mRNA transcripts were detected by quantitative RT-PCR in the drug-resistant cells while transcripts encoding anti-apoptotic genes such as: BCL2, BCLXL and MCL1 were observed at higher levels in the drug-sensitive FL5.12 cells. The percentage of cells that were side-population positive was increased in the drug-resistant cells compared to the parental line. Drug-resistance and sidepositive population cells have been associated with cancer stem cells (CSC). Our studies suggest mechanisms which could allow the targeting of these molecules to prevent drug-resistance.

Abstract

OBJETIVO: Este estudo teve como objetivo verificar a relação entre o bem-estar subjetivo, independência funcional e desempenho de membros inferiores (força muscular, velocidade de marcha e equilíbrio) de idosos em seguimento ambulatorial, em relação ao sexo e a grupos etários. MÉTODOS: Foram avaliados 125 idosos de ambos os sexos com idade mínima de 60 anos, atendidos em um ambulatório de geriatria. Os instrumentos utilizados foram: 1) Medida da Independência Funcional (MIF) para avaliar a dependência funcional; 2) Short Physical Performance Battery (SPPB) para medir o desempenho físico; 3) Bem-Estar Subjetivo (BES): questões sobre a saúde e satisfação com a vida. RESULTADOS: A amostra utilizada foi de conveniência, com predomínio do sexo feminino, que apresentou maior comprometimento funcional. As correlações do bem-estar subjetivo com o teste de desempenho não demonstraram diferenças entre os sexos, contudo os idosos mais velhos apresentaram maior nível de satisfação que os idosos mais jovens. A saúde percebida também foi mais satisfatória entre os idosos mais velhos. Entretanto, a saúde percebida comparada mostrou melhores resultados nos idosos com moderado a bom desempenho físico. CONCLUSÃO: Os resultados sugerem que indivíduos mais velhos apresentam maior satisfação com a vida e melhor saúde percebida. Além disso, o bom desempenho físico foi uma variável de relevância para melhor saúde percebida quando comparada a outras pessoas.OBJECTIVE: The aim of this study was to investigate the relationship between subjective well-being, functional independence and lower-limb performance (muscle strength, gait velocity and balance) among elderly people undergoing outpatient follow-up, stratified by sex and age groups. METHODS: We evaluated 125 elderly people, aged 60 years and over, who received care at a geriatric outpatient clinic. The instruments used were: 1) Functional Independence Measure (FIM) to evaluate functional dependence; 2) Short Physical Performance Battery (SPPB) to measure physical performance; and 3) Subjective Well-Being (SWB) with questions about health and satisfaction with life. RESULTS: A convenience sample was used, with predominance of females (who had greater functional impairment). The Spearman correlation coefficients for subjective well-being and the performance tests varied from -0.16 to 0.31 for men and -0.09 to 0.29 for women, therefore there were no differences between the sexes. However, the older participants had a higher level of satisfaction than the younger participants. Perceived health was also more satisfactory among the older participants, however comparative perceived health was better among the elderly participants with moderate to good physical performance. CONCLUSION: The results suggest that older individuals have greater satisfaction with life and better perceived health. Moreover, good physical performance was an important variable for better perceived health when compared to other people.

OBJETIVOS: Analisar, entre idosos com ocorrência de quedas, a relação entre as variáveis sociodemográficas, clínicas, físicas e funcionais segundo as variáveis critério desempenho físico e número de quedas. MÉTODOS: Estudo transversal com 72 idosos (76,3±8,3 anos) que sofreram quedas no último ano, sendo 65,3% mulheres, atendidos no ambulatório de geriatria de um hospital universitário de Campinas, SP. Os idosos foram divididos em grupo com pior (PDF) e grupo com melhor desempenho físico (MDF), e em grupo com uma queda (1Q) e grupo com duas quedas ou mais (2Q). As características sociodemográficas, clínicas, físicas e funcionais foram as variáveis independentes do estudo. Foi realizada análise de comparação entre os grupos. RESULTADOS: Os idosos do grupo PDF tiveram maior faixa etária, maior número de doenças e menor independência na maior parte das tarefas da dimensão motora em relação ao grupo MDF. Os idosos do grupo 2Q apresentaram maior número de doenças, menor força de preensão manual e menor independência na tarefa "controle das fezes" na dimensão motora da medida de independência funcional (MIF) em relação ao grupo 1Q. CONCLUSÕES: Entre idosos que já caíram, piores níveis de desempenho físico relacionam-se com maior faixa etária, mais doenças e menos independência funcional. Além disso, ter sofrido quedas recorrentes relaciona-se com mais doenças, menos força muscular e menos independência funcional na tarefa de controle de fezes.OBJECTIVES: The aim of this study was to determine the relationship between sociodemographic, clinical, physical and functional data according to physical performance and number of falls among older fallers. METHODS: Cross-sectional study carried out among 72 older adults (76.3 ±8.3 years) with a history of falls in the past year, 65.3% of which were women. The participants received care at the geriatric outpatient clinic of a university hospital in Campinas, SP. They were divided into the following groups: worst physical performance (WPP) and best physical performance (BPP), one-time fallers (1F) and frequent fallers (2F). Sociodemographic, clinical, physical and functional characteristics were considered as independent variables. Comparison analysis between the groups was conducted. RESULTS: The WPP group was older and had a higher number of illnesses and less independence in most motor dimension tasks compared to the BPP group. The 2F group had a higher number of illnesses, less handgrip strength and less independence in the bowel management task in the motor dimension of the Functional Independence Measure (FIM) compared to the 1F group. CONCLUSIONS: Among older adults fallers, poor physical performance is associated with more advanced age, more illnesses and less functional independence. Moreover, recurrent falls are associated with more illnesses, less muscle strength and less functional independence in the bowel management task.

Abstract

There is still disagreement on whether peritoneal dialysis (PD) should begin with a full dose (Full_Dial) or with incremental doses (Incr_Dial) to compensate for the amount of Kt/V no longer supplied by the residual renal function (RRF). The aim of this study is to assess the effects of an Incr_Dial protocol on the choice of dialysis modality, RRF, and adequacy. The Incr_Dial protocol in our center is as follows: for patients with a glomerular filtration rate (GFR)5 ml min(-1), PD is initiated with two exchanges per day (continuous ambulatory PD (CAPD)) or four sessions per week (ambulatory PD (APD)); and hemodialysis (HD) is initiated with two sessions per week. The PD dose is then increased in proportion to the reduction in the GFR as follows: GFRor =5 and3 ml min(-1)=3 CAPD exchanges or five APD sessions; GFR3 ml min(-1)=full dialysis dose (Full_Dial). The effects of the Incr_Dial protocol on the choice of dialysis modality were assessed on 87 patients (pts) (age: 69.3+/-13.1 years) who initiated dialysis between 1 January 2004 and 31 May 2007. The effects of Incr_Dial on RRF and dialytic adequacy were assessed in 11 pts treated with two CAPD exchanges per day for a total of 106 months (mean+/-s.d. 9.7+/-6.5), and then treated with three CAPD exchanges per day for an additional 105 months (9.4+/-8.3). The use of Incr_Dial determined the choice of PD in 27 of 44 pts (61.4%) without indications or contraindications to HD or PD. CAPD was chosen by 20 of these pts (74.1%), whereas APD was preferred by 6 of the 8 pts switched from Incr_Dial to Full_Dial. During Incr_Dial, a significant reduction in the loss of GFR of 2.4+/-3.1 ml min(-1) year(-1) was observed when compared to the pre-dialysis period. Incr_Dial allowed for adequate clearance, as confirmed by the Kt/V (2.07+/-0.2), protein nitrogen appearance (1.17+/-0.13), and biochemical parameters. Ultrafiltration (UF) with icodextrin (772+/-166 ml per exchange) provided a daily UF of 517+/-296 ml day(-1) and remained unchanged when the duration of the dwell time increased significantly from 12.3+/-1.4 to 17.5+/-2.6 h.
