Radical resection of pancreas during pancreatic ductal adenocarcinoma has a favorable effect on the patient’s survival rate and disease prognosis. Performance of PV/SMV resection during pancreatoduodenectomy is a routine procedure in leading pancreatology centers, increasing the possibility of R0 resections. Purpose of the study. Is to evaluate the criteria for the diagnosis of vascular invasion at the preoperative stage, to increase the proportion of R0 resections. The study included 73 patients with locally advanced tumors of the pancreatic head operated on in the pancreatic surgery department from 2019 to 2022. The range of operations included: Whipple pancreatoduodenectomy, pyloruspreserving pancreatoduodenectomy, and total pancreatoduodenectomy. The presence or suspicion of vascular invasion, the size, and the stage of the tumor at the preoperative stage were diagnosed according to the data of MSCT, MRI, ultrasonography, and endosonography. Histological examination was carried out according to the recommendations of the College of American Pathologist 2020. The first group 44 of patients who were made pancreatoduodenectomy with resection of PV/SMV, and the second group 29 patients without vascular resection. In the second group, a positive venous (smv) resection margin, R1 resection, had all patients according to the planned histological examination. During the preoperative examination, vascular invasion was suspected in 68,2% of patients in the first group, and 58.6% in the second, 58,6%, (x2 = 0,696; p = 0,403). According to histological examination, damage to the vascular wall was confirmed in 81,8% of cases among the first group of patients. A comparative assessment of the indicators of the two groups indicates that there is no significant difference in tumor size (t = 1,38; p = 0,085), localization, intraoperative data (operation time) (t = 1,19; p = 0,117), the number of postoperative complications (x2 = 0,027; p = 0,869), and the histological type of the tumor. Only intraoperative blood loss had statistical significant. Conclusions. The nowaday preoperative examination protocol allows diagnosing vascular or suspicion of it with sufficient probability and planning adequate surgical intervention. Performing a resection of PV/SMV during pancreaticoduodenectomy with pre- or intraoperative suspicion of vascular invasion allows to increase the number of R0 resections and improve the prognosis of the disease.

Objective. To elaborate the measures system, which permit to reduce the occurrence of pancreatic fistula and other severe complications after performance of pancreato–duodenectomy. Materials and methods. There were analyzed the results of treatment of 327 patients, in whom pancreato–duodenectomy was performed. In accordance to the scheme proposed, using the elaborated scale of risk for the postoperative pancreatic fistula occurrence with estimation of sarcopenia presence and application of certain prophylactic measures 98 patients were operated in period from November 2018 to December 2020 yr. (the main group). Into the control group 229 patients were included, operated on in the clinic from January 2015 to October 2018 yr. without estimation of risk for the pancreatic fistula occurrence and presence of sarcopenia. The method of pancreato–jejunoanastomosis formation was selected by operating surgeon. Results. Postoperative complications have occurred in 94 (41.0%) patients of the control group and in 28 (28.6%) patients of the main group (c 2 = 4.56, p=0.03). Clinically significant postoperative pancreatic fistula of B Degree have occurred in 9 (9.2%) patients of the main group, what was statistically significantly lower, than in the control group, in which postoperative pancreatic fistula of B or C Degree have occurred in 64 (27.9%) patients (c 2 = 11.6, p=0.0007). Lethality was 2.2% in the control group and 1.02% – in the main one. Conclusion. Introduction of the measures system elaborated have permitted to lower the postoperative pancreatic fistula rate statistically significantly from 27.9 to 9.2%, and of other postoperative complications – from 41.0 to 28.6% and lethality from 2.2 to 1.02%.

Slowing of motor-evacuation function of the stomach after pancreatoduodenal resection: a modern approach to the problem

Мета. Визначення впливу саркопенії на виникнення ускладнень після резекційних оперативних втручань на підшлунковій залозі (ПЗ) з приводу її злоякісних пухлин. Матеріали і методи. Проведено ретроспективний аналіз результатів лікування 87 пацієнтів, яким були виконані радикальні оперативні втручання. За допомогою передопераційної комп’ютерної томографії (КТ) розраховували в одиницях Хаунсфільда (од. Х.) середнє значення щільності (Hounsfield Unit Average Calculation - HUAC) та сумарний індекс (Total Psoas Index - TPI) великого поперекового м’яза на рівні ІІІ поперекового хребця. Результати. У 35 (40,2%) пацієнтів виявили саркопенію, застосовуючи показник HUAC. Післяопераційні ускладнення виникли у 21 (60%) хворого з саркопенією, померли 2 (5,7%) хворих. Післяопераційні ускладнення виникли у 16 (30,8%) хворих без саркопенії, помер 1 (1,9%) хворий. Застосовуючи показник TPI, саркопенію діагностували у 38 (43,7%) хворих, із них ускладнення виникли у 21 (55,3%), померли 2 (5,3%). Ускладнення виникли у 16 (32,6%) хворих без саркопенії, помер 1 (2,0%) хворий. Висновки. Саркопенія достовірно впливає на частоту післяопераційних ускладнень, її виявлення може бути використано для покращення відбору пацієнтів перед виконанням резекції ПЗ з приводу її злоякісних новоутворень.

Проаналізовані результати діагностики й хірургічного лікування 231 хворого з приводу пухлинного ураження лівого анатомічного сегмента підшлункової залози (ПЗ) в період з 2009 по 2014 р. Відзначені особливості клінічного перебігу пухлинних утворень цієї локалізації, можливості різних методів інструментального дослідження в їх діагностиці та диференційній діагностиці. Розроблений ефективний діагностично-лікувальний алгоритм, визначені діагностичні можливості існуючих та нових інструментальних методів дослідження. Обгрунтовані показання, наведені методики, впроваджені в практику, оцінені результати застосування резекційних втручань на лівому анатомічному сегменті ПЗ, в тому числі з використанням лапароскопічного доступу.
