La stérilisation est essentielle dans la prévention des infections nosocomiales et pour la sécurité des patients. Cette étude a pour objectif d'évaluer le respect des normes du processus de stérilisation des dispositifs médicaux et textiles opératoires dans hôpitaux du Mali et du Sénégal, en Afrique de l'ouest. Il s'agissait d'une étude prospective descriptive réalisée sur une période de quatre mois. Les données ont été recueillies par entretien avec les surveillants des unités de stérilisation centrale et par observation directe des pratiques de stérilisation. Elles ont été saisies et analysées avec le logiciel Epi info3.3.2. Des produits de nettoyage et de désinfection conformes aux normes en vigueur étaient disponibles dans 3 des 7 hôpitaux enquêtés. L'usage d'un emballage secondaire était pratiqué dans 2 hôpitaux au Sénégal. Au Mali, la moitié des hôpitaux stérilisait leurs dispositifs métalliques à la vapeur d'eau tandis qu'au Sénégal tous les hôpitaux le faisaient. Les unités de stérilisation évaluées dans les deux pays n'avaient pas encore mis en place un système d'assurance qualité. Dans 6 des hôpitaux, la place du pharmacien en unité de stérilisation était occultée et le personnel n'avait pas bénéficié de formation selon les bonnes pratiques de Stérilisation. Les insuffisances relevées ont permis de formuler des recommandations pour une meilleure pratique de la stérilisation et partant, l'amélioration de la qualité des soins dans les deux pays. Objectif : Évaluer l'impact de l'usage des gants médicaux sur l'observance de l'hygiène des mains et promouvoir leur usage approprié. Matériel et Méthodes: Il s'agissait d'une étude interventionnelle avec une évaluation avant et après intervention. Sa mise en œuvre a été réalisée entre avril 2010 et octobre 2011 au Centre Hospitalier et Universitaire du Point G. Le recueil du consentement éclairé et les observations ont été effectués auprès du personnel soignant ayant un contact direct avec les patients avant et après intervention. Ces observations ont été discrètes mais ouvertes, menées auprès des soignants préalablement informés du but et du programme des activités. Résultats : Au total, 143 fiches de consentement éclairé ont été retournées sur une prévision de 274 fiches, soit 52,19%. Les observations ont donné un taux d'observance globale de 52,05% avant intervention contre 42,97% après intervention (p = 0,0017). Le taux global de port de gants était de 23,30% et 27,20% respectivement avant et après intervention (p = 0,12). Conclusion : Les résultats des deux évaluations avant et après intervention ont montré une amélioration du taux global de port des gants qui n'était pas significative mais sans impact sur l'observance de l'hygiène des mains. COVID-19 was declared a pandemic in March 2020. For case management, Mali has created several treatment sites including the site of the CHU Gabriel Touré. Aims: The objective of the study was to analyse drug prescriptions for the COVID-19 treatment at the CHU Gabriel Touré. Methods: We performed a retrospective and descriptive study from April to September 2020. Drug prescriptions and hospital records were used to collect data. The median age was 44 years, 75.90% of patients had at least one pathology associated with COVID-19. The number of prescriptions was 333 comprising 870 lines of prescriptions including 33.21% for standard treatments, and 66.79% for associated pathologies. with 86.23% available at the Hospital Pharmacy. Chloroquine, dosed at 250mg, was administered at 500mg twice a day. The national guidelines from the treatment of COVID-19 recommends 200mg of chloroquine in two doses. Vitamin C was prescribed for all patients although not included in the national guidelines. The class of drugs for the blood and blood-forming organs was the most prescribed (31.49%). The average cost of treatments was 65,602 ± 106,858 FCFA with a maximum of 567,860 FCFA. An evaluation of prescriptions in other treatment sites is necessary. [ABSTRACT FROM AUTHOR]

Background: During the 2014-2015 Ebola outbreak in West Africa, the lack of infection prevention and control (IPC) measures in health care facilities amplified human-to-human transmission and contributed to the magnitude of this humanitarian disaster.Case ReportIn the summer of 2014, the Geneva University Hospitals (HUG; Geneva, Switzerland) conducted an IPC assessment and developed a project based on the local needs and their expertise with the support of the Swiss Agency for Development and Cooperation and the Humanitarian Aid Unit (SDC/HA; Bern, Switzerland). The project consisted of building local capacity in the production of alcohol-based hand-rub solution (ABHRS) based on the World Health Organization (WHO; Geneva, Switzerland) formula in non-Ebola health facilities at the peak of the outbreak in Liberia (Fall 2014) and during recovery in Guinea (September 2015) to promote safer care. Twenty-one pharmacists in Liberia and 22 in Guinea were trained and one years' worth of laboratory equipment, chemical products, containers for personal use, and bioethanol were delivered to 10 hospitals per country with more than 8,000 100 ml bottles of solution produced at the end of the project.DiscussionHand hygiene using hand-rub solution is a critical component of safer care, especially in health care settings lacking runnable water. Throughout the Ebola outbreak, it was a timely moment to promote hand-rub solution and to reinforce IPC measures in non-Ebola health facilities. During the project implementation, a substantial challenge was the unavailability of bioethanol in Liberia and Guinea. In the long run, sustainability of the production can become an issue as it depends heavily on the local government's financial and political commitment, the capacity to create an on-going demand for hand-rub solution in health facilities, the local purchase and replacement of the materials and chemical products, as well as the availability of continuous local partners' support., Conclusion: The project demonstrated that it was feasible to build local capacity in ABHRS production during an emergency and in limited-resource settings when materials and training are provided. Future programs in similar contexts should identify and address the factors of sustainability during the implementation phase and provide regular, long-term technical support. Background: Health-care-associated infections are a major threat to patient safety worldwide. Transmission is mainly via the hands of health-care workers, but compliance with recommendations is usually low and effective improvement strategies are needed. We assessed the effect of WHO's strategy for improvement of hand hygiene in five countries., Methods: We did a quasi-experimental study between December, 2006, and December, 2008, at six pilot sites (55 departments in 43 hospitals) in Costa Rica, Italy, Mali, Pakistan, and Saudi Arabia. A step-wise approach in four 3-6 month phases was used to implement WHO's strategy and we assessed the hand-hygiene compliance of health-care workers and their knowledge, by questionnaire, of microbial transmission and hand-hygiene principles. We expressed compliance as the proportion of predefined opportunities met by hand-hygiene actions (ie, handwashing or hand rubbing). We assessed long-term sustainability of core strategy activities in April, 2010., Findings: We noted 21,884 hand-hygiene opportunities during 1423 sessions before the intervention and 23,746 opportunities during 1784 sessions after. Overall compliance increased from 51.0% before the intervention (95% CI 45.1-56.9) to 67.2% after (61.8-72.2). Compliance was independently associated with gross national income per head, with a greater effect of the intervention in low-income and middle-income countries (odds ratio [OR] 4.67, 95% CI 3.16-6.89; p<0.0001) than in high-income countries (2.19, 2.03-2.37; p<0.0001). Implementation had a major effect on compliance of health-care workers across all sites after adjustment for main confounders (OR 2.15, 1.99-2.32). Health-care-workers' knowledge improved at all sites with an increase in the average score from 18.7 (95% CI 17.8-19.7) to 24.7 (23.7-25.6) after educational sessions. 2 years after the intervention, all sites reported ongoing hand-hygiene activities with sustained or further improvement, including national scale-up., Interpretation: Implementation of WHO's hand-hygiene strategy is feasible and sustainable across a range of settings in different countries and leads to significant compliance and knowledge improvement in health-care workers, supporting recommendation for use worldwide., Funding: WHO, University of Geneva Hospitals, the Swiss National Science Foundation, Swiss Society of Public Health Administration and Hospital Pharmacists., (Copyright © 2013 World Health Organization. Objective: To assess the feasibility and effectiveness of the World Health Organization hand hygiene improvement strategy in a low-income African country., Design: A before-and-after study from December 2006 through June 2008, with a 6-month baseline evaluation period and a follow-up period of 8 months from the beginning of the intervention., Setting: University Hospital, Bamako, Mali. Participants. Two hundred twenty-four healthcare workers., Methods: The intervention consisted of introducing a locally produced, alcohol-based handrub; monitoring hand hygiene compliance; providing performance feedback; educating staff; posting reminders in the workplace; and promoting an institutional safety climate according to the World Health Organization multimodal hand hygiene improvement strategy. Hand hygiene infrastructure, compliance, healthcare workers' knowledge and perceptions, and handrub consumption were evaluated at baseline and at follow-up., Results: Severe deficiencies in the infrastructure for hand hygiene were identified before the intervention. Local handrub production and quality control proved to be feasible, affordable, and satisfactory. At follow-up, handrubbing was the quasi-exclusive hand hygiene technique (93.3%). Compliance increased from 8.0% at baseline to 21.8% at follow-up (P < .001). Improvement was observed across all professional categories and medical specialities and was independently associated with the intervention (odds ratio, 2.50; 95% confidence interval, 1.8-3.5). Knowledge enhanced significantly (P < .05), and perception surveys showed a high appreciation of each strategy component by staff., Conclusions: Multimodal hand hygiene promotion is feasible and effective in a low-income country. Access to handrub was critical for its success. These findings motivated the government of Mali to expand the intervention nationwide. This experience represents a significant advancement for patient safety in developing countries.
